The Effects of Task-Related Circuit Exercise Program Combined with Sensorimotor Training on Balance and Walking in Persons with Stroke : A pilot study (감각운동 훈련을 병행한 순환식 과제 지향 운동프로그램이 뇌졸중 환자의 보행 및 균형에 미치는 영향 : 예비연구)

  • Purpose: The purpose of this study was to identify whether task-related circuit exercise program combined with sensorimotor training for 4 weeks could improve the balance and gait in stroke patients. Method: Fifteen stroke patients who had agreed with the study were randomly divided into 3 groups categorized as task-related circuit exercise program combined with sensorimotor training group (experimental group 1, n=5), task-related circuit exercise program group (experimental group 2, n=5), and control subjects performed conventional physical therapy (control group, n=5). The balance and gait were assessed by BT-4 force platform system, Berg Balance Scale, 10meter Walk Test and Smart Step at before training and after training. Wilcoxon signed rank test was used to analyze change before and after intervention in intra-group. Kruskal Wallis H test, Mann-Whitney U test and Bonfferoni correction were used to analyze changes of all variables in inter-groups. Result: The experimental group 1 showed significant improvements in postural sway area, BBS scores, walking velocity and plantar pressures of affected foot, whereas the experimental group 2 showed significant improvements in BBS scores, and the control group were no significantly different in all variables following training. The changes of postural sway area and BBS scores in the experimental group 1 were significantly greater than them of the control group. The changes of postural sway area in the experimental group 1 was significantly greater than that of the experimental group 2. Conclusion: The result of this study suggest the task-related circuit exercise program combined with sensorimotor training is an effective intervention to improve balance and gait in stoke patients.

Effects of Manual Intervention and Self-Corrective Exercise Models of the General Coordinative Manipulation on Balance Restoration of Spine and Extremities Joints

  • The purpose of this study was conducted in order to analyze the effects of the manual intervention and self-corrective exercise models of general coordinative manipulation(GCM) on the balance restoration of spine & extremities joints with distortions and mal-alignment areas. The subjects were the members who visited GCM Musculoskeletal Prevent Exercise Center from March 1 2012 to December 31 2013 because of spine & extremities joints distortion and mal-alignments, poor posture, and body type correction. All subjects were diagnosed with the four types of the GBT diagnosis. And according to the standards of the mobility vs stability types of the upper & lower body, they were classified into Group 1(40 persons) and Group 2(24 persons). For every other day for three times a week, GCM intervention models were applied to all subjects for four weeks, adding up to 12 times in total. Then the balance restoration effects were re-evaluated with the same methods. The results are as follows. 1) Balance restoration effects of VASdp(Visual analysis scale pain & discomfort) and ER(Equilibrium reaction: ER) came out higher in GCM body type(GBT) II III IV of Group 1. 2) In case of balance restoration effects in Moire and postural evaluation areas, Group 1 was higher and cervical and scapular girdle were higher in Group 2. The balance restoration of the four GBT types was significant in all regions(p<.05), and the scapular girdle came out as high in the order of GBTII IV I. 3) In case of thoracic-lumbar scoliosis and head rotation facial asymmetric cervical scoliosis ribcage forward, the balance restoration effects of the upper body postural evaluation areas came out the highest in Group 1 and Group 2, respectively. The balance restoration effects of the four GBT types were significant in all regions(p<.05), and came out the highest in lumbar scoliosis GBTIII I, ribcage forward and thoracic scoliosis GBTII IV. 4) The balance restoration effects of the lower body postural evaluation areas came out higher in Group 1 and Group 2 for pelvis girdle deviation patella high umbilicus tilt and hallux valgus foot longitudinal arch: FLA patella direction, respectively. The balance restoration effects of the four GBT types were significant in all regions(p<.05), and came out the highest in pelvis girdle deviation GBTIII I and patella high-direction GBTIV II I. 5) The balance restoration effects between the same GBT came out significant (p<.05) in all evaluation areas and items. The conclusions of this study was the manual intervention and self-corrective exercise models of the GCM about the mal-alignment of the spine & extremities joints across the whole body indicated high balance restoration effects(p<.05) in spine & extremities joints in all evaluation areas.

Effects of complex exercises in pressure-pain threshold, pain and postural alignment of women university students with shoulder pain (복합운동이 어깨통증을 경험한 여대생들의 압통역치, 통증 및 자세정렬에 미치는 영향)

  • The purpose of this study was to examine if complex exercises could improve pressure-pain threshold and postural alignment in university students. Subjects in the CON group did not receive an intervention. Subjects in the EXP1 group were applied stretching exercises after applying hot packs on their both shoulders. Subjects in the EXP2 group were applied complex exercises including stretching, stabilization exercises of shoulder girdle and correction of wrong postures. Changes in pressure-pain threshold in the EXP1 group and EXP2 group were greater than that in the CON group (p<0.05). Changes in VAS scores in the EXP1 group and EXP2 group were greater than that in the CON group (p<0.05). Changes of postural alignment did not significantly differ among three groups (p>0.05). The results of this study indicates that complex exercises including stretching exercises, stabilization exercises of shoulder girdle and correction of wrong postures about neck and scapular can be an effective in reduction of shoulder pain and increase of pressure-pain threshold in women university with shoulder pain.

Comparison of Thoracic Reposition Error in Individuals With or Without Habitual Slouched Posture

  • In this study, we present the difference in thoracic reposition sense in young people (24.00 ±2.20 years old) with and without habitual slouched posture in two target positions of half flexion and half extension. People with habitual slouched posture (n = 20; 11 men and 9 women) and people without slouched posture (n = 20; 10 men and 10 women) were recruited from three universities. Thoracic spine (T-spine) repositioning errors were measured in two target positions (half flexion and half extension). People with habitual slouched posture showed significantly higher thoracic repositioning error in the extension target position than did people without slouched posture (P<0.05). There was no difference in repositioning error in the flexion target position between the two groups (P>0.05). In conclusion, people with habitual slouched posture show lower T-spine repositioning sense in extension than do people without slouched posture. A rehabilitation program to treat habitual slouched posture, such as postural correction education, should be implemented for individuals with decreased position sense of the T-spine.

  • Structural Integration(SI), known popularty as 'rolfing', is a systematic programme of connective tissue manipulation. thomas w. Myers who has been working with Ida Rolf's recipe since 1975 presents an alternative and developed version(the Anatomy Trains 12-session recipe(ATR)) based on longitudinal myofascial continuities. In comparison with Ida Rolf's recipe, AR has the different base of myofascial meridian. while they have the same outline, principle, and intention. We intend to introduce an effective approach to postural correction and myofascial treatment through discussion on ATR.

Effect of Self-Stretching Exercises on Postural Improvement in Patients with Chronic Neck Pain Caused by Forward Head Posture (자가 신장운동이 전방머리자세를 동반한 만성 목통증 환자의 자세개선에 미치는 영향)

  • PURPOSE: The purpose of this study was to provide clinical basic data to reduce pain and improve function by comparing neck muscle activity and neck alignment using self-stretching and passive stretching exercises for chronic neck pain caused by forward head posture. METHODS: The subjects were divided into 15 subjects assigned to perform self-stretching exercise and 15 subjects assigned to perform passive stretching exercise. The intervention was conducted for a total of 4 weeks. The muscle activity in the neck was measured by surface electromyography (EMG) before intervention, and craniovertebral and cranial rotation angles were measured by X-ray. The 4-week intervention was conducted and the above items re-measured in the same manner and analyzed. RESULTS: Muscle activity within both groups after intervention using self-stretching or passive stretching exercise was significantly different (p < .05)(p < .01). Neck alignment of both groups was significantly different (p < .001)(p < .01). Further, muscle activities of the upper trapezius and splenius capitis muscles showed significant differences (p<.05). Lastly, neck alignment showed statistically significant difference (p < .05). CONCLUSION: Self-stretching exercise activated motor nerves as a posture correction exercise, thereby improving inhibition of muscle activity, muscle contraction delay, and pathological conditions of the muscle. For future research, interventions of self-stretching exercise will be needed for patients with chronic back pain accompanied by forward head posture, and various clinical studies on postural improvement of forward head posture by maintaining a normal muscle tone state are needed.

Immediate Effects of a Postural Correction Garment Designed for Postural Kyphosis on Adolescents With Thoracic Hyperkyphosis: A Pilot Study

  • The purpose of this study was to investigate the changes in the flexicurve kyphosis index (KI), the flexicurve lordosis index (LI) and the distance from the inferior angle of the scapula to the nearest vertebral spinous process (DS), as a dependent variable of scapular protraction, after applying of adolescents with thoracic hyperkyphosis using three different garments. A repeated measures design was used. Ten adolescents ($15.8{\pm}1.0$ years) with thoracic hyperkyphosis ($40.1{\pm}3.7$ Cobb angle) were recruited from a university hospital. A flexicurve ruler was used to measure KI and LI and a scoliometer was used to measure DS under three different conditions: wearing-a hospital garment (HG), wearing-a sham garment (SG), and wearing-an experimental garment (EG). KI under EG condition was significantly decreased compared with that wearing the HG. However, there was no significant difference between wearing the SG and HG. LI when wearing the EG was significantly increased compared with that when wearing HG. However, there was no significant difference under SG and HG conditions. DS when wearing the EG was significantly decreased compared with wearing HG. However, there was no significant difference between the SG and HG. The results of this study show that the EG was effective in decreasing KI, but not effective in decreasing LI. Hence, the effect of the EG for correcting sagittal spine angle in adolescents with thoracic hyperkyphosis is still debatable. However, since we showed that DS decreases in the EG, this method could be applied in correcting the scapular protraction.

  • Objectives: The purpose of this study is to report the effect of Postural Yinyang Correction of Temporomandibular Joint (Functional Cerebrospinal Therapy, FCST) on a patient with Osteoarthritis of the hip caused by Hip dysplasia and Lumbar spinal stenosis. Methods: A patient with Osteoarthritis of the hip caused by Hip dysplasia and Lumbar spinal stenosis was treated at Dept. of Acupuncture & Moxibustion, ○○ University Korean Medicine Hospital from Nov 8th, 2019 to Dec 6th, 2019 and received a Korean-Western medical treatment mainly managed with FCST. This study was measured with VAS (Visual Analogue Scale), ODI (Oswestry Disability Index) and questionnaire. Results: After treatment, the patient's pain was controlled and gait ability was improved, also VAS, ODI and questionnaire score were improved. Conclusions: Korean-Western Medical Treatment mainly managed with FCST may be helpful in controling pain with Osteoarthritis of the hip caused by Hip dysplasia and Lumbar spinal stenosis, but the further researches are needed.

Evaluation of the Usefulness of Ancillary Devices for Patients with Acute Lumbar Pain During Magnetic Resonance Imaging (자기공명영상검사 시 급성 요추 통증 환자를 위한 보조기구의 유용성 평가)

  • The purpose of this study was to evaluate the convenience and image quality of patients with acute lumbar pain patients at a general hospital in Daejeon using ancillary devices for postural changes and correction. The results of the study are summarized as follows. First, the Turbo S pin Echo technique(TS E) using ancillary equipment has the highest image evaluation rating with an average score of 4.440, which is highly valuable on a diagnosis. Second, the average score for patient the questionnaire 'When using ancillary equipment, I feel that my body is calibrated to side without bias.' was shown as 4.440, which is very useful for the correction of the patient's body when using ancillary equipment. Finally, Breath Hold technique(BH) is very effective in shortening test time of acute lumbar pain patients, because it can reduce test time 86.4% faster than Turbo Spin Echo technique(TSE). The results of the study showed that the use of ancillary equipment to perform the test through the side lying postures helped to reduce the pain and control the patient's breathing, and the diagnostic value of the image was high.

Effect of Physical Therapy Based Tailored Exercise Program on Pain, Accident incidence Rates, and Lost Days of Work in Manufacturing Worker: Single Subject Design (제조업 근로자의 근골격계 질환 예방을 위한 물리치료 기반 맞춤형 운동프로그램이 통증, 재해율, 및 근로손실에 미치는 영향: 단일사례 연구)

  • PURPOSE: The purpose of this study was to investigate the effect of a physical therapy-based tailored exercise program on pain, accident incidence rates, the number of work days lost, and economical loss cost for workers in an automobile parts manufacturing company. METHODS: A total of 530 workers with musculoskeletal symptoms were given a physical therapy-based tailored exercise program twice a week, for one hour a day. This exercise program consisted of movement pattern correction, muscle stretching and strengthening, and postural correction exercises, according to principles of movement impairment syndromes and medical training therapy. From 2011 to 2016, the lost days of work, accident incidence rates, and loss cost were examined. The pain measured by VAS (visual analogue scale) and the number of workers participating in the exercise program from 2014 to 2016 were also measured. The single subjects A-B design was applied and analyzed. RESULTS: After applying the exercise program, pain decreased and the number of workers participating in the program increased. Accident incidence rates, number of work days lost, and economical loss cost decreased. There was a significant correlation between the number of workers who received exercise therapy by year and accident incidence rates, lost days of work, and economical loss cost (p<.05). CONCLUSION: It is necessary to expand the physical therapy-based tailored exercise program to prevent musculoskeletal disorders because it has a positive effect on both workers and employers.
