Perceived Social Support and Adaptation to the Maternal Role in First-time Mothers during the Postpartum Period (산욕기 초산모가 지각한 사회적 지지와 어머니 역할 적응과의 관계연구)

  • The relationship between perceived social support and adaptation to maternal role for first-time mothers was investigated in this descriptive correlational study. A nonprobable sample of 90 first-time mothers were selected, who had uncomplicated perinatal experiences and delivered healthy and term newborns as well. The data was collected during a home interview at 4-6weeks postpartum. The outcome of adaptations was defined as the level of sensitivity in parent-infant interactions and of the self confidence in infant care. The perception of social support in the primiparous was assessed by the NSSQ during the postpartum. The results obtained from this study are summarized as follows : 1. The mean score of the perceived total functional support was $116.6{\pm}37.5$ points (affective : 38.1 affirmative : 39.3, aid : 39.3), and the score of the total network support was $45.2{\pm}13.9$ points (size : 4.9, duration :19.8 frequency : 20.4). These scores tended to be slightly low. 2. The mean score of the self confidence on the infant care activity as the subjective aspect of the maternal role adaptation (MRA) was 56.5 points (86.9%), whereas that of the sensitivity of the mother-infant interaction of the MRA was 78.9 points (63.2%). 3. The subjective aspect of the MRA has showed a positive relation ship with the aid dimension of the functional support. And the objective aspect of the MRA also showed a positive relationship with the total functional support and the total network support. However the correlating degrees were slightly low. In conclusion, the primiparous mothers perceived that they had received a small amount of social support during the postpartum period, suggesting the need of various kinds of social support to promote the MRA for the primiparous.

A STUDY TO DETERMINE THE LEVEL OF OBSTETRIC KNOWLEDGE OF PREGNANT WOMEN AND POSTPARTUM MOTHERS (병원을 이용하는 임산부들의 산과적 지식정도에 관한 일 조사 연구)

  • The purpose of this study was to investigate the level of obstetric knowledge of pregnant women and postpartum mothers, and to contribute the data to community health teaching program as well as maternity nursing. 385 pregnant women and postpartum mothers visited at or admitted to Seoul Adventist Hospital were sampled. The results of the study were as follows. 1. 97.9% of the subjects were between the age of 21 to 35, most of them (82.9%) were housewives with high school education and with a middle class family background. 2. The first symptoms of pregnancy were amenorrhea (70.4%), nausea & vomiting (23.9%), change of skin color (1.6%). 3. 77.9% of the subjects did not know how to deal with morning sickness. 4. 52.9% did not know when the sex determination of the fetus-occurs. 5. Concerning the fetus position; 77.1% of them believed it lies head down. head up (12.0%). and not knowing (10.9%). 6. 73.2% recognized the need for a well balanced food during pregnancy. 7. As to the dangerous symptoms during pregnancy: vaginal bleeding was considered the 1st, edema, the and, continuous abdominal pain, the 3rd. 82% of them would confer with physician in case of vaginal bleeding. As to the type of activities restricted by pregnancy: hard exercise was considered first, travel the second, and coitus during 1st and last trimester of pregnancy the 3rd. 8. 82.4% of them visited antenatal clinic at the second or third month of amenorrhea and 88.5% realized the regular physical examination imperative. 9. The purpose of urine test was recognized correctly by 44.7%, blood test: 89.4%, B. P. and weight check ; 69.9%. 10. The need of breast care during pregnancy was recognized by 76.9% of the subject. 11. 75.8% realized that prenatal exercise was needed. The higher the education the better the understanding of the need for it before delivery. 12. The concept of postpartum period was understood correctly by 54.4%. 13. The state of uterus after delivery was understood correctly by 49.1% only. 14, Regarding the management of colostrums: 48.3% knew it was to feed infants, and it was to squeeze and discard 43.4%. 15. Dealing with postpartum exercise; 67.8% answered it was needed, 9.1% not needed, not heard about 23, 1%.

A Study on the Correlation between Auricular Tenderness Testing on Mammary Gland Zone and Postpartum Breast Pain (산모에서 산후 유방통과 이혈 압통점과의 관계 연구)

  • Objectives : The objectives of this study were to examine the specificity and the sensitivity of tenderness testing on the ear about the postpartum breast pain, and to improve the efficacy of the tenderness test. Methods : Thirty women who visited in the hospital for recuperating 2-14 days after childbirth participated in this study. They answered the questionnaire of the severity of breast pain and their auricular acupuncture points were tested by a long metal probe with round tip 1.1mm in diameter. We analyzed the relationship between auricular tenderness testing on mammary gland zone[CW6] and postpartum breast pain with validity and significance as grouping 8 diagnosis methods. Results : When we consider above 'moderate pain' or 'severe pain' in the auricular tenderness testing as a positive sign, and above 'evere pain' or 'very severe pain' in the self-reported breast pain score as a positive symptom, the diagnostic efficacy was highest. The sensitivity for tenderness testing was 0.62-0.75, and the specificity was 0.64-0.72, and the sensitivity and the specificity in the contralateral ear were a little higher than in the ipsilateral ear. Conclusions : These results suggest that auricular tenderness testing can be objectified clinically and be used in diagnosis.

Clinical trial of Herbal Decoction for Sitz Bath on Postpartum Mother's Perineal Healing (한방좌욕제가 산모의 회음절개술후 동통에 미치는 효과에 대한 임상연구)

  • Purpose: This study was designed to verify the effect of Herbal Decoction for Sitz Bath on postpartum mother's perineal healing. Methods: Women who had given birth vaginally and had an episiotomy were included in study. The primary outcome for study was severity of pain, discomfort, rated on a 10-cm visual analog scale from 0("no pain") to 10("worst pain ever"). As a secondary outcome, we also evaluated side effect and "feeling". Results: In clinical trial, perineal pain, perineal discomfort, after-pain were significantly low at postpartum 3th days, respectively. And mothers were satisfied with "Herbal Decoction for Sitz Bath". Conclusion: According to these results, we finally concluded that "herbal decoction for sitz bath" for perineal care could be effective in healing the perineum.

Combination Therapy of Conventional and Korean Medicine for Multiple Vertebral Compression Fractures Associated with Postpartum Osteoporosis: A Case Report

  • Multiple vertebral compression fractures are a rare condition and are a serious consequence of postpartum osteoporosis. This report describes the case of a 35-year-old woman who had given birth to her first child 2 months before the onset of pain. Magnetic resonance imaging showed compression fractures of 7 vertebrae. The patient was treated with both conventional, and Korean medicine methods including acupuncture and herbal prescriptions. The patient's progress was assessed using self-reported symptoms, scale scores and laboratory test results. Her pain was gradually alleviated and biochemical inflammation marker levels improved, but her functional status remained severely impaired. Clinical practitioners who treat women in the postpartum period must be aware of osteoporosis and potential vertebral fractures and need to consider Korean medicine as an alternative therapy to help such patients.

A Study of the Change of Body Weight and BMI during Some Pregnant and Postpartum Periods Based on the Sasang Constitution (일부 산모의 사상체질에 따른 임신.출산전후의 체중 및 BMI 변화에 관한 고찰)

  • Purpose: As body weight management is significantly important for women and newborn infants during pregnant and postpartum periods, there have been studies about changes of body weight during those periods. However, there are not enough studies about it which is based on the Sasang Constitution. The following is a result of the study which analyzes the changes with Sasang Constitution. Methods: This study investigated those who understand and agree this study#s purposes out of 109 primiparae who did normal delivery, who gave birth to a healthy infant and who are breastfeeding and who were treated by S postpartum care center from March 2005 to November 2005. The result of survey was collected from the questionnaires that included postpartum pattern and QSCC II. Results: There are remarkable changes of body weight and BMI based on the Sasang Constitution in pregnant and postpartum periods and they appear to be in order of Taeumin, Soyangin, Soeumin. In terms of the amount of milk according to the Constitution, there are no key changes between Soeumin and Taeumin, but there is a significance between Soyangin and Soeumin and Soyangin and Taeumin. In terms of the average body weight of infants according to the Constitution, there are no essential changes between Soeumin and Soyangin, but there is a magnitude between Soeumin and Taeumin and Soyangin and Taeumin. Conclusion: The changes of the body weight and BMI based on the Sasang Constitution appear to be the smallest for Soeumin, and the largest for Taeumin. The amount of milk from mothers did not affect on the changes and the weight of newborn infant did affect on the changes.

Effects of Breast Massage on Breast Pain, Breast-milk Sodium, and Newborn Suckling in Early Postpartum Mothers (유방마사지가 산욕초기 어머니의 유방 통증, 유즙 소디움, 신생아 흡유 횟수에 미치는 효과)

  • Purpose: In this study the effects of breast massage on breast pain, breast-milk sodium, and newborn suckling in early postpartum mothers were investigated. Methods: The design was a non-synchronized nonequivalent control group pretest-post-test design. Sixty postpartum mothers who were admitted to a postpartum care center and had problems with breastfeeding were recruited. Of these mothers, 44 were assigned to the intervention group and received two 30-minute breast massages within 10 days of postpartum period. The others were assigned control group and received only routine care. Breast pain was measured using a numeric pain scale and number of times newborns suckled was observed throughout breastfeeding. Breast milk was self-collected to evaluate breast-milk sodium. Results: Mean age of postpartum mothers was 30 years old. Compared to the control group, women in the intervention group reported significant decreases in breast pain (p<.001), increases in number of times newborns suckled after the first and second massage (p<.001), and a decrease in breast-milk sodium after the first massage (p=.034). Conclusion: Breast massage may have effects on relieving breast pain, decreasing breast-milk sodium, and improving newborn suckling. Breast massage can be used to solve breast problems. Further research is needed to validate our findings.

A Literature Review on the Effects of Chuna Manipulation for Postpartum Pelvic Pain and Low Back Pain (산후 골반통에 추나 수기 요법의 효과에 대한 문헌 고찰)

  • Objectives: The purpose of this review is to assess the effects of Chuna manipulation for postpartum pelvic pain and low back pain. Methods: To investigate the effects of Chuna manipulation for postpartum pelvic pain and low back pain, we searched the seven database (Korean Obstetrics and Gynecology, Oasis, National Digital Science Library, Pubmed, Cochrane Library, Embase, China National Knowledge Infrastructure) and manually checked related clinical trials up to May, 2019. Results: Seven studies were included in this review. Chuna and other manipulation (Osteopathy and Tuina) were effective in relieving postpartum pelvic pain and improving Oswestry Disability Index. Conclusion: This review suggests Chuna manipulation produces clinically relevant benefits for postpartum women with pelvic pain and low back pain. Further research may change estimates of effect, and larger, high-quality randomized controlled trials with robust comparison groups are recommended.

A Study on the Postpartum Depression Experience : Q-Methodological Approach (산후 우울 경험에 관한 연구 -Q 방법론적 접근-)

  • The purpose of this study is to explore types of postpartum depression and to understand the nature and structure of the postpartum depression by using Q-methodological approach. As a way of research, 55 statements concerning postpartum depression were selected through individual interviews with postpartum mothers and literature review. 30 women were chosen as a subject group for the study, with opinions shown in 55 statements divided into 9 scales by forced distribution. PC QUANL Program was used for analysis and Q-factors were analyzed by using principal component analysis. As a result, postpartum depression experience was classified into 5 types. There are "Role-Strain Type", "Unattributional Depression Type", "Psychosomatic Symptoms Type", "Self-Compassion Type", and "Role-Crisis Type". Type I was named "Role-Strain Type", referring to the strain generally experienced by mothers with regard to the new role as a mother and as a social member. Type II was named "Unattributional Depression Type", referring to the symptom experienced by people who were in a state of vanity and a sense of loss. They often break into tears for no specific reasons. In case of Type III, people in a state of "Psychosomatic Symptoms Type" develop physical symptoms after suffering from inherent emotional conflict. Type IV was named "Self-Compassion Type" refers to the symptom shown by those who feel pity for their children and for themselves. And they show inability to cope with the reality properly. Type V was named "Role -Crisis Type", which is experienced by people who have a burden and a severe fear of their own job and their children in their mind, also showing serious conflict with maternal role. Futhermore, it was carried out to examine structure of postpartum depression in terms of degree of depression and adjustment ability. Type I showed mild degree of depression and relatively good adjustment ability. Type II showed broad range of degree in depression and moderate adjustment ability. Type III showed moderate depression and relatively low adjustment ability. Type IV revealed relatively serious degree of depression and the lowest adjustment ability. Type V revealed very serious degree of depression and the lowest adjustment ability. As a result, considering the structure of postpartum depression. Type I is considered to be a normal depression sympton which most mothers generally experience, followed by Type II, Type III, Type IV and Type V, each of which show increasingly worse degree of depression and lower adjustment ability. In conclusion, it seems to be it is necessary to understand distinct symptoms of postpartum depression and to examine the characteristics and structure of those types, so that it could lead to more individual nursing approach.cteristics and structure of those types, so that it could lead to more individual nursing approach.

Research on the Interior Environment Planning of a Postpartum Care Center. (산후 조리원 실내 환경 계획에 관한 연구)

  • The postpartum care center is a facility which made its debut only about 10 years ago but has now changed the nation's postpartum care culture. Women delivered of a child use the postpartum care center at this time since the low rate of childbirth has become a social phenomenon. However, while the postpartum care center is not supported legally and systematically, anybody can run it after they only file a report on businessman status with the tax office. So there are disordered wave of postpartum care startups which do not have basic facilities. Thus, this study is intended to suggest an interior environment plan considering an efficient space and users' benefits of a postpartum care center which has a characteristic difference from that of ordinary facilities in terms of targeted persons or operations but for which standards are not clear in terms of the Health and Welfare Law and the Medical Service Law. This study was conducted by investigating the actual condition of the entrance space, living space, common-use space, nursing space, administration and attached space in the six postpartum care centers selected from Seoul and Kyeonggi-do region and suggesting the direction of improvements, thus establishing the direction of interior designs and an interior environment plan. Hopefully, this study would be used as basic data on interior building plans, thus being helpful in enhancing the satisfaction of the postpartum care center user and operating the facility.
