목적: 관절경하 후격막 통과 도달법을 이용한 후방십자인대 결절종의 제거술시 전방 도달법 보다 우수한 접근성 및 제거의 용이성과 그 치료 결과에 대하여 분석하였다. 대상 및 방법: 후방십자인대 결절종의 관절경하 후격막 통과 도달법을 이용하여 제거술을 시행한 7예에 대하여 조사하였다. 증상을 동반한 후방십자인대 결절종에서 동반손상 유무, 전방 접근법과 후격막 통과 도달법의 수술시야의 차이 등에 대하여 비교 분석하고, 결과의 평가는 Lysholm 슬관절 지수를 이용하였다. 결과: 증상이 동반된 7예의 후방십자인대 결절종 중 4예에서 동반 손상이 있었으며, 또한 외상과 연관된 경우도 3예이었다 임상평가 결과, Lvsholm 슬관절 지수는 술 전 평균 93.3점에서 술 후 평균 97.8점으로 향상을 보였다. 결론: 후방십자인대 결절종의 관절경적 제거술시 수술시야의 확보 및 후방구조물의 이상 유무를 확인하기 위해서는 후격막 통과 도달법이 효과적인 술식이라 생각된다.
Objectives : Burst fracture of the lower lumbar spine(L3-L5) is rare and has some different features compare to that of thoracolumbar junction. Lower lumbar spine is flexible segments located deeply, and has physiologic lordosis. All of these contribute to making surgical approach difficult. Generally, lower lumbar burst fracture is managed either anteriorly or posteriorly with various fixation and fusion methods. But there is no general guideline or consensus regarding the proper approach for such lesion. We have tried to find out the influencing factors for selecting the surgical approach through the analysis of lower lumbar burst fractures treated for last 4 years(1994.3-1998.3). Method : This study includes 15 patients(male : 10, female : 5, age range 20-59 years with mean age of 36.7 years, L3 : 8 cases, L4 : 5 cases, L5 : 2 cases). Patients were classified into anterior(AO) and posterior operated(PO) groups. We investigated clinical findings, injured column, operation methods, and changes in follow-up radiologic study (kyphotic angle) to determine the considerable factors in selecting the surgical approaches. Results : There were 5 AO and 10 PO patients. Anterior operation were performed with AIF with Kaneda or Z-plate and posterior operation were done with pedicle screw fixation with PLIF with cages or posterolateral fusion. Canal compression was 46.6% in AO and 38.8% in PO. The degree of kyphotic angle correction were 10.7 degree(AO) and 8.5 degree(PO), respectively. There was no statistical difference between anterior and posterior operation group. All patients showed good surgical outcome without complications. Conclusion : Anterior operation provided good in kyphotic angle correction and firm anterior strut graft, but it difficulty arose in accessing the lesions below L4 vertebra. While posterior approach showed less correction of kyphotic angle, it required less time and provided better results for accompanied adjacent lesion and pathology such as epidural hematoma. The level of injury, canal compression, biomechanics, multiplicity, and pathology are considered to be important factors in selection of the surgical approach.
Purpose: This paper aims at improving inefficiency of an existing posterior preference articulation method proposed for dual response surface optimization. The method generates a set of non-dominated solutions and then allows a decision maker (DM) to select the best solution among them through an interval selection strategy. Methods: This paper proposes an iterative posterior preference articulation method, which repeatedly generates the predetermined number of non-dominated solutions in an interval which becomes gradually narrower over rounds. Results: The existing method generates a good number of non-dominated solutions not used in the DM's selection process, while the proposed method generates the minimal number of non-dominated solutions necessitated in the selection process. Conclusion: The proposed method enables a satisfactory compromise solution to be achieved with minimal cognitive burden of the DM as well as with light computation load in generating non-dominated solutions.
Objective: Aneurysms arising from the posterior inferior cerebellar artery(PICA) are uncommon. We review literature on that and surgical results on aneurysmal treatment by choice of surgical approach. Methods: On the basis of radiologic findings & charts, we review retrospectively the surgical results of 12 cases from Mar 1999 to Dec 2003. Results: The mean age of the 12 patients was 55.8(ranged from 36 to 71) and female was predominant (female:male = 8:4). Locations of PICA aneurysms revealed variously(vertebral artery - PICA junction: 8, lateral medullary segment: 2, PICA - anterior inferior cerebellar artery common trunk: 1, telovelomedullary : 1). Surgical approaches & treatments were attempted in 11 cases and embolization was done in 1 case(Far lateral transcondylar or supracondylar approach & clipping: 9, Far lateral transcondylar or supracondylar approach and trapping: 2, suboccipital approach & clipping: 1). The surgical result were 8 of 12 patients were good outcome, 1 of 12 was severely disabled and 3 of 12 were died. Conclusion: First, we choose surgical approach by the laterality of aneurysms and surgical or interventional treatment is attempted as soon as possible. The PICA aneurysm is regarded as having a relatively good surgical outcome without drilling of the posterior arch of the atlas.
Objective : Traditionally, standard access to the posterior fossa was a suboccipital craniectomy. After introducing the technique and benefit of posterior fossa craniotomy, several surgeons have used this procedure to manage various pathologies in posterior fossa in children. Though this approach offers several advantages over conventional suboccipital craniectomy, the elevation of bone flap is not easy in adult because of the irregular contour of inner bone surface and tight adhesion of dura to the skull. The aim of this article is to describe the technical aspects of this approach and to delineate the important landmarks for a safe and quick performance in adult. Methods : We report on our series of twenty-five adult patients who underwent craniotomy for posterior fossa pathologies between 2002 and 2005. The maximal follow-up period was 4 years. Results : None of the patients complained of persistent headache and no CSF leaks have occurred postoperatively. We have encountered only two cases of transient subcutaneous CSF retention after craniotomy. Three-dimensional reconstruction of the CT scan confirmed the well-fitted suboccipital bone contour after craniotomy. And patients were satisfied with the cosmetic results. Conclusion : This method is safer and simpler. It restores normal anatomical planes and improves protection of the contents of the posterior fossa. The authors recommend craniotomy as an alternative method of access to the posterior fossa in all age groups.

Background: Recently, ultrasound has been commonly used. Ultrasound-guided interscalene brachial plexus block (IBPB) by posterior approach is more commonly used because anterior approach has been reported to have the risk of phrenic nerve injury. However, posterior approach also has the risk of causing nerve injury because there are risks of encountering dorsal scapular nerve (DSN) and long thoracic nerve (LTN). Therefore, the aim of this study was to evaluate the risk of encountering DSN and LTN during ultrasound-guided IBPB by posterior approach. Methods: A total of 70 patients who were scheduled for shoulder surgery were enrolled in this study. After deciding insertion site with ultrasound, awake ultrasound-guided IBPB with nerve stimulator by posterior approach was performed. Incidence of muscle twitches (rhomboids, levator scapulae, and serratus anterior muscles) and current intensity immediately before muscle twitches disappeared were recorded. Results: Of the total 70 cases, DSN was encountered in 44 cases (62.8%) and LTN was encountered in 15 cases (21.4%). Both nerves were encountered in 10 cases (14.3%). Neither was encountered in 21 cases (30.4%). The average current measured immediately before the disappearance of muscle twitches was 0.44 mA and 0.50 mA at DSN and LTN, respectively. Conclusions: Physicians should be cautious on the risk of injury related to the anatomical structures of nerves, including DSN and LTN, during ultrasound-guided IBPB by posterior approach. Nerve stimulator could be another option for a safer intervention. Moreover, if there is a motor response, it is recommended to select another way to secure better safety.
목적: 관절와상완 관절내 주사의 도달법에 있어서, 현재 임상에서 선호되고 있는 맹검 도달법에 의한 전방 도달법과 초음파 장비를 이용한 후방 도달법의 주사 정확도를 비교하고자 하였다. 대상 및 방법: 본원에서 견관절의 관절와상완 관절내 주사를 시행하였으며, 시술 전-후의 임상 경과에 관한 의무기록 및 초음파 영상의 확인이 가능한 연속적인 95예를 대상으로 하였다. 맹검법을 사용하여 전방 도달을 시행한 52예(I군), 초음파 장비를 이용하여 후방 도달을 시행한 43예(II군)였다. 후방 관절내에서 주사액의 존재가 초음파로 확인된 경우에 관절내 주사의 성공으로 판정하였다. 주사 주입전과 주사 후 2주의 견관절의 운동 범위를 측정하였다. 시술 후 2주 추시시 환자의 주관적인 증상 호전 정도를 기록하였다. 결과: 맹검 전방도달법(I군)에서의 관절내 주사의 정확도는 80.8%였으며, 초음파 유도하 후방 도달법(II군)에서의 성공률은 90.7%였다. 모든 환자에게서 관절와 상완 관절내 주사의 도달법에 상관 없이, 견관절의 운동 범위의 증가를 보였다. 시술 후 2주 추시 관찰 시, 양호 이상의 만족을 보인 환자는 73.7%(70/95)였다. 결론: 맹검법에 의한 전방 도달법을 이용한 관절와상완 관절내 주사의 성공률은 80.8%였다. 초음파 장비를 이용한 후방 도달법을 이용한 관절와상완 관절내 주사의 성공률은 90.7%였다. 동일한 시술자에 의한 두 가지 방법의 비교 연구에서, 초음파 유도하 후방 도달법이 보다 우월한 정확도를 보였다.
The aneurysm arising from the posterior cerebral artery is relatively uncommon, and has been reported in 0.7- 2.2% of whole cerebral aneurysm. Moreover, only a few cases of the giant aneurysm over 2.5cm in diameter arising from the distal portion of the posterior cerebral artery have been reported. Neurologic complications may occur after surgical treatment of this aneurysm due to difficult surgical approach and neurovascular complexity around the brain stem. Authors experienced a case of the thrombosed giant aneurysm with approximately 2.7cm in diameter arising from the P2 segment of the left posterior cerebral artery. Complete removal of the thrombus in the aneurysmal sac and direct neck clipping was performed via left subtemporal approach.
We report a case of T4 bursting fracture with paraparesis that recovered by posterior approach. A 47-year-old man presented with paraparesis (grade III) which had progressed rapidly after motor cycle accident. After sacrificing the T4 nerve root (right), posterior interbody fusion using cage following T4 corpectomy and T3-4, T4-5 discectomy was performed. After operation, lower extremities motor power improved and he could walk after one month. And this is the first report of posterior approach using cage by corpectomy and two level discectomy in case of upper thoracic burst fracture in Korea.
We report a case of anterior spinal epidural hematoma, after removal of cervical meningioma by posterior approach, which occurred in a 61-year-old man who presented with left hemiparesis and hypalgesia. A cervical mass surgically confirmed as meningioma was removed by posterior approach. 3 hours after operation, the patient revealed quadriparesis with respiratory difficulty. We herewith report a rare case of anterior spinal epidural hematoma which caused a catastrophic aggrevation of postoperative course.
