This study was written to discover the changes that may exist in the contents of medical services after introduction of health insurance system, and to identify the net-effect of health insurance system on medical services. Uncomplicated nornmal delivery and appendectomy patients were divided into 4 groups, the non-insured in pre-NHI periods(group A), the insured of health insurance for employees in pre-NHI periods(group B), the insured of regional health insurance for city residents in post-NHI periods(group C) and the insured of health insurance for employees in post-NHI periods(group D). The mehtod of matching was applied to control for major demographic differences among these 4 groups of each disease. In pre-NHI period, the medical services and the variation of medical services of the non-insured were compared with those of the insured. The difference between the change of medical services from group A to those of group C, and the change of medical services from group B to those group D is defined as the net-effect of health insurance. The results are as follows. First, in length of stay after delivery or operation, total length of stay, some laboratory examination, amount of several drugs used in appendectomy patients, frequency of sitz bath in delivery patients, there was net-effect of health insurance in increasing direction. Second, length of stay after delivery or operation, total length of stay, some laboratory examination, amount of several drugs used in appendectomy patients and frequency of sitz bath in delivery patients were significantly more in the insured than in the non-insured group in pre-NHI period. Third, the variation of medical services of post-NHI period was not less then those of pre-NHI period. Fourth, antenatal care on which the third party does not pay and the patient pays for all, was diffrerent by socioeconomic and educational level of patients.

The purpose of the study was to examine the effects of the lesson using Multiple Intelligence on self-esteem in the unit of Resources management and environment in Technology${\cdot}$Home Economics of middle school. In this study, the lesson using Multiple Intelligence theory for six hours was developed and applied to a class of 37 students of middle school as an experimental group. Lecture-style lesson was applied to the other class of 37 students as a control group. Significant differences between pre-test and post-test scores of self-esteem were found in the experimental group. Post-test scores of self-esteem were higher than pre-test scores of self-esteem in the experimental group. There were no significant differences between pre-test and post-test scores of self-esteem in the control group. It was found that the lesson using Multiple Intelligence theory was effective in improving self-esteem of middle school students.

Relapse is one of the most serious problem in alcoholism treatment area. But there is no proper post-treatment management program for preventing relapse in Korea. The purposes of this study are the analysis of several alcoholism treatment programs and recommendation about consideration for developing our alcoholism treatment program to prevent relapse in our country. Five different alcoholism treatment programs were collected for this study, two programs were collected by participant observation and three were collected by instruction manuals, references, and briefing materials. The followings are the criteria for this analysis of the five treatment programs. 1. the specific characteristics of the treatment program 2. the criterion of the patients, the duration and period (or the treatment, the special activities, and goals of the treatment. 3. the common characteristics of the five treatments. The common characteristics of those treatment programs are; 1 preparation for post-treatment from the beginning of the treatment (hospitalization) 2. assessment of relapse related factors 3. assessment of the relapse-warning signs 4. development and plan of individualized intervention strategy 5. practice of the plan The common characteristics of those treatment programs are 1. groups are classified the recovery training groups and the self-help groups 2 all kind of treatment programs carried out as groups rather than individuals. 3. usefulness which apply to various settings and various types 4. multidisciplinary and interdisciplinary approach As results, those programs were developed as post-detoxication management program, follow up program, and continued care program and their theoretical framework was from social learning theory. Those programs used the principles of cognitive-behavioral therapy for the classes, groups, and social skill training. These finding can be used for developing culturally appropriate alcoholism treatment programs focused on relapse prevention.
As the innovation of Information technology in the Public sector is in progress, the "Efficient adaptation and management of information system Act" (passed the National Assembly in December 30, 2005) was established to support the information system planning and development which are systematic and integrated in the enterprise view point. The public sector should adopt Information Technology Architecture into it's organization by the act. This paper reports on the issues, solutions and CSFs (Critical Success Factors) considering the adoption of ITA through the implementation for the case of the Korea Post. To adopt ITA program to the Public sector successfully, Korea Government should set the clear goals for IT A, set the standards for IT A project pricing, and collaborate with the system integration industry to make successful IT A program cases. The case study of Korea Post of MIC will be useful in developing and managing ITA programs for all agencies and organizations considering adoption of ITA.
Intercostal nerve blockade with local anesthetics has been used extensively in the past to provide pain relief following thoracotomy. Its popularity fell, for a period, probably due to increasing use of epidural analgesia. More recently, interest has focused on intercostal nerve block with the introduction of variously sited catheters. Two epidural catheters were placed under direct vision, in the intercostal spaces just above and below the wound by feeding the catheters posteriorly from the wound edges, superficial to the parietal pleura. Bupivacaine 0.25%. Was infused continuously at a rate of 5 ml/hour through each of the two intercostal catheters. Each catheter was primed with 10 ml/hour through each of the two intercostal catheters. Each catheter was primed with 10 ml of 0.25% bupivacaine. Postoperative vital signs resembled preoperation data. Arterial carbon dioxide pressure ($PaCO_2$) was unchanged and arterial oxygen pressure ($PaO_2$) was increased during two days after surgery because oxygen was administered at 21/min. Forced vital capacities (FVC) and forced expiratory volume in 1 second ($FEV_1$) were decreased the day of operation but restored to preoperative value from second operation day. VAS were increased on operation day but decreased from second operation day. Motion range of arms were not impaired. We concluded that continuous intercostal nerve block through catheters placed during thoracotomy in the adjacent intercostal spaces is a simple and effective method for management of the post-thoracotomy pain.
Objectives: One way to prevent deaths due to rabies is the timely utilization of post-exposure prophylaxis (PEP). Therefore, in addition to an understanding of the epidemiological distribution of animal bites, it is necessary to explore the factors leading to delays in PEP initiation. Methods: This cross-sectional study was conducted in Iran in 2011, and included 7097 cases of animal bites recorded at the Rabies Treatment Center of the Shiraz University of Medical Sciences using the census method. Logistic regression was used to identify factors associated with delays in PEP. Results: Among the patients studied, 5387 (75.9%) were males. The prevalence of animal bites in Fars province was 154.4 per 100 000 people. Dogs were the most frequent source of exposure (67.1%), and the most common bitten part of the body was the hands (45.5%). A delay in the initiation of PEP was found among 6.8% of the studied subjects. This delay was more likely in housewives (odds ratio [OR], 4.66; 95% confidence interval [CI], 2.12 to 10.23) and less likely in people with deep wounds (OR, 0.65; 95% CI, 0.43 to 0.97). Conclusions: Although all animal bite victims received complete PEP, in some cases, there were delays. Further, the type of animal involved, the depth of the bite, and the patient's occupation were the major factors associated with a delay in the initiation of PEP for rabies prevention.

The Maritime Traffic Safety Audit Scheme was institutionalized in 2009 to provide for professional surveys, measurement and the evaluation of navigational hazards that might occur in coastal waters in the course of marine work. Related researches that dealt with the reform of the Maritime Traffic Safety Audit Scheme found that there were no rules or regulations for verifying whether marine work conformed to audit results and reviewing the exactitude of ship handling simulations which were a crucial part of the scheme. According to the necessity of adopting an ex post facto management system in the field of maritime traffic, this study analyzed the similar audit acts such as the Natural Environment Impact Audit Scheme and the Road Traffic Safety Audit Scheme, and proposed specific articles for revising Maritime Safety Act in order to break out from the institutional inertia of the current Maritime Traffic Safety Audit Scheme. It is expected that a newly proposed legal system for verifying the performance of audit results and the exactitude of audits will help improve maritime traffic safety by eradicating potentially hidden hazards related to marine work.
Purpose: To analyze health programs of the PHCP (Primary Health Care Posts) Method: From August 2006 to July 2007, data on the general quality and health program of the PHCP was requested by official letter and replies were received via E-mail. From December 8 to December 30, 2007, data from 1,268 (66.8%) PHCP out of 1,897 PHCP were analyzed using SPSS 12.0 Win program. Results: The average population covered by each PHCP is 878.3 people. For the health and special programs, Community Health Practitioners report high motivation for programs on health promotion, management of chronic illness, social welfare (40-50%). Demand by the residents was reported at 10% and increases in the health of the residents were attributed to high interest and demand. Volunteer work was 83.3% for bathing, 54.5% for equipment support and 46% for exercise programs. As elders make up 30% of the population in rural areas, there is an increasing demand for volunteer work in bathing programs. Conclusions: As the number of elders in the population increases and there is an increased need for more medical treatment for older people who are sick, the role of PHCP must be strengthened to include visits to homes of community residents. Where financial support for the PHCP is difficult, it is necessary to develop sound data on demographic characteristics of the population in order to develop efficient and effective health promotion programs. The finding that 54.7% of the population need management of chronic illness has difficulty in seeing a physician indicates a need to enhance the health care delivery system by strengthening the role of the Community Health Practitioners and including them in the civil service system to ensure stability of the PHCP.
This study explored the factors affecting the acceptance of public recording in SNS post. Using SPSS 21.0 program and AMOS 21.0 program, major results were derived through exploratory factor analysis, confirmatory factor analysis, correlation analysis, and path analysis. The results are as follows: First, the risk of personal information leakage on SNS posting has a significant negative impact on the attitude toward SNS posting. Second, the security of the SNS posting has a significant effect on the attitude toward the SNS posting. Third, the concern about privacy of SNS posting has a significant negative impact on the attitude toward SNS posting. Fourth, the attitude toward SNS posting has a significant effect on the intention to accept the SNS posting. The above results suggest that the SNS post recording should be able to collect opinions of SNS users from a long-term viewpoint.
Background: Postoperative pain management is crucial for patients undergoing total knee arthroplasty (TKA). There have been many recent clinical trials on post-TKA peripheral nerve block; however, they have reported inconsistent findings. In this meta-analysis, we aimed to comprehensively analyze studies on post-TKA analgesia to provide evidence-based clinical suggestions. Methods: We performed a computer-based query of PubMed, Embase, the Cochrane Library, and the Web of Science to retrieve related articles using neurothe following search terms: nerve block, nerve blockade, chemodenervation, chemical neurolysis, peridural block, epidural anesthesia, extradural anesthesia, total knee arthroplasty, total knee replacement, partial knee replacement, and others. After quality evaluation and data extraction, we analyzed the complications, visual analogue scale (VAS) score, patient satisfaction, perioperative opioid dosage, and rehabilitation indices. Evidence was rated using the Grading of Recommendations Assessment, Development, and Evaluation approach. Results: We included 16 randomized controlled trials involving 981 patients (511 receiving peripheral nerve block and 470 receiving epidural block) in the final analysis. Compared with an epidural block, a peripheral nerve block significantly reduced complications. There were no significant between-group differences in the postoperative VAS score, patient satisfaction, perioperative opioid dosage, and rehabilitation indices. Conclusions: Our findings demonstrate that the peripheral nerve block is superior to the epidural block in reducing complications without compromising the analgesic effect and patient satisfaction. Therefore, a peripheral nerve block is a safe and effective postoperative analgesic method with encouraging clinical prospects.
