A 3-year-old castrated male Maltese dog, weighing 4.8 kg was referred with hindlimb ataxia and right forelimb proprioceptive deficits were shown for 20 months. Chiari-like malformation and syringomyelia diagnosed through MRI at a local animal hospital and Knuckling of right forelimb and reluctance to walk were managed with steroid. The medical management was getting ineffective to manage for the symptoms one month before referred. Physical and neurological examinations, radiography, computed tomography, and magnetic resonance imaging were performed and diagnoses of caudal occipital malformation syndrome (COMS) and subsequent syringomyelia (SM) were made. Given that pharmacological treatment was previously ineffective, surgical intervention was recommended. Foramen magnum decompression with duraplasty using $Lyoplant^{(R)}$ was performed. Three days post-surgery, the dog showed improved gait and activity. After 2 months, the dog received no additional prescription medications. At the 12-month follow-up after surgery, the dog showed no clinical problems or recurrences, despite complete cessation of pharmacological treatment. In present report, we applied $Lyoplant^{(R)}$ as a dural graft has been carried out in a dog with COMS. Surgical decompression with $Lyoplant^{(R)}$ was an effective long-term (12-month) treatment for COMS without the need for any pharmacological treatment.

Background: The submucous cleft palate (SMCP) is a type of cleft palate that may result in velopharyngeal insufficiency (VPI). Palate muscles completely separate oral and nasal cavities by closing off the velopharynx during functional processes such as speech or swallow. Also, hypernasality may arise from anatomical or neurological abnormalities in these functions. Treatments of this issue involve a combination of surgical intervention, speech aid, and speech therapy. This case report demonstrates successfully treated VPI resulted from SMCP without any surgical intervention but solely with speech aid appliance and speech therapy. Case presentation: A 13-year-old female patient with a speech disorder from velopharyngeal insufficiency that was caused by a submucous cleft palate visited to our OMFS clinic. In the intraoral examination, the patient had a short soft palate and bifid uvula. And the muscles in the palate did not contract properly during oral speech. She had no surgical history such as primary palatoplasty or pharyngoplasty except for tonsillectomy. And there were no other medical histories. Objective speech assessment using nasometer was performed. We diagnosed that the patient had a SMCP. The patient has shown a decrease in speech intelligibility, which resulted from hypernasality. We decided to treat the patient with speech aid (palatal lift) along with speech therapy. During the 7-month treatment, hypernasality measured by a nasometer decreased and speech intelligibility became normal. Conclusions: Surgery remains the first treatment option for patients with velopharyngeal insufficiencies from submucous cleft palates. However, there were few reports about objective speech evaluation pre- or post-operation. Moreover, there has been no report of non-surgical treatment in the recent studies. From this perspective, this report of objective improvement of speech intelligibility of VPI patient with SMCP by non-surgical treatment has a significant meaning. Speech aid can be considered as one of treatment options for management of SMCP.
This research was performed to investigate the effects of participation in aqua exercise on blood composition and vascular compliance of mentally retarded persons. The targets of this research consisted of a control group of 12 people and an aqua exercise group of 12 people after selecting 24 mentally retarded males and aqua exercise group carried out an exercise program for 60 minutes per day and five days a week for 9 weeks. Weight, body fat percentage, blood composition and vascular compliance were measured in a preliminary inspection and the same items were measured in a post-inspection. The results of this measurement were as follows. Firstly, aqua exercise group showed a significant decrease in weight and body fat percentage (p<.05). Secondly, for blood composition, aqua exercise group showed a notable decrease in Total-Cholesterol, Triglyceride, and LDL-Cholesterol (p<.05) and a significant increase in HDL-Cholesterol (p<.05). Thirdly, aqua exercise group showed a notable increase in blood composition for both upper limbs and lower limbs (p<.05). Taken these results together, it was considered that a participation of mentally retarded persons in aqua exercise would prevent adult diseases and would have an affirmative influence on improvement of the quality of life.
For this research, 27 out of 52 patients with chronic low back pain involving sacroiliac joint pain were classified into the experimental group to conduct pelvic exercise program and traditional physical therapies in parallel, and the remaining 25 ones were classified into the control group to only apply traditional physical therapies for 6 weeks. After that, their clinical samples were randomly extracted. Before the experiment, both of the experimental group and the control group had the Oswestry disability index test to see how big their pain was and their hip joint angles were analyzed during walking. After the post-test, finally, the results of the Oswestry disability index test and hip joint angles while walking before and after the experiment were compared between two groups to know the effect of pelvic stability exercise program. Within-group results and between-group results both displayed significantly reduced low back pain, and when comparing hip joint angles of the experimental group, there were differences between mid stance phase, terminal stance phase, pre swing phase and early swing phase of the right hip joint, and mid stance phase, terminal stance phase, pre swing phase and early swing phase of the left hip joint. In the control group, there were significant differences between mid stance phase, terminal stance phase and early swing phase of the right hip joint, and loading response phase, mid stance phase, terminal stance phase and pre swing phase of the left hip joint. as a result, pelvic stability exercise program is helpful to gait rhythm on stance phase and swing phase although effective to decrease Oswestry disability index including pain.
The aim of the study is to examine the effects of the peer relationship promotion convergence program on self-esteem and sociality of children using a community child center. A nonequivalent control group pre-post design was used. Participants were 52 children (experimental group=26, control group=26), 4-6th grade in elementary school from 5 rural community child center. The experimental group participated in 12 sessions of the program for 50 min, once a week. The data were analyzed with descriptive statistics, a chi-square test, and an independent t-test using SPSS WIN 21.0 program. After completion of the program, the experimental group showed a significant improvement in self-esteem(t=2.43, p=.018) and sociality (t=2.03, p=.047) compared to the control group. The result shows that the program is beneficial for improving self-esteem and sociality of low-income, school-age children. Therefore, the program is recommended the intervention for improvement of self-concept and sociality of children.
This sturdy proposed and tested whether the six core treatment processes of Acceptance and Commitment Therapy (ACT) reduce self-criticism and dichotomous thinking in Evaluative concerns perfectionists and even diminish depression and ultimately verify it. The sample consisted of 22 subjects who displayed high EC perfectionism and depression. The subjects were randomly assigned to ACT groups or control groups (n-11/each). The ACT program was administered in eight sessions. All participants completed scale about EC perfectionism, self-criticism, dichotomous thinking, depression and acceptance & action at the pre-test, post-test, and the six week follow-up. The results showed that evaluative concerns perfectionism, self-criticism, dichotomous thinking, and depression decreased more in the treatment group than in the control group, whereas acceptance & action increased in the former. Finally, the implications and limitations of this study, along with suggestions for future study were discussed.

In the movies of the Coen brothers, it seems like an entertainment movie as typical black comedy genre through irony and happening, but in fact, it inserts a lot of social problems in the film to show that they cynically express their social agenda from a contemplative view. In their movie "The Big Lebowski (1998)", it seems like they are creating comical content through the main characters' unaffected attitude. However, it is director's excellent director of the sub-text that expresses American social issues such as Vietnam war, post-modernism and an obscurantist policy and au fond the comedy about the historical facts of mass production of social maladjustment into black comedy. We expect to contribute to make a step forward in the Korea film industry by analyzing such movies that has the cultural power of influence.
This study conducted non-equality control post-design similar experimental study in order to investigate the effect of self-directed practice through self-reflection by using videos on the basic nursing performance ability, confidence in performance, learning self-efficacy, and practice satisfaction. The experimental group was 57 students and the control group was 56 students in the Department of Nursing in G region. The data was collected from April 1, 2017 to June 14, 2017. For analyzing data, the SPSS 22.0 program was used and the homogeneity test was analyzed with ${\chi}^2$ test, t-test, and Fisher's exact test. Also, independent-test was used. According to the result, all of the experimental groups including the basic nursing performance ability, confidence in performance, learning self-efficacy(t=15.304, p<.001), and practice satisfaction(t=15.304, p<.001) were improved and showed significant differences compared to the control group. As a result, it is predicted that the practice method has a positive effect on participating in self-study and it contributes to active participation by increasing learning motivation of learners. Thus, it is suggested to be applied to various practical training to improve basic nursing skills.

Pulmonary mucormycosis is a very rare but often fatal opportunistic fungal infection caused by the order Mucorales in class Zygomycetes. Reported overall mortality exceeds 70% and the diagnosis is often made post-mortem. We experienced 2 cases of typical form of pulmonary mucormycosis. One patient was a poorly controlled diabetic and the other suffered from acute Iymphocytic leukemia (ALL). The former was diagnosed by a bronchoscopic biopsy and the latter by a pathologic examination from the percutaneous drain of a subphrenic abscess. Both of them underwent a surgical excision of the involved lung tissue. The patient with diabetes mellitus was successfully treated by surgical resection and discharged without complications. The other with ALL underwent a second operation and was transferred to the department of internal medicine for further management of his relapse of lettkemia. Recent literat re suggests that early aggressive diagnostic effort and treatment including surgical resection in the case of localized forms of the disease results in a good prognosis.

To improve the effectiveness and efficiency of public health personnel training, we evaluated not only how appropriate the students felt the objectives, contents, methods and multimedia used in the train ing courses, but also how much the students accomplished the objectives and applied skill and knowledge to their own works. We selected 5 courses for the study : Tuberculosis control, Radiological technique, Public health information, Immunization, Mental health management courses used by Kirkpatrick's evaluation model. Reaction evaluation was carried out in final day by questionnaire. The results showed that all of them were very satisfied with educational input and curricula, learn Ing environment. Secondly, we measured the degree of learning achievement on pre and post training by questionaire of specific behavioral objectives. The degree of learning achievement was statistically higher just after training than pre training (paired t-test, p<0.01). Thirdly, evaluation of behavioral change to job was conducted to find out how much students applied skill and knowledge to their own job in 3 months after training by questionnaire. The results of behavioral change evaluation showed that 43.5% of the students who were performing job related with the training courses in 3 months after training applied the learned skill and knowledge to their own job quite well and 37.8% of them applied relatively well, therefore total 81.4% of them applied to their own job. And effectiveness of training for the above mentioned students showed that 41.9 % of them had improved or enforced their jobs after training, 35.5% of them had had no remarkable changes, and 15.7% had newly applied the learned skill and knowledge to their jobs. For evaluating the degree of usefulness of material predistribution in two weeks before training, we compared experimental groups with control groups. The results showed that general reactions are helpful but the degree of learning achievement is no discrepancy.
