Embarrassment; a concept analysis (당혹감(embarrassment); 개념분석)

  • Purpose: The concept of embarrassment as an unpleasant emotion is important in caring for those who are in the continuum of illness and health requiring body care and facing sexual problems. In spite of its nursing implication, embarrassment has not been focused in nursing research. Therefore, the purpose of this paper is to develope a conceptual analysis of embarrassment. This analysis would help to understand comprehensively an embarrassing situation in which an intimate relationship between nurse and patient regarding body care of the patient could develope. Futhermore, it would be a basis for developing strategies for prevention of embarrassment which could lead for both patient and nurse to emotionally vulnerable situation because of their failure of self-presentation (impression management). Method: The concept analysis model suggested by Walker and Avant (1988) is used to clarify what is meant by the term 'embarrassment'. Result: This analysis defines the attributes of embarrassment such as follows. First, embarrassment is often attributed to situational factors interrupting a smooth interaction such as intimacy, confusion, vulnerability, exposure of interaction and characteristics of audience. Second, embarrassment is closely related with cognitive factor such as fear of losing face resulted from a behaviour being out of line. Third, embarrassment is closely associated with dispositional factor such as embarrassability. Fourth, embarrassment is an unpleasant and unwanted emotion arising reactively after an interaction had occurred. Fifth, embarrassment encloses physical, physiological and behavioral aspects such as a variety of unique and easily noticeable reactions and a pattern of verbal and non-verbal behaviour for coping in an embarrassing situation. The antecedents were normally socialized adolescents with normal cognitive ability, concern of losing face, embarrassability, embarrassing events related with situational intimacy, confusion, vulnerability, exposure of interaction and characteristics of audience, physical, behavioral blunders resulting in a failure of impression management. The consequences were an emotional coping behaviour, audience's embarrassment responses, and verbal and non-verbal coping strategies of interactional participants. Conclusion: It is hoped that this analysis will stimulate further exploration of this concept and study for developing systematic assessment and nursing practice that diminishes embarrassment of interactional participants.

  • Since arterial disease in the upper extremity is less common than that in the lower extremity, experimental and numerical investigations related to upper extremity have been rarely performed. We created a three-dimensional model of the arteries, larger than approximately 1 mm, in a Korean adult's left hand (from brachial to digital arteries), from 3T magnetic resonance imaging (MRI) data. For the first time, a three-dimensional computational fluid dynamic method was employed to investigate blood flow velocity, blood pressure variation, and wall shear stress (WSS) on this complicated artery system. Investigations were done on physiological blood flows near the branches of radial and deep palmar arch arteries, and ulnar and superficial palmar arch arteries. The flow is assumed to be laminar and the fluid is assumed to be Newtonian, with density and viscosity properties of plasma.

Responses of Systemic Arterial Blood Pressure and Heart Rates to Graded Increase of Positive Lung Inflation (폐내압증가시(肺內壓增加時)의 동맥혈압(動脈血壓) 및 심박변동(心搏變動)에 관(關)하여)

  • Effects of graded increase of positive lung inflation upon heart rates and arterial blood pressure were observed in the anesthetized dogs to analyze the mechanical and neural regulatory factor in response to the positive inflation of the lung. The results obtained were summarized as followings: 1) When the low grade of positive lung inflation was employed under the mild to moderate anesthesia, central venous pressure was linearly increased while heart rate was decreased. After bilateral vagotomy, central venous pressure was obviously increased while heart rate was constant. 2) When the high degree of positive lung inflation was employed, changes of central venous pressure and heart rate were not significant. 3) The low grade of intrapulmonary pressure increase caused reflex tachycardia in phase 2 and overshooting in phase 4 in response to the systemic arterial blood pressure change. 4) On the other hand, the high degree of intrapulmonary pressure increase caused paradoxical bradycardia in phase 2 and lack of overshooting in phase 4 in response to the systemic arterial blood pressure change. 5) It may be noted that the experimental model employed in the present study is a useful tool to evaluate and analyze the neural and mechanical regulatory factor in response to the graded increase of the positive lung inflation.

Simulation of lesion-to-liver contrast difference curves in Dynamic Hepatic CT with Pharmacokinetic Compartment Modeling (Pharmacokinetic Compartment Modeling을 이용한 나선식 CT 에서의 간암-간 대조 곡선의 Simulation)

  • Contrast-enhanced CT has an important role in the assessment of liver lesions. However, the optimal protocol to get most effective result is not clear. The main principle for deciding injection protocol is to optimize lesion detectability by rapid scanning when lesion-to-liver contrast is maximum. For this purpose, we developed a physiological model of contrast medium enhancement based on the compartment modeling and pharmacokinetics. Blood supply to liver was modeled in two paths. This dual supply character distinguishes the CT enhancement of liver from that of the other organs. The first path is by hepatic artery and the second is by portal vein. It is assumed that only hepatic artery can supply blood to hepatocellular carcinoma (HCC) compartment. It is known that this causes the difference of contrast enhancement between normal liver tissue and hepatic tumor. By solving differential equations for each compartment simultaneously using computer program Matlab, CT contrast-enhancement curves were simulated. Simulated enhancement curves for aortic, hepatic, portal vein, and HCC compartments were compared with mean enhancement curves from 24 patients exposed to the same protocols as simulation. These enhancement curves were in a good agreement. Furthermore, we simulated lesion-to-liver curves for various injection protocols, and analyzed the effects. These may help to optimize the scanning protocols for good diagnosis.

The Effects of Kamihaepyoijin-tang on the Respiratory Patterns and Tracheal Tissues in Allergic Asthma (가미해표이진탕이 알레르기 천식 백서의 호흡 양상과 기관조직에 미치는 영향)

  • The present study was made with the aim of finding out the Kamihaepyoijin-tang's effect on the allergic asthma using the Egg-White Implant(EWI) method model recommended by de Siqueira et al. Kamihaepyoijin-tang has widely been prescribed and used in Oriental Medicine for controlling and curing of cough, asthma, nasal obstruction. Previous studies have demonstrated that Kamihaepyoijin-tang had analgesic, anticonvulsive effects. But it is uncertain whether Kamihaepyoijin-tang could inhibit the allergic reaction of rats with experimentally induced allergic asthma using EWI method. With such aim. the respiratory patterns and eosinophil infiltration in the tracheal mucosa of the rats were mainly observed. The results were summerized as follows ; 1. The frequency of malformed respiratory patterns in Kamihaepyoijin-tang treated groups were decreased as compared with the control group. 2. The Ratio of malformed respiratory patterns in Kamihaepyoijin-tang treated groups were significantly decreased as compared with the control group(p<0.02). 3. The eosinophil infiltration in Kamihaepyoijin-tang treated groups were decreased as compared with the control group. 4. The eosinophil infiltration that inside of trachea tissue in Kamihaepyoijin-tang treated groups were significantly decreased as compared with the control group(p<0.05). Depending upon above results, it is considered that Kamihaepyoijin-tang has the inhibitory effects on the allergic asthma of rats and suggested that it could be used in relieving patients of the symptoms which are caused by allergic asthma.

  • Oriental pear was used as treatment of asthma, control of blood pressure tonic medicine vasoaction, diabetes in oriental medicine. In this study, it was investigated that pear phenolic compound effects on cardiovascular system as blood pressure and renin and atrial natriuretic peptide(ANP) in plasma, Cardiac hypertrophy. The experiments were performed on Sprague Dawley rats, 2K1C hypertension model was prepared by constricting the left renal artery with a sliver clip. Animals were then divided into four groups, 5mg/Kg(PPC-A) 10mg/Kg(PPC-B) 15mg/Kg(PPC-c) and control group. Pear phenolic compound solution were supplied with them for 3weeks, a day's interval. The results are that The blood pressure was significantly decreased at 15days in PPC-A group than control group. The plasma ANP was increased in PPC-A and PPC-C group, and cardiac hypertrophy was significantly decrease in PPC-C group compared with control group.

Differences in Behaviors of Utilization on Western and Oriental Medical Care according to Korea National Health and Nutrition Examination Survey (양.한방 의료 이용 선택에 영향을 미치는 요인 - 국민건강영양조사 2기, 3기 자료 분석)

  • The objective of this study is to examine the difference about manner of utilization and satisfaction on western and oriental medical care. The data came from the National Health and Nutrition Examination Survey in 2001 and 2005. The Andersen model of health behavior was employed to make this analysis. The major statistical methods used in this analysis are chi-square test and logistic regression. The major findings are as follows; Medical care is totally increased in 2005 in comparison with 2001, but medical care in the oriental side is decreased. There is no significant variables in predisposing factors such as sex, age, education level and spouse. Of health behavior factors, the average persons in health state are more favorable in oriental care rather than western care. Oriental care is favorably taken in health insurance subscribers and residents in a large city of enabling factors. The patients with musculoskeletal disease are more dependent in oriental care than western medical care. In conclusion, the findings show that it is largely related to patients' health state, medical insurance, living area and disease types to take oriental medical care. These characteristics should be considered in establishing policies of the oriental medical care in the future.

Effect of Ginseng-Berry Extract on the Improvement of Blood Microcirculation and Skin Brightness (인삼열매 추출물의 혈행개선과 피부톤 개선에 미치는 영향)

  • Several studies have demonstrated that ginseng-berry extract has several beneficial properties, including anti-inflammatory, antioxidant, and vasodilation properties. Ginseng-berry extract has also been shown to have the great potential against skin aging. Its beneficial mechanism against skin aging, however, has not been examined in detail. Also, the effects of ginseng-berry extract on microcirculation and skin cellular responses have not been investigated. Inhibition of skin microcirculation is the primary cause of many adverse biological effects, which is responsible for the skin aging and darkening. We investigated the beneficial effects of ginseng-berry extract on blood circulation, transcutaneous oxygen pressure in vivo model and also on skin microcirculation, cellular response and skin brightening effect in clinical trial. We found that oral administration of ginseng-berry extract markedly increased blood flow rate and transcutaneous $O_2$ pressure, but decreased transcutaneous $CO_2$ pressure. Also, it improved skin tone on cheeks, as is skin brighteness. These results suggest that ginseng-berry extract is a potent candidate for the treatment of skin aging and brightening by improving skin microcirculation.

Effects of Kamigingansikpung-tang on Hypertension Model by DOCA-salt (가미진간식풍탕(加味鎭肝熄風湯)이 DOCA-salt로 유발된 고혈압(高血壓) 병태(病態) 흰쥐 모델에 미치는 영향)

  • Kamigingansikpung-tang(KGST) has been used for many years as a therapeutic agent for acute stage of cerebrovascular disease and hypertension in oriental medicine. But the effect of KGST on hypertension and vascular system is not well-known. This study was done to investigate the effects of KGST on hypertension. The results were obtained as follow: KGST showed scavenging activity on DPPH free radical. KGST showed the inhibitory effect on ROS and ACE, and the accelerated SOD activity. KGST significantly decreased the blood pressure and pulse in DOCA-salt induced hypertensive rat. KGST significantly decreased the levels of aldosterone in DOCA-salt induced hypertensive rat. KGST significantly decreased the levels of dopamine, epinephrine in DOCA-salt induced hypertensive rat. KGST significantly decreased the levels of potassium(K+) and chloride(Cl-) in DOCA-salt induced hypertensive rat. KGST significantly decreased the levels of uric acid and creatine in DOCA-salt induced hypertensive rat. KGST has an effect on inhibiting cell damage of the heart, liver, kidney, and adrenal gland. results suggest that KGST might be effective in treatment and prevention of hypertension.

Anti-inflammatory Effect of Cinnamomi Ramulus on Collagen Induced Arthritis;a Model for Rheumatoid Arthritis in DBA/1J Mice and Cytokine Production in Raw264.7 Cells (계지(桂枝)의 collagen 유발 관절염에 대한 소염 효과;DBA/1J mouse에서의 병태 관찰 및 RAW264.7에서의 cytokine 분비측정)

  • In order to examine anti-inflammatory effect of Cinnamomi Ramulus (CR) alcohol extract on rheumatoid arthritis, the present study investigated the viability and TNF-${\alpha}$ production in Raw264.7 cells treated with CR and collagen induced arthritis in DBA/1J mice which were orally administered with CR prior to immunization. The results are as follows: CR extract at $20{\mu}g/ml$ inhibited the viability of Raw264.7 by 50%. CR at 15 and $20{\mu}g/ml$ showed a significant decrease in TNF-${\alpha}$ levels from Raw264.7 cells treated with LPS. CR administration decreased arthritic index in DBA/1J mice immunized with bovine collagen type II but it did not reach statistical significance. CR administration significantly decreased spleen weights obtained from mice in 6 weeks after immunization. CR administration significantly decreased serum anti-type II collagen antibody levels compared with control group. CR administration decreased serum IL-6 levels compared with control group but it did not reach statistical significance.
