Scopolamine patch is an effective anticholinergic and antiemetic agent for motion sickness in S. Korea. Since this medication is Over The Counter (OTC) medication and may cause serious side effects when misused. Therefore, we evaluate the safety and efficacy for scopolamine patch in this study. We selected and surveyed total 43 patients who purchased scopolamine patch from three different pharmacies located at S and C Express Bus terminals and P Port at Seoul, Cheongju, and Busan. In the result, 11 patients (8${\sim}$15 year old:25.6%) were correctly used scopolamine patch with a children dose and 2 patients were misused. 26 (15${\sim}$60 year old: 60.4%) and 4 (over 6o year old: 9.3%) patients were correctly used with a adult dose. 33 patients (69.9%)answered that they knew the directions well how to use scopolamine patch correctly. Only 24.2% (n=8) patients learned the directions by pharmacist's consultation. Most patients (45 frequencies with duplicate counts) had some experienced side effects and among those drowsiness is the most common one. In conclusion, scopolamine patch as a non-prescription drug (OTC) should be monitored by pharmacist with correct drug consultation.
A 77-year-old male patient was admitted to the hospital with acute appendicitis and underwent emergency appendectomy under inhalation anesthesia without any complications. After the operation, the patient was placed on epidural analgesia with the mixture of fentanyl-bupivacaine and scopolamine patch for nausea and vomiting. The patient experienced central anticholinergic syndrome (CAS) with hallucination, confusion, somnolence, dysarthria and delirium at 20 hours after patch placement and recovered from all symptoms of CAS in 24 hours after the removal of the patch. The CAS occurs when central cholinergic sites are occupied by specific drugs and also as a result of an insufficient release of acetylcholine. The diagnosis of CAS is often determined by a process of exclusion and not actually made until a positive therapeutic response to physostigmine, a centrally active anticholiesterase agent, has taken place. Treatment of CAS includes prompt removal of the patch, cleansing of the area, and physostigmine administration. The scopolamine patch should be used with extreme caution in elderly patient.
Forsythiaside is a polyphenolic constituent of the fruits of Forsythia suspense Vahl which are widely used as anti-inflammatory herbal raw materials in traditional Chinese medicine. In the present study, the authors assessed the effects of forsythiaside on learning and memory impairments induced by scopolamine using a passive avoidance and the Morris water maze tests in mice. Drug-induced amnesia was induced by scopolamine treatment (1 mg/kg, i.p.). Forsythiaside (10 mg/kg, p.o) administration significantly prevented scopolamine-induced step-through latency reduction in the passive avoidance test and scopolamine-induced increased escape latency in the Morris water maze test (p<0.05). Moreover, in an ex-vivo study, forsythiaside treatment (10 mg/kg, p.o) significantly reduced the increase of thiobarbituric acid reactive substance levels induced by scopolamine (p<0.05). Taken together, the present study suggests that forsythiaside could be useful for the treatment of cognitive impairment, and that its beneficial effects are mediated, in part, by its antioxidative properties.
We investigated the effect low salt (2 or 4% salt) concentrations jeotgal made from Todarodes pacificus on the learning and memory impairments in scopolamine-induced (2 mg/kg, i.p.) dementia rats. Rats treated with oral BF-7 (200 mg/kg, p.o.) as a positive control and Todarodes pacificus jeotgal had significantly reduced scopolamine-induced memory deficits in the passive avoidance test. The Morris water maze test or treatment with 2% salt jeotgal made from Todarodes pacificus significantly ameliorated the scopolamine-induced memory deficits in the formation of long- and short-term memory. The acetylcholine content and acetylcholinesterase acitivity paralleled the results of the behavior experiment. There were no significant differences in the brain acetylcholine contents of the experimental groups, while the brain acetylcholine content of the group treated with 2% salt Todarodes pacificus jeotgal was higher than that of the control group. The inhibitory effect of 2% salt jeotgal made from Todarodes pacificus on the acetylcholinesterase activity in the brain was lower than that of the control group. These trends were similar to those of the gamma-aminobutyric acid content. We suggest that Todarodes pacificus jeotgal enhances learning memory and cognitive function by regulating cholinergic enzymes.
Murraya koenigii leaves commonly known as 'curry patta' are routinely added to Indian gravy and vegetarian dishes by south Indian as a favourate condiment. The present study was undertaken to investigate the effects of Murraya koenigii leaves (MKL) on memory in rats. Elevated plus-maze and Hebb-Williams maze served as the exteroceptive behavioral models for testing memory. Diazepam-, scopolamine- and ageing-induced amnesia served as the interoceptive behavioral models. MKL fed orally to various groups of young and aged rats with diet containing 2, 4 and 8% w/w of MKL for 30 days consecutively were investigated. The MKL diets produced a significant dose-dependent improvement in memory scores of young and aged rats and significantly reduced the amnesia induced by scopolamine (0.4 mg/kg, i.p.) and diazepam (1 mg/kg, i.p.). Therefore, it would be worthwhile to specifically investigate the therapeutic potential of MKL in the management of dementia patients.
We examine whether Phellodendron amurense (PA) and its major alkaloid compound, berberine (BER), improved memory defects caused by administering scopolamine in rats. Effects of PA and BER on the acetylcholinergic system and pro-inflammatory cytokines in the hippocampus were also investigated. Male rats were administered daily doses for 14 days of PA (100 and 200 mg/kg, i.p.) and BER (20 mg/kg, i.p.) 30 min before scopolamine injection (2 mg/kg, i.p.). Daily administration of PA and BER improved memory impairment as measured by the passive avoidance test and reduced the escape latency for finding the platform in the Morris water maze test. Administration of PA and BER significantly alleviated memory-associated decreases in cholinergic immunoreactivity and restored brain-derived neurotrophic factor and cAMP-response element-binding protein mRNA expression in the hippocampus. PA and BER also decreased significantly the expression of proinflammatory cytokines such as interleukin-$1{\beta}$, tumor necrosis factor-${\alpha}$ and cyclooxygenase-2 mRNA in the hippocampus. These results demonstrated that PA and BER had significant neuroprotective effects against neuronal impairment and memory dysfunction caused by scopolamine in rats. These results suggest that PA and BER may be useful as therapeutic agents for improving cognitive functioning by stimulating cholinergic enzyme activity and alleviating inflammatory responses.

To compare and analyze the improvement effects of white ginseng extract, red ginseng extract, and black ginseng extract on cognitive dysfunction and memory impairment caused by scopolamine in rats. In the cognitive behavioral test, the tendency of the SCOP+B group to overcome the escape time delay induced by scopolamine administration was observed, unlike the SCOP group. The frequency on plat form was significantly increased in the group treated with ginseng extracts compared to the SCOP group. As a result of measuring the duration time on goal quadrant, the time spent in the quadrant was significantly increased in the SCOP+B group compared to the SCOP group. In the hippocampus, the SCOP-treated group significantly decreased the activity of AChE compared to the normal group, but the ginseng extract-treated groups significantly increased it compared to the SCOP group. After sacrificing the rats after the behavioral test, the expression of PSD95 protein in the excised brain was significantly decreased in the SCOP group compared to normal, but it was observed that the SCOP+R and SCOP+B groups were significantly increased compared to the SCOP group. CREB1 protein expression was significantly increased in the SCOP+R group, and the expression of Cdk5 was significantly increased in the SCOP+B group. Ginseng extracts significantly restored the memory damaged by scopolamine suggesting that red ginseng increased the expression of CREB1 and PSD95 proteins, and black ginseng increased the protein expression of Cdk5 and PSD95 to induce memory recovery.

The purpose of this study is to investigate the memory enhancing effect and underlying molecular mechanism of arabinoxylan (AX), a major component of dietary fiber in wheat against scopolamine (SCO)-induced amnesia in Sprague-Dawley (SD) rats. Diverse behavior tests including Y-maze, Morris water maze, and passive avoidance tests were performed to measure cognitive functions. SCO significantly decreased the spontaneous alterations in Y-maze test and step-through latency in passive avoidance test, whereas increased time spent to find the hidden platform in Morris water maze test compared with the sham control group. In contrast, oral administration of AX (25 mg/kg and 50 mg/kg) effectively reversed the SCO-induced cognitive impairments in SD rats. Furthermore, AX treatment up-regulated the expression of brain-derived neurotrophic factor (BDNF) in the cortex and hippocampus via promoting activation of cAMP response element binding protein (CREB). Therefore, our findings suggest that AX can improve SCO-induced learning and memory impairment possibly through activation of CREB and up-regulation of BDNF levels, thereby exhibiting a cognition-enhancing potential.
Dianthus superbus (D. superbus) is a traditional crude drug used for the treatment of urethritis, carbuncles and carcinomas. The objective of this study was to confirm the cognitive enhancing effect of D. superbus in memory impairment induced mice and to elucidate the possible potential mechanism. Effect of D. superbus on scopolamine induced memory impairment on mice was evaluated using the Morris water maze and passive avoidance tests. We also investigated acetylcholinesterase (AChE) activity and brain-derived neurotropic factor (BDNF) expression in scopolamine-induced mice. HPLC-DAD analysis was performed to identify active compounds in D. superbus. The results revealed that D. superbus attenuated the learning and memory impairment induced by scopolamine. D. superbus also inhibited AChE levels in the hippocampi of the scopolamine-injected mice. Moreover, D. superbus increased BDNF expression in the hippocampus. Eight compounds were identified using HPLC-DAD analysis. The content of 4-hydroxyphenyl acetic acid was higher than contents of other compounds. These results indicated that D. superbus improved memory functioning accompanied by inhibition of AChE and upregulation of BDNF, suggesting that D. superbus may be a useful therapeutic agent for the prevention or treatment of Alzheimer's disease.
Recently, a traditional medicine called Gouteng-san, which consists of eleven herbs, was reported to be effective in treating vascular dementia with a double-blind, placebo-controlled study. Gout-eng-san is also used for patients with vascular dementia in combination with Si-Wu-Tang. The effect of Gouteng-san and Si-Wu-Tang on deficit of learning behavior was investigated using step-down passive avoidance task in mice. Hot-water extract of Gouteng-san (1.5 and 6 g/kg, p.o.) significantly prolonged the step-down latency shortened by scopolamine. The extract of Uncaria hook (150 mg/kg, p.o.), one of the component herb of Gouteng-san, significantly prevented the decrease in the latency after scopolamine. Hot-water extract of Si-Wu-Tang (1.5 and 6 g/kg of dried herbs, p.o.) prevented dose-dependently scopola-mine-induced disruption qf learning behavior. Si-Wu-Tang also prevented the ischemia-induced deficit of learning behavior. Both hot water extract of peony and angelica (1.5 g/kg, p.o.), which are component herbs qf Si-Wu-Tang, prevented the scopolamine-induced learning behavior deficit. Scopolamine (10 uM) suppressed long-term potentiation (LTP) of population spike in the CA1 region of the rat hippocampal slices. Peoniflorin (0.1~ 1uM) extracted from paeony root significantly ameliorated scopolamine-induced inhibition of LTR These results suggest that improvement of deficit of learning behavior by Gouteng-san and Si-Wu-Tang is mediated by direct and/or indirect activation of the cholinergic system in the brain.
