The Effect of Thin Teflon on TLD Response for in vivo Dosimetry of Radiotherapy (생체 내 흡수선량 측정을 위한, 얇은 테프론의 TLD 반응감도에 대한 효과성)

  • The purpose of this study was to evaluate the performance of the teflon encapsulated TLD rod, which may be used in nuclear medicine for the direct in vivo measurements of radiation dose. We analyzed the influence of teflon encapsulation for measuring absorbed dose. An experiment was carried out to evaluate and observe the response of a LiF TLD-100 rod in a thin-wall teflon capsule at different depths in a solid phantom. An adult anthropomorphic phantom was used to measure the absorbed dose using thin teflon encapsulated TLD. The measurements of PDD-, and TMR in solid phantom and athe bsorbed dose in humanoid phantom performed with normal TLD were compared with values obtained by teflon encapsulated TLD. It was demonstrated that the difference of TL response of LiF in phantom with and without teflon thin-wall capsule was less than 3% under the same conditions beyond the build-up region. However, significant differences were observed near the phantom surface because of the build-up effect caused by the thin-wall thickness of the teflon capsule. Thus, our study showed that the contribution of teflon thin-wall capsule to TLD response for the megavoltage photon beams was negligible and that it did not significantly effect dose measurement. The teflon encapsulated TLD described in this work has been proven to be appropriate for in vivo dosimetry in therapeutic environments.

Verification of Stereotactic Target Point Achieved by Acquisition of MR Image in Actual Treatment Position of Radiosurgery (정위적 방사선 수술시 치료위치에서의 정위적 표적점 확인을 통한 자기공명영상 획득의 정확도 연구)

  • To make practical application of the MR image for stereotactic radiosurgery, the target point-achieved by acquisition of MR image in stereotactic radiosurgery planning system must agree with the actual isocenter of irradiation in real treatment. And the amount of distortion of the MR image must be known to make a correction for the agreement. A radish containing abundant water content was chosen as a homogeneous phantom for the purpose of verification of the agreement in this experiment. A dosimetric film was firmly attached to the small specially fabricated acryl plate and needle puncture was made through the film just into the acryl plate and a drop of oil was dropped into the hole of the film. The acryl plate with film was inserted into the radish and the dorp of oil represented the target point in MR image. After the image acquisition by stereotatic radiosurgery planning system, we achieved stereotactic coordinate of the target point represented by the oil drop. And we proceeded to actual irradiation to the target point according to the procedure of stereotactic radiosurgery. After the irradiation, the film in the radish was developed and processed and the degree of coincidence between the center of the radiation distribution and the target point represented by the hole in the film was measured. The discrepancy between two points was under 0.5 mm. so we could confirm good coincidence in homogeneous phantom such as radish. On the other hand, authors tried to use our home-made device for estimation of distortion of MR image.

Feasibility Study of Improved Patch Group Prior Based Denoising (PGPD) Technique with Medical Ultrasound Imaging System

  • The purpose of this study was to quantitatively evaluate image quality using intensity profile, coefficient of variation (COV), and peak signal to noise ratio (PSNR) with respect to noise reduction techniques in the ultrasound images. For that purpose, we compared with the median filter, Rudin-Osher-Fatemi (ROF), Anscombe and proposed patch group prior based denoising (PGPD) techniques. To evaluate image quality, the Shepp-Logan phantom and the ultrasound image were acquired using simulation and experiment, respectively. According to the results, the difference of intensity profile using PGPD technique is lowest compared with original Shepp-Logan phantom. In simulation, the measured COV was 0.249, 0.198, 0.198, 0.177, and 0.080 using noisy, median, ROF, Anscombe and PGPD technique, respectively. Also, in experimental image, the measured COV was 0.245, 0.230, 0.231, 0.242 and 0.187 using noisy, median, ROF, Anscombe and PGPD technique, respectively. Especially, when we used PGPD technique, the PSNR has highest value in both simulation and experiment. In this study, we performed simulation and experiment study to compare various denoising techniques in the ultrasound image. We can expect the PGPD technique to improve in medical diagnosis with excellent noise reduction.

Patient Exposure Dose Reduction in Coronary Angiography & Intervention (심혈관조영술 및 중재술 시 환자 선량 감소방안)

  • This study, the method of reducing the exposure dose by changing the geometrical requirements among the preceding studies and the method of directly wearing a protector on the patient were used to expose the patient. A comparative experiment was conducted on the method of reducing the dose and the most effective method for reducing the exposure dose was investigated. Using the phantom, the dose of the lens, thyroid gland, and gonad gland in the 5 views most used in coronary angiography and intervention accumulated 5 times for 10 seconds at 60~70 kV, 200~250 mA as an automatic controller of the angiography system, and measured by Optically Stimulated Luminescent Dosimeter(OSLD). SID 100 cm and Cine 15 f/s as a control group the experiment was conducted by dividing the experimental group into 3 groups: a group lowered to Cine 7.5 f/s, a phantom protector, and a group lowered to 95 cm SID. As a result of the experiment, showing decrease in exposure dose compared to the control group. Lowering the cine frame may be the simplest and most effective method to reduce the exposure dose, but there is a limit that it cannot be applied if the operator judges that the diagnostic value is small or feels uncomfortable with the procedure. Conclusion as fallow reducing the exposure dose by directly wearing protector is the next best solution, and it is hoped that the conclusions obtained through this study will help reduce the exposure dose to unnecessary organ.

Optimal Echo phase of FLASH sequence for Brain Enhancement scan of mouse at 9.4T MRI system (9.4T MRI FLASH Sequence에서 마우스의 뇌 조영증강 검사를 위한 적정 Echo phase)

  • The objective of study was to investigate the optimal echo phase for mouse brain enhancement scan using fast low angle shot (FLASH) sequence of 9.4T magnetic resonance imaging (MRI). For quantification based on this method, an MR phantom experiment and clinical research were done. The phantom experiment was conducted by fabricating three phantoms with different molar concentration of gadolinium to create changes in echo phase of 9.4T FLASH sequence used in mouse brain scans. In the phantom experiment, SSI was 25~27 [arbitrary units, a.u.] in each of 33 phases from $6{\pi}$ to $28{\pi}$, while RSP was 30~100 mmol. MPSI was 47~52 [a.u], while MPP, where MPSI is seen, was 0.8~9 mmol. EPMS was 80.8~108.0%, while ASIMP was formed between 21.1 and 31.8 [a.u]. In the clinical research, Finally, the occurrence rate of artifact that expressed -1 nd +1. The present study was able to quantify the degree of enhancement at FLASH sequence of 9.4T MRI, as well as identify the optimal echo phase during mouse brain enhancement scan.

Evaluation of the Image Quality According to the Pre-set Method in PET/CT Image (PET/CT 영상 획득 시 사전설정법 차이에 따른 영상 질 평가)

  • Purpose: The result of exam using an imaging device is very closely related with the image quality. Moreover, this image quality can be changed according to the condition of image acquisition and evaluation method. In this study, we evaluated the image quality according to the difference of pre-set method in PET/CT image. Materials & Methods: PET/CT Discovery STe16 (GE Healthcare, Milwaukee, USA), Chest PET phantom (Experiment 1) and 94 NEMA phantom (Experiment 2) were used. Phantom were filled with $^{18}F$-FDG maintaining hot sphere and background ratio to 4:1. In the case of experiment 1, we set the radio activity concentration on 3.5, 6.0, 8.6 kBq/mL. In the case of experiment 2, we set the radio activity concentration on 3.3, 5.5, 7.7, 9.9, 12.1, 16.5 kBq/mL. All experiments were performed with the time-set method for 2 minutes 30 seconds per frame and the count-set method with one hundred million counts in 3D mode after CT transmission scan. For the evaluation of the image quality, we compared each results by using the NECR and SNR. Results: In the experiment 1, both the NECR and SNR were increased as radioactivity concentration getting increased. The NECR was shown as 53.7, 66.9, 91.4. and SNR was shown as 7.9, 10.0, 11.7. Both the NECR and SNR were increased in time-set method. But the count-set method's pattern was not similar with the time-set method. The NECR was shown as 53.8, 69.1, 97.8, and SNR was shown as 14.1, 14.7 14.4. The SNR was not increased in count-set method. In experiment 2, results of both the NECR and SNR were shown as 45.1, 70.6, 95.3, 115.6, 134.6, 162.2 and 7.1, 8.8, 10.6, 11.5, 12.7, 14.0. These results were shown similar patten with the experiment 1. Moreover, when the count-set method was applied, the NECR was shown as 42.1, 67.3, 92.1, 112.2, 130.7, 158.7, and SNR was shown as 15.2, 15.9, 15.6, 15.4, 15.5, 14.9. The NECR was increased but SNR was not shown same pattern. Conclusion: Increment of administered radioactivity improves the quality of image unconcerned with the pre-set method. However, NECR was not influenced by increment of total acquisition counts through simple increasing scan duration without increment of administered activity. In case of count-set method, the SNR was shown similar value despite of increment of radioactivity. So, the administered activity is more important than the scan duration. And we have to consider that evaluation of image quality using only SNR may not be appropriate.

The Impact of Signal Intensity and Image Distortion Magnetic Resonance Imaging in the Orthopedic Prosthetic Metal (자기공명영상에서 정형보철 금속이 신호강도와 영상왜곡에 미치는 영향)

  • Used as an ingredient in the hospital for orthopedic prosthetic stainless and titanium metal the same size as on the MRI diagnostic value of imaging were compared. Center of images, background high band portion of the video signal is converted into a weighted intensity values Normal images and compared. The area of normal slice and also the distortion of images and cross-sectional imaging of a range of quantitative and sagittal planes were compared. As a result, the periphery high band signal intensity values of Stainless video phantom 2, pig bone 1.8, Titanium imaging of phantom 1.7 has higher value than Normal video pig bone 1.3 times the signal strength rivers. MRI distortion of the shape and the distortions of the range, if the cross-sectional area compared to Normal Slice Stainless case of phantom 65.8 %, pig bone 61.5 %, Titanium distortion phantom 23.1 %, pig bone 38.5 % of the range of community found. In this experiment, as a result, MRI was found to be Titanium more diagnostic value than the specimen with respect to the signal intensity weighted value and low distortion range, Stainless.

Entrance Skin Dose and Image Quality Evaluation According to Use Grid Radiography for the Extremity in FPD System (FPD System에서 상.하지 촬영 시 격자에 따른 환자 선량 및 화질 평가)

  • By accessing the current status of FPD system use in the hospitals located in Seoul and Gyeonggi Province as well as the entrance skin dose and the image quality evaluation realized by C-D Phantom, and the image assessment by the medical professionals regarding the radiography for the extremity, the following results were derived. 1. According to the evaluation made in the actual use of FPD system (12 machines), the grid ratio varied from 8:1 to 13:1, and 6 machines used the grid ratio with 12:1, realizing the largest number. Among the machines, there were 8 machines that allowed a removable grid while 3 machines did use a removable grid (25.0%). 2. When it came to the equipments used for the experiment, it showed that the amount of the entrance skin dose increased from 4.13 times up to 4.79 times with the grid use. 3. The difference in the entrance skin dose depending on the changes in the exposure condition(0.5times or 2.0times) was not significantly different regardless of the patients' thickness. 4. In terms of the image quality depending on C-D Phantom, the grid use was distinguished well. However, the images were well distinguishable as the exposure condition got increased. 5. In the clinical assessment, the grid use was less effective for the Hand PA, which was considered to shoot a thin body part. It was evaluated that the grid use was preferred for the Knee AP, which was shooting for a relatively thick body part. Nonetheless, 3 out of 5 people said that they would not use the grid if the entrance skin dose to reduced.

Changes in CT Number and Noise Level according to Pitch in Spiral Image Acquisition (나선형영상획득에서 Pitch에 따른 CT 감약계수와 잡음의 변화)

  • In this study, a self-made customized phantom was used to quantitatively measure the change in CT number and noise according to the change of pitch. In order to acquire an image using the phantom, the inside of the phantom was filled with sterile distilled water. Inside the glass tube, a solution obtained by diluting the ratio of normal saline and contrast medium to 100%(NS), 400:1, 200:1, 100:1, 50:1, respectively, was placed and imaged. At this time, the pitch was divided into steps of 0, 0.35, 0.7, 1.05, and 1.4 for each dilution ratio of the solution and imaged, respectively. One-way ANOVA analysis were performed to verify whether the mean of the CT number and noise values measured in all ROIs by dilution ratio showed a significant difference according to the change in pitch. As a result of the experiment, there was no statistically significant difference in the change of the CT number according to the change in the pitch for each dilution ratio, but the noise value tended to increase with the increase of the pitch, and showed a statistically significant difference. In the spiral image acquisition of CT, noise can be changed to a significant level depending on the pitch. Therefore, it will be necessary to set the quality evaluation items and criteria for CT images using the spiral image acquisition method.

A Study of Cancer Incidence Rate due to Photoneutron Dose during Radiation Therapy for Prostate Cancer Patients (전립샘암 환자의 방사선 치료 시 광중성자 선량으로 인한 암 발생률의 연구)

  • The purpose of this study was to study the probability of cancer occurrence due to photoneutron dose exposure of the colon and thyroid gland, which are normal organs, in 3D CRT, IMRT 5 portals, and IMRT 9 portals, which are radiotherapy methods for prostate cancer. The total prescribed dose for prostate cancer was 6600 cGy, 220 cGy per dose, and 30 divided irradiations were applied for the total number of times. After setting up the Rando phantom on the treatment table (couch) of the medical linear accelerator used in the experiment, an optically stimulated luminescence albedo neutron dosimeter was placed on the corresponding area of the large intestine and thyroid gland of the phantom for measurement. During 3D CRT of prostate cancer, the probability of secondary cancer due to photoneutron dose to the colon and thyroid gland, which are normal organs, was 1.8 per 10,000 people. And IMRT 5 portals were 8.7 per 10,000 people, which was about 5 times larger than 3D CRT. IMRT 9 portals derived the result that there is a probability that 1.2 people per 1,000 people will develop cancer. Based on this study, the risk of secondary radiation exposure due to the dose of photoneutrons generated during radiation therapy is studied, and it is thought that it will be used as useful data for radiation protection in relation to the stochastic effect of radiation in the future.
