  • The foot or lower extremity is rarely the initial site in adult-onset dystonia, whereas dystonia in children often begins in the foot. Isolated lower extremity dystonia in adults is rarely on a primary basis. Oral anti-dystonic medications have been found unsatisfactory in providing adequate symptomatic relief. On the other hand, botulinum toxin injections have been reported as beneficial. It is also known that personalized orthosis can be an effective solution for patients of dystonia. The purpose of this report is to demonstrate a case of primary focal foot dystonia that was effectively treated with botulinum toxin injection and the custom-made ankle-foot orthosis.

  • The emergence of new therapeutic agents for non-small cell lung cancer (NSCLC) implies that histologic subtyping and molecular predictive testing are now essential for therapeutic decisions. Histologic subtype predicts the efficacy and toxicity of some treatment agents, as do genetic alterations, which can be important predictive factors in treatment selection. Molecular markers, such as epidermal growth factor receptor (EGFR) mutation and anaplastic lymphoma kinase (ALK) rearrangement, are the best predictors of response to specific tyrosine kinase inhibitor treatment agents. As the majority of patients with NSCLC present with unresectable disease, it is therefore crucial to optimize the use of tissue samples for diagnostic and predictive examinations, particularly for small biopsy and cytology specimens. Therefore, each institution needs to develop a diagnostic approach requiring close communication between the pulmonologist, radiologist, pathologist, and oncologist in order to preserve sufficient biopsy materials for molecular analysis as well as to ensure rapid diagnosis. Currently, personalized medicine in NSCLC is based on the histologic subtype and molecular status. This review summarizes strategies for tissue acquisition, histologic subtyping and molecular analysis for predictive testing in NSCLC.

Hearing Rehabilitation with Combined Electroacoustic Stimulation and Ossiculoplasty

  • We report a case of hearing rehabilitation following combined cochlear implantation and ossiculoplasty. A 71-year-old patient visited the clinic for right-sided mixed hearing loss. We targeted neural and conductive components, performing two different operations simultaneously. At two months post-operative, the patient showed satisfactory results with respect to hearing threshold and speech comprehension. Our experience suggests that careful evaluation of patients and consideration of the diverse array of available treatment strategies can be used to provide personalized rehabilitation with maximal effectiveness. To the best of our knowledge, this is the first report to take such an integrated approach to treat hearing impairment and is thus likely to have clinical importance for otologists.

  • Radiation therapy (RT) is a very important treatment for cancer that irradiates a large amount of radiation to lead cancer cells and tissues to death. The progression of RT in the aspect of personalized medicine has greatly advanced over the past few decades in the field of technical precision responding anatomical characteristics of each patient. However, the consideration of biological heterogeneity that makes different effect in individual patients has not actually applied to clinical practice. There have been numerous discovery and validation of biomarkers that can be applied to improve the efficiency of radiotherapy, among which those related to genomic information are very promising developments. These genome-based biomarkers can be applied to identify patients who can benefit most from altering their therapeutic dose and to select the best chemotherapy improving sensitivity to radiotherapy. The genomics-based biomarkers in radiation oncology focus on mutational changes, particularly oncogenes and DNA damage response pathways. Although few have translated into clinically viable tools, there are many promising candidates in this field. In this review the prominent mutation-based biomarkers and their potential for clinical translation will be discussed.

  • The Korean Thyroid Imaging Reporting and Data System (K-TIRADS) is an ultrasound-based risk stratification system for thyroid nodules that has been widely applied for the diagnosis and management of thyroid nodules since 2016. This review article provides an overview of the use of the K-TIRADS compared with other risk stratification systems. Moreover, this review describes the challenges in the clinical application of the K-TIRADS, as well as future development directions toward the personalized management of patients with thyroid nodules.

설명가능한 인공지능을 활용한 안면 특징 분석 기반 사상체질 검출 (Sasang Constitution Detection Based on Facial Feature Analysis Using Explainable Artificial Intelligence)

  • Objectives The aim was to develop a method for detecting Sasang constitution based on the ratio of facial landmarks and provide an objective and reliable tool for Sasang constitution classification. Methods Facial images, KS-15 scores, and certainty scores were collected from subjects identified by Korean Medicine Data Center. Facial ratio landmarks were detected, yielding 2279 facial ratio features. Tree-based models were trained to classify Sasang constitution, and Shapley Additive Explanations (SHAP) analysis was employed to identify important facial features. Additionally, Body Mass Index (BMI) and personality questionnaire were incorporated as supplementary information to enhance model performance. Results Using the Tree-based models, the accuracy for classifying Taeeum, Soeum, and Soyang constitutions was 81.90%, 90.49%, and 81.90% respectively. SHAP analysis revealed important facial features, while the inclusion of BMI and personality questionnaire improved model performance. This demonstrates that facial ratio-based Sasang constitution analysis yields effective and accurate classification results. Conclusions Facial ratio-based Sasang constitution analysis provides rapid and objective results compared to traditional methods. This approach holds promise for enhancing personalized medicine in Korean traditional medicine.

Frailty, Sarcopenia, Cachexia, and Malnutrition in Heart Failure

  • With global aging, the number of patients with heart failure has increased markedly. Heart failure is a complex condition intricately associated with aging, organ damage, frailty, and cognitive decline, resulting in a poor prognosis. The relationship among frailty, sarcopenia, cachexia, malnutrition, and heart failure has recently received considerable attention. Although these conditions are distinct, they often exhibit a remarkably close relationship. Overlapping diagnostic criteria have been observed in the recently proposed guidelines and position statements, suggesting that several of these conditions may coexist in patients with heart failure. Therefore, a comprehensive understanding of these conditions is essential, and interventions must not only target these conditions individually, but also provide comprehensive management strategies. This review article provides an overview of the epidemiology, diagnostic methods, overlap, and prognosis of frailty, sarcopenia, cachexia, and malnutrition in patients with heart failure, incorporating insights from the FRAGILEHF study data. Additionally, based on existing literature, this article discusses the impact of these conditions on the effectiveness of guideline-directed medical therapy for patients with heart failure. While recognizing these conditions early and promptly implementing interventions may be advantageous, further data, particularly from well-powered, large-scale, randomized controlled trials, are necessary to refine personalized treatment strategies for patients with heart failure.

The Tumor Suppressor, p53, Negatively Regulates Non-Canonical NF-κB Signaling through miRNA-Induced Silencing of NF-κB-Inducing Kinase

  • NF-κB signaling through both canonical and non-canonical pathways plays a central role in immune responses and inflammation. NF-κB-inducing kinase (NIK) stabilization is a key step in activation of the non-canonical pathway and its dysregulation implicated in various hematologic malignancies. The tumor suppressor, p53, is an established cellular gatekeeper of proliferation. Abnormalities of the TP53 gene have been detected in more than half of all human cancers. While the non-canonical NF-κB and p53 pathways have been explored for several decades, no studies to date have documented potential cross-talk between these two cancer-related mechanisms. Here, we demonstrate that p53 negatively regulates NIK in an miRNA-dependent manner. Overexpression of p53 decreased the levels of NIK, leading to inhibition of the non-canonical NF-κB pathway. Conversely, its knockdown led to increased levels of NIK, IKKα phosphorylation, and p100 processing. Additionally, miR-34b induced by nutlin-3 directly targeted the coding sequences (CDS) of NIK. Treatment with anti-miR-34b-5p augmented NIK levels and subsequent non-canonical NF-κB signaling. Our collective findings support a novel cross-talk mechanism between non-canonical NF-κB and p53.

"아시아인 건강을 위한 한국인 게놈" : 한국인 유전체 프로젝트의 상업화 전략 ("The Korean Genome for Asian Health": A Commercialization Strategy of the Korean Genome Projects)

  • 인간 유전체 프로젝트의 초안 발표 이후 여러 한국인 유전체 프로젝트들이 추진되었다. 그 결과 등장한 한국인 유전체를 둘러싼 흥미로운 담론 중 하나는 "한국인 유전체" 서열 분석을 통해 "아시아인 맞춤의학"을 구현할 수 있다는 주장이다. 본 논문은 이를 한국 유전체 학자들이 자국민에 대한 유전체 자료를 상업화하려는 노력 가운데 발전시킨 전략으로 인지하고, 이 "아시아인 건강을 위한 한국인 게놈" 전략이 출현하게 된 배경을 역사적으로 검토한다. 이 글은 한국 유전체 프로젝트들의 전략이 탈식민 국가들에서 빈번하게 발견되는 "유전체 주권"(genome sovereignty) 정책이 2000년대 초반 이후 한국에서 주요 정책 의제로 부상한 아시아 지역주의와 결합하여 등장한 산물이라고 주장한다. 이를 통해 이 연구는 그간 범아시아 SNP 컨소시엄(Pan-Asian Single Nucleotide Polymorphism Consortium)을 중심으로 논의된 유전체학과 아시아인의 구성에 관한 과학기술학 연구가 국소적인 아시아인 관념과 아시아 지역주의를 가진 싱가포르의 경험을 지나치게 일반화해왔음을 지적한다. 이와 함께 한국 유전체학 거버넌스에서 과학기술학자들이 맡을 수 있는 역할에 대해서도 고민해 볼 기회를 제공할 것이다.
