The use of platelet rich plasma in the treatment of immature tooth with periapical lesion: a case report

  • This study describes the treatment of an immature permanent tooth with periapical lesion which was treated with regenerative approach using platelet rich plasma (PRP). The root canal of immature human permanent tooth with periapical lesion was gently debrided of necrotic tissue and disinfected with 2.5% NaOCl, and then medicated with triple antibiotic paste comprised of ciprofloxacin, metronidazole, and tetracycline. When the tooth was asymptomatic, PRP and mineral trioxide aggregate (MTA) were placed into the root canal. Six months after PRP treatment, radiographical examination revealed resolution of the radiolucency and progressive thickening of the root wall and apical closure. Our findings suggest that PRP can be used for the treatment of immature permanent teeth with periapical lesion, as part of a regenerative endodontic treatment procedure.

  • In this paper, the Judgment 2007DA76290 of the Korean Supreme Court was analysed in two points of the legal theory and litigation. The judgment arouses some issues of medical malpractice liability. They includes the concept of the complications and permanent lesion and the difference between them, some problems in a judge's applying the requirements for the physician's tort liability to the medical malpractice situations, the theory of obligation de moyens related with the burden of proof of the negligent conduct for a physician's liability for misperformance of contract, the influence of a patient's physical conditions on the physician's liability, the breach of duty to disclose in selecting the safer one of the treatment methods bringing about the complications or leaving the permanent lesion and so on. In the situations of the case referred to above, the plaintiff should have tried to establish that a reasonable physician in the specific situation of the case would have substituted the safer method of treatment for the method in the case. If the plaintiff had succeeded in establishing it, he or she could have recovered even the physical harm resulting from the permanent lesion brought about by the complications of the specific treatment in the case. The plaintiff failed to do so and recovered only the emotional distress which the patient suffered owing to the physician's breach of the duty to disclose. Therefore the legal malpractice of the counsel might be found in this case.

THE SPONTANEOUS ERUPTION OF DISPLACED PERMANENT TOOTH BY PERIAPICAL LESION OF PRIMARY TEETH (유치의 치근단 병소에 의해 변위된 영구치의 자발적 맹출)

  • The infection of primary teeth occurs by caries or trauma. But, if it is not treated on time, the complication may occur such as enamel hypoplasia, discoloration, impaction, displacement and dilaceration of permanent successors. The periapical lesion on primary tooth could displace the permanent successors in any direction. Treatment options of displaced tooth are observation after extracting the primary teeth, surgical exposure orthodontic traction, transplantation and extraction. In these cases, displacement of permanent central incisor caused by the periapical lesion of primary teeth was observed. The spontaneous eruption after extraction of primary teeth was expected considering degree of displacement, development of root and direction of eruption. This case report shows spontaneous eruption of displaced permanent tooth germ was achieved with periodic examination after extraction of primary teeth.

  • Primary teeth and the permanent successor must be understood as interdependent units, where each one of them interacts with and depends upon the other. The spread of pulpal inflammation in primary teeth to the periradicular tissues can lead to early eruption, enamel hypoplasia or hypocalcification, developmental arrest of permanent successor. Also the periapical inflammation cause permanent tooth displacement in various direction. We describe here two clinical cases of displaced permanent successor caused by periapical lesion of mandibular primary molar in children.

Cystic lesion between a deciduous tooth and the succeeding permanent tooth: a retrospective analysis of 87 cases

  • Objectives: The purpose of this study is to investigate the characteristics of dentigerous and radicular cysts that occur between deciduous and succeeding permanent teeth and to propose considerations for differential diagnosis of cysts at the treatment planning stage in the outpatient clinic. Materials and Methods: A total of 87 patients with a cystic lesion located between a deciduous tooth and the succeeding permanent tooth participated in the study. Twelve variables were analyzed to diagnose such a cyst. For data analysis, Fisher's exact test was used to determine the statistical significance of the variables. Results: Of the total 87 patients who participated in this study, 69 were diagnosed with dentigerous cysts and 18 were diagnosed with radicular cysts. Seven of the 12 differential factors analyzed in this study were statistically significant: age, location, symptoms, dental caries, endodontic treatment, delayed eruption, and size. Conclusion: Several criteria can be considered for diagnosis of dentigerous cysts or radicular cysts. Age, location, presence of symptoms and dental caries, previous endodontic treatment, cystic size, and delayed eruption of impacted permanent teeth are reliable factors that should be considered when diagnosing dentigerous and radicular cysts.

Safety of Total and Near-total Thyroidectomy (갑상선 전 절제술 및 근전 절제술의 안전성에 대한 고찰)

  • To clarify the safety of both total and near-total thyroidectomy, and to guide a selectionof an adequate type of surgical treatment of thyroid diseases, 192 consecutive total or near-total thyroidectomy cases were reviewed. They were divided into two groups: ont, the total thyroidectomy group(Group T,N=111) and the other, the near-total thyroidectomy group (Group NT, N=81). In both groups, complication rates, associations of complication rates with extents of surgery and stage of lesion were observed. Complication rate was significantly higher in Group T (53.6% vs 12.3%, p<0.05). But the rate of permanent complications such as permanent hypoparathyroidism and recurrent laryngeal nerve injury was remarkably low(4.5% in Group T, 6.0% in Group NT) and shows no significant difference in both groups. There was no permanent complication in cases where any type of neck dissection had not been performed regardless of the type thyroidectomy. But among whom underwent central compartmental neck dissection(CCND) and functional neck dissection(FND), 4(4.4%) and 4(6.4%) cases showed permanent complications. There was no statistical significance in differences between Group I and NT. In cases who underwent concomittant classical radical neck dissection(RND), 3(25.5%) showed permament complications. In this subgroups, complications were significantly higher in Group T(p<0.005). Complications were also directly related to the stage of the lesion. Only one patient showed permanent complication in 74 intracapsular lesions but 9 permanent complications were observed in 118 advanced lesions. We could clarify both total and near-total thyroidectomy were safe operations and the complications were related to accompanying neck dissections and the disease status rather than total or near-total thyroidectomy itself. Thus, we think that for the cases where higher complication rates are expected, such as locally advanced thryoid cancers or the cases which required wider neck dissection, the near-total thyroidectomy would be a preferrable method.

COMPARATIVE STUDY ON THE RATE OF DENTAL ENAMEL DEMINERALIZATION USING A QLF (Quantitative Light-induced Fluorescence를 이용한 법랑질 탈회 속도에 관한 비교 연구)

  • The objective of this study was to compare the rate of in vitro demineralization of bovine permanent (BP), human deciduous (HD) and human permanent (HP) enamel. Twenty aye flattened and polished enamel samples for each group (BP, HD, HP) were immersed in a demineralizing solution (0.1 mol/L lactic acid, 0.2% Carbopol 907, and 50% saturated hydroxyapatite) for 1, 2, 4 or 8 days. All 25 samples from each group were subjected to Quantitative light induced fluorescence analysis (QLF) and 5 samples from each group were randomly selected for Transverse Microradiography analysis (TMR). Integrated mineral loss (IML) and lesion depth (LD) were determined by TMR. The fluorescence radiance (FR) of sound enamel $(FR_S)$, demineralized enamel $(FR_D)$ were determined by QLF and FR ratio $(FR_D/FR_S)$ was calculated. Bovine enamel samples showed significant correlation between FR ratio and lesion depth(p<0.05) and deciduous enamel samples does not showed significant correlation between FR ratio and lesion depth(p>0.05). Permanent enamel samples showed significant correlation between FR ratio and lesion depth(p<0.05) The constant of demineralization time between FR ratio from regression analysis were as follows: bovine enamel was -4.643(p<0.05) deciduous enamel was -5.421(p<0.05) and permanent enamel was -4.435(p<0.05).

  • The pulp infection of primary tooth is often caused by dental caries or trauma. But, if it is not managed properly, it can be produce the periapical lesion. The periapical lesion can cause some complications such as enamel hypoplasia, displacement, root dilaceration and impaction. Treatment options of displaced permanent successor are clinical and radiological follow-up after extraction of primary teeth, surgical opening, orthodontic traction, transplantation and extraction. In these cases, the premolars with abnormal eruption path caused by periapical lesion of the primary tooth have shown successful spontaneous eruption just as a result of extraction of infected primary tooth and space maintenance.

  • The cariostatic effect of fluoride had been established by many epidemiologic and experimental studies. But, there are still different views on the mechnism of cariostasis and remineralization, especially about the proper fluoride concentration. The purpose of this study is to ascertain the remineralization of caries lesion and influence of fluoride concentration which affect remineralization by a study based on dynamic mechanism. The subjects, sound permanent teeth without demineralization or crack, were immersed in lactic acid buffered demineralization solution for 4 days. Dental caries with surface zone and subsurface lesion were artificially produced. All specimens were immersed in lactic acid buffered remineralization solution which had fluoride concentrations of 1 ppm, 2 ppm, 3 ppm for 10days. Final conclusions were obtained by observing the specimens for every 10 days under polarized microscopy. 1. Remineralization of caries lesion as well as demineralization of enamel were produced by changing the degree of saturation of lactic acid buffer solution. 2. Remineralization of caries lesion was facillitated by fluoride ion in lactic acid buffer solution. but, remineralization of the entire caries lesion was not increased as fluoride ion concentration increased.

  • A dieulafoy lesion, which is an unusual cause of gastrointestinal bleeding that can be fatal in children. Dieulafoy lesions are characterized by an abnormally large eroded submucosal artery that is commonly located in the lesser curvature of the proximal stomach. In most cases, permanent hemostasis is achieved by endoscopic epinephrine injection, however, some patients require other endoscopic treatment modalities, embolization or surgery. We report here a case of a Dieulafoy lesion in an 11-year-old boy who had recurrent bleeding from the lesion in the duodenal bulb after endoscopic epinephrine injection and surgical ligation, that was successfully treated using coil embolization.
