The Effects of Catheter Revision and Mupirocin on Exit Site Infection/Peritonitis in CAPD Patients (복막 투석 환자에서 도관 관련 감염 및 복막염에 대한 Mupirocin과 도관 전환술(Catheter revision)의 효과)

  • Background: Exit site/tunnel infection causes considerable morbidity and technique failure in CAPD patients. We presently use a unique revision method for the treatment of refractory ESI/TI in CAPD patients and mupirocin prophylaxis for high risk patients. Materials and Methods: We reviewed 139 CAPD patients about the ESI/TI from October 1993 to February 1999 at Yeungnam University Hospital. At the beginning of the ESI. we usually started medications with rifampicin and ciprofloxacin and then changed the antibiotics according to the sensitivity test. If the ESI had persisted and there were TI symptoms (purulent discharge, abscess lesion around exit site). we performed catheter revision(external cuff shaving, disinfection around tunnel and new exit site on opposit direction) with a combination of proper antibiotics. We applied local mupirocin ointment at the exit site three times per week to the 34 patients who had the risk of ESI starting from October 1998. Results: The total follow-up was 2401 patient months(pt. mon). ESI occurred on 105 occasions in 36 out of 139 patients, and peritonitis occurred on 112 occasions in 67 out of 139 patients. The total number of incidences of ESI and peritonitis was 1 per 23.0 pt. mon and 1 per 2l.6 pt.mon. The most common organism responsible for ESI was Staphylococcus aureus (26 of 54 isolated cases, 48%), followed by the Methicillin resistant S. aureus(MRSA) (13 cases, 24%). Seven patients(5: MRSA. 2: Pseudomonas) had to be treated with a revision to control infection. Three patients experienced ESI relapse after revision. One of them improved with antibiotics, while another needed a second revision and the remaining required catheter removal due to persistent MRSA infection with re-insertion at the same time. But, there was no more ESI in these 3 patients who were received management to relapse (The mean duration: 14.0 months). The rates of ESI were significantly reduced after using mupirocin than before(1 per 12.7 vs 34.0 pt.mon, P<0.01). Conclusions: In summary, revision technique can be regarded as an effective method for refractory ESI/TI before catheter removal. Also local mupirocin ointment can play a significant role in the prevention of ESI.

Pediatric Peritoneal Dialysis in Korea : Practical Solution to the Problems of Peritoneal Dialysis for Children (한국 소아복막투석의 현황 - 다기관 공동연구 결과보고 -)

  • Purpose : To find and solve the common problems of peritoneal dialysis(PD) by analysing the clinical data of pediatric PD performed in Korea. Methods : 264 cases of CAPD and acute PD had been performed from Nov.1987 to Oct. 1997 in 17 institutions of pediatric nephrology in Korea. Results : CAPD was performed in 114 cases. The mean age of the patients was $10.5{\pm}6.6$ years and male to female ratio was 1.4:1. The original renal diseases of ESRD were proven in 92 cases($80\%$). The common renal disease of ESRD were FSGS($17\%$), reflux nephropathy ($11\%$), chronic glomerulonephritis($9.6\%$). Mean duration of CAPD was $20{\pm}16.9$ months. Peritonitis was the most common complication and incidence was one episode/18.2 patient-months. Other complications were exit site infection in 10 cases, obstruction in 7 cases, leakage of dialysate in 6 cases. The most common etiologic organism of peritonitis was staphylococcus aureus and the next was staphylococcus coagulase(-). Acute PD was performed in 150 cases. Most common underlying causes were congenital heart disease, hemolytic uremic syndrome, sepsis and dehydration. The mean duration was $10.3{\pm}11.3$ days. The most common complication was peritonitis($20.6\%$). The most common etiologic organism was staphylococcus aureus and coagulase(-), acinetobactor and pseudomonas. Conclusion : Reflux nephropathy should be emphasized in early diagnosis and treatment to prevent ESRD. Incidence of congenital anomaly($7\%$) as a original disease of ESRD was relatively low in Korea. Growth status was not significantly improved after CAPD. In acute PD, the incidence of peritonitis was rapidly increased at 2weeks after beginning of dialysis.

Acute carbohydrate engorgement in the Korean indigenous cattle, a case report (한우에서 급성탄수화물 과식증에 의한 폐사 증례)

  • This case occurred following accidental consumptions of toxic levels of feedstuffs in the Korean indigenous cattle which were pregnant for 3-6 months. Grossly, poorly defined bluish coloration in the ventral sac of the rumen, reticulum and omasum was visible on the surface of the stomach. The severely affected ruminal walls were perforated and blood-tinged exudate was observed and death followed due to acute diffuse peritonitis. There were copious amount of porridgelike rumen contents, which has a distinct fermentative odor. The epithelia were easily wiped off and hyperemic in patch. Histopathologically, chemical rumenitis with neutrophilic infiltration and opportunistic fungal infection was evident in the mucosa and submucosa. Multifocal necrotic foci with fungal hyphae on the liver and severe congestion on the renal medulla were noted. There was no evidence of heavy metal and aflatoxin Bl in the suspected feedstuffs. This case was diagnosed as acute carbohydate engorgement in cattle.

A Case of Intussusception with Acute Appendicitis

  • In children presenting to hospital with gastrointestinal symptoms, diseases such as intussusception and acute appendicitis require particular attention and careful examination. Early diagnosis and proper treatment are important because of possible severe complications such as peritonitis and death. Intussusception and appendicitis share similar clinical manifestations. More importantly, the presence of acute appendicitis together with intussusception in children is very rare. We describe an interesting case of a 38-month-old boy who presented with abdominal pain in the right lower quadrant. His vital signs were stable and laboratory test findings showed no specific alterations. We detected tenderness in the right lower quadrant. A computed tomography scan showed an ileocolic intussusception with no strangulation and diffuse wall thickening of the appendix trapped within the intussusception. The patient underwent an appendectomy and manual reduction.

Bar Suture for the Treatment of Bovine Abomasal Displacement (소의 제4위전위증 교정을 위한 봉봉합법)

  • Surgical treatment was performed in 507 heads out of 1078 dairly cows which were diagnosed as abomasal displacement. Out of 507 heads. bar suture was performed in 280 heads. Twelve heads out of 280 heads(4.9%) recurred. Incorrective suture of the reticulum or pylorus instead of abomasum occurred in 3 cows. The cows in which abdominal wall was sutured but the suture was not inserted into the abomasum were 4 heads. The cows in which bar was broken are 4 heads. A cow suffered peritonitis after bar suture. Bar suture technique is thought to be a good method as a first choice for the correction of abomasal displacement.

  • Ovarian cyst is found in 32% of necropsies in neonates, and can be visualized during gestation by ultrasonography. The clinical evolution of these Cysts is variable, but in most cases the prognosis is favorable. Ovarian torsion, bleeding, rupture, and peritonitis have been described as complications. We report a newborn girl with torsion of ovarian cyst. A cystic mass measuring $41.9{\times}31.9mm$ on left side of abdomen was identified at 32 weeks of gestation by fetal ultrasonography. Surgery was performed after birth.

IscR Modulates Catalase A (KatA) Activity, Peroxide Resistance, and Full Virulence of Pseudomonas aeruginosa PA14

  • We have identified the iscR (PA3815) gene encoding an iron-sulfur cluster assembly regulator homolog as one of the genes required for peroxide resistance in Pseudomonas aeruginosa PA14. Here, we present the phenotypic characterization of an iscR deletion mutant in terms of KatA expression, stress responses, and virulence. The iscR null mutant exhibited reduced KatA activity at the posttranslational level, hypersensitivity to hydrogen peroxide, and virulence-attenuation in Drosophila melanogaster and mouse peritonitis models. These phenotypes were fully restored by multicopy-based expression of katA. These results suggest that the requirement of IscR in P. aeruginosa is related to the proper activity of KatA, which is crucial for peroxide resistance and full virulence of this bacterium.

D-dimer Analysis in Dogs With Hypercoagulable Diseases

  • Total 283 dogs were enrolled in this study (control group: 140, patient group: 143). In the patient group, 143 dogs with underlying diseases including immune medicated hemolytic anemia (IMHA) (7), lymphoma (30), hyperadrenocorticism (HAC) (16), trauma (10), pyometra (8), bone fracture (38), peritonitis (13), meningoencephalitis (12) and mitral regurgitation (9) were enrolled in this study. Compared with healthy group, lymphoma, trauma, HAC, and IMHA group showed significantly (P < 0.01) high values of D-dimer and the highest levels in the IMHA group. Additionally, we evaluated the D-dimer level after a week of enoxaparin treatment, and the results showed D-dimer levels of post treatment group were significantly decreased compared to the pre-treatment group in lymphoma, HAC, trauma and IMHA diseases. In the high level D-dimer group, post D-dimer values after enoxaparin treatment had significantly decreased (P < 0.01) compared to levels prior to treatment.

Surgical Treatment of the Abdominal Aortic Aneurysm (복부 대동맥류의 외과적 치료)

  • We experienced 12 patients with the abdominal aortic aneurysm during last 31 years [Dec. 1958 \ulcornerSep. 1989]. Among them, 10 patients were reviewed. They were all male. The age ranged from 34 to 80 years with the mean age of 59.4 years. The etiology of the aneurysm was atherosclerotic in 8, mycotic in 1, and aortitis in 1. The location of the aneurysm was infrarenal in 8, and suprarenal in 2 cases. Aneurysmectomy and Dacron Y-graft interposition in 8 cases, and lease with Teflon Y-graft were made. In another 1 case, long thoracoabdominal bypass surgery was made. The operative mortality was 30%[3cases]. The postoperative complications were respiratory complications[3cases], acute renal failure[2cases], bleeding[lease], mechanical ileus[ lease], and peritonitis[lease].

  • In oral & maxillofacial surgery, careful management is necessary to avoid infection for old aged person or patient with diabetes mellitus(DM). We experienced a 69 years old female patient with DM, who has liver abscess and appendicitis, caused by Klebsiella pneumoniae, was developed into left mid-facial abscess and peritonitis. We report this case along with review of article for we have gained fairly good results in our attempt to resolve the patient's abscess by means of I & D of the left mid-face and percutaneous catheter drainage of the stomach, supplemented with fluid and antibiotic therapy.
