소아 복통에서 소아과 의사에 의한 초음파 검사의 유용성 (Usefulness of Ultrasonographic Examination by a Pediatrician in Children with Abdominal Pain)

  • 목적: 초음파 검사는 복통 환자의 선별 검사로 널리 사용되고 있다. 소아의 복통 환자에서 소아청소년과 의사에 의해 시행된 초음파 검사의 유용성에 대해 조사하였다. 방 법: 2008년 11월부터 2010년 6월까지 복통을 주소로 복부 초음파 검사를 받았던 소아 환자의 의무기록을 후향적으로 분석하였다. 결 과: 대상 환자는 628명(남자 325명, 여자 303명)이었고, 평균 연령은 $8.08{\pm}4.61$세였다. 급성 복통이 427명(68.0%), 1개월 이상의 만성 복통이 201명(32.0%)이었다. 복통의 위치는 미만성 36.9%, 배꼽주위 24.4%, 상복부 21.0%, 우하복부 8.1%, 하복부 3.0%, 우상복부 2.9%, 좌하복부 2.4%, 좌상복부이 1.3%였다. 327명(52.1%)에서 특이 소견이 없었고, 이상 소견이 있었던 301명(47.9%) 중 장간막 림프절염 170명, 소장염 또는 대장염 55명, 장중첩증 19명, 급성 충수돌기염 16명, 담관낭종 10명, Henoch-Sh$\ddot{o}$lein purpura 8명, 췌장염 2명이었다. 39명(6.2%)에서 복부 컴퓨터 단층 촬영 또는 자기공명촬영 검사가 병행되었는데 33명에서 동일한 결과가 나왔다. 복부 컴퓨터 단층 촬영을 통해 충수돌기염 2명, 췌장염 1명, Henoch-Sch$\ddot{o}$nlein purpura 1명이 진단되었다. 그러나 복부 초음파 검사에서 충수돌기염이 의심되었던 2명은 복부 컴퓨터 단층 촬영에서 진단이 되지 않았으나 수술을 통해 최종 충수돌기염으로 확진되었다. 전체 환자 중 624예(99.4%)에서 초음파 검사 결과와 최종 진단이 일치하였다. 결론: 소아과 의사에 의한 초음파 검사는 소아 복통의 감별진단에 신속하고 정확한 정보를 줌으로써 선별 검사로써 가치가 높으며 비침습적이고 방사선 조사가 없는 장점이 있다.

출산 전후 여성의 양육관련 신념 및 부모 효능감에 대한 연구 (A Study on Childrearing Belief and Parental Efficacy)

  • The purpose of this study was to explore the relationship between childrearing belief and parental efficacy of women before and after childbirth. For this purpose 253 pregnant women and 256 mothers with infants under one year of age were contacted and asked to fill in a packet of survey questionnaire at their visits to obstetrician, gynecologist and pediatrician. The questionnaire includes questions about what mothers value in childrearing, their expectations for their children and parental efficacy. Data collected were analyzed using SAS PC program. It was reported that comparing to mothers, pregnant women view childrearing in more ideal lights and expect their children to be more exceptional. Also working mothers reported comparatively lower parental efficacy than full-time mothers suggesting that social support system to minimize pressure coming from work for working mothers with young children should be provided and the parental education methods to enhance the parental efficacy should be sought.

How to Manage the Pediatric Nutritional Support Team: Updates

  • Pediatric patients in hospital are at risk of malnutrition at admission and even during their hospitalization. Although the concept of nutritional support team (NST) was introduced to hospitals for optimal nutritional care since 1960s and the benefits of pediatric NST have been proven by many studies and reports in terms of patient clinical outcome and cost saving, the pediatric NST is not widespread yet. The pediatric NST composed of pediatricians, dieticians, pharmacist, and nutrition support nurses as core members dedicated to nutritional care in children should be independent of central NST or other disciplines, but closely cooperate with other teams in hospitals. There is no doubt that a multidisciplinary NST is an effective way to provide appropriate nutritional support to an individual patient. Therefore, the implementation of the pediatric NST in hospitals should be recommended to provide optimum nutritional support including enteral tube feeding and parenteral nutrition and to assess pediatric patients at risk of malnutrition.

  • 최근들어 우리나라에서 소아 염증성 장질환 특히 크론병의 유병률이 매우 빠르게 증가하고 있다. 염증성 장질환의 진단을 위해 자세한 병력 청취와 혈액 검사 그리고 대장 내시경 검사가 필요하다. 대부분의 환자에서 복통, 설사, 체중감소, 혈변 등의 전형적인 증상을 보이지만 비전형적인 발현으로 장기간 진단에 어려움을 겪을 수 있어 주의를 요한다.

  • The incidence of chemothrapy related among pediatric cancer patient was 90.1%. Adequate oral intake and nutrition have been shown to be important. These consideration prompted the decision to survey by means of a questionaire. The questionaire were included nausea-vomiting peak time, causing factor, coping method, education need, diet pattern change and food preference. Results are fellow 1. Almost(90.1%) pediatric cancer patient experienced nausea-vomiting during chemotherapy and required coping method or reducing method. 2 . The food preference form were Identified. Those were fluid form, cold and small amout and frequentry eating form. The patients preferred noodles, chickens, soap, juice. The results of the survey indicate that nasea-vomiting relief nursing intervention are required pediatric cancer patient received chemotherapy. Health care personnels recognize the pediatric cancer patient's diet pattern and encourage the nutritional counselling. The care of patient should be multidisciplinary team approach and the nurse occupies a key position with in this team, which includes the pediatrician, nutrionist.

  • We often overlook the importance of several safety issues such as identification of patients, timeout procedure, hand hygiene, handoff communication, and many others. This ignorance, along with many other issues, leads to medical error being ranked as a third leading cause of death in the U.S. Consequently, quality improvement (QI) has become one of the major subjects in healthcare despite a relatively short history. Improving quality is about making healthcare safe, effective, patient-centered, timely, efficient, and equitable. Understanding the need and methodology of QI as well as participation is now essential for physicians. Although basic QI methodology has not changed, one of the most fascinating changes in recent QI is conducting large-scale QI projects through multicenter networks. Prospective multicenter QI projects utilizing the Korean Neonatal Network are a substantial initiation of pediatric QI in Korea. The Korean Pediatric Society should set ambitious goals for QI activities for every primary care pediatrician and pediatric subspecialist.

  • Urinary incontinence is the most common urinary symptom in children and causes considerable anxiety in children and parents. In most cases, there is no underlying organic pathology and the longterm prognosis is excellent. Despite these reassuring facts, children with wetting problems can be a challenge for primary pediatrician. This is probably because the problem is poorly understood, there is no readily identifiable medical pathology, and because treatment is usually time consuming and arduous. There is a great demand for treatment because wetting is an unpleasant symptom that can cause a stress and anxiety in the family. There may also be other coexisting problems such as urinary tract infection, constipation, soiling, and behavioral or emotional difficulties. Despite the frequency and vexing nature of voiding dysfunction, physicians may not always obtain a careful history to identify and to treat children properly with this condition. This article addresses the comprehensive diagnostic and therapeutic approach to such children.

  • Special formulas are those in which one of the basic nutrients (usually the protein and/or carrbohydrate) has been changed to an alternative nutrient that an individual baby may better tolerate. The following are specialized formulas : Soy formulas, hypoallergenic formulas, Lactose free formulas, Premature baby formulas, Low phosphate formulas, Formulas for inborn errors of metabolism. Less is known about the long-term effects of feeding babies these special formulas and each formula has its unique properties and indications. The pediatrician must acquaint themselves with all commercially available infant formulas so as to provide the parent with reliable and unbiased information about them.

  • Constipation is one of the most common symptoms that a child visits pediatrician. The general approach to the child with functional constipation includes the following steps; disimpaction, maintenance and withdrawal of medication. There are many drugs which can be applied to children; osmotic agents (lactulose, sorbitol, magnesium hydroxide/citrate, polyethylene glycol with/without electrolytes, sodium phosphate, glycerin), stimulants (senna, bisacodyl, caster oil), lubricant (mineral oil), bulking agent (psyllium, cellulose, glucomannan). At each stage of treatment, one or some of these drug can be applied to the purpose. The author tries to summarize recent studies on drugs for constipation in child, and finally introduces new dugs for constipation which is under investigation.

  • Type 2 diabetes mellitus in children and adolescents has been increasing for last 10 years. The increase in frequency of type 2 diabetes appears to parallel the increase in prevalence and severity of obesity in children and adolescents. The metabolic syndrome, cluster of potent risk factors for atherosclerotic cardiovascular disease and type 2 diabetes, consists of insulin resistance, obesity, hypertension and hyperlipidemia. The atherosclerotic cardiovascular disease are rarely seen in the young, but the pathologic processes and risk factors are associated its development have been shown to begin during childhood. In pediatrician it is important to recognize early and treat aggressively for prevention of future cardiovascular disease in children and adolescents with metabolic syndrome.
