In case of endodontic treatment and extensive restoration of severe dental caries, trauma, and developmental defect, esthetic restoration of primary incisors and permanent anterior teeth for children and adolescents period is delicate matter for pediatric dentists. Existing restorative methods for anterior teeth have retentive and esthetic limitations for badly damaged teeth, especially for the adolescent anterior teeth. Therefore, the preparative stage for setting the permanent prosthesis as well as the retention and esthetics have to be considered. In this case, esthetic restoration for badly destroyed anterior teeth was tried with fiber-reinforced post and the result was satisfactory.

Restoration of severly carious, malformed or traumatically fractured primary incisors is one of the most difficult challenges in restorative dental care for preschoolers. In restoring primary anterior teeth, four types of complete coverage crowns have been attempted over the years. Stainless steel crowns, open-faced stainless steel crowns, acid-etched crowns(strip crowns), and preformed polycarbonate crowns have been the choices. While these restorations have been acceptable for many patients, they all have very distinct limitations. The ideal full coronal restoration for a primary incisor requires durability, retention, and esthetics. This paper reported veneered stainless steel crown restoration of primary anterior teeth by silicoater technique. Veneered stainless steel crown restoration by silicoater technique is considered to be a durable, retentive, and esthetic restoration.

The use of stainless steel crowns are indicated for restoration of primary or permanent molars with proximal dental caries, extensive dental caries, or previous pulp treatment with increased danger of tooth fracture. Stainless steel crowns were introduced by Humphrey in 1950. For their improved durability, longevity, and success rate, they have been strongly considered for restoring extensive and multi-surfaced dental caries of molars in pediatric dentistry. However, they also have shortcomings, such as possibility of pulpal exposure or damaging proximal surface of adjacent teeth. In addition, when oversized stainless steel crowns are used, eruption of the adjacent permanent teeth may be disturbed by their prominent margin. As a means to compensate the shortcomings of stainless steel crowns, use of orthodontics bands may be considered. It is an alternative restoration method, where an orthodontic band is placed on a tooth first and cavity is restored with filling material, such as composite resin, glass ionomer, or amalgam. The use of an orthodontic band is indicated for molar restoration with cervical dental caries, extensive dental caries, enamel hypoplasia, or previous pulp treatment. Because it requires shorter chair time compared to stainless steel crown, its application is very useful for children with poor behavior. However, restoration using an orthodontic band requires good oral hygiene after its application. This case report illustrates the conservative restoration of primary molars and permanent molars with extensive dental caries using orthodontic bands.

Clinical performance of resin-based composite material depends on case selection and on the proficiency of the operator. Especially, composite resin restoration on permanent first molar in children have more limitations clinically than adult patients according to stage of tooth eruption and patient's compliance. This study was designed to evaluate the clinical performance of posterior composite resin restoration in children after 5 years. 35 teeth of 16 patients who were received composite resin restoration on permanent first molar in Department of Pediatric dentistry, Pusan National University Hospital between January 2001 and December 2001 were evaluated based on Modified USPHS criteria. From the finding in this study, following conclusions can be made. 1. 6 teeth(17%) of 35 teeth was replaced, so 5-years survival rate of posterior composite resin restoration is 82.9%. 2. As results of each evaluation criteria, on color match, anatomic form, surface roughness, sensitivity/ discomfort, ideal A grade score was 86.2%, 93.1%, 86.2%, 86.2%, clinically accepted B grade score was 13.8%, 0%, 13.8%, 10.3%. On marginal adaptation and marginal discoloration, A grade score was 13.8%, 44.8% and B grade score was 79.3%, 34.5% and secondary caries rate was 20.7%. 3. 69.1% of teeth (20 teeth) was clinically accepted on all evaluation criteria.

This study was performed to evaluate the restoration combined with resin infiltration (RI) of early cavitated smooth surface caries lesion in terms of microleakage. Flowable resin and resin-modified glass ionomer cement (RMGIC) were compared. Sound 20 extracted 3rd molars were divided into 2 groups randomly. Artificial decalcified lesion was induced. Cavities were prepared on the mesial and distal surfaces, and randomly set as experimental and control group. RI was applied to the experimental group before cavity restoration. The control group was restored without RI. In group I and II, flowable resin and RMGIC was used for restoration respectively. After thermocycling and silver nitrate immersion, microleakage was assessed by μ-CT. Depth of microleakage was lower in experimental group than control group only in group II (p = 0.05). Microleakage depth was lower in group II than group I in both experimental and control groups (p = 0.05). RI pretreatment before restoration of early cavitated caries lesions might reduce the microleakage and help long-term maintenance of restoration. In this study, RMGIC was less polymerization shrinkage. Restoration with RMGIC after RI pretreatment reduced the microleakage of the restoration compared to the flowable resin.

Post-endodontic restorations are both important and challenging for clinical success in endodontically treated posterior teeth. Several options have been proposed to restore endodontically treated molars. In pediatric dentistry, restoration using conventional single crowns, especially for partially erupted molars with insufficient retentive tooth structure, has proven to be difficult. However, the endocrown presents a conservative and esthetic restorative alternative to conventional crowns with post-and-core, as it acquires additional retention within the pulp chamber. The tooth preparation consists of a circular, equigingival, butt-joint margin and a central retention cavity in the pulp chamber that helps to construct both the crown and core as a single unit. This case report describes the esthetic and conservative endocrown restorations of erupting permanent first molars with extensive coronal destruction.

Pre-eruptive intracoronal resorption (PEIR) is a developmental defect in the crown of a pre-eruptive tooth. The purpose of this study was to investigate the relationship between the size of the pre-eruptive buccal pit radiolucency, which is suspected as PEIR on a panoramic radiography, and the need for restoration after the eruption of a mandibular first molar. The experimental group included 35 mandibular first molars, in which lesions requiring definite restoration were observed during eruption. The control group consisted of 64 sound mandibular first molars after eruption. The sex, age, tooth position, tooth formation stage, size of the pre-eruptive buccal pit radiolucency, and restoration methods in the experimental group and control group were examined. Compared with the control group, the experimental group showed a statistically significant difference in the size of the buccal pit before eruption. The buccal pit size for predicting the need for restoration was further examined by receiver operating characteristic curve analyses, and the area under the curve was $0.813{\pm}0.047$. If radiolucency is observed at the buccal pit of the mandibular first molar before eruption, periodic observations and post-eruption examinations are required.

Since January 2019, insurance coverage for caries treatment of permanent teeth using composite resin in children aged 12 and under has started in South Korea. The purpose of this study was to compare the pattern of permanent molar composite resin restoration aged 12 and under before and after the insurance coverage from January 2009 to March 2020 in the Department of Pediatric Dentistry at Kyung Hee University Dental Hospital at Gangdong. Since the insurance coverage was started in 2019, the frequency of permanent molar composite resin restoration aged 12 and under has increased more than twice. There was a significant change in frequency of resin restoration by shape of cavity comparing before and after the insurance coverage, whereas there was no significant difference in changes by patient age, gender and position of tooth. As accessibility to the permanent tooth composite resin restoration is increased by the insurance coverage, which shows a fair survival rate, it is expected that it could be able to secure oral health in the early permanent dentition aged 12 and under.

Children who have severely destructive anterior primary dentition, as in nursing-bottle caries, in trauma, in rampant caries and in developmental defects, present the dentist with one of the most perplexing situations in dentistry. Especially, children with managed behavor difficultly is very severly situations. This paper reported a new technique for the utilization of resin post and strip crown to indirect methods on severely destructive primary anterior teeth. We name it "indirect short resin post crown" in this paper. Indirect technique be considered to be a simple, a retentive, intensive, color-stable and esthetic restoration.

The purpose of this study were to investigate the chief complain and dental treatment needs in handicapped patient. This study examines treatment records of 1025 patients in free dental clinic for handicapped patients during 10 years from 1999 to 2008. The results were as follows : 5.8 average visit per patient; mean patient age was 25; 544 patients was younger than 20. Handicapped patients classified according to types of disability. Crippled disorder were 19.1%, brain disorder were 4.5%, visual disorder were 3.1%, auditory disorder were 4.1%, speech disorder were 0.9%, mental retardation were 67.1%, and developmental disorder were 25.1%. Performed treatments were 322 scaling, 13 fluoride varnish, 727 preventive resin restoration, 1296 resin restoration, 600 amalgam restoration, 46 GI restoration, 612 extraction, 289 pulp treatment of primary teeth, 75 pulp treatment of permanent teeth and 138 stainless steel crown restoration. Many handicapped patients have some difficulty to dental treatment. They have limited access to dental care, which is compounded by a shortage of skilled dental professionals who are willing to treat these population and financial problems.
