A Study on a Comparative Analysis of 2D and 3D Planning Using CT Simulator for Transbronchial Brachytherapy (전산화단층모의치료기를 이용한 경기관지 근접치료환자의 치료계획에 관한 고찰)

  • Purpose: Transbronchial brachytherapy used in the two-dimensional treatment planning difficult to identify the location of the tumor in the affected area to determine the process analysis. In this study, we have done a comparative analysis for the patient's treatment planning using a CT simulator. Materials and Methods: The analysis was performed by the patients who visited the hospital to June 2012. The patient carried out CT-image by CT simulator, and we were plan to compare with a two-dimensional and threedimensional treatment planning using a Oncentra Brachy planning system (Nucletron, Netherland). Results: The location of the catheter was confirmed the each time on a treatment planning for fractionated transbronchial brachytherapy. GTV volumes were $3.5cm^3$ and $3.3cm^3$. Also easy to determine the dose distribution of the tumor, the errors of a dose delivery were confirmed dose distribution of the prescibed dose for GTV. In the first treatment was 92% and the second was 88%. Conclusion: In order to compensate for the problem through a two-dimensional treatment planning, it is necessary to be tested process for the accurate identification and analysis of the treatment volume and dose distribution. Quantitatively determine the dose delivery error process that is reflected to the treatment planning is required.

Measurement of the Compression Force and Thickness applied during Mammography (유방촬영술에서 압력과 두께 측정)

  • The purpose of this study was to compare the compression force and thickness of the compressed breast between mediolateral oblique and craniocaudal mammography. This population consisted of 204 paired mediolateral oblique and craniocaudal mammography obtained on one mammographic unit by certified mammography technologists during 2 month period. The women examined were $30{\sim}59$ years old with breast implants, the patients were classified into 3 groups according to age, $30{\sim}39$, $40{\sim}49$, and $50{\sim}59$, prior breast cancer, mastectomy or breast deformity were excluded. The digital readout of compressed breast compression force and thickness was recorded. Mammographic positioning was assessed using standard criteria. The mean compression force of the compressed breast on the craniocaudal projection was less than the mean compression force on the mediolateral projection(14 versus 13.88 daN, p<0.05). The mean thickness of the compressed breast for mediolateral projection was 41.46 mm and that for the craniocaudal projection was 39.50 mm(p<0.05). The compressed breast is higher or thicker on mediolateral oblique than on craniocaudal mammography.

  • Lesch-Nyhan syndrome is a disease caused by metabolic disorder of purine. General muscle stiffness and hyposomia are shown from infancy and symptoms can include involuntary or irregular movements of arms and legs, mental retardation, and compulsive self-mutilating behaviors. Self-mutilating behaviors begin at approximately the first year or sometimes at late teens. The patients bite their lips, especially lower lip, tongue, buccal mucosa, hands and fingers. Tongue and lips can be injured or mutilated in severe cases. As the patient gets older, self-mutilating behaviors become more serious and extensive and secondary infection of injured areas is possible. Periodic soft tissue damage due to self-mutilating may evolve to cancer. Medical treatment, appliance treatment, extraction of tooth and surgical operation was attempted to control self-mutilaing behaviors. We hereby report the case of child Lesch-Nyhan syndrome patient who has self-inflicted labial damage as chief complaint. When patient was treated with conservate therapy, such as removable or fixed appliance, the frequency of labial damage could be subdued and yielded favorable results.

  • Hepatitis B is a serious public health problem leading to chronic infection and liver cancer. Quantitation of circulating hepatitis B virus (HBV) is important for monitoring disease progression and for assessing the response to antiviral therapy. In this study, by using Real-Time PCR and novel Micro-PCR assay method, we measured HBV concentration in the clinical sample. A total of 120 serum samples from patients with HBV infection collected was in Dankook university hospital to compare the detection limit, sensitivity, specificity and reproducibility of the two assay methods. These findings of this study suggest that Micro-PCR and Real-Time PCR assay methods are comparable to each other in there detection limit, sensitivity, and reproducibility for HBV DNA quantitation. However, Micro-PCR assay is more efficient than Real-Time PCR method, because Real-Time PCR is not so time - consuming, technically easy and need to reagent of a small quantity. It will be useful for rapid and reliable clinical diagnosis of HBV in many countries.

Dosimetric Evaluation of Static and Dynamic Intensity Modulated Radiation Treatment Planning and Delivery (세기조절방사선치료에서 조사방법이 빔 파라미터 및 선량에 미치는 영향에 대한 연구)

  • The two commonly used methods in delivering intensity modulated radiation therapy (IMRT) plan are the dynamic (sliding window) and static (stop and shoot) mode. In this study, the two IMRI delivery techniques are compared by measuring point dose and dose distributions. Using treatment planning system, clinical target volume (CTV) was created as a sphere with various diameter (3 cm, 7 cm, 12 cm). Two IMRT plans were peformed to deliver 200 cGy to the CTV in dynamic and static mode. The two plans were delivered on a phantom and central point dose and dose distributions were measured. The central point dose differences between static and dynamic IMRT delivery were 0.2%, 0.2% and 0.4% when the diameter of CTV was 3 cm, 7 cm, and 12 cm, respectively. The differences In volume receiving 90% of the proscribed dose were 2.7%, 2.2%, and 2.9% for the diameter of CTV was 3 cm, 7 cm, and 12 cm, respectively. For lung cancer patients, the differences in central point dose were 0.2%, 0.2%, and 0.4% when the volume of CTV was 35.5 cc, 296.8 cc, and 903.5 cc, respectively. The differences in volume receiving 90% of the prescribed dose were 2.7%, 4.8%, and 9.1% when the volume of CTV was 35.5 cc, 296.8 cc, and 903.5 cc, respectively. In conclusion, it was possible to deliver IMRT plans using dynamic mode of MLC operation although the loaves are In motion during radiation delivery.

Dosimetric Effects of Intrafractional Organ Motion in Field-in-Field Technique for Whole-Breast Irradiation

  • Purpose: We evaluated the motion-induced dosimetric effects on the field-in-field (FIF) technique for whole-breast irradiation (WBI) using actual patient organ motion data obtained from cine electronic portal imaging device (cine EPID) images during treatment. Materials and Methods: Ten breast cancer patients who received WBI after breast-conserving surgery were selected. The static FIF (SFIF) plan involved the application of two parallel opposing tangential and boost FIFs. To obtain the amplitude of the internal organ motion during treatment, cine EPID images were acquired five times for each patient. The outside contour of the breast (OCB) and chest wall (CW) contour were tracked using in-house motion analysis software. Intrafractional organ motion was analyzed. The dynamic FIF (DFIF) reflecting intrafractional organ motion incorporated into the SFIF plan was calculated and compared with the SFIF in terms of the dose homogeneity index (DHI90/10) for the target and V20 for the ipsilateral lung. Results: The average motion amplitudes along the X and Y directions were 1.84±1.09 mm and 0.69±0.50 mm for OCB and 1.88±1.07 mm and 1.66±1.49 mm for CW, respectively. The maximum motion amplitudes along the X and Y directions were 5.53 and 2.08 mm for OCB and 5.22 and 6.79 mm for CW, respectively. Significant differences in DHI90/10 values were observed between SFIF and DFIF (0.94 vs 0.95, P<0.05) in statistical analysis. The average V20 for the lung in the DFIF was slightly higher than that of the SFIF in statistical analysis (19.21 vs 19.00, P<0.05). Conclusion: Our findings indicate that the FIF technique can form a safe and effective treatment method for WBI. Regular monitoring using cine EPID images can be effective in reducing motion-induced dosimetric errors.

  • The objectives of this Paper is to implement a diagnostic classifier of differential laryngeal diseases from acoustic signals acquired in a noisy room. For this Purpose, the voice signals of the vowel /a/ were collected from Patients in a soundproof chamber and got mixed with noise. Then, the acoustic Parameters were analyzed, and hierarchical neural networks were applied to the data classification. The classifier had a structure of five-step hierarchical neural networks. The first neural network classified the group into normal and benign or malign laryngeal disease cases. The second network classified the group into normal or benign laryngeal disease cases The following network distinguished polyp. nodule. Palsy from the benign laryngeal cases. Glottic cancer cases were discriminated into T1, T2. T3, T4 by the fourth and fifth networks All the neural networks were based on multilayer perceptron model which classified non-linear Patterns effectively and learned by an error back-propagation algorithm. We chose some acoustic Parameters for classification by investigating the distribution of laryngeal diseases and Pilot classification results of those Parameters derived from MDVP. The classifier was tested by using the chosen parameters to find the optimum ones. Then the networks were improved by including such Pre-Processing steps as linear and z-score transformation. Results showed that 90% of T1, 100% of T2-4 were correctly distinguished. On the other hand. 88.23% of vocal Polyps, 100% of normal cases. vocal nodules. and vocal cord Paralysis were classified from the data collected in a noisy room.

Effect of Sparassis crispa Extracts on Immune Cell Activation and Tumor Growth Inhibition (꽃송이버섯(Sparassis crispa) 추출물의 면역세포 활성화 및 항암 효과)

  • Sparassis crispa is an edible mushroom with medicinal properties that contains more than 40% ${\beta}$-glucan. The role of S. crispa in regulating the functional activation of macrophages has yet to be fully elucidated. The objective of this study was to investigate the molecular mechanism underlying the immune-stimulatory function of S. crispa soluble ${\beta}$-glucan and extracts on macrophages. In this study, we showed that S. crispa soluble ${\beta}$-glucan was able to stimulate TNF-${\alpha}$ and IL-$1{\beta}$ production through NF-${\kappa}B$ activation in Raw 264.7 cells. We also showed that S. crispa extracts could not only enhance TNF-${\alpha}$ production in Raw 264.7 cells, but also suppress tumor growth in vivo. All of our results suggest that S. crispa could be developed as a promising immunostimulatory principle, applicable to cancer patients.

The Objective Measurement of the Lung Parenchyma Motion for Planning Target Volume Delineation (폐 부위 Planning Target Volume(PTV)설정시 폐 움직임의 객관적 측정)

  • Purpose : To quantify the movement of lung Parenchyma for ICRU 50 Planning Target Volume (PTV) delineation of the lung region. Materials and Method : Fluoroscopic observations and measurements are Performed on 10 patients with chest region cancer who have normal putmonary functions We have divided the lung region into 12 parts for the right lung, 10 parts for the left lung and four to five Points of lung parenchyma were selected for anatomical analysis Points, Fluoroscopic images are sent to a computer and then movements are measured. Results : Both lowe lobes showed the longest longitudinal movements because of breathing (average 14.1mm, maximum 22.1mm), while anteroposterior displacement showed the smallest value. Lateral movements of the lung parenchyma averaged 6.6mm, and the maximum value was 9.1mm, (both hilar regions showed maximum values because of cardiac motion) Conclusion : We could quantify the lung movements by measuring parenchyma displacements. The movements of both upper lobes were less than those of the middle and upper lobes in longitudinal and transverse movements. Optimal margins can be selected for PTV delineation using these results.

Effects of YideungJetong-Tang on Peripheral Neuropathy Induced by Taxol and Compression Injury in the Rat Sciatic Nerve (이등제통탕(二藤除痛湯)이 Taxol 처리 및 좌골신경의 압박 손상 후 유발된 랫드의 말초신경병증에 미치는 영향)

  • Background: Most antitumor agents have the side effect of chemotherapy-induced peripheral neuropathy (CIPN). Cancer patients who take antitumor agents suffer from CIPN, but there is no known treatment for it. Unlike the central nerve system, the peripheral nerve can self-repair, and the Schwann cell takes this mechanism. Objectives: In this study, we researched the effect of YideungJetong-Tang (YJT) extract on taxol-induced sciatic nerve damage, through in vitro and in vivo experiments. Also, we studied the effect of YJT extract on neurite recovery and anti-inflammatory effect after compression injury of sciatic nerve in vivo. Methods: Vehicle, taxol and taxol+YJT were respectively applied on sciatic nerve cells of rat in vitro, then the cells were cultured. The sciatic nerve cells and Schwann cells were then observed using Neurofilament 200, Hoechst, ${\beta}$ -tubulin, S-$100{\beta}$, caspase-3 and phospho-Erk1/2. CIPN was induced by taxol into the sciatic nerve of rat in vivo, then YJT extract was taken orally. The axons, Schwann cells and neurites of the DRG sensory nerve were then observed using Neurofilament 200, ${\beta}$-tubulin, Hoechst, S-$100{\beta}$, phospho-Erk1/2 and caspase-3. YJT was taken orally after sciatic nerve compression injury, and the changes in axon of the sciatic nerve, Schwann cells and TNF-${\alpha}$ concentration were observed. Results: The taxol and YJT treated group showed significant effects on Schwann cell recovery, neurite growth and recovery. In vivo, YJT compared with control group showed Schwann cell structural improvement and axons recovering effect after taxol-induced Schwann cell damage. After sciatic nerve compression injury, recovery of distal axon, changes of Schwann cell distribution, and anti-inflammatory response were observed in the YJT. Conclusions: Through this study, we found that after taxol-induced neurite damage of sciatic nerve in vivo and in vitro, YJT had significant effects on sciatic nerve growth and Schwann cell structural improvement. In vivo, YJT improved recovery of distal axons and Schwann cells and had an anti-inflammatory effect.
