We propose a new early rehabilitation training system for postural control using a tilting bed and a force plate. The conventional rehabilitation systems for postural control cannot be applied to the patients lying in bed because the rehabilitation training using those systems is possible only when the patient can stand up by himself or herself. Moreover, there has not existed any device that could provide the sense of balance or the sensation of walking to the patients in bed. By using a tilting bed, a visual display, and a force plate, we have developed a new rehabilitation training system for balance control of the patients in bed providing sense of balance and the sensation of walking to the patient. Through the experiments with real people, we verified the effectiveness of the new early rehabilitation training system. The results showed that this system is an effective system for the early rehabilitation training and that our system might be useful as clinical equipment.
This study aimed to provide basic data for 3D printing in the medical health field by developing upper wear fixation device (UWFD), an auxiliary device for shortening chest AP examination time on emergency room beds and non-contact with patients. The standard of hooks was modeled according to the bed frame using the Autodesk Fusion 360. It was printed with Form2 (Formlabs, Somerville, MA, USA), as SLA (stereo lithography apparatus) method, and was washed and hardened using Form Wash and Form Cure. The completed UWFD conducted an online survey on 4 items of stability, convenience, availability, preference and general characteristics. The total stability average was 3.93±0.80, the total convenience average was 3.93±0.68, the total availability average was 4.01±0.89, and the total preference average was 3.80±1.08. This study was significant in suggesting improvements in the general X-ray examination process in the emergency room by designing and making aids to easily fixing the patient's top to the frame of the emergency bed while meeting promptness and non-contact with the patient.
본 논문에서는 다수의 체압 센서가 내장된 의료용 침대의 원격 케어에 관해 기술한다. 낙상은 환자의 안전을 심각하게 위협하고 건강을 해치는 요인중의 하나이다. 본 연구에서는 이를 극복하기 위해 새로운 침대가 개발하였다. 침대 시스템은 동작이 가능한 건반, 건반의 동작을 관리하는 건반 제어기, 체압을 측정하는 센서, 센서의 값을 송수신하는 센서 제어기, 이를 확인하여 알고리즘에 따라 자동 혹은 수동으로 동작하게 하는 메인 제어기, 이 모든 정보를 총괄하는 서버로 이루어져 있다. 침대 시스템은 센서를 통해 환자의 위치를 확인하여 환자가 낙상의 위험이 있다고 판단하게 되면 메인 제어기를 통해 무선으로 서버에 경고를 보내 간호인 혹은 간호사가 환자의 위험한 상태를 인지할 수 있도록 한다. 서버는 유무선 단말에서 송신된 상태 데이터를 전송받아 침대 시스템이 정상적으로 동작하고 있는지 모니터링할 수 있다. 건반의 제어기는 건반형 메커니즘을 구동하고 체압 센서가 연결된 욕창 예방 제어를 자동으로 행하여 환자의 압력이 가해지는 부위를 물리적으로 분리해 욕창을 예방한다. 메인 제어기는 환자의 침상 존재를 확인하여 서버에 전송한다. 결론적으로 제안된 시스템은 사용자의 상태를 스마트 모니터링하고 원격 케어를 행할 수 있게 된다.
Purpose: This study examines areal composition of ward applying to 4 bedroom in provincial medical center. Methods: The existing five-bed patient rooms, general type of multiple-bed rooms in Korea causes many problems for the amenity of patients. We should reconsider their inconvenience carefully and try to provide the right to keep their privacy and enjoy amenity. The number of patients of multi-bed rooms is very critical point to improve the environmental condition of the patient rooms. This study separate 5 bedroom group and 4 bedroom group. Net area from space program was surveyed and analyzed. Results: The result of this study can be summarized into two points. The first one is that Group-4(4 bedroom) ward has more 23% patient's area and -23% convenience area than Group-5(5 bedroom). The second one is that Group-4 has more single bedroom and Group-5 has more dayroom. Implications: Consideration Should be taken into account for the effective bedroom composition and allocation in ward. This Study hopefully may serve as a stepping stone for the standard design of space program in ward.
Purpose: This study was done to develop a postural-stability patient transfer technique for care helpers in nursing homes and to evaluate its effectiveness. Methods: Four types of patient transfer techniques (Lifting towards the head board of the bed, turning to the lateral position, sitting upright on the bed, transferring from wheel chair to bed) were practiced in accordance with the following three methods; Care helpers habitually used transfer methods (Method 1), patient transfer methods according to care helper standard textbooks (Method 2), and a method developed by the author ensuring postural-stability (Method 3). The care helpers' muscle activity and four joint angles were measured. The collected data were analyzed using the program SPSS Statistic 21.0. To differentiate the muscle activity and joint angle, the Friedman test was executed and the post-hoc analysis was conducted using the Wilcoxon Signed Rank test. Results: Muscle activity was significantly lower during Method 3 compared to Methods 1 and 2. In addition, the joint angle was significantly lower for the knee and shoulder joint angle while performing Method 3 compared to Methods 1 and 2. Discussion: Findings indicate that using postural-stability patient transfer techniques can contribute to the prevention of musculoskeletal disease which care helpers suffer from due to physically demanding patient care in nursing homes.
In this paper, an Intelligent Bed Robot System (IBRS) is proposed, that is a special bed equipped with robot manipulator. To assist a patient using IBRS, pose and motion estimation process is fundamental. It is designed to help the elderly and the disabled for their independent life in bed without other assistants. For this purpose, we use the pressure sensor distributed mattress for detecting the change of motion on the bed. Using that data, we control the robot arm to move to the appropriate position and serve to the user. In addition, we can estimate the user's intention based on the change of pressure and use those data to control the robot arm guide.
본 연구는 가정용 침대 위에서 발생한 심정지 환자에게 가슴압박을 수행할 경우 환자의 체중과 구조자의 체중에 따른 가슴압박 정확도의 차이를 확인하고자 하였다. 2023년 01월 07일부터 01월 19일까지 대한심폐소생협회의 KBLS 교육과정을 이수한 N대학교와 S대학교의 응급구조(학)과 학생 36명을 대상으로 대상자의 무릎 위치에서 매트리스가 눌리는 깊이, 마네킹의 무게에 의해 매트리스가 눌리는 깊이를 측정하였고, 2분간 연속된 가슴압박을 수행하는 동안 가슴압박 깊이, 속도, 압박 위치 불량, 가슴압박 깊이 50 mm 미만 횟수, 모든 변수를 고려한 정확도를 측정하였다. 연구결과 가정에서 소아 심정지를 발견한 경우 환자를 바닥으로 이동시켜 가슴압박을 진행하고, 성인 심정지를 발견한 최초목격자는 환자를 무리하게 침대 밑으로 내려 가슴압박을 진행하기보다는 침대 위에서 진행하는 것을 권장한다.
핵의학은 방사성 동위원소 추적자를 인체에 투여하여 질병의 형태학적 정보와 생물학적 기능 정보를 얻고 평가한 Amyloid PET Tracer 개발로 베타아밀로이드 신경반 확인을 통해서 알츠하이머 진단의 정확도를 높일 수 있다. 그러나 20분이라는 긴 영상획득 시간은 환자에게 힘든 시간 일 수 있다. PET/CT 검사는 환자의 움직임에 민감하며, 검사 결과에도 일부분 영향을 미칠 수 있다. 이에 본 연구에서는 체내 방사성 의약품의 분포를 시간에 따른 list mode acquisition 방법을 통해 영상의 정량적 평가에 영향을 미치지 않으면서 적절한 영상획득 시간 연구하였다. list mode는 기존의 frame mode에 비해 시간에 대한 정보가 포함되어 연구자들이 원하는 시간에 대한 영상을 별도로 reconstruction 할 수 있어 data 분석에 용이하다. 연구 방법은 5 min frame/bed, 10 min frame/bed, 15 min frame/bed, 20 min frame/bed 로 리스트 모드를 이용하여 시간별 재구성 영상을 획득하여 SNR(signal to noise ratio) 과 LPR(lesion-to-pons uptake ratio)의 차이와 판독의 차이를 비교 분석하여 적정한 영상획득시간을 알아보고자 하였다. 정량적 분석 결과 list mode로 측정 시 정량적 평가 결과, 영상획득 시간이 증가함에 따라 관심 영역 6개에서 SUVmean 값은 감소하였으나 5 min/bed 영상에서 가장 많은 차이를 보였고 10 min/bed 그리고 15 min/bed 순으로 SUVmean 값의 차이가 감소하였다. 따라서 15 min/bed 에서의 SUVmean 값은 영상의 평가에 영향을 끼치지 않을 만큼의 차이가 없다는 결과값이 나왔다. LPR 값의 차이는 없었다. 정성적 분석 결과, PET 영상획득시간에 따른 판독 소견의 변화는 없었으며 시간에 따른 영상 재구성의 정성적 분석 점수의 유의한 차이가 없었다. 연구 결과 F-18 fluteme tamol PET/CT 검사 시 15 min/bed 과 20 min/bed 영상이 유의한 차이가 없으므로 환자의 상태에 따라 LIST MODE를 통해 선택적으로 15 min/bed 을 사용하여 영상획득 시간을 줄이는 것이 임상적으로 유용하다고 할 수 있다.
As we become an aging society, the number of elderly patients continues to increase. Pressure sores that can easily occur in patients with trauma cause serious socio-economic problems. In general, prevention of bedsores through predicting the patient's posture is being developed. Developed method usually use artificial intelligence techniques to estimate the patient's posture by measured pressure images in the mattress. In this method, it has a problem the reduction of estimation accuracy when posture of patient is changed. Therefore, it is necessary to use the filter of pressure images in the position transition of patient. In this paper, we propose an algorithm to predict the patient's posture, and an algorithm to reduce the ambiguity that can occur in the patient's posture transition section. By obtaining stable data through this algorithm, learning/prediction stability of the neural network can be expected, and prediction performance is improved accordingly. Through experiments, the effectiveness of the algorithm was verified.
Most of the vascular procedures performed for various diagnoses and treatments of various abdominal intervention procedures performed by the Department of Radiology and Angiography are performed by puncture of the femoral artery. For this reason, patients should undergo blood-related tests such as prothrombin time (PT) and partial thromboplatin time (PTT). Therefore, many patients are instructed to take precautions such as putting a sandbag on the puncture site to prevent delayed hemorrhage after hemostasis of the femoral artery puncture site, and not to bend the leg of the treated area for about 3 hours. Because of this, many patients have complained of pain during the procedure and inconvenience during the absolute bed rest time in the ward. The purpose of this study was to compare the safety of balloon ancillary devices with sandbags placed on the hemostasis site to prevent delayed hemorrhage after arterial puncture. We compared the safety of each patient with the results of medical records in consideration of the problem that the patient could not press with the focus, the position of the patient was changed depending on the patient's body shape, and the problem of falling down according to the location of the puncture site. As a result, the use of a balloon type ancillary device improves the effect of continuous hemostasis, reduces discomfort during the patient's absolute stabilization time, increases the patient's satisfaction, and is a good alternative to the existing sandbag.
