의학이 발달하여 치료의 질이 높아지더라도 고객만족을 위한 의료서비스의 질이 높아지지 않으면, 더 이상 고객의 만족도가 올라가지 않는다. 이는 병원의 재이용 또한 감소하여 경쟁력이 둔화된다. 의료정보화의 초점이 의료진에서 고객에게 옮겨간 이유라고 할 수 있다. 이에 본 연구는 의료서비스 중 정보 접점인 웹기반 정보제공이 증가하고 있는 부산에 소재한 개인병원 웹사이트를 분석하였다. 통계처리는 웹상에서 분석가능한 변인들로 SPSS for 21.0 Version을 사용하여 교차분석(cross analysis)을 실시하였다. 이 연구의 분석결과로 연구문제로 제시된 의료서비스 외적 접점요소와 디자인 접점요소에 따라 진료과목별로 유의미함을 보였다. 이는 진료과목별 접점이 다르다는 의미이다. 본 연구결과가 웹사이트 제작 시 진료과목별 특성을 잘 고려하여 의료서비스 질과 경쟁력을 높여 환자의 경험 만족도를 높이기를 기대한다.
The aim of this research is to study and develop enabling technologies for home healthcare device with ubiquitous network. The motivation of this paper is to enable healthcare in home, to development the device for smart home health care. To achieve the aim, we must develop the prototype platform based on home gateways, distributed context user interface based on UPnP and support for information sharing with high speed power line communication and mobile infra-structures. And IPv6 is the base technology of this platform. In this paper, we concern that physical health, mental health and medical emergencies is all of home healthcare. With the smart device, we evaluate the connectivity, automatic information extraction and private data exchange and event driven message. The result of this paper is demonstration of smart device for ubiquitous communication in a healthcare application such as patient monitoring device and several information services. In conclusion, home healthcare will support more healthy and easy living for a human.
본 논문은 SAW태그를 이용하여 환자의 위치를 파악하고, 바이오 센서를 활용하여 재택환자의 상태를 집안의 사람들에게 쉽게 전달하며, 위급상황에 대처가능한 시스템의 개발에 관한 것이다. 일반적으로 집안에서 환자가 가장 많이 다치는 장소는 화장실이고, 개인의 프라이버시가 지켜져야 하는 점과 환자의 상태를 확인하고 위급시 대처해야 하는 부분이 첨예하게 대립하는 지점이므로, 개인의 프라이버시를 지키면서 재택환자의 상태를 모니터링가능한 방법을 고안하였고, 관련 시스템을 제안한다. 또한 SAW 태그를 이용하여 사용자의 위치도 확인가능한 위치 확인 시스템도 연동할 수 있도록 제안하였다. SAW태그는 송신기에서 오는 전파를 그대로 전송하며 자신의 고유 아이디를 전달할 수 있어 3차원 위치 측정에 유용하고, 이를 사용자의 의복에 부착할 경우 입체적인 위치를 측정할 수 있어 생체신호와 같이 정보를 전달할 경우 요양원 같이 많은 환자를 관리하는 종합적인 환자관리 시스템으로 활용이 가능한 장점이 있다. 본 연구에서는 상기의 상태 및 위치 측정 시스템을 구현하여 정상적으로 구동되는 것을 확인하였다.

Purpose: This study presents the process of designing workbooks for advance care planning appropriate for the Korean cultural setting and describes actual case studies. Methods: This study focused on single inductive case studies of the utilization of an advance care planning workbook and recruited individual participants. Results: The workbook for adolescents contained six sessions and the workbook for children contained seven sessions. The workbook sessions led to four major discoveries: 1) considering the Korean cultural context, discussions on life and death must be held indirectly; 2) the role of the counselor as a supporter is crucial for the workbook to be effective; 3) the workbook must be accessible regardless of the seriousness of the illness; and 4) patients must be able to make their own choice between the workbook versions for children and adolescents. Six facilitating factors improved engagement: 1) the role of the counselor as a supporter; 2) building trust with the patient; 3) affirming freedom of expression on topics the patient wished to avoid talking about; 4) having discussions on what private information to keep secret and to whom the information can be disclosed; 5) discovering and regularly discussing relevant topics; and 6) regular communication and information-sharing with the patient's medical service providers. Conclusion: It is necessary to build on actual case studies regarding workbooks for children and adolescents in order to expand the usage of these workbooks to all relevant medical institutions in Korea.
본 논문에서는 DNA 생체정보와 소유자 기반의 hardware RFID(Radio Frequency Identification) 스마트카드, 그리고 Software 인증 분야인 PKI 전자서명을 도입한 다중 사용자 인증시스템을 제시한다. 이는 현 시스템의 인가딘 자의 접근 방법인 ID or password 가 안전한 방법이 아니므로 논문[1] 에서 제안한 사항을 다음과 같이 개선하였다. 즉, 사용자 인증 카드인 two card(the biometric registered seal card and the DNA persional ID card) 대신 하나의 RFID 스마트카드로 사용자 인증을 할 수 있고, 카드 분실시 사용자 정보 노출의 위험을 저가의 RFID로 해결한다. 또한 DNA personal ID 민으로 일난성 쌍둥이, 수혈한 환다, 암세포에서 돌연변이가 발생한 경우의 사용자 인증이 어려운 경우까지 사용자 ID 에 대응하는 일회용 의사난수와 DNA 정보로 사용자 인증을 해결하였다. 그러므로 현 생체 정보 사용자 인증시스템의 단점인 패턴 매칭과 패턴 비교의 에러르 정확한 digital DNA 생체정보로 안전하게 스마트카드에 저장하여 터미널에 로그온하는 local applications에 적용할 수 있다. 스미트카드내 RFID는 사용자를 판독, 추적, 관리할 수 있으므로 카드 분실시 카드 위치를 추적하고 개인 정보를 관리할 수 있으며, 어떠한 개인 DNA 정보도 노출되지 않는다. 현 PKI 전자서명의 비밀키 안전성을 해결한다. 뿐만 아니라 이러한 시스템은 생체정보의 RFID 스마트카드 사용 확대 계기로, 신용카드, 신분증, 그리고 여권 등에서도 이용할 수 있다. 제시한 시스템의 안전성을 통계학적 분석으로 보여진다.
4차 산업혁명시대로 접어들면서 많은 기술들의 발전이 이루어지고 있으며 다양한 위협요소들이 생겨나고 있다. 이러한 위협요소에 대응하기 위한 연구가 많은 분야에서 이루어지고 있다. 다양한 분야의 발전중에서도 의료기술과 지능형 자동차의 발전으로 인한 위협요소는 의료에 대한 잘못된 정보로 인한 생명에 대한 위협과 지능형 자동차를 통한 사람의 안전한 운행을 방해하여 생명을 위협하는 요소들이 가장 큰 위협요소로 대두되고 있다. 본 논문에서는 환자의 정보가 중요한 만큼 환자의 의료 기록에 대한 안전성과 신뢰성이 있는 기술을 위하여 블록체인의 기술 종류 중 프라이빗 블록체인을 사용하여 환자의 의료 기록에 대한 안전성과 효율성, 확장성을 높이는 방법과 자동차 시스템을 해킹하여 운전자의 생명을 위협하고 개인정보 및 위차파악으로 사생활 문제점에 대한 해결과 사물인터넷에서의 위변조를 방지하기 위하여 블록체인 기술을 이용한 정보보호 기법을 제안한다.

Background: Many patients use online resources to educate themselves on surgical procedures and make well-informed healthcare decisions. The aim of our study was to evaluate the quality and readability of online resources exploring shoulder arthroplasty. Methods: An internet search pertaining to shoulder arthroplasty (partial, anatomic, and reverse) was conducted using the three most popular online search engines. The top 25 results generated from each term in each search engine were included. Webpages were excluded if they were duplicates, advertised by search engines, subpages of other pages, required payments or subscription, or were irrelevant to our scope. Webpages were classified into different source categories. Quality of information was assessed by HONcode certification, Journal of the American Medical Association (JAMA) criteria, and DISCERN benchmark criteria. Webpage readability was assessed using the Flesch reading ease score (FRES). Results: Our final dataset included 125 web pages. Academic sources were the most common with 45 web pages (36.0%) followed by physician/private practice with 39 web pages (31.2%). The mean JAMA and DISCERN scores for all web pages were 1.96±1.31 and 51.4±10.7, respectively. The total mean FRES score was 44.0±11.0. Only nine web pages (7.2%) were HONcode certified. Websites specified for healthcare professionals had the highest JAMA and DISCERN scores with means of 2.92±0.90 and 57.96±8.91, respectively (P<0.001). HONcode-certified webpages had higher quality and readability scores than other web pages. Conclusions: Web-based patient resources for shoulder arthroplasty information did not show high-quality scores and easy readability. When presenting medical information, sources should maintain a balance between readability and quality and should seek HONcode certification as it helps establish the reliability and accessibility of the presented information. Objectives: This study aimed to determine the prevalence of odontogenic cysts, tumors, and other lesions among reports in the archives of the Department of Oral and Maxillofacial Surgery at the Faculty of Dentistry affiliated with Kocaeli University collected over a four-year period. Materials and Methods: In this retrospective study, patient records from the archive of the Department of Oral and Maxillofacial Surgery from 2014 to 2018 were reviewed. Patient demographic information (age and sex) and lesion location were recorded and analyzed. Results: From a total of 475 files reviewed, odontogenic cyst was confirmed in 340 cases (71.6%), and odontogenic tumor was confirmed in 52 cases (10.9%). Regarding odontogenic cyst type, the most common was radicular cyst (216 cases), followed by dentigerous cyst (77 cases) and odontogenic keratocyst (23 cases). Among odontogenic tumors, the most frequent was odontoma (19 cases), followed by ossifying fibroma (18 cases) and ameloblastoma (9 cases). Giant cell granuloma was also reported in 35 cases. Conclusion: The distribution pattern of odontogenic cysts and tumors in our retrospective study is relatively similar to that reported in the literature. Complete clinical reports for final diagnosis of these lesions and routine follow-up examinations are very important for treatment.
Purpose: The purpose of this study was to examine the influencing factors of the patient leaning phenomenon in tertiary hospitals. Based on the results of this study, we intended to find implications for improving the problems of the delivery system imbalance in tertiary hospitals caused by patient leaning phenomenon. Methodology/Approach: Qualitative studies were conducted, using focus group interviews and in-depth interviews. The focus group interviews were conducted for 12 users of tertiary hospitals by 2 groups. And in-depth interviews were conducted for 6 tertiary hospital managers. This was considered to be the most effective approach to gather diverse and in-depth information about the influencing factor of patient leaning phenomenon in tertiary hospitals. Findings: In focus group interviews, the reason for choosing tertiary hospitals was the reliability of the hospital(physician, reputation, etc.). And the effect of the policy to strengthen coverage of National Health Insurance and private medical insurance was relatively small. In other words, we found that the individual's desire to receive medical services suitable for one's health status and disease condition was the biggest factor, rather than the cost and policy factors. Practical Implications: We suggested that the appropriate medical care provision should be strengthened according to the role and function of medical institutions. In addition, the education system needs to be reorganized to activate the referral program, expand community medical capabilities, and foster quality primary medical care.
최근 고령화로 인한 치매환자의 증가로 그들의 배회행동과 실종예방을 위한 대책이 시급하다. 이러한 문제를 해결하기 위해 다양한 인증 방법과 위치 탐지 기법들이 소개되고 있으나 개인인증의 보안성 문제와 실내·외를 전반적으로 확인할 수 있는 시스템은 찾아보기 어려웠다. 본 연구에서는 프라이빗 블록체인 환경에서 손목 밴드 형태의 웨어러블 디바이스를 활용해 개인인증, 기본적 건강 상태 파악 및 실내·외의 전반적인 위치를 파악할 수 있는 시스템을 제안하고자 한다. 이 시스템에서 개인인증은 위변조가 어렵고 개인식별성이 높은 ECG를, 실내는 저전력, 비접촉 및 자동 송수신 방식으로 사용이 용이한 블루투스 비콘을, 실외는 GPS 위성의 의사거리 오차를 보정한 DGPS를 활용하여 치매환자의 위치를 파악함으로써 배회행동 및 이상징후를 탐지하고자 한다. 이를 통해 재가나 요양시설 등에서 생활하는 치매환자의 배회행동 및 이상징후 시 신속한 대처와 실종예방에 기여하고자 한다.
