  • Objective: The purpose of this study was to evaluate the efficacy of frozen mixed double-embryo transfer (MDET; the simultaneous transfer of day 3 and day 5 embryos) in comparison with frozen blastocyst double-embryo transfer (BDET; transfer of two day 5 blastocysts) in patients with repeated implantation failure (RIF). Methods: A total of 104 women with RIF who underwent frozen MDET (n = 48) or BDET (n = 56) with excellent-quality embryos were included in this retrospective analysis. All frozen embryo transfers were performed in natural cycles. The main outcome measures were the implantation rate, clinical pregnancy rate, multiple pregnancy rate, and miscarriage rate. These measures were compared between the patients who underwent MDET or BDET using the chi-square test or the Fisher exact test, as appropriate. Results: The implantation and clinical pregnancy rates were significantly higher in patients who underwent MDET than in those who underwent BDET (60.4% vs. 39.3%, p=0.03 and 52.1% vs. 30.4%, p=0.05, respectively). A significantly lower miscarriage rate was observed in the MDET group (6.9% vs. 10.7%, p=0.05). In addition, the multiple pregnancy rate was slightly, but not significantly, higher in the MDET group (27.1% vs. 25.0%). Conclusion: MDET was found to be significantly superior to double blastocyst transfer. It could be regarded as an appropriate approach to improve in vitro fertilization success rates in RIF patients.

  • A splanchic artery occlusion for 90 min followed by reperfusion of the mesenteric circulation resulted in a severe form of circulatory shock characterized by endothelial dysfunction, severe hypotension, marked intestinal tissue injury, and a high mortality rate. The effect of rutin, a flavonoid having antiprostanoid, anti-inflammatory, antithrombotic, antioxidant effect, were investigated in a model of splanchnic artery occlusion (SAO) shock in urethane anesthetized rats. Occlusion of the superior mesenteric artery for 90 min produced a severe shock state resulted in a fatal outcome within 120 min of reperfusion in many rats. Rutin was given as a bolus (1.28 mg/kg) 10 min prior to reperfusion. Administration of rutin significantly improved mean arterial blood pressure in comparison to vehicle treated rats (p<0.05). Rutin treatment also resulted in a significant attenuation in the increase in plasma amino nitrogen concentration, intestinal myeloperoxidase activity, intestinal lipid peroxidation, infiltration of neutrophils in intestine and thrombin induced adherence of neutrophils to superior mesentric artery segments. These results suggest that rutin provides beneficial effects in part by preserving endothelial function and attenuating neutrophil accumulation in the ischemic reperfused splanchnic circulation.

  • Background: Few studies have reported the results of arthroscopic coracoplasty concomitantly conducted with subscapularis tear. Therefore, this study was conducted to examine and compare the outcomes of arthroscopic subscapularis repair after arthroscopic coracoplasty using either the subacromial approach or rotator interval approach. Methods: We retrospectively reviewed 51 patients who underwent coracoplasty with subscapularis repair. The patients were grouped according to whether the subacromial approach group (24 patients) or rotator interval approach group (27 patients) was used during coracoplasty. Preoperative and postoperative visual analogue scale scores, American shoulder and elbow surgeons scores, Korean shoulder scores, and range of motion (ROM) were assessed. Assessment of repaired rotator cuff tendon integrity was performed at 1 year after surgery using either magnetic resonance imaging or ultrasonography. Results: At final follow-up, overall functional scores and ROM improved significantly in both groups when compared with preoperative values (p>0.05). The re-tear rates were not significantly different between groups; however, the rotator interval approach group showed a significant increase in ROM compared with that in the subacromial approach group (p<0.05). Conclusions: Arthroscopic coracoplasty conducted concomitantly with subscapularis repair can provide a satisfactory outcome. There were no significant differences between the two approach groups regarding final functional scores and re-tear rates. However, the rotator interval approach group showed a greater increase in ROM at final follow-up, especially in external rotation.

  • PURPOSE: The purpose of this study is to compare the effects of aerobic exercise and combination exercise on physical fitness, body composition, and depression and to observe the relationship between obesity and depression. METHODS: A total of 40 obese people were divided into an aerobic exercise group ($n_1$=20) and a combination exercise group ($n_2$=20) that was performed swiss ball exercise and aerobic exercise. Each group exercised three times per week for eight weeks. Outcome measures were basic physical fitness, body composition, and depression. RESULTS: After completion of the exercise, there was a significant difference in overall physical fitness factors of percentage body fat (%body fat), strength, muscle endurance, flexibility, cardiovascular endurance in the combination exercise group (p<.05). Strength, muscle endurance, flexibility, and cardiovascular endurance were more improved in the combination exercise group than aerobic exercise group (p<.05). However, changes of %body fat and psychological factor between the two groups were not significant. CONCLUSION: Combination exercise program has an effect on the solution of obesity in physical development and strength. Therefore, it seems to that use with better perform both aerobic exercise and Swiss ball exercise. In addition, the Swiss ball exercise, which is an easy, convenient and economical self-exercise that can be done at home, is recommended due to its positive effects on physical establishment and fitness.

  • Although liposarcoma is the second most common soft tissue sarcoma in adults, the incidence of liposarcoma in the head and neck is low. The histologic nature of liposarcoma is correlated clinically with treatment outcome, but histologic classification of liposarcoma is controversial. Well-differentiated liposarcoma and atypical lipoma are pathologic synonyms because they are identical lesions both morphologically and karyotypically. They represent the lowest grade lesions in the spectrum of liposarcoma. The terms "atypical lipoma" were introduced specifically to describe well-differentiated liposarcomas occurring in the extremities, because of their better prognosis in comparison with their retroperitoneal counterparts. We present 1 case of atypical lipomatous tumor of the neck.

  • This study empirically analyzed the effects of the Top Management Team (TMT) on organizational performance. We verified whether the age heterogeneity, job heterogeneity (core career, core function and major), and process (communication and integration) of the TMT affect organizational performance (management performance and healthcare service quality evaluation level). We collected data about 473 members of the 2006 TMT in 81 medical institutions. We also utilized statistics of organizational performance from the Ministry for Health, Welfare and Family Affairs and the Korean Institute of Hospital Management. Results of the study showed that the age heterogeneity of TMT exerted a negative effect on the healthcare service quality evaluation level, while the process exerted a positive effect. However, the age heterogeneity, job heterogeneity, and process had no influence on management performance. We discussed the implications of such outcome of the investigation in comparison with the former studies on TMT and organizational performance, and presented its restrictions and future plans.

  • The present paper study investigates the relationship between social exclusion and donation intention among specific social groups in Korea. Social exclusion refers to non-participation in social experiences by the socially disadvantaged. Data were analyzed using two sources; first was the evidence of behaviors arising from social exclusion of the university students and then socially excluded reactions of the elderly responses from the survey were compared with the first research findings. The reason of using multi-sources of data is that the outcome from the experimental design of the university student is imperative to clarify what the conclusions will be the same result with the other demographic characteristic of the elderly. The research design was three excluded elderly individuals of a self-excluded group and two other excluded groups divided such as "ignored" and "rejected" individuals to compare the differences among three groups of different sources of exclusion. The conclusion of this study is that those with high social exclusion exhibited a more negative donation intention than those with lower social exclusion, but that those who perceived themselves as self-excluded were more likely to give donations than those excluded by others, regardless of the level of their social exclusion.
