An anatomical study on the aortic sinus in swine (돼지 대동맥동에 대한 해부학적 연구)

  • This study was aimed to obtain the anatomical information on the location of ostia of left and right coronary artery in 3 weeks old and 6 months old hybrid swine. The each intercommissural distance of 6 months group was twice than 3 weeks old group. The largest sinus was right aortic sinus followed by left and posterior sinus. All left coronary artery ostia in left aortic sinus were located near the right aortic sinus as well as lower than the ostia of right one. Most of the right coronary artery ostia were located at the level of supravalvular ridge of right aortic sinus. In addition the right ostia had more variation than left ones. Comparing to the sites of 3 weeks old pigs, the sites of the right ostia in 6 months group were more variable. These data suggest that the locations of coronary ostia were different with the sites of human's, and changes of the location may be occurred during the growth.

  • Background: Conservative treatment is performed for isolated anterior wall of the maxillary sinus fractures, in many cases when the fracture is clinically not severe and asymptomatic. Despite the absence of symptoms, complications such as sinusitis, rhinitis, and chronic purulent secretion may develop; therefore, successful reduction is required. We attempted to reduce the risk of complications using an alternative technique: reduction of the fracture with two urinary balloon catheters inserted through the maxillary ostium and fixation using fibrin glue, which minimizes the damage to the bony fragments and sinus mucosa. Methods: In this study, 38 patients who were diagnosed with an isolated anterior wall of the maxillary sinus fracture at our hospital between January 2014 and January 2017 were enrolled. The fracture site was exposed via the Caldwell-Luc approach followed by reduction through the insertion of two urinary balloon catheters using a nasal endoscope and fixation with fibrin glue. The sex, cause of fracture, physical examination, and presence of complications were examined and patient's medical records and facial bone computed tomography scans were analyzed. Results: Radiological evaluation showed that there was no evidence of collapsed reduction fragments. Although some patients had remaining symptoms of hypoesthesia (15%; 3 patients), there were no complications such as infection, rhinitis, sinusitis, and chronic purulent secretion at the surgical site. Conclusion: In this study, we present an alternative surgical technique using two urinary balloon catheters and fibrin glue for the successful reconstruction of an isolated anterior wall of the maxillary sinus fracture. This technique enables precise restoration with a reduced risk of complications.

  • The coronary sinus (CS) is the venous drainage system of the heart. CS ostium atresia is a rarely seen cardiac malformation. Congenital atresia of the CS is usually found together with persistent left superior vena cava (LSVC) and other cardiac malformations. However, isolated congenital atresia of the CS is very rare. We present a rare case of isolated congenital atresia of the CS connecting the left atrium and coronary veins without persistent LSVC in a 58-year-old female.

Studies on Development of Reticulum and Abomasum with Age, and Their Relationship in Korean Native Young Goats (한국 재래유산양(幼山羊)의 제2위와 제4위의 일령 별 발달과 상호관계에 관한 연구)

  • This experiment was carried out to investigate the relationship of morphological characteristics between reticulum and abomasum of Korean native young goats age from 2 days to 150 days. Number of traits investigated in the reticulum in this experiment were 12[body weight, chest girth, body length, right and left reticulum length(R.L.), upper and lower reticulum length(U.L.), reticulum weight(R.W.), reticulum area(R.A.), upper and lower length of one polygon located at central part of reticulum(U.P.C.R.), right and left length of one polygon located at central part of reticulum(R.P.C.R.), thickness of polygon wall located at central part of reticulum(T.P.C.R.), thickness of polygon wall located at middle part of reticulum(T.P.M.R.), and thickness of polygon wall locared at edge part of reticulum(T.P.E.R.)] and items for abomasum were 12[length of between ostium omaso-abomasicum part and pylosica part in the abomasum(L.B.O.P.), broadest outer part of the abomasum(B.O.A.), weight of abomasum(W.O.A.), area of abomasum(A.O.A.), number of plicae abomasi in the abomasum(N.P.A.A.), thickness of abomasum well at cranial part(ostium omasoabomasicum) in the abomasum(T.A.C.A.), thickness of abomasum well at central part in the abomasum(T.A.P.A.), thickness of abomasum wall at light upper area of pylosica part in the abomasum(T.A.L.A.), length measured from the longest plica abomasi in the abomasum(L.L.P.A.), broadest measured from the longest plica abomasi in the abomasum(B.L.P.A.), area measured from the longest plica abomasi in the abomasum(A.L.P.A.), weight of longest plica abomasi in the abomasum(W.L.P.A.)]. The results were summarized as follows: 1. Number of coefficient of correlation obtained among 12 traits of the abomasum and 12 of the reticulum were 144, and coefficient of correlation of 114 were significant(P〈0.05). 2. Trait of abomasum weight have high correlation with 12 traits of reticulum. 3. Correlation coefficients and regression equation between body weight. VS. abomasum weight(r$_1$), and upper and lower length of one polygon located at central part of reticulum(U.P.C.R.) VS. abomasum weight(r$_2$) were r$_1$=0.8954$^{**}$ and Y=10.703+3.374X, r$_2$=0.8430$^{**}$ and Y=5.689+4.311X, respectively. 4. Correlation coefficients and regression equation between chest girth VS. abomasum weight(r$_1$), and body weight VS. abomasum weight(r$_2$) were r$_1$=0.8708$^{**}$ and Y=-17.219+1.227X, r$_2$=0.8589$^{**}$ and Y=- 17.616+1.290X, respectively.

Surgical Angioplasty of Left Main and Proximal Left Anterior Descending Coronary Artery (좌주관상동맥및 좌전하챙지기시부의 수술적 혈관 성형술)

  • Surgical angioplasty of isolated stenosis of the left main coronary artery(LMCA) restores a more physiologic flow to the myocardium, allows percutdneous transluminal coronary angioplasty (PTCA) of distal coronary stenoses at a later stage, and is a less time consuming and convenient procedure than the conventional coronary artery bypass grafting(CABG) . Between Jul. 1994 and Dec. 1995, 7 surgical angioplasty had been performed. LMCA stenoses involved ostium in 2 patients, middle third in 3, and dis- tal third in 2. In 2 patients, the origin of left anterior descending coronary artery was involved in conjunction with LMCA. T e additional coronary artery stenoses were found in 2 cases. One patient was emergently operated after coronary angiography following his cardiac arrest. LMCA was approached anteriorly in all patients. The pulmonary artery was transected in 3 patients for a better exposure. The onlay patch consisted or autologous or bovine pericardium. There was no postoperative myocardial infarction or mortality. Left ventricular functions were well preserved in all patients. Postoperative coronary angiography revealed widely patent LMCA in 5 cases, and mild narrowing of distal anastomotic sites in 2 cases. Provided that well defined indications are followed correctly, surgical angioplasty can be a safe alternative to conventional CABG.

Closure of secundum atrial septal defect: comparison between percutaneous and surgical occlusion (이차공 심방 중격 결손의 폐쇄: 경피적 방법과 수술적 방법의 비교)

  • Purpose : This study was performed to compare the safety, efficacy and clinical results of the Amplatzer septal occluder (ASO) for closure of secundum atrial septal defect (ASD) with surgery. Methods : One hundred fifteen patients diagnosed as isolated secundum ASD in Gil Medical Center, Gachon University of Medicine from January 2000 to July 2006 were included. Seventy patients underwent surgical repair of ostium secundum ASD. Forty-five consecutive patients were treated with percutaneous closure using ASO. We compared the mortality, morbidity, hospital stay, and efficacy between two groups. Results : Male to female ratio was 1:2.4. The mean age and the size of defects were not statistically different. No mortality occurred in either group. The success rate was 97.8% in the device group and 100% in the surgical group. The overall rate of complications was higher in the surgical group than in the device group (64.0 vs. 15.6%, P<0.05). Hospital stay was shorter in the device group than in the surgical group ($4.2{\pm}1.2$ vs. $12.4{\pm}4.7days$, P<0.0001). Residual shunt rates were more frequent in the device group (8.9%) than in the surgical group (4.3%) at discharge. All residual shunts disappeared at 3 months follow-up. Conclusion : Percutaneous closure of ASD using ASO is a safe and effective alternative to surgical repair. The indications of percutaneous ASD closure with ASO would be expanded by accumulation of experiences and evolutions of device.

The Effect of Acute Sinusitis on the Ultrastructure and Sialic Acid Distribution on the Sinus Mucosa Cell Surface of the Rabbit (실험토끼 상악동염이 상피세포 표면의 미세구조변화와 Sialic acid의 분포에 미치는 영향)

  • Experimatal maxillary sinusitis was induced in New Zealand white rabbits by blocking the maxillary sinus ostium. The distribution of lectin receptors was explored in the mucosa with induced maxillary sinusitis using colloidal gold label complex with lectin WGA purified from wheat germ (Triticum vulgaris). The lectin WGA gold complex, shown to recognize GlcNac (N-acetylglucosamine) and NeuNAc (N-acetylneuraminic acid) regions, was applied to detect binding sites in Lowicryl HM 20 sections and viewed under the electron microscope. An increased height of the cylindric cells, ciliary loss and hyperplasia of the secretory cells were observed. Examination of normal sinus mucosa labeled with gold-labeled lectins showed the distribution of sialoglycoconjugates to be mainly in the ciliary layer and the granules in the secretory cells. Inflamed mucosa had increased labeling intensity of gold-labeled WGA in the cilia and the secretory granules. These results indicate that lectin WGA receptors are located in the cilia and secretory granules. Specific changes in the lectin binding pattern were apparent in the inflamed mucosa in the experimentally induced acute sinusitis, in comparison with normal mucosa, conceivably as a part of host defense reactions.

Palliative Surgery in Two Dogs with Non-Curative Palatal Tumors (두 마리 개에서 완치가 어려운 구개 종양의 완화 수술 적용 증례)

  • Two dogs were presented with a history of anorexia, respiratory distress, or epistaxis. On physical examination, the dogs showed difficulty breathing and open-mouth breathing and large masses were found on the hard palate and soft palate. Cardiac arrest happened in case 2 and the dog was stabilized with cardiopulmonary resuscitation and oxygen therapy. Computed tomography demonstrated that the mass occupied the oropharynx, intrapharyngeal ostium, and laryngopharynx including the hard palate and soft palate in case 1. Palliative surgery was decided to improve swallowing and breathing with owner's consent in two dogs. Buccal mucosal flaps were performed for reconstruction of defects using rotational and single-pedicle advancement flaps and bilateral 90 degree transposition flaps in cases 1 and 2, respectively. Histopathology results described the oral masses as amelanotic melanoma in two dogs. The owners reported that there was normal swallowing and breathing at 7 days postoperatively in two dogs. In case 1, recurrent tumor was identified caudal to the hard palate 4 weeks after surgery. The owner did not allow further treatment and the dog became lost to follow-up at 2 months postoperatively. In case 2, there was no clinical or radiographic evidence of a local recurrence or distant metastasis at 3 weeks after surgery. The owner informed that the dog died suddenly with no signs of anything particularly wrong at 7 weeks after surgery.

Coronary Artery Disease Affected by Moyamoya Disease - A case report - (관상 동맥 질환을 동반한 모야모야 병 1례의 증례 보고)

  • Moyamoya disease is an unusual cerebrovascular disorder characterized by occlusive intimal dysplasia of the distal internal carotid and proximal cerebral arteries as well as other collateral arteries. However, moyamoya diseases are recently being reported as a systemic process. We experienced one case of coronary artery occlusive disease affected by moyamoya disease. The patient was a 35-year-old female, experiencing intermittent NYHA class ll dyspnea and exertional chest pain for 6 months and right paresthesia for 1 month before admission. Cerebral artery angiogram showed abnormal cerebrovascular systems and confirmed moyamoya disease with cerebral infarction of the left frontal lobe. In coronary artery angiogram, left coronary artery was not visualized due to total occlusion of the left main ostium and left coronary blood flow was supplied from normal right coronary artery. CABG was performed with OPCAB. Both internal mammary arteries were used for LAD and LCx. Intraoperative coronary artery findings showed intimal hyperplasia and no definite thrombi, and nondiseased coronary arteries were good and patent. We concluded that this patient's coronary artery disease was affected by moyamoya disease, and moyamoya disease should be evaluated in the extracerebral cardiovascular system.

Postoperative Evaluation of Congenital Atrial Septal Defect in Adult (성인 심방중격결손증의 술후평가)

  • Preoperative and postoperative evaluation was done in 60 adult patients above 20 years of age among 152 patients, who had operation for the congenital atrial septal defect from January 1984 to June 1993 in the Department of Thoracic and Cardiovascular Surgery, Chonbuk National University Hospital. Anatomically, the secundum type defect was present In 58 cases and the other two were ostium primum type defect. The defect were closed directly in 45 cases and with patches in 15 cases. Preopera ive EKG findings revealed regular sinus rhythm in 81%, arrhythmia 19%, RVH 32%, RAD 30%, Postoperative findings were regular sinus rhythm in 76%, arrhythmia 24%, RVH 25%. RAD 25%. In hemodynamic data, the mean SPAP in patients over 40 years old was 41.91 $\pm$ 14.33mmHg compared to 31.46 $\pm$ 13.77 mmHg in patients under 40 years old, and the difference between the two groups was stastistically significant. And then the patients were classified into two groups (group A sinus group, and group B : arrhythmia group) according to their preoperative rhythm. The patients in group B were older, had greater value of mean SPAP and Rp/Rs. Mean Qp/qs was higher in group A. But the difference between two groups about the mean Qp/qs and Rp/Rs was not significant. Postoperative complications such as arrhythmia, pneumothorax or hemothorax occurred in 7 cases, but there was no operative mortality. The functional improvement were remarkable in half of the patients who underwent surgery.
