Hydroxyapatite-coated implant: Clinical prognosis assessment via a retrospective follow-up study for the average of 3 years

  • PURPOSE. This research evaluated clinical outcomes of two types of hydroxyapatite (HA)-coated implants: OT (Osstem TS III-HA, Osstem implant Co., Busan, Korea) and ZM (Zimmer TSV-HA, Zimmer dental, Carlsbad, USA). MATERIALS AND METHODS. The research was conducted on 303 implants (89 of OT, 214 of ZM), which were placed from January 16, 2010 to December 20, 2012. The prognosis was evaluated in terms of success rates, survival rates, annual marginal bone loss, and implant stability quotients (ISQ). The samples were classified into immediate, early, conventional, and delayed groups according to the loading time. RESULTS. Overall, there were no significant differences between OT and ZM in success rates, survival rates, and annual marginal bone loss, except for the result of secondary stability. OT showed $77.83{\pm}8.23ISQ$, which was marginally higher than $76.09{\pm}6.90ISQ$ of ZM (P<.05). In terms of healing periods, only immediate loading showed statistically significant differences (P<.05). Differences between OT and ZM were observed in terms of two indices, the annual marginal bone loss ($0.17{\pm}0.58mm/year$ < $0.45{\pm}0.80mm/year$) and secondary stability ($84.36{\pm}3.80ISQ$ > $82.48{\pm}3.69ISQ$) (P<.05). OT and ZM did not have any statistically significant differences in early, conventional, and delayed loading (P>.05). CONCLUSION. OT (97.75%) and ZM (98.50%) showed relatively good outcomes in terms of survival rates. In general, OT and ZM did not show statistically significant differences in most indices (P>.05), although OT performed marginally better than ZM in the immediate loading and 1-stage surgery (P<.05).

COMPARATIVE STUDY ON THE FRACTURE STRENGTH OF METAL-CERAMIC VERSUS COMPOSITE RESIN-VENEERED METAL CROWNS IN CEMENT-RETAINED IMPLANT-SUPPORTED CROWNS UNDER VERTICAL COMPRESSIVE LOAD

  • Statement of problem. Fracture of the tooth-colored superstructure material is one of the main prosthetic complications in implant-supported prostheses. Purpose. The purpose of this in vitro study was to compare the fracture strength between the cement-retained implant-supported metal-ceramic crowns and the indirect composite resinveneered metal crowns under the vertical compressive load. Material and methods. Standard implants of external type (AVANA IFR 415 Pre-mount; Osstem Co., Busan, Korea) were embedded in stainless steel blocks perpendicular to their long axis. Customized abutments were fabricated using plastic UCLA abutments (Esthetic plastic cylinder; Osstem Co., Busan, Korea). Thirty standardized copings were cast with non-precious metal (Rexillium III, Pentron, Walling ford, Conn., USA). Copings were divided into two groups of 15 specimens each (n = 15). For Group I specimens, metal-ceramic crowns were fabricated. For Group II specimens, composite resin-veneered (Sinfony, 3M-ESPE, St. Paul, MN, USA) metal crowns (Sinfony-veneered crowns) were fabricated according to manufacturer's instructions. All crowns were temporary cemented and vertically loaded with an Instron universal testing machine (Instron 3366, Instron Corp., Norwood, MA, USA). The maximum load value (N) at the moment of complete failure was recorded and all data were statistically analyzed by independent sample t-test at the significance level of 0.05. The modes of failure were also investigated with visual analysis. Results. The fracture strength of Sinfony-veneered crowns ($2292.7{\pm}576.0N$) was significantly greater than that of metal-ceramic crowns ($1150.6{\pm}268.2N$) (P < 0.05). With regard to the failure mode, Sinfony-veneered crowns exhibited adhesive failure, while metal-ceramic crowns tended to fracture in a manner that resulted in combined failure. Conclusion. Sinfony-veneered crowns demonstrated a significantly higher fracture strength than that of metal-ceramic crowns in cement-retained implant-supported prostheses.

이중미세나사산 임플란트의 임상적 및 방사선학적 1년 평가 (Clinical and radiographic evaluation of implants with dual-microthread: 1-year study)

  • Purpose: The stability of periodontal condition and marginal bone level were important to achieve long-term success of dental implant treatment. The aim of this study was to evaluate periodontal conditions and marginal bone loss around 67 GSII(OSSTEM, Seoul, Korea) dental implants with dual-microthread at the neck portion, 1 year after prosthetic loading. Materials and methods: Sixty-seven GS II dental implants in 27 patients(mean age; $47.4{\pm}14.0$ years) who received implant treatments at Pusan National University Hospital, were included in this study. Thirteen US II(OSSTEM, Seoul, Korea) implants with smooth neck design were selected for the control group. Periodontal and radiographic evaluations were carried out at baseline, 6 months and 12 months after prosthetic loading. Results: In the GS II group, plaque index(PI), gingival index(GI) and probing depth(PD) increased as time passed. In the US II group, GI and PD increased. Although marginal bone level was lower in the US II group in all evaluation periods, the changes between the periods were not statistically significant(p>0.05). In each period, periodontal parameters were not statistically significant between groups. Conclusion: One year after prosthetic loading, GS II and US II dental implants showed similar periodontal conditions and marginal bone response, and were within the criteria of success.

Long-term Retrospective Clinical Study Comparing Submerged Type with External Hex Connection and Non-submerged Type with Internal Morse Taper Connection Implants

  • Purpose: This study was aimed to compare the survival and success rates, and long-term crestal bone loss according to the use of 2 connection types of dental implants (submerged-USII and non-submerged-SSII; Osstem $Implant^{(R)}$) by analyzing the change in alveolar bone height after 1 year under load and during final follow-up period. Materials and Methods: Between December 2004 and August 2008, patients with two types of Osstem implants (USII and SSII) were retrieved retrospectively. A total of 92 patients with 284 implants (USII=60, SSII=224) was finally selected. Their mean follow-up period was 7.5 years. The mesial and distal alveolar crestal bone changes were measured using radiographic images and the average was calculated at 1 year after loading and during final follow-up period. Result: Among the 284 implants, 4 USII and 7 SSII implants were removed, indicating 93.3% and 96.9% survival rates. Of the survived implants, mean crestal bone loss 1 year after loading was 0.39 mm for USII and 0.19 mm for SSII (P=0.018). During the final follow-up, mean crestal bone loss was 0.63 mm and 0.35 mm for USII and SSII, respectively, without statistical significance (P=0.092). According to the criteria for the success and failure of the implant by Albreksson and colleagues, final success rate was estimated as 86.7% for USII and 91.5% for SSII, respectively. Conclusion At 1 year after loading, the average crestal bone loss was significantly different between USII and SSII; however, both types met the criteria for implant success. During the final follow-up, both groups showed insignificant bone resorption patterns and did not show any pathological clinical symptoms. Therefore, both implants exhibited high long-term stability.

기성 지대주와 맞춤형 CAD-CAM 지대주의 적합 및 나사 안정성 비교 (A comparative study on the fit and screw joint stability of ready-made abutment and CAD-CAM custom-made abutment)

  • 연구 목적: 본 연구에서는 기성 지대주와 맞춤형CAD-CAM 지대주의 적합성 및 나사 사이의 안정성 대해 알아보고자 한다. 연구 재료 및 방법: 오스템 임플란트 시스템을 이용하였다. 동일회사에서 제작된 기성 지대주(Transfer abutment, Osstem Implant Co. Ltd, Busan, Korea) 및 맞춤형CAD-CAM 지대주(CustomFit abutment, Osstem Implant Co. Ltd, Busan, Korea)와 국내 외주 제작된 맞춤형CAD-CAM 지대주(Myplant, Raphabio Co., Seoul, Korea)를 5개씩 선택, 제작하였으며 나사는 각 회사에서 제공되는 것을 사용하였다. 제조사의 지시에 따라 고정체와 지대주를 30 Ncm으로 조인 후 초기 풀림 토크를 3회 반복 측정한 후 통계 분석하였다(${\alpha}=.05$). 고정체-지대주 연결체를 에폭시 레진에 매몰 후 습식 절삭 및 연마하여 계면 사이의 적합성을 FE-SEM으로 관찰하였다. 결과: 지대주의 초기 풀림 토크 값은 각각 기성 지대주(Transfer abutment)에서 $26.0{\pm}0.30Ncm$, 동일 회사에서 제작된 맞춤형CAD-CAM 지대주(CustomFit abutment)에서 $26.3{\pm}0.32Ncm$, 외주로 제작된 맞춤형CAD-CAM 지대주(Myplant abutment)에서 $24.7{\pm}0.67Ncm$였다. 국내 외주 제작된 맞춤형CAD-CAM 지대주에서 유의적으로 낮은 초기 풀림 토크 값을 보였으며(P=.027), 고정체-지대주 계면에서 변연 간극이 관찰되었다. 결론: 본 실험의 한계 내에서 국내 외주 제작된 맞춤형CAD-CAM 지대주는 동일 회사에서 제작된 기성 지대주나 맞춤형CAD-CAM 지대주에 비해 더 낮은 나사 안정성 및 적합성을 보인다고 볼 수 있다.

임플란트의 형태가 초기 안정성에 미치는 영향 (Influence of Implant Designs on Initial Stability)

  • 임플란트-골 계면상에 미세동요 없이 회복기를 가지는 것은 골융합을 위해서 필수적이다. 이를 위해 임플란트의 초기 안정성은 성공적인 예후를 위해서 중요한 요건이다. 충분한 골질과 골량은 임플란트의 초기 안정성과 조기 실패 방지를 위해 중요한 요소이지만, 임플란트가 갖는 외형적 특성이 초기 안정성에 미치는 영향에 대한 연구는 부족하다. 이에 본 연구는 임플란트의 형태가 초기 안정성에 미치는 영향을 알아보고자 하였다. 골질에 따라 피질골과 해면골의 두께가 서로 다른 모형골에 straight body의 US II, GS II, SS II system과 tapered body의 GS III system (OSSTEM Implant Co., Seoul, Korea) 임플란트를 식립하고 식립 회전력과 공진 주파수 및 동요도를 측정하였다. 이번 실험에서 초기 안정성에 주요한 영향을 미치는 요소는 골질로 나타났다. 임플란트의 형태에 따라 이중 나사선을 가지는 GS II 임플란트가 초기 안정성은 높게 나타났으며 tapered 한 임플란트가 straight 임플란트에 비해서 높은 초기 안정성을 얻을 수 있음을 알 수 있었다. 또한 tapered 임플란트 식립 시 임플란트의 외형과 조화된 식립구를 형성하는 것이 피질골이 두꺼운 경우 피질골에 과도한 압축력을 감소시키고 반대의 경우 해면골로부터 좀 더 많은 골고정을 얻을 수 있음을 알 수 있었다.

RBM 표면 테이퍼형태 임프란트의 단기간 후향적 임상 평가 (Short-Term Retrospective Clinical Study of Resorbable Blasting Media Surface Tapered Implants)

  • Purpose: The aim of the present study was to evaluate the clinical outcome of resorbable blasting media surface tapered implant. Methods: 169 Osstem$^{(R)}$ GS III dental implants in 73 patients who received implant treatments at Seoul National University Bundang Hospital, were included in this study. The incidence of biological and prosthetical complications has been carefully analysed for each implant. Results: The short-term implant survival rate was 97.63%, success rate 94.7%. The prevalence of biological complications was 15.38% and the prevalence of prosthetic complications was 13.04%. The mean value of crestal bone loss was $0.28{\pm}0.57$ mm. The relationship between loading periods and marginal bone loss was small and not statistically significant. In mandible, marginal bone loss was larger than in maxilla, no statistically significant. Also, length and diameter of implant had no relationship with marginal bone loss. Conclusion: We suggest that this implant system could achieve successful and stable results.

수산화인회석 코팅 임프란트의 골치유과정에 관한 실험적 연구 (Experimental study about the bony healing of hydroxyapatite coating implants)

  • Introduction: Hydroxyapatite(HA) coating implant can accelerate osseointegration, however, there are many controversies. Materials and Methods: This study examined the early osseointegration of two types of hydroxyapatite coated implants. Twelve adult male miniature pigs (Medi Kinetics Micropigs, Medi Kinetics Co., Ltd., Busan, Korea) were used in this study. In the implants placed in the mandible, a histomorphometric evaluation was performed to evaluate the bone-implant contact (BIC) ratio. Results: The BIC ratio increased with time. TS III HA and Zimmer HA were not significantly different (P>0.05). At 8 weeks, the BIC of Zimmer HA was higher than TS III HA, but there was no significant difference (P>0.05). Conclusion: HA coated implants will accelerate early osseointegration.

Osstem USII plus 임플란트의 단기간 성공률 및 변연골 흡수량 평가 (Success rate and marginal bone loss of Osstem USII plus implants; Short term clinical study)

  • 연구 목적: 임플란트 경부까지RBM 표면 처리를 한 Osstem$^{(R)}$ USII plus system (Osstem Co., Seoul, Korea)의 임상적 유용성을 평가하기 위하여 연세대학교 치과대학 병원에 내원한 환자 중 이 system을 이용하여 치료 받은 32명의 환자에게 식립된 88개의 임플란트를 대상으로 기능 부하 후 1년까지의 임상적, 방사선학적 결과를 후향적으로 분석하였다. 연구 재료 및 방법: 진료 기록부를 통해 전신 조건, 성별, 연령, 식립 위치, 식립 된 임플란트의 직경 및 길이 등을 조사하여 그에 따른 분포 및 생존율의 차이와 함께 이들 항목이 변연골 흡수량에 영향을 미치는지 조사하였다. 방사선사진 분석으로 임플란트 식립 시, 보철물 장착 후 6개월 내지 1년간의 변연골 흡수량을 측정하여 분석하였다. 각 항목과의 연관성 분석을 위하여 독립 t-검정(independent sample t-test)을 이용하였으며(${\alpha}$=.05), 보철물 장착 전과 보철물 장착 후 12개월 동안의 변연골 흡수량 비교를 위하여 일원 반복측정 분산 분석(one-way repeated-measures ANOVA)을 이용하였다(${\alpha}$=.05). 결과:총 32명에게 식립된 88개의 임플란트 중에서 실패한 것은 없었으며 누적 생존율은 100%로 나타났다. 임플란트 식립 시부터 보철물 장착까지의 변연골 흡수량은 0.24 mm이었으며, 보철물 장착 후 12개월까지의 변연골 흡수량은 평균 0.19 mm로 임플란트 식립 시부터 보철물 장착 12개월까지의 총 변연골 흡수량은 0.43 mm였다. 상악에 식립된 경우와 하악에 식립된 경우의 변연골 흡수량을 비교했을 때는 통계적으로 유의한 차이를 보이지 않았으나 구치부에 식립된 경우가 전치부에 식립된 경우에 비해 변연골 흡수량이 크게 나타났으며 이는 통계적으로 유의한 차이가 있었다. 결론: 이상의 결과를 토대로 보철물 장착 12개월까지의 기간 동안 임플란트 고정체의 경부까지 RBM 표면 처리를 한 외측 연결형의 국산 임플란트의 단기간 임상적인 성공률은 만족스러운 결과를 보였으며 변연골 흡수량도 임플란트 성공 기준에 부합하였다.

Evaluation of the correlation between insertion torque and primary stability of dental implants using a block bone test

  • Purpose: Implant stability at the time of surgery is crucial for the long-term success of dental implants. Primary stability is considered of paramount importance to achieve osseointegration. The purpose of the present study was to investigate the correlation between the insertion torque and primary stability of dental implants using artificial bone blocks with different bone densities and compositions to mimic different circumstances that are encountered in routine daily clinical settings. Methods: In order to validate the objectives, various sized holes were made in bone blocks with different bone densities (#10, #20, #30, #40, and #50) using a surgical drill and insertion torque together with implant stability quotient (ISQ) values that were measured using the Osstell Mentor. The experimental groups under evaluation were subdivided into 5 subgroups according to the circumstances. Results: In group 1, the mean insertion torque and ISQ values increased as the density of the bone blocks increased. For group 2, the mean insertion torque values decreased as the final drill size expanded, but this was not the case for the ISQ values. The mean insertion torque values in group 3 increased with the thickness of the cortical bone, and the same was true for the ISQ values. For group 4, the mean insertion torque values increased as the cancellous bone density increased, but the correlation with the ISQ values was weak. Finally, in group 5, the mean insertion torque decreased as the final drill size increased, but the correlation with the ISQ value was weak. Conclusions: Within the limitations of the study, it was concluded that primary stability does not simply depend on the insertion torque, but also on the bone quality.
