A Study on the Status of Patients with Infertility in a Department of Oriental Gynecology for the last Three Years and on the Future Policy of Infertility Treatment in Oriental Medicine (최근 3년간 일개 불임전문 한방병원에 내원한 불임환자의 실태분석과 한방불임치료정책에 대한 제언)

  • Objectives : This study was to analyze the status of patients with infertility in the department of oriental gynecology in an oriental medical hospital and propose infertility treatment in oriental medicine. Methods : This study was presented with 2,153 cases of infertile women who had visited the department of oriental gynecology in one oriental hospital in Seoul from 2002 to 2004. Results : The total number infertile patients has decreased year by year. The age range of patients was getting higher and the age group between 30 and 35 took up about one half of the total patients. The duration of infertility for less than 3 years held about 50-60% of the total patients, and the mean duration of infertility was $39.18{\pm}29.57$ months (2004). The most common occupations of females were professionals or administrative managers. The secondary infertility was increasing and unexplained infertility increased from 38.2% (in 2002) to 54.4% (in 2004). However the duration of treatment was shorter. This shows that the age range of the patients having oriental-medicine treatment is gradually getting higher and the number of the patients having oriental-medicine treatment and compliance is decreasing. Conclusions : More studies about objectification of oriental medicine need to precede any other studies. Then, it is thought to be necessary to rebuild cooperative relationships between oriental & western medicine through systematic research and political supports and to establish firm foundations of oriental medicine.

A study on the current status of oriental medical care services in health centers and implications for its expansion (보건소장의 한방진료에 대한 태도와 관련 요인 분석)

  • Currently the oriental medical care services in the health centers is getting popularity because of their unique aspects which western medicine can not cover. This study was conducted to speculate the current status of oriental medical care services in health centers and possibility of how to effectively provide the oriental medical care services(or traditional medical care services) in health centers. For the study, the survey questionnaires were distributed to all 269 health centers and 138(51.3%) of them were collected. Eight of the collected were inadequate for the analysis and 130 of 269 questionnaires were finally chosen for the study. The SPSS/PC WIN 8.0 was used for the statistical analysis. The results are as follows. First, 91 out of 130 sample health centers(70%) are providing oriental medical care services. The results show that the willingness of the directors and the public awareness in community are the most important factors to provide oriental medical care services in health centers. In contrast, the lack of cooperative efforts between western and oriental medical doctors and the lack of government supports are considered as factors that intervene the oriental medical care services in health centers. About 80% of the sample health centers respond that the government supports is needed for oriental medical care services in health centers. Second, it was asked to the directors of 39 health centers which do not provide oriental medical care services regarding their future plan to include the services. About 70% of health centers respond that no plan is available now. They acknowledge that the demand on the services is the most important factor to consider the oriental services as their future medical services. Third, 69.2% of the oriental medical doctors in health centers are public health doctor. 95.6% of the sample health centers have space for the oriental medical care services in their facilities and some health centers provide the services through the private clinics nearby facilities. Finally, the surveyors consider both budgetary constraints and difficulty in recruiting doctors as barriers for the effective oriental medical care services in health centers. Finally, t-test and LSD were employed to find out the difference among several groups. The analysis shows statistically significant difference among groups about their recognition on health care policy, health care system, and effectiveness of oriental medical care services. To be conclude, the study shows the necessity of oriental medical care services in health centers. To do so, the government supports, cooperative efforts between oriental and western medical doctors, and providing job security for doctors should proceed to provide effective oriental medical care services in health centers.

A Qualitative Study on Traditional Korean Medicine Treatment for Child Patients with Night Crying - With a Focus on Descriptions by Main Fosterers - (야제 환아의 한의치료에 대한 질적 연구 - 주양육자의 서술을 중심으로 -)

  • Objectives This study sets out to examine treatment experiences of child patients with night crying based on the traditional Korean medicine. Also, the study traces the process in which clients chose traditional Korean medicine treatment, and proposes a possible future direction of Korean medicine treatment. Methods The investigator conducted a semi-standardized open-ended question interview with two main fosterers of a child who received traditional Korean medicine treatment for night crying, and examined two fosterers' experiences of a child with the traditional Korean medicine treatment. Their interviews were recorded with their facial expressions and acts for analysis. Collected data was analyzed based on the Grounded theory. Results The participants felt a sense of responsibility, worry, and fear for their children with night crying. They had easy access to traditional Korean medicine doctors who were in the community and had positive experiences with traditional Korean medicine treatment. They reported that some people had been forced to use traditional Korean medicine, had misunderstandings about traditional Korean medicine, and had prejudice that night crying is not a disease. They felt there was a room for improvement in terms of the high medical expenses and the absence of a cooperative system between Oriental and Western medicine. Conclusions As the first qualitative study on night crying based on the traditional Korean medicine treatment, this study proposed a future direction for traditional Korean medicine treatment for night crying in the shoes of fosterers. The findings showed that the fosterers chose traditional Korean medicine treatment for night crying child based on their feelings, accessibility and experiences with traditional Korean medicine. Fosterers felt a need for improvement of the general public's perceptions about Korean medicine treatment on night crying, and a cooperative system between Oriental and Western medicine.

Korean Herbal Medicine on Liver Function : A Systematic Review in Korean Literature (한약 복용이 간기능에 미치는 영향 : 국내 문헌에 대한 체계적 고찰)

  • Background : The safety of Korean herbal Medicine (KHM: prescribed herbal medicine by doctors of traditional Korean medicine) is an important issue in Korea. Although both fields. western medicine and traditional Korean medicine. have been studied on the safety of herbs and KHM, their results were not concordance with each other. Objectives : This study aims to review the influence of KHM on liver function in Korean literature systematically. Additionally, we tried to estimate the change of data of liver function test (LFT) and the incidence of liver injury (LI) after the use of KHM. Methods : Systematic literature searches were performed on 4 major databases of Korea from their inception to May 2008. Screening and selection of the studies and the extraction of data were performed independently by two authors. There were no restrictions on the types of publication including grey literatures. Results : Forty studies were included. Only sixteen studies were performed prospectively and fifteen studies collected data from outpatients. Only 8 studies reported the occurrence of LI after the use of KHM. Nineteen cases of LI showed no or mild symptoms and the elevation of LFT was not high. All of LI patients used conventional western drug and KHM concomitantly. and causality of LI was not assessed properly. The incidence of LI related to the use of KHM was estimated as 0.59%-0.76% from all data of these studies. The conflicting results were shown on the change of alanine aminotransferase (ALT) and total bilirubin (TB) after the use of KHM. Conclusions : KHM might be a minor cause of LI in Korea. However the results are not strongly supported as enough to make the safety issue clear because of the limitations of original studies. More rigorous studies are required for answering the safety issue of KHM with the cooperative investigation of both fields of Korean traditional medicine and western medicine.

Systematic Review of the Cause of Drug-Induced Liver Injuries in Korean Literature (약인성 간손상의 원인물질에 관한 국내연구의 체계적 고찰)

  • Background: The safety of herbal products is an important issue in Korea. It is more complicated because of the concomitant use of conventional western drugs and herbal medicine, including prescribed medicine, folk herbs and food supplements. Although both western and traditional Korean medical doctors have studied on the safety of HMP, their results did not show relevant or consistent conclusions because of the poor quality of studies, including bias. Objectives: The aim of this study was to review the studies in Korea related to drug induced liver injury (DILI) systematically. We tried to estimate the proportion of causative materials of DILI and evaluate the clinical difference of DILIs from different materials. Methods: Systematic searches were performed on MedRIC and MEDLIS in Korea. The extraction of data as well as selective screening of the studies was carried out independently by two of the authors. There were no restrictions on the types of publication, including grey literature. Results: Twenty-one articles were included. Of them only 7 adapted prospective design and only 6 were published in peer-reviewed journals. The proportion of conventional drugs associated with hepatotoxic injuries in all DILI ranged from 15.8% to 83.3% and that of herbal medicine was from 28.9% to 44.7%. However, the criteria for herbal medicine were not clearly defined and concomitant medications were not fully investigated in most studies. There were limited objective data in the clinical differences of liver injuries from different materials and their results were conflicting. Conclusions: The causality assessments regarding DILI of herbal medicine were not performed properly and causative materials were misclassified in most of the studies published in Korea. These make the safety issue still ambiguous because of the limitations and lack of objectivity of the studies. More rigorous studies are required for clearly addressing these conflicting issues with cooperative investigation between traditional Korean and western medicine.

A study on the validity to make a diagnosis of Soeumin by QSCC II (Questionnaire for the Sasang Constitution Classification II) (사상체질분류검사지(四象體質分類檢査紙)(QSCC) II 에 의한 소음인(少陰人) 진단(診斷)의 타당성(妥當性) 연구(硏究))

  • The purpose of this study is to examine validity to Soeum scales by QSCC II. For this study, the objects are 336 patients who were made a diagnosis of Soeumin and 422 patients of other constitutions in Oriental Medicine & Western Medicine Cooperative Health Examination Center KyungHee University Medical Center. A systematic analysis has been made and the results are as follows. 1. We made a frequency analysis in 758 examinees' questions and concluded that 35 questions, among which 30 questions were existing and 5 were new, had statistical significance in making a diagnosis of Soeumin by more than 10% in comparison with answer frequency of other constitutions. 2. Because confidence coefficient of 0.8011 revised Soeum scales(35) rises up as compared with that of 0.6319 existing Soeum scales(51), I think revised Soeum scales reflect trait of Soeumin well and it is liable to shorten question time by controling questions. 3. Soeumin's discrimination probability in Soeumin was 55.4% by QSCCII, but that rises up to 64.3% by discriminant function made by revised Soeum scales. So there is statistical significance in discriminant function. 4. When questions in QSCCII are divided into body-spirit(體形氣像), looks-attitude(容貌詞氣), temper- talent(性質才幹), continuous mind(恒心), avarice(心慾), physiological symptoms(生理的 證) and pathological symptoms(病理的 證), response frequency and constitution discrimination probability falls in questions about avarice and pathological symptoms.

A Case of Painful Ophthalmoplegia Patient Diagnosed as Tolosa-Hunt Syndrome (Tolosa-Hunt Syndrome으로 인한 통증성 안구운동 장애 치험 1례)

  • ■ Objectives The purpose of this clinical study is to evaluate the effect of Traditional Korean Medicine(KM) on a patient with Tolosa-Hunt syndrome. ■ Methods We treated a 58 years old male patien t with Tolosa-Hunt syndrome who had several clinical symptoms such as ocular pain, limitaion of ocular movement and unilateral ptosis. We used herbal medication, acupuncture, electro-acupuncture(EA), cupping, moxibustion, bee-venom injection. Then we evaluated the improvement by Numerical Rating scale(NRS), Distance of eye opening and ratio of pupil movement. ■ Results We demonstrated the decrease of pain score and improvement of equal ocular movement(EOM) and ptosis after the KM treatment. ■ Conclusion This study proved the effect of KM treatment on Tolosa-Hunt syndrome. It could cooperative treatment would be more effective than mono treatment of western medicine.

  • 1. PURPOSE This is a quantitative analysis of Sasang Constitution by measuring human body except face. This study is carried out for the purpose of searching out the theory of Sa-cho and the morphologic theory of Sasang Constitution. 2. METHOD (1) SUBJECTS Subjects are 338 person came to Dept. of Sasang Constitutional Medicine (KyungHee Oriental Medical Center) and Oriental Medicine and Western Medicine Cooperative Health Examination Center (KyungHee Medical Center) from October 1997 to Jauary 1998. The mean age is $41.7{\pm}12.39$. (2) CLASSIFICATION OF SASANG CONSTITUTION QSCCII(Questionnaire for the Sasang Constitution Classification) is used for the Classification of Sasang Constitution. The distributions of Sasang Constitution Classification are Soeumin 83, Soyangin 98, and Taeumin 157. (3) MEASURING INSTRUMENT & ITEMS Length, Breadth, Circomferece, and Skinfold thickness of each body segment are measured by Martin's measuring instrument and Skinfold Caliper. Measured items of this study is based on Lohman's methods and is added new items. (4) DATA ANALYSIS The Volume, Weight, Rate, and Index of each group are calculated by using measuring items. The difference of each group is analyzed by SPSS 7.0 for Win. 3. RESULT (1) THE MORPHOLOGIC CHARACTERISTIC of TAEUMIN First of all, the Taeumin has a emormous morphology. His trunk is huge but head-neck and limbs is small. The volume and weight of head-neck is smaller than other body segments but the volume and weight of upper-abdomen is bigger than ones. (2) THE MORPHOLOGIC CHARACTERISTIC of SOEUMIN The Soeumin has a small morphology. His head-neck rate is bigger than Taeumin's but his trunk rate is smaller than Taeumin's. And his lower limbs rate is bigger than other constitution but it's not a significant difference. His neck is thinest and longgest than others. His chest is thin and narrow. His lower-abdomen rate is bigger than other constitution but it's difference is not significant. (3) THE MORPHOLOGIC CHARACTERISTIC of SOYANGIN Soyangin has a middle size at head-neck and chest part. But the rate of upper limb is higer than other constitution. And the rate of lower-abdomen part is lower than other constitution. but that's not significant.

  • I . Objective and significance of the study To design a data acquisition chart, which facilitates data collection and analysis. The chart is also useful for solving problems that arise from personal variations in clinical symptoms and filling the knowledge base of an expert system. II. Content and scope 1. Collect the diagnosis knowledge of cerebrovacular diseases from doctors and analyze it. 2. Design a data acquisition chart. 3. Compare ODS and doctors with respect to their diagnosis results 4. Select patients who are determined to suffer from cerebrovascular diseases using CT(computed tomographic) scan, collect clinical data from them. III. Results and Application The chart be used for data collection and analysis in different medical hospitals, The results of data analysis facilitates collecting clinical data about other diseases and implementing the knowledge base. Also, the collected data serves as a tool for medical education, and cooperative diagnosis of oriental and western medical doctors.

Data Mining and Construction of Database Concerning Effects of Vitis Genus (산머루 관련 정보수집 및 데이터베이스의 구축)

  • The database for the oriental medicine had been existed in documentation in past times and it has been developed to the database type for random accesses in the information society. However, the aspects of the database are not so diversified and the database for the bio herbal material exists in widened type dictionary style. It is a situation that the database which handles the in-depth raw herbal medicines is not sufficient in its quantity and quality. Korean wild grape is a deciduous plant categorized into the Vitaceae and it was found experimentally that it has various medical effects. It is one of the medical materials with higher potentiality of academic study and commercialization recently because it has a bigger possibility to be applied into diverse industrial fields including the medical product for health, food and beauty. We constituted the cooperative system among the Muju cluster business group for Korean mountain wild grapes, Physiology Laboratory in Kyung Hee University Oriental Medicine and Medical Classics Laboratory in Kyung Hee University Oriental Medicine with a view to focusing on such potentiality and a database for Korean wild grapes was made a touchstone for establishing the in-depth database for the single bio medical materials. First of all, the literatures based on the North East Asia in ancient times had been categorized into the classical literature (Korean literature published by government organization, Korean classical literature, Chinese classical literature and classical literature fro Korean and Chinese oriental medicine) and modern literature (Modern literature for oriental medicine, modern literature for domestic and foreign herbal medicine) to cover the eastern and western research records and writings related to Korean wild grapes and the text-mining work has been performed through the cooperation system with the Medical Classics Laboratory in Kyung Hee University Oriental Medicine. First of all, the data for the experiment and theory for Korean wild grape were collected for the Medline database controlled by the Parliament Library of USA to arrange the domestic and foreign theses with topic for Korean wild grapes and the network hyperlink function and down load function were mounted for self-thesis searching function and active view based on the collected data. The thesis searching function provides various auxiliary functions and the searching is available according to the diverse searching/queries such as the name of sub species of Korean wild grape, the logical intersection index for the active ingredients, efficacy and elements. It was constituted for the researchers who design the Korean wild grape study to design of easier experiment. In addition, the data related to the patents for Korean wild grape which were collected from European Patent Office in response to the commercialization possibility and the system available for searching and view was established in the same viewpoint. Perl was used for the query programming and MS-SQL for database establishment and management in the designing of this database.
