긴장성두통에 대한 한약치료: 체계적 문헌고찰 및 메타분석 (Herbal Medicine for Tension-type Headache: Systematic Review and Meta Analysis of Randomized Controlled Trials)

  • Objectives: People with tension-type headache generally take pain relievers, but long term dependency causes problems as well as side effects. The present study aimed to provide clinical evidence of herbal medicine for tension-type headache by systematic review of randomized controlled trials on the effect of herbal medicine for tension-type headache. Methods: Randomized or quasi-randomized controlled trials that verified effects of herbal medicine intervention on patients with tension-type headache were included in the study. A literature search of English, Japanese, Chinese, Korean databases was performed. The selected literature were assessed by Jadad scale and Risk of Bias. Results: Herein, 40 reports were selected from a total of 157. Meta-analyses of 2 trials indicated that the effective rate was significantly higher in the herbal medicine treatment group, as compared to the placebo control (risk ratio: 1.49, 95% confidence interval (CI): 1.23 to 1.80, p<0.0001, I2=0%). Four trials that compared herbal medicine with routine care with routine care only group showed a significantly higher effective rate of benefits for herbal medicine with routine care, as compared to routine care only (RR: 1.57, 95% CI: 1.18 to 2.10, p=0.002, I2=0%). Conclusions: The effective rate was significantly higher than control and adverse events were less in the treatment group. Pattern analysis (辨證) indicated that the studies on wind-fire pattern (風火證) were highest. Yougeun-bang (柔筋方) in prescription and Paeoniae Radix Alba (白芍藥) in medicinal herbs were most used. This finding could be widely utilized in future clinical practice and form the basis for the development of clinical practice guidelines in advance.

  • A study on Bang-Yak-Hap-Py$\breve{u}$n, one of the most widely-known Korean medicine books, has been conducted and the findings are as follows:the political and social background at the end of Yi-dynasty amidst the inflow of western civilization and the aggression of foreign forces can be seen as a historical period of trial, when civilization and independence had to be preserved at the same time, during the period, the social trend was to pursue a rapid practical solution rather than a profound theory. Responding to this trend, simple and easy medical science became to prevailand the representative medicine book of this kind was Bang-Yak-Hap-Py$\breve{u}$n. 1. Bang-Yak-Hap-Py$\breve{u}$n was written comprising the characteristics of simple and easy medical science with a view to popularizing-collection of logics, easy readability, medical science in time, etc. 2. Bang Yak-Hap-Py$\breve{u}$n, rich and grand in content, came out of Dong-$\breve{U}$i-Bo-Gam at the right moment as a pragmatic guide book of Korea medical practice symbolizing Korean spirit. 3. Do-yun Whang whose pen name was Hye-Am was born in Changwon, Kyungnam-do as the 18th descendant of Changwon Whang family. He engaged in medical practice in Seoul and wrote and edited Boo-Bang-Py$\breve{u}$n-Ram. $\breve{U}$i-Jong-Son-Ik, $\breve{U}$i-J$\breve{u}$ng-Son-Ik-Bu-Yeo, $\breve{U}$i-Bang-Whan-Tu, ect. and made his son, Pil-Su follow him and let him edit Bang-Yak-Hap-Py$\breve{u}$n, which put $\breve{U}$i-Bang-Whal-Tu and Son-Ik-Bon-Cho together. 4. The construct of Bang-Yak-Hap-Py$\breve{u}$n can be subdivied into three parts: herbs of 34 classes and 514 kinds were arranged on the upper part and $\breve{U}$i-Bang-Whal-Tu was arranged on the middle end lower part. Such arrangement can be very effective in clinics. 5. Bang-Yak-Hap-Py$\breve{u}$n as a curative book of Korean medical science has been a necessary another ego for many Korean medical practioners. It also played a great role of enhancing Korean oriental medical science to be widely used by Koreans. On the other hand, it produced a side effect of emergence of employed medical practioner who lacked in scholastic aptitude. 6. Tens or newly edited Bang-Yak-Hap-Py$\breve{u}$n have been issued. Among them, Jung-Jeong, Bang-Yak-Hap-py$\breve{u}$n, J$\breve{u}$ng-Mak-Bang-Yak-Hap-Py$\breve{u}$n. By$\breve{u}$n-J$\breve{u}$ng-Bang-Yak-Hap-Py$\breve{u}$n, and By$\breve{u}$n-J$\breve{u}$ng-Bang-Yak-Jung-Jeon have been widely known. Lately, Bang-Yak-Hap-Py$\breve{u}$n which includes more symptoms and remedies or is translated into Korean came out in several kinds. 7. The number of korea classical medicinal books quoted in Bang-Yak-Hap-Py$\breve{u}$n is 68, among which Dong-$\breve{U}$i-Bo-Gam Occupies the top in the list enlisting 69 medicinal treatment. This Shows that Dong-$\breve{U}$i-Bo-Gam gave birth to Bang-Yak-Hap-Py$\breve{u}$n.

대황 추출물의 항위염 작용과 유효성분에 관한 연구 (A Study on the Antigastritic Effects of Rheum Species Extracts and Their Active Components)

  • 현재 대황은 지표성분의 규격이 한국, 일본과 중국이 다르고, 유통시장에서 대황과 종대황이 혼재되어 있으며 다양한 기원식물이 혼재되어 수입되고 있다. 따라서 대황의 효능을 현대 약리학적으로 해석하는 과정을 통해 약재의 표준화, 과학화 및 위염에 대한 지표성분 검색에 기여할 수 있을 것으로 사료되어 Rheum tanguticum(당고특대황)과 그 비교생약으로 Rumex cripus(양제근), Rheum officinale(약용대황), Rheum palmatum(장엽대황) 및 Rheum undulatum(종대황)의 물 및 70% 에탄올 추출물을 비교하였다. 그 결과, 흰 쥐를 이용한 위염 모델에서 양제근 70% 에탄올 추출물(73.2%)와 종대황 물 추출물(71%)이 각각 70% 이상으로 우수한 위손상 억제활성을 보여주었지만, 당고특대황의 70% 에탄올 추출물 투여시 91.8%로 실험군 중 가장 우수한 위염 억제 효과를 확인할 수 있었다. 공격인자로써의 산을 중화시킬 수 있는 능력을 알아보기 위한 제산력 시험법으로는 당고특대황 물 추출물과 70% 에탄올 추출물이 각각 약 14.2%, 약 15.8%로 억제함을 알 수 있었다. 이 결과로 보아 양성대조군으로 사용한 hydrotalcite(49.9%)보다는 제산작용이 약하였지만 과량 혹은 장기간 복용하면 효과가 있을 것으로 생각된다. 소화성궤양, 위암 및 변연부 B세포 림프종 발생에 있어 중요한 원인 인자로 밝혀졌고, 기능성 소화불량증, 위의 과형성 용종, 철결핍성 빈혈 등 기타 질환과의 연관성들이 제시되고 있는 H. pylori에 대한 colonization 억제 활성을 확인한 결과 당고특대황의 70% 에탄올 추출물 $100{\mu}g/mL$에서 H. pylori에 대한 항균작용(+)을 확인할 수 있었다. DPPH 용액을 이용하여 free radical의 소거능을 $IC_{50}$으로 나타낸 결과, 당고특대황은 물 및 70% 에탄올 추출물에서 $IC_{50}$$25.9{\mu}g/mL$, $5.8{\mu}g/mL$로 양성대조군으로 사용한 L-ascorbic acid ($IC_{50}=6.5{\mu}g/mL$)와 유사한 free radical 소거능을 나타내었다. 이 결과로 당고특대황의 70% 에탄올 추출물이 휜 쥐 위염모델의 위손상을 가장 우수하게 억제하였으며 항산화 활성 또한 가장 우수한 것으로 확인되었다. 당고특대황의 성분인 chysophanol, chrysophanol-8-O-glc, desoxyrhaponticin desoxyrhapontigenin, emodin, isorhaponticin, 2-methoxy-4-hydroxyanthraquinone-5-O-glc, physcion, piracetannol-3'-O-glc, resveratrol, rhaponticin 및 rhapontigenin을 이용하여 실험한 결과로는 효성분들 중 physcion $100{\mu}M$과 rhaponticin $50{\mu}M$$100{\mu}M$에서는 실험에 사용한 성분 들 중 비교적 우수한 colonization 억제작용(+)을 나타내었고, DPPH radical 소거 활성은 isorhaponticin, piracetannol-3'-O-glc, resveratrol 및 rhaponticin에서 $IC_{50}$이 각각 $129.5{\mu}M$, $94.1{\mu}M$ $80.1{\mu}M$$104.6{\mu}M$로 확인되었다. 실험결과를 토대로 항위염 효과가 있는 우수한 약용식물로써 천연물 의약품개발 가능성을 확인하였고 나아가 한약재의 효능을 과학적으로 검증하고 한약재 표준화에 기여할 수 있을 것으로 기대된다.

한인진(韓茵蔯)의 치주염세균에 대한 항균효과 및 항염효가 (Antimicrobial Effect on the Periodontal Pathogens and Anti-inflammatory Effect of Artemisiae Iwayomogii Herba)

  • Objectives : The purpose of this study was to evaluate on the antimicrobial effect on the periodontal pathogens and anti-inflammatory effect of Artemisiae Iwayomogii Herba. Artemisiae Iwayomogii Herba has been used for treating as Artemisiae Capilaris Herba in Korea. Methods : Artemisiae Iwayomogii Herba was prepared by extracting medicinal herb with water. We investigated antimicrobial activity by the minimun inhibitory concentration (MIC) test. We also investigated inhibition of IL-$1{\beta}$-induced collagenase-l(MMP-l), stromelysin-1(MMP-3), interleukin-6 gene expression in human gingival fibroblasts. Results : The antimicrobial effect of Artemisiae Iwayomogii Herba was evaluated with MIC against periodontopathogens; Porphyromonas gingivalis 2561, W50, A7A1-28, 9-14K-1, Prevotella intermedia28, and Actinobacillus actinomycetemcomitans Y4, MICs of Artemisiae Iwayomogii Herba were 0.156 mg/ml, 0.625 mg/ml, 0.313 mg/ml, 1.25 mg/ml, 10 mg/ml and 10 mg/ml. The anti-inflammatory effect of Artemisiae Iwayomogii Herba was evaluated with Influence of herbs on the IL-$1{\beta}$-induced expression of MMP-1, MMP-3, interleukin-6, IL-$1{\beta}$ increased MMP-1, MMP-3, interleukin-6 mRNA levels. Artemisiae Iwayomogii Herba significantly inhibited IL-$1{\beta}$-induced MMP-1, MMP-3, interleukin-6 gene expressions in a dose-dependent manner. Conclusions : These results suggested that Artemisiae Iwayomogii Herba might reduce the excessive proteolytic capacity of the gingival fibroblast during inflammation and could be developed a new drug in periodontitis.

한약재 4종 복합추출물의 해마신경세포 보호를 통한 기억력 개선 (New Four-herb Formula Ameliorates Memory Impairments via Neuroprotective Effects on Hippocampal Cells)

  • 본 연구는 동의보감을 근거로 선별된 백수오, 지황, 원지 및 석창포로 구성된 복합추출물의 해마신경세포에 대한 보호 및 기억력의 개선효과를 살펴보았다. 복합추출물의 신경보호효과를 검증하기 위해 HT22해마신경세포의 생존율, 세포독성 및 활성산소를 분석하였으며, 허혈성 뇌손상 마우스모델을 이용하여 기억에 대한 동물행동학적 변화와 단백질 발현을 측정하였다. 해마신경세포에서 복합추출물의 전 처리는 glutamate에 의해 유도된 활성산소의 축적을 억제하였으며 세포사멸을 감소시켰다. 허혈성 뇌손상 마우스모델에서 복합추출물은 동물행동학적으로 공간 및 단기 기억능력을 개선시켰다. 뇌허혈로 인해 증가된 p38 MAPK의 인산화는 복합추출물에 의해 현저히 감소하는 반면, 감소된 PI3K와 CREB의 인산화는 현저히 증가하였다. 이를 면역조직화학분석을 통해 복합추출물을 투여한 그룹이 해마에서 발현되는 CREB의 인산화가 현저히 증가되는 것을 확인하였다. 이상의 결과는 복합추출물이 CREB 단백질과 관련된 신경보호 신호기전을 조절함으로써 인지기능을 개선시키는 것으로 사료된다.

익수영진고가미방의 지황박, 복령피 배합과 발효에 따른 제제가 면역활성에 미치는 영향 (The effect of prescriptions prepared by adding medicinal herbs(Rehmannia glutinosa Residue, Poria cocos Bark) and fermenting herbal materials based on formulas Iksuyoungjingo on immunological activity)

  • Objectives : This study was carried out to enhance the activity of Iksuyoungjingo, which has the tonify Qi, nourish Yin effect of oriental medicine, and to study the effect of the prescriptions on the immunological activities. Methods : Immunosuppression was induced by methotrexate (MTX) 2 mg / kg, the experimental group was divided into IYGgami-I, IYGgami-I-F, IYGgami-II and IYGgami-II-F groups. Each prescription was administered with drinking water for 20 days, and body weight was measured every 5 days during this period. Leukocyte, $TNF-{\alpha}$, IL-2, IL-6, IgE, spleen weight and body weight were measured Results : In the changes of $TNF-{\alpha}$, IL-2 and IL-6 as pro-inflammatory elements, all of the experimental groups showed a significant increase compared to the control group. In the IgE changes, the IYGgami-I-F, IYGgami-II and IYGgami-II-F groups showed a significant decrease compared to the control group. In the changes of spleen weight, the IYGgami-II-F group showed a significant increase compared to the control group. In the changes of WBC and lymphocytes, the IYGgami-I-F group showed a significant increase compared to the control group. Conclusions : From the above results, it can be observed that the efficacy against immunity is exerted in all of the preparations, and it was confirmed that the efficacy was maintained constant even when utilizing the Rehmannia glutinosa Residue and Poria cocos Bark, and that a more beneficial effect can be exerted in the effectiveness when the fermentation is carried out.

오가피와 삼채 등 생약재 추출물의 근력 활성 효과 (Muscle Strength Activity of Herbal Medicine Extracts of Acanthopanax Sieboldianus and Allium Hookeri)

  • 우리는 한방과 민간요법에서 전통적으로 근력증대와 근감소증 예방, 근육합성 촉진, 연골과 인대 관절강화 등에 사용되어온 천연물 약재를 선정하였다. 이와 관련된 효능이 보고된 총 아미노산을 분석하고 근력약화 및 근감소 현상도 생체의 노화촉진 증상의 일환으로 간주하여 이들 추출물의 항산화 활성과 관련된 총 폴리페놀 및 플라보노이드 함량을 분석하였다. 인삼 및 삼채의 가공제조 과정에서 주종 Arginine 함량의 변화는 건조 백삼 114.07mg/100ml 대비 증숙 홍삼 99.48mg/100ml 및 건조 삼채 118.90mg/100ml 대비 증숙 삼채 54.69mg/100ml으로 감소하였다. 섬오가피 야생 뿌리와 가시오가피 지상부 잔가지 추출물의 항산화효과 DPPH 및 ABTS 라디칼 소거 활성이 현저하게 높았다. 한편 DPPH활성은 건조백삼과 건조 삼채에 비하여 증숙 홍삼과 증숙 삼채가 높았으나, ABTS 활성은 건조백삼 41.96% 대비 증숙 홍삼 66.88%로 증가를 보였으나 건조 삼채 77.48% 및 증숙 삼채 77.52%의 활성은 유사하였다.

동병하치 확산을 위한 전략적 방향과 이행방안 (Strategic Direction and Road Map of Expanding Prevention of Winter Disease in the Summer)

  • Objectives : The purpose of this study was to propose appropriate strategic directions and road maps for successful achievement of programs preventing winter disease in the summer. Methods : Details on programs preventing winter disease in the summer such as clear concept, theoretical basis, current status, intervention or available prescriptions and indication/contraindication/caution were prepared through the related journal review, upon which an observational study was devised and done for simulation to find out even a trivial problem and to guarantee the safety beforehand. The experimental group was divided into 5 groups by the size of pill and the way ginger is treated; 1cm pill with ginger group, 3cm pill group without ginger, 3cm pill group dipped into ginger, 3cm pill group applying ginger to acupoints and 3cm pill group with ginger Results 1. program preventing winter disease in the summer was defined as representative winter diseases such as common cold, influenza, chronic asthma, chronic bronchitis, allergic rhinitis, emphysema, chronic gastritis and rheumatoid arthritis, and preventive care in the summer, reinforcing deficient yang qi of five viscera by using exuberant yang qi from summer heat. 2. It was based upon historically established theories which is 'nourishing yang qi in the spring and summer', 'long summer, namely rainy spell in the summer overwhelms the winter, because of earth winning water according to the five phases theory' and 'To replenish yang qi is major principle to treat winter diseases, which can be most appropriately and timely applied to the patient with deficient yang qi of five viscera inherently, especially in the three dog days of the summer, because of exuberant exterior yang qi and deficient interior yang qi in the five viscera'. 3. In the adjacent China and Taiwan, acupoint applying method in the three dog days named 'San Fu Tie' have been stirring a boom throughout the nation, in which Xiaochuan Gao was used as a basic prescription and it mainly was applied at bilateral $BL_{13}$, $_{15}$ and $_{17}$ for about 4 hours. As far as domestic current status, the necessity of adopting the above method prior to Herbal formula was also recognised, because not a few koreans have apprehension for the safety of it including medicinal herbs and are reluctant to take it any more due to negative advertisement of narrow minded doctors' association. 4. Indication of acupoint applying method in the three dog days included most of winter diseases such as common cold, influenza, chronic asthma, chronic bronchitis, allergic rhinitis, emphysema, chronic gastritis. contraindication was pregnant woman and the weak such as infants and the old. More attention was paid to grasp firmly the normal reaction following the treatment for preventing side effect and teasing blister. recommendation was also given to abstain from food inducing phlegm and dampness such as meat, shrimp and crab as well as cold drinks and foods 5. In the simulation observational study based upon the above findings following review the related articles, no blister was shown on the acupoints icluding bilateral $BL_{13}$, $_{15}$ and $_{17}$ in every experimental group during 24hr observation following the acupoint applying treatment with pills made by modified and devised prescription. At 4 hr, the effectiveness of it reached a peak showing redness and mild tenderness and there is little difference between groups 3cm pills groups regardless of the way ginger was treated. abdominal distention and growling was found in all the volunteers during the treatment at CV 8. Strategic directions and road maps : Through successful fulfillment of the program preventing winter disease in the summer, Korean traditional medicine should be integrated into mainstream national health care services. Cultural access was thought to be as important as Scientific EBM approach. First of all, To evoke potential cultural homogeneity from campaigns and press advertisement was needed for promoting public awareness about preventing winter disease in the summer by enhancing immunity via acupoint applying treatment in the three dog days, and then indigenous name as Sambokcheop, protocol, Clinical Research Form for data collection of it should be developed and prepared. Once the first step was taken this summer, through a thorough data collection and scrutinized scientific evaluation, drawbacks should be compensted for and the efficacy and safety should be substantiated.

일본 '고증파(考證派)' 의학에 관한 연구 (A Study on The 'Kao Zheng Pai'(考證派) of The Traditional Medicine of Japan)

  • 1.The 'Kao Zheng Pai'(考證派) comes from the 'Zhe Zhong Pai(折衷派)' and is a school that is influenced by the confucianism of the Qing dynasty. In Japan Inoue Kinga(井上金峨), Yoshida Koton(古田篁墩 $1745{\sim}1798$) became central members, and the rise of the methodology of historical research(考證學) influenced the members of the 'Zhe Zhong Pai', and the trend of historical research changed from confucianism to medicine, making a school of medicine based on the study of texts and proving that the classics were right. 2. Based on the function of 'Nei Qu Li'(內驅力) the 'Kao Zheng Pai', in the spirit of 'use confucianism as the base', researched letters, meanings and historical origins. Because they were influenced by the methodology of historical research(考證學) of the Qing era, they valued the evidential research of classic texts, and there was even one branch that did only historical research, the 'Rue Xue Kao Zheng Pai'(儒學考證派). Also, the 'Yi Xue Kao Zheng Pai'(醫學考證派) appeared by the influence of Yoshida Kouton and Kariya Ekisai(狩谷掖齋). 3. In the 'Kao Zheng Pai(考證派)'s theories and views the 'Yi Xue Kao Zheng Pai' did not look at medical scriptures like the "Huang Di Nei Jing"("黃帝內經") and did not do research on 'medical' related areas like acupuncture, the meridian and medicinal herbs. Since they were doctors that used medicine, they naturally were based on 'formulas'(方劑) and since their thoughts were based on the historical ideologies, they valued the "Shang Han Ja Bing Lun" which was revered as the 'ancestor of all formulas'(衆方之祖). 4. The lives of the important doctors of the 'Kao Zheng Pai' Meguro Dotaku(目黑道琢) Yamada Seichin(山田正珍), Yamada Kyoko(山田業廣), Mori Ritsi(森立之) Kitamura Naohara(喜多村直寬) are as follows. 1) Meguro Dotaku(目黑道琢 $1739{\sim}1798$) was born of lowly descent but, using his intelligence and knowledge, became a professor as a Shi Jing Yi(市井醫) and as a professor for 34 years at Ji Shou Guan(躋壽館) mastered the "Huang Di Nei Jing" after giving over 300 lectures. Since his pupil, Isawara Ken(伊澤蘭軒) taught the Lan Men Wu Zhe(蘭門五哲) and Shibue Chusai(澀江抽齋), Mori Ritsi(森立之), Okanishi Gentei(岡西玄亭), Kiyokawa Gendoh(淸川玄道) and Yamada Kyoko(山田業廣), Meguro Dotaku is considered the founder of the 'Yi Xue Kao Zheng Pai'. 2) The family of Yamada Seichin(山田正珍 $1749{\sim}1787$) had been medical officials in the Makufu(幕府) and the many books that his ancestors had left were the base of his art. Seichin learned from Shan Ben Bei Shan(山本北山), a 'Zhe Zhong Pai' scholar, and put his efforts into learning, teaching and researching the "Shang Han Lun"("傷寒論"). Living in a time between 'Gu Fang Pai'(古方派) member Nakanishi Goretada(中西惟忠) and 'Kao Zheng Pai' member Taki Motohiro(多紀元簡), he wrote 11 books, 2 of which express his thoughts and research clearly, the "Shang Han Lun Ji Cheng"("傷寒論集成") and "Shang Han Kao"("傷寒考"). His comparison of the 'six meridians'(3 yin, 3 yang) between the "Shang Han Lun" and the "Su Wen Re Lun"("素問 熱論") and his acknowledgement of the need and rationality of the concept of Yin-Yang and Deficient-Replete distinguishes him from the other 'Gu Fang Pai'. Also, his dissertation of the need for the concept doesn't use the theories of latter schools but uses the theory of the "Shang Han Lun" itself. Ritzi left works on pure investigation, paid much attention to social problems, and through 12 years of poverty treated all people and animals in all branches of medicine, so he is called a 'half confucianist half doctor'(半儒半醫). 5) Kitamurana Ohira(喜多村直寬, $1804{\sim}1876$) learned scriptures and ancient texts from confucian scholar Asaka Gonsai(安積艮齋), and learned medicine from his father Huai Yaun(槐園), He became a teacher in the Yi Xue Guan in his middle ages, and to repay his country, he printed 266 volumes of "Yi Fang Lei Ju"("醫方類聚") and 1000 volumes of "Tai Ping Yu Lan"("太平禦覽") and devoted it to his country to be spread. His works are about 40 volumes including "Jin Qui Yao Lue Shu Yi"("金匱要略疏義") and "Lao Yi Zhi Yan"(老醫巵言) but most of them are researches on the "Shang Han Za Bing Lun". In his "Shang Han Lun Shu Yi"("傷寒論疏義") he shows the concept of the six meridians through the Yin-Yang, Superficial or internal, cold or hot, deficient or replete state of diseases, but did not match the names with the six meridians of the meridian theory, and this has something in common with the research based on the confucianism of Song(宋儒). His first son Tanbano Shigeaki(丹波重明) inherited the Shi Yao Yuan(施藥院) and the third son Tanbano Masatada(丹波雅忠) inherited the Dian You Tou(典藥頭). Masatada's descendents succeeded him for 25 generations until the family name was changed to Jin Bao(金保) and five generations later it was changed again to Duo Ji(多紀). The research scholar Taki Motohiro was in the third generation after the last name was changed to Taki, and his family kept an important part in the line of medical officers in Japan. Taki Motohiro(多紀元簡 $1755{\sim}1810$) was a teacher in the Yi Xue Guan where his father was residing, and became the physician for the general Jia Qi(家齊). He had a short temper and was not good at getting on in the world, and went against the will of the king and was banished from Ao Yi Shi(奧醫師). His most famous works, the "Shang Han Lun Ji Yi"("傷寒論輯義") and "Jin Qui Yao Lue Ji Yi"("金匱要略輯義") are the work of 20 years of collecting the theories of many schools and discussing, and is one of the most famous books on the "Shang Han Lun" in Japan. The younger sibling of Yuan Yin(元胤 $1789{\sim}1827$), Yuan Jian(元堅 $1795{\sim}1857$) expounded ancient writings at the Yi Xue Guan only after he reached middle age, was chosen for the Ao Yi Shi(奧醫師) and later became a Fa Yan(法眼), Fa Yin(法印) and Yu Chi(禦匙). On the Taki family's founding and working of the Yi Xue Guan Yasuka Doumei(矢數道明) said they were "the people who took the initiative in Edo era kampo medicine" and evaluated their deeds in the fields of 'research of ancient text', the founding of Ji Shou Guan(躋壽館) and medical education', 'publication business', 'writing of medical text'. 5. The doctors of the 'Kao Zheng Pai' based their operations on the Edo Yi Xue Guan, and made groups with people with similar ideas to them, making a relationship 'net'. For example the three families of Duo Ji(多紀), Tang Chuan(湯川) and Xi Duo Cun(喜多村) married and adopted with and from each other and made prefaces and epitaphs for each other. Thus, the Taki family, the state science of the Makufu, the tendency of thinking, one's own interests and glory, one's own knowledge, the need of the society all played a role in the development of kampo medicine in the 18th and 19th century.

  • 1. The 'Kao Zheng Pai(考證派) comes from the 'Zhe Zhong Pai' and is a school that is influenced by the confucianism of the Qing dynasty. In Japan Inoue Kinga(井上金娥), Yoshida Koton(吉田篁墩) became central members, and the rise of the methodology of historical research(考證學) influenced the members of the 'Zhe Zhong Pai', and the trend of historical research changed from confucianism to medicine, making a school of medicine based on the study of texts and proving that the classics were right. 2. Based on the function of 'Nei Qu Li '(內驅力) the 'Kao Zheng Pai', in the spirit of 'use confucianism as the base', researched letters, meanings and historical origins. Because they were influenced by the methodology of historical research(考證學) of the Qing era, they valued the evidential research of classic texts, and there was even one branch that did only historical research, the 'Rue Xue Kao Zheng Pai'(儒學考證派). Also, the 'Yi Xue Kao Zheng Pai'(醫學考證派) appeared by the influence of Yoshida Kouton and Kariya Ekisai(狩谷掖齋). 3. In the 'Kao Zheng Pai(考證派)'s theories and views the 'Yi Xue Kao Zheng Pai' did not look at medical scriptures like the "Huang Di Nei Jing"("黃帝內經") and did not do research on 'medical' related areas like acupuncture, the meridian and medicinal herbs. Since they were doctors that used medicine, they naturally were based on 'formulas'(方劑) and since their thoughts were based on the historical ideologies, they valued the "Shang Han Ja Bing Lun" which was revered as the 'ancestor of all formulas'(衆方之祖). 4. The lives of the important doctors of the 'Kao Zheng Pai' Meguro Dotaku(目黑道琢) Yamada Seichin(山田正珍), Yamada Kyoko(山田業廣), Mori Ritsi(森立之) Kitamura Naohara(喜多村直寬) are as follows. 1) Meguro Dotaku(目黑道琢 1739${\sim}$1798) was born of lowly descent but, using his intelligence and knowledge, became a professor as a Shi Jing Yi(市井醫) and as a professor for 34 years at Ji Shou Guan mastered the "Huang Di Nei Jing" after giving over 300 lectures. Since his pupil, Isawara Ken taught the Lan Men Wu Zhe(蘭門五哲) and Shibue Chusai, Mori Ritsi(森立之), Okanishi Gentei(岡西玄亭), Kiyokawa Gendoh(淸川玄道) and Yamada Kyoko(山田業廣), Meguro Dotaku is considered the founder of the 'Yi Xue Kao Zheng Pai'. 2) The family of Yamada Seichin(山田正珍 1749${\sim}$1787) had been medical officials in the Makufu(幕府) and the many books that his ancestors had left were the base of his art. Seichin learned from Shan Ben Bei Shan(山本北山), a 'Zhe Zhong Pai' scholar, and put his efforts into learning, teaching and researching the "Shang Han Lun"("傷寒論"). Living in a time between 'Gu Fang Pai'(古方派) member Nakanishi Goretada(中西惟忠) and 'Kao Zheng Pai' member Taki Motohiro(多紀元簡), he wrote 11 books, 2 of which express his thoughts and research clearly, the "Shang Han Lun Ji Cheng"("傷寒論集成") and "Shang Han Kao"("傷寒考"). His comparison of the 'six meridians'(3 yin, 3 yang) between the "Shang Han Lun" and the "Su Wen Re Lun"("素問 熱論) and his acknowledgement of the need and rationality of the concept of Yin-Yang and Deficient-Replete distinguishes him from the other 'Gu Fang Pai'.
