Influence of Prescribed Herbal and Western Medicine on Patients with Abnormal Liver Function Tests: A Retrospective Quasi-Experimental Study

  • Objectives: The aim of this study was to investigate the safety and the efficacy of Korean herbal, western and combination medicine use in patients with abnormal liver function tests. Methods: We investigated nerve disease patients with abnormal liver function tests who were treated with Korean herbal, western and combination medicine at Dong-Eui University Oriental Hospital from January 2011 to August 2011. We compared aspartic aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP) and total bilirubin (T-bil) levels before and after taking medicine and excluded patients who had liver-related disease when admitted. Results: AST and ALT were decreased significantly in patients who had taken herbal, western medicine. AST, ALT and ALP were decreased significantly in patients who had taken combination medicine. Compare to herbal medicine, AST, ALT and ALP were decreased significantly in patients who had taken western medicine, and ALT and ALP were decreased significantly in patients who had taken combination medicine. There were no significant differences between western and combination medicine. Conclusions: This study suggests that prescribed Korean herbal medicine, at least, does not injure liver function for patients', moreover, it was shown to be effective in patients with abnormal liver function tests.

  • A liver is responsible for drug metabolism in a body. 10% of all medicine side effects lead to liver injury. Herb medicine is not an exception to this rule but all Herb medicines do not lead to liver injury absolutely. As for two times of occurrence of the drug induced liver injury during treatment of a CVA patient, we gave him Herb medicines in conjunction with Western medicines and Western medicines alone for each case. We meet with the result that giving the patient Herb medicines in conjunction with Western medicines reduces the duration of decreasing AST, ALT level as compared with giving him Western medicines alone. So we are now reporting the result.

식도염(食道炎)에 대(對)한 동서의학적(東西醫學的) 고찰(考察) (The study on oriental and western medicine of esophagitis)

  • We arrived at the following conclusions after we have studied esophagitis through the literatures of oriental and western medicine. 1. The western medical causes of acute esophagitis are corrosiveness chemical material, esophageal or gastric disease, trauma, blister stomatitis, filamentous fungus infection and uremia of chronic patient etc, and the oriental medical causes are qi and blood stagnation, blood stasis and stagnation, stagnant phlegm by coldness, heating, dyspepsia and food poisoning etc. 2. The western medical causes of chronic esophagitis are malfunction of lower esophageal sphincter, esophageal tom chink and hernia, increase of gastric pressure by overeating, fatness, pregnancy and ascites etc, and the oriental medical causes are asthenic cardiac qi, hepatic qi attacking stomach by seven kinds of depression, cold-damp stagnation and insufficiency of gastric qi by overeating, excessive drinking and sexual indulgence etc. 3. The main symptoms of acute esophagitis are severe chest pain, instantly vomiting, swallowing pain etc, and chronic esophagitis are occasionally light chest pain, heart bum, anorexia, dysphagia, dizziness, general body weakness etc. These symptoms are come under thoracic obstruction, acid regurgitation, vomiting and chest pain of oriental medicine. 4. The western medical diagnoses of acute and chronic esophagitis have used radiation test, esophageal endoscopy, esophageal pressure test and biopsy etc, and the oriental medical diagnoses have used syndrome differentiation by four examination of inspection, listening and smelling examination, inquiring, pulse-taking and palpitation. 5. The western medical treatments of acute esophagitis have regarded preservation stability of esophagus as a principle, and the oriental medical treatments mainly have used expelling pathogen of expelling cold and regulating qi, cooling and removing stasis, promoting blood circulation to remove blood stasis, eliminating phlegm and regulating qi. 6. The western medical treatments of chronic esophagitis have regarded decrease flowing backward of gastric juice as a purpose, and the oriental medical treatments mainly have used strengthening body resistance of replenishing and strengthening cardioqi, dispersing stagnated hepatoqi, expelling cold and dehygrosis, invigorating stomach and nourishing qi.

급성기 뇌졸중 환자의 한양방 협진치료에 대한 임상적 고찰 (A Clinical Study of Combined Eastern and Western Treatment of Acute Stage Stroke)

  • Object : This is a study of acute stage stroke patients designed for comparison of combined Eastern and Western medical treatments and Oriental treatments alone. Methods : 51 patients were diagnosed by Brain-CT and Brain-MRI scan as having suffered stroke. They had entered Dong-eui hospital within seven days of attack, and remained over seven days, all between November 2002 and August 2003. Patients were divided into two groups: a group treated with both Eastern and Western medical treatments, and a group treated only with Oriental medical treatments (but examinations were done and medications were given in hypertension, Diabetes Mellitus and so on). Results : The Eastern-Western medical treatment group showed significant changes in NIHSS. A rate of improvement figure of 24.94% was obtained for the Eastern-Western medical treatment group, and for the Oriental medical group a figure of 7.84% was obtained. Conclusion : The East-West medical treatment group had significant rate of development that measure for NIHSS by treated comparatively dependent patients. And oriental medical treatment group had significant rate of development that measure for NIHSS by treated comparatively independent patients. The NIHSS measure for the Eastern-Western medical treatment group shows a significant rate of improvement for relatively dependent patients, and the NIHSS measure for the Oriental medical treatment group shows a significant rate of development for relatively independent patients.

요통(腰痛) 환자(患者)의 한방단독치료와 한양방협진치료의 효과 비교 (Comparison of Treatment Effect between Oriental Medicine Theraphy and Oriental and Western Medicine Combination Theraphy on Low Back Pain Patients)

  • Objective: This study was designed to investigate the difference of treatment effect between Oriental Medicine theraphy and Oriental and Western Medicine combination theraphy on low back pain patients. Methods: We divided 40 low back pain patients into 2 groups. The Group I was treated with Oriental Medicine theraphy and the Group II was treated with Oriental and Western Medicine combination theraphy. We evaluated visual analogue scale(VAS) score before and after 3, 5, 7 days of each treatment. Results: The VAS score of Group I and Group II after 3, 5, 7 days of each treatment was significantly lower than before treatment. But there was no significant difference between two groups after 3, 5, 7 days of each treatment. Conclusions: In this study, there was no significant difference between the two groups after 3, 5, 7 days of each treatment on low back pain patients. Further studies are needed for the comparison of Oriental Medicine theraphy and Oriental and Western Medicine combination theraphy.

한약-양약 병용 투여에 관한 논문 동향 분석 (Analysis of Studies on Combined Medication of the Oriental and the Western Medicines)

  • Objectives : In this study, we survey papers about the combined therapy of oriental medicine and western medicine in oriental medicine viewpoint. Methods : First, we collected research papers on combined treatment, and we found 70 papers in the two internet sites, 'OASIS' and 'KISTI', using the keywords 'combined treatment', 'collaborated diagnosis', 'oriental-western medicine', and 'western-oriental medicine'. Then we analyzed them according to year, oriental-western medicine, diseases, research methods and subjects. Results & Conclusions : Researches for combined therapy had some problems as follows : First, studies have been made only for limited diseases like cancer, infection, stroke, etc. Second, methods of research are also limited. Some methods such as in vivo, in vitro and case-report are used repeatedly. Third, only the positive aspects of the combined therapy are proposed. We don't find the side effects of the combined therapy at all. To solve these problems, more objective and balanced studies such as RCT for various diseases should be performed.

일부 한.양방병원 뇌혈관질환 환자의 진료결과 및 만족도의 비교연구 -한양방협진 진료프로토콜의 적용을 중심으로- (The Clinical Interchange between Western Medicine and Oriental Medicine: with the Stroke Patient Outcomes Research)

  • Objectives : This study was done to assess the effects of the clinical interchange between the Western Medicine and the Oriental Medicine for ischemic stroke patients. The patient outcomes include changes in neurologic function by modified NIH stoke scale, stroke pattern identification scale, and patient satisfaction, Methods : For the assessment of effects, this study was performed with 178 inpatients who had undergone the stroke care at three hospitals (W Hospital adopted western therapy, S Oriental Hospital adopted Sasang constitution medicine therapy, and H Oriental Hospital adopted mixed therapy according to a joint protocol on Western Oriental medical care) from November 1997 to December 1998. Patients were interviewed or written with self-entered questionnaire forms, and clinical data were obtained, Physicians or oriental doctors wrote clinical questionnaire forms according to the care process. Results : The patient outcomes within three hospitals at 2 stages (at admission and discharge in the modified NIH stroke scale. at admission and second weeks during admission in the stroke pattern identification scale) were found to be decreased, Especially in the results of hierarchical multiple regression analysis, the degree of improvement of modified NIH stroke scale of the stroke patients at W Hospital was significant large than it at S Oriental Hospital. Also, the degree of improvement of stroke pattern identification scale at W Hospital was significantly large than it at other two hospitals. However, the patient's satisfaction score at three hospitals wasn't significantly different. Conclusions : The result of this study suggested that the joint clinical research of Western & Oriental medical practitioners was possible even if there was a conflict between Western Medicine and Oriental Medicine. Therefore Western & Oriental medical practitioners share a mutual responsibility to apply evidence-based practice, to seek scientific empirical proof through randomized clinical trials between the multicenter.

  • Good health and longevity is the goal of huamn beings. Recently, 'Dietary treatment' has become influential as one of the means for it in western medicine. Whereas in oriental medicine, 'Dietary treatment' was not recognized as therapeutic method but care of health. in this paper, the viewpoints of 'Dietary treatment' in oriental and western medicine was compared and searched for new possibilities in oriental medicine. And the results were as follows. 1. In oriental and western medicine, food was obviously recognized as a source of nourishment, and moreover oriental medicine took even a human soul into consideration. 2. Western medicine made much of nourishment and was analytical and therapeutic-centered. on the other hand, oriental medicine took a serious view of prevention and care of health. 3. Oriental medicine considered that intake of food was a adoption of Gi(氣) and then it helped a circulation of Gi and beneficial for the production of Jeong(精). 4. The principles of diet in oriental medicine was reasonable combination of food, balance of Oh-Mi(五味), temperance of food and intake by physical constitution.

한양방 협진에 대한 한의대생들의 인식도 (A Study on the Recognition of Students of Oriental Medical School on Cooperative System between Oriental and Western Medicine)

  • The purpose of this study is to examine the recognition of the student of oriental medical school on cooperative system between Oriental and Western medicine and to provide basic information for the development of oriental medicine. In order to look at the level of recognition on cooperative system between Oriental and Western medicine, from September 1st to 15th of 2005, this study had been conducted through personal interview and questionnaires to 600 students who were attending the Department of Oriental Medicine (in both prep and regular courses) in D University, located in the City of Daegu. The data has been analyzed using statistic program, the SPSS WIN 12.0. Statistical analysis tools used for this study were frequency analysis, cross-over analysis and the t-test. The results are as follows; The students of oriental medical school had relatively high level of recognition on the basic concept, interests, necessity and potential for cooperative system between Oriental and Western medicine. However, they had negative understandings on the issue of unification of the two medical systems and it's possibility in the future. The students were optimistic about possible merits of cooperative system between Oriental and Western medicine, especially in the field of rehabilitation. On the reasons for lack of development of cooperative system between Oriental and Western medicine, the students listed prejudices existing on both sides firstly, followed by their contrasting approach on human illness, lack of legal and institutional support system, the indifferences of doctors and indifferences of oriental medical doctors. In addition, students understood that the dualism existing in our current medical system is aggravating mutual distrust between the two sides, causing the confusion of patients on the choice of medical facilities, and raising their medical bills. Therefore, in order to vitalize collaboration between Oriental and Western medicine in new health care environment, the following measures should be needed decreasing the prejudices between the two medical spheres with open mind improving educational programs in Western and Oriental medical schools; promoting joint academic research or exchange programs between the schools, and increasing government effort to minimize legal and institutional restrictions cooperative system between Oriental and Western medicine.

Helicobacter pylori의 한약 치료에 대한 연구 (A Study of Oriental Herbal Medicine on the Treatment of Helicobacter Pylori Infections)

  • Objectives : Many studies have shown that helicobacter pylori (H. pylori) infection is associated with gastroduodenal diseases. The purpose of this report was to evaluate recently published research on the influence of oriental herbal medicine on H. pylori infections. Methods : Recently published literature were systematically compared with their findings of how oriental herbal medical treatment affects H. pylori-associated disease. Results : The eradication rate of H. pylori in oriental herbal medicine groups was 66.93% while it was 66.02% in western medicine groups. In oriental herbal plus western medicine groups, interestingly, the rate increased to 84.78%. On the other hand, the total treatment efficacy rate of H. pylori in oriental herbal medicine groups was 91.27%. The treatment efficacy rate in oriental herbal plus western medicine groups rose to a record 93.22%, which was 15.34% higher than the rate in western medicine groups. In addition, the rate of adverse effects was 2.71%, 4.85%, 15.80% in oriental herbal medicine, western medicine, and oriental herbal plus western medicine groups, respectively. Diarrhea was most frequently observed in oriental herbal medicine groups, while nausea was most frequently observed in the other groups. Conclusions : The results of this study showed that herbal medicinal treatment can increase the rate of H. pylori eradication and improve H. pylori-related gastrointestinal symptoms. These findings suggest that herbal medicine can solve the problems including side effects due to antibiotic resistance of standard triple therapy.
