A therapeutic following confirming symptom(辨證施治) has a proper characteristic in oriental medicine, which can be grasped but by pathological body reflection not by histological or molecular level etiology. It is also able to be selected a prescription by means of clinical trials which promote human body's natural resurgent. Whereas, a therapeutic regimen prescription(定病傳方) has a merit in prescribing a disease to the point. which has been developed and established a therapy method naturally for thousands years. Then, which of the two is the better method in clinic? The answer is a co-exist of the two therapy methods in a matter of developing deciding therapeutic cooperation. To tie a knot, two methods need the opposite one.

1. Objective: Gastric Stuffiness (心下痞證) in the Soeumin Taeeum symptomatology (少陰人 太陰病) in Sasang Constitutional Medicine shows an interesting evolutionary process. Originally found in the texts of Sanghanlun (傷寒論), the perspectives regarding the symptoms of Gastric Stuffiness were expanded by many medical scholars and Lee Jema. To comprehend the Jema's concept of Taeeum symptomatology and utilize his prescription clinically, it deserves to study on the Gastric Stuffiness in Donguisuseowon (東醫壽世保元). 2. Methods: Many texts including Sanghanlun (傷寒論) and Dongeuibogam (東醫寶鑑) were reviewed in order to track the evolutionary process concerning the symptoms of Gastric Stuffiness. Also, Lee Jema's works including the different versions of Donguisuseowon (東醫壽世保元) were reviewed to examine the development of Lee's concepts regarding the Gastric Stuffiness in the Soeumin Taeeum symptomatology. 3. Results and Conclusions: 1) Most of the previous concepts on Gastric Stuffiness were generally similar to those found in Sanghanlun, especially in that they addressed Yang deficiency of the middle triple energizeer as the central pathogenic process. 2) Lee Jema's views on the epigastric discomfort in the Soeumin Taeeum symptomatology evoloved over time and culminated in the finalized pathology and therapeutics appearing in his last work, the Shinchuk version of Donguisuseowon (東醫壽世保元). 3) 4 kinds of prescription which invented by Lee Jema to treat the Gastric Stuffiness in the Soeumin Taeeum symptomatology can be matched to the prescription of Sanghanlun respectively. However through evolutionary process of Jema's prescriptions, the indication of them expanded: not only to treat Gastric Stuffiness but to diverse symptoms of Soeumin.

In the recent, increased concern has been focused on the pharmacology and clinical utility of herbal extracts and derivatives as a drug or adjunct to chemotherapy and immunotherapy. Salicis Radicis Cortex, A decoction has been mainly used for improvement of ozena and a diuretic effect in oriental medicine, but there was no study on the molecular mechanism of Salicis Radicis Cortex as an immunomodulator. Here we investigated the role of the aqueous extract of Salicis Radicis Cortex in the expression of inflammatory mediators, surface molecule, and related receptors in vitro and in vivo. In murine macrophage RAW 264.7 cells and peritoneal macrophages of C57BL/6N mice, water extract of Salicis Radicis Cortex increased the production of secretary TNF-alpha and Nitric oxide, and the expression level of CD14, LPS co-receptor and CD86, co-stimulatory molecule compared to negative natural extract ex vivo. Moreover, i.p. injection of water extract of Salicis Radicis Cortex significantly increased the secretion level of IFN-gamma and TNF-alpha, IL-2, IL-4 and IL-5 in serum of mice in vivo. Taken together, these results suggest that Salicis Radicis Cortex may regulate the immune response by secreting Th1 and Th2 types of cytokines in vivo and the possibility of its as natural immunostimulator.

Background Clinical practice guidelines (CPG) are systematically developed statements to assist practitioners and patients on healthcare decisions. They provide recommendations for the average patient, which should take into account individual clinical judgment and the patient's values and expectations. Ephedra has sympathomimetic effect and has been used for weight loss worldwide. However, its safety is controversial especially in autonomic and cardiovascular systems. Therefore, the need of appropriate CPG for ephedra prescription in obesity was advocated in Korean Traditional Medicine. Methods The committee comprised of specialists of obesity, oriental herbology, oriental cardiology, constitutional medicine. The committee collected all relevant references about adverse effect and safety of ephedra in the forms of meta-analysis, systematic review, randomized controlled trial, case-control study and observational study from international and domestic databases and paper journals. 11 English- and 5 Korean-language references were gathered and categorized by PICO (Patient, Intervention, Comparison, Outcome) method. We could not complete strength of recommendation which should be clarified according to the evidence grade estimation. Result The first version of CPG for ephedra prescription in obesity was issued by Korean Oriental Association for Study of Obesity. It includes topics of introduction, pharmacokinetics, side effects and adverse events, constitutional aspect and recommendations for dose, indication and contraindication. Conclusion There should be periodic upgrade of this CPG from now on. Although there are some drawbacks in this version of CPG, it has significance as the first CPG in Korean Traditional Medicine.
Objectives : The purpose of this study was to investigate the interpretation of the Gwibi-tang(歸脾湯, GBT) prescription in order to obtain the traditional significance. Methods : We have analyzed the interpretation on the GBT prescription through translations and comparisons based on classic books about the oriental medical prescriptions. Results : 1. The source of GBT in Dongeuibogam(東醫寶鑑) is Guchiryuyo(口齒類要), Jeongcheryuyo(正體類要) and Gyojubuinyangbang(校註婦人良方) etc. 2. Meaning of "GwiBi(歸脾)" in the GBT name is returning to the Bi(脾) which is origin of generating Gi(氣) and Blood(血). 3. GBT is composed of Sagunja-tang(四君子湯), Hwanggi-tang(黃芪湯) and Danggwibohyeol-tang(當歸補血湯) etc. 4. According to wide range of symptom, GBT can be varied by adding or subtracting herbs. Conclusions : In this study, we have demonstrated various methodologies. This paper will be useful to the future researchers and clinicians to conduct a study on herbal medicines such as the GBT.

In order to investigate the therapeutic effect of SSC, NC/Nga animal model resembling the AD-like symptoms were used to measure the changes in cytokines and histology. SSC prescription group showed significant decrease in the atopic dermatitis clinical index by 40.2% compared to that of the control. The SSC prescription had significant effect on immune cells that are related to inducing AD symptoms. SSC prescription also increased the ratio of immune cells in DLN that were not directly involved in AD symptoms. SSC prescription group showed significant decrease in the level of cytokines within spleen cells and DLN. The prescription also decreased the level of immunoglobulin IgE levels in serum by 25.3%. The thickness of epidermis and dermis as well as the precipitation of erythrocytes were also observed. The results indicate the therapeutic effect of SSC in the treatment of atopic dermatitis through immune modulation. The study will provide a broader applications in the treatment of atopic dermatitis. Particularly, skin regeneration effect and supplemental use of topical application of SS in atopic dermatitis treatment had been reported previously, and further investigation on the dose dependent effect as well as skin irritation studies of SS should be followed.
Salt is a vital inorganic substance to human body and is seasoning in food. It is an absolute alternating factor on fermented food. What's more, in the oriental medicine, it is widely used to raise the pharmacological effect as a component of a prescription when processing a medicine, and when taking the medicine. Though, Importance of salt is often unaware and it is not used in the right way. focusing on "東醫寶鑑 內景篇 Donguibogam-Naegyeong" I categorized the methods of salt's prescription, dose, processing, seasoning, etc. The contents were compared and studied as well. When salt is used as a medicine, It is used as a main medicine, complimentary(aid) medicine, Jjim-Jil(hot towel or bath etc), vomiting medicine and gargling water. when taking the salt water with other medicine, depending on the type of the medicine, boiled salt water, warm alcohol or salt water, mixing powder medicine with salt water etc The methods are diverse. The reason to process medicine with salt or salt water is to bring the pharmacological Qi(energy) down to increase remedial value. When processing a medicine, kneading dough with salt water and the case using food ingredient as medicine salt was used as seasoning. Kneaded mud with salt were used to cover medicine or on the outer surface. This proves that it prevents the dryness and helps the medicine cooked even. Like this, salt is vital and highly-valued medicine in the oriental medicine. learning the right method of using salt and if it were used properly It is considered that it would increase the Pharmacological effect.
