Risk Factors of On-Pump Conversion during Off-Pump Coronary Artery Bypass Graft

  • Background: Off-pump coronary artery bypass grafting (OPCABG) procedures can avoid the complications of an on-pump bypass. However, some cases unexpectedly require conversion to cardiopulmonary bypass during OPCABG. The risk factors associated with a sudden need for cardiopulmonary bypass were analyzed. Methods: This retrospective study included 283 subjects scheduled for OPCABG from 2001 to 2010. These were divided into an OPCABG group and an on-pump conversion group. Preoperative, operative, and postoperative variables were compared between the 2 groups. Results: Of the 283 patients scheduled for OPCABG, 47 (16%) were switched to on-pump coronary artery bypass grafting (CABG). The mortality of the both the OPCABG and on-pump conversion groups was not significantly different. The major risk factors for conversion to on-pump CABG were congestive heart failure (CHF) (odds ratio [OR], 3.5; p=0.029), ejection fraction (EF) <35% (OR, 4.4; p=0.012), and preoperative beta-blocker (BB) administration (OR, 0.3; p=0.007). The use of intraoperative (p=0.007) and postoperative (p=0.021) inotropics was significantly higher in the conversion group. The amount of postoperative drainage (p<0.001) and transfusion (p<0.001) also was significantly higher in the conversion group. There were no significant differences in stroke or cardiovascular complications between the groups over the course of short-term and long-term follow-up. Conclusion: Patients who undergo OPCABG and have CHF or a lower EF (<35%) are more likely to undergo on-pump conversion, while preoperative BB administration could help prevent conversions from OPCABG to on-pump CABG.

  • Background: Off-pump coronary artery bypass (OPCAB) is performed worldwide, but significant risks are associated with conversion to on-pump surgery. Therefore, we evaluated the composite outcomes between an OPCAB group and a conversion group. Methods: From January 2008 to December 2012, 100 consecutive patients underwent OPCAB at Hallym University Sacred Heart Hospital, of whom 84 underwent OPCAB without adverse events (OPCAB group), and 16 were converted to on-pump surgery (conversion group). Early morbidity, early and long-term mortality, and major adverse cardiac and cerebrovascular events (MACCEs) were the primary and long-term composite endpoints. Results: The mean follow-up period was $55{\pm}26months$, with 93% of the patients completing follow-up. The composite outcomes in the OPCAB and conversion groups were as follows: early morbidity, 2.3% versus 12.5%; early mortality, 4.7% versus 0%; long-term mortality, 14.3% versus 25.0%; and MACCEs, 14.3% versus 18.8%, respectively. No composite endpoints showed statistically significant differences. Preoperative acute myocardial infarction (AMI) was identified as an independent risk factor for conversion (p=0.025). Conclusion: The conversion group showed no statistically significant differences in early mortality and morbidity, MACCEs, or long-term mortality compared with the OPCAB group. The preoperative diagnosis of AMI was associated with an increased number of conversions to on-pump surgery.

  • In this paper, the similarity criterion and dimensionless conversion method combined with the elasticity condition and Hooke's law are used to derive the functional relationship of the maximum effective value of the vibration velocity between the prototype pump and the model pump. The seawater circulation pump of a nuclear power plant is used as the prototype pump, and the model pump is obtained by performance conversion and choosing the appropriate scale, and the vibration state of the model pump under different flow rates is measured and analyzed. The vibration data of the model pump through the function relationship to find out the vibration parameters of the prototype model pump, and compare with the vibration data of the seawater circulation pump in reality. It can be seen that with the increase of flow rate, the maximum effective value of the vibration velocity of both model and prototype decreases and then increases, and the relative error is small, the maximum value is 7.7757%. Therefore, it can be considered that the functional relationship of model pump converted to prototype pump derived in this paper can be used to analyze the vibration of the actual seawater circulation pump of coastal nuclear power plant.

Microfluidic Suction Pump based on Restoring Force of Elastomer for Liquid Transportation in Microfluidic System (미세유체시스템의 유체이송을 위한 탄성체의 복원력을 이용한 흡입형 미세유체펌프)

  • This paper presents a disposable passive suction pump that uses the restoring force of an elastomeric chamber for liquid transportation in a microfluidic system. The proposed suction pump can be operated by finger pressure without any peripheral equipment. To adjust the generated suction pressure, five different displacements of the suction chamber ceiling, two different chamber shapes, and five different elastic moduli of the elastomer were considered. For a cylindrical chamber with a 5 mm height and 5 mm radius, the generated suction pressure and flow rate increased almost linearly up to about 31 kPa and $160.8{\mu}L/min$, respectively, depending on the chamber deformation. A maximum suction pressure of $42.9{\pm}0.7kPa$ was obtained for a hemispherical chamber with a 2.1 mm height and 5 mm radius.

Chromatic Dispersion Compensation via Mid-span Spectral Inversion with Periodically Poled $LiNbO_3$ Wavelength Converter at Low Pump Power

  • Mid-span spectral inversion (MSSI) has to utilize high optical pump power, for its operation principle is based on a nonlinear optical wavelength conversion. In this paper, a low pump-power operation of MSSI-based chromatic dispersion compensation (CDC) has been achieved successfully, for the first time to our knowledge, by introducing a noise pre-reduction scheme in cascaded wavelength conversions with periodically poled $LiNbO_3$ waveguides at a relatively low operation temperature. As preliminary studies, phase-matching properties and operation-temperature dependence of the wavelength converter (WC) were characterized. The WC pumped at 1549 nm exhibited a wide conversion bandwidth of 59 nm covering the entire C-band and a conversion efficiency of -23.6 dB at 11 dBm pump power. CDC experiments were implemented with 2.5 and 10 Gb/s transmission systems over 100 km single-mode fiber. Although it is well-known that the signal distortion due to chromatic dispersion is not critical at a 2.5 Gb/s transmission, the clear recovery of eye patterns was identified. At 10 Gb/s transmission experiments, eye patterns were retrieved distinctly from seriously distorted ones, and notable improvements in bit-error rates were acquired at a low pump power of 14 dBm.

  • We demonstrate wavelength conversion of 2.5Gb/s optical signals by four-wave mixing (FWM) in a semiconductor optical amplifier (SOA). We investigate the effect of input pump and signal powers on the coversion efficiency, optical signal-to-noise ratio (OSNR) and extinction ratio to be a measure of performance in a wavelength converter. As a result, we show that the maximum bit error rate (BER) performance can be obtained by co promising among high-vonversion efficiency (minimum Pprobe), high-OSNR (maximum Pprobe) and low-cross-gain saturation effects (Pprobe kept at least 6dB weaker than Ppump). In our experiment, we obtain optimum performance at +3 dBm pump power and -6dBm signal power. The power penalty incurred in the wavelength conversion can be minimized by careful selection of the input pump and signal powers. We show that about 0.5dB power penalty for 3.2nm wavelength coversion at 10-10 BER is achievable.

  • The Prouductivity of pump dredger varies very much depend on soil conditions is one of the main factors affecting construction period. Therefore the acculate evaluation of dredging capability of pump dredger considering soil conditions is a main task in dredging projects. After assesing the results of pump dredging on several Gwangyang port projects, some data of Japan standard for pump dredger are similar to those of actual results and a pump dredging capability table is prepared for future evaluation purposes.

Efficiency Increase and Input Power Decrease of Converted Prototype Pump Performance

  • The performance of a prototype pump converted from that of its model pump shows an increase in efficiency brought about by a decrease in friction loss. As the friction force working on impeller blades causes partial peripheral motion on the outlet flow from the impeller, the increase in the prototype's efficiency causes also a decrease in its input power. This paper discusses results of analyses on the behavior of the theoretical head or input power of a prototype pump. The equation of friction-drag coefficient for a flat plate was applied for the analysis of hydraulic loss in impeller blade passages. It was revealed that the friction-drag of a flat plate could be, to a certain degree, substituted for the friction drag of impeller blades, i.e. as a means for analyzing the relationship between a prototype pump's efficiency increase and input power decrease.

Single-photon Detection at 1.5 ㎛ Telecommunication Wavelengths Using a Frequency up-conversion Detector (주파수 상향변환 검출기를 이용한 1.5 ㎛ 통신파장대역의 단일광자 측정)

  • We present a low jitter frequency up-conversion detector based on quasi-phase matched sum frequency generation in a periodically poled $LiNbO_3$ waveguide for efficient single-photon detection at 1.5 ${\mu}m$ telecommunication wavelengths. The maximum detection efficiency and the noise count rate using the pump power of 300 mW and the pump wavelength of 974 nm are about 7% and 480 kHz, respectively. We also characterize the timing jitter of the frequency up-conversion detector by analyzing the time distribution of the detection outputs for photons generated through a picosecond pump pulsed spontaneous parametric downconversion. The minimum timing jitter was measured to be about 39.1 ps. Coincidence measurement with a narrow time window for pulsed up-conversion photons can eliminate the unwanted noise counts and maximize signal to noise ratio.

  • A new energy conversion system which converts wind energy directly to heat instead of transforming it to electric power beforehand is suggested in this study. The new energy conversion system is composed of two big parts divided by their functions. One of them is a wind turbine part, and another is the heat energy conversion part. The object of this study is confined only to the heat energy conversion part, so the wind turbine is replaced with an electric motor for the convenience of experiment. In the experimental process, pressure difference at the hydraulic pump, revolution speed of the hydraulic pump, temperature at a few points on the oil circuit and the water circuit are measured at time intervals of five minutes. And integral values of input energy to the system and stored energy in the system is investigated.
