The helideck structure must satisfy the safety requirements associated with various environmental and accidental loads. Especially, there have been a number of fire accidents offshore due to helicopter collision (take-off and/or landing) in recent decades. To prevent further accidents, a substantial amount of effort has been directed toward the management of fire in the safety design of offshore helidecks. The aims of this study are to introduce and apply a procedure for quantitative risk assessment and management of fires by defining the fire loads with an applied example. The frequency of helicopter accidents are considered, and design accidental levels are applied. The proposed procedures for determining design fire loads can be efficiently applied in offshore helideck development projects.

On offshore platforms, oil and gas leaks are apt to be the initial events of major accidents that may result in significant loss of life and property damage. To prevent accidents induced by leakage, it is vital to perform a case-specific and accurate risk assessment. This paper presents an integrated method of Ddynamic Qquantitative Rrisk Aassessment (DQRA)-using the Decision Making Trial and Evaluation Laboratory (DEMATEL)-Bayesian Network (BN)-for evaluation of the system vulnerabilities and prediction of the occurrence probabilities of accidents induced by leakage. In the method, three-level indicators are established to identify factors, events, and subsystems that may lead to leakage, fire, and explosion. The critical indicators that directly influence the evolution of risk are identified using DEMATEL. Then, a sequential model is developed to describe the escalation of initial events using an Event Tree (ET), which is converted into a BN to calculate the posterior probabilities of indicators. Using the newly introduced accident precursor data, the failure probabilities of safety barriers and basic factors, and the occurrence probabilities of different consequences can be updated using the BN. The proposed method overcomes the limitations of traditional methods that cannot effectively utilize the operational data of platforms. This work shows trends of accident risks over time and provides useful information for risk control of floating marine platforms.

The aim of this study is to research attributes of fishermen's occupational accidents for investigating the measure of risk control on situational condition in the Korean offshore and coastal fishing vessel. Using data of fishermen's occupational accidents are from National federation of fisheries cooperatives for 2013. The results were as belows; Occupational accident occurrence rate was 29.5‰, slips & trips and struck by object and contact with gear were shown severe occurrence pattern. Occupational accident occurrence rate of offshore fisheries was 130.2‰, coastal was 16.9‰, specially the risk rates were severely high in several type of danish seine, stow net and offshore trap. Death rate by accidents was 10.6‰ and by fall into the water in occurrence pattern was 5.5‰.

The shipbuilding and offshore industry is a large and complex assembly industry, which causes many safety accidents. The major accidents in the shipbuilding and offshore industry workplaces are stenosis, falling objects, dust, fire, explosions, and gas poisoning. The accident by worker in this industry mainly has three factors: frequent movement, narrow work space, and increased use of subcontractors. To control these factors, it is necessary to monitor the worker's location and work status. In this paper, a worker location monitoring system using inaudible sound wave was designd that can be used in environments with many metal barriers. The process included deriving stakeholder requirements, transforming to system requirements, designing system architecture, and developing prototype. The prototype was validated by third-party testing agency. As a result, it satisfied the designed performance and verified its feasibility.

Release of hazardous and flammable gas is a significant contributor to risk. The ignition of flammable gas clouds can lead to explosion accidents in the offshore installations. A gas detector, which is one of active protect systems, brings the module into a safe state through emergency shut down processes and reduces the damage by eliminating the dangerous releases. It is critical to understand the gas release, the wind field, and the complex geometry of installations to determine gas detector placement. In this paper, the Gas detector Location Index (GLI) which is a novel index for optimal detector location determination to efficiently prevent explosion accident using probabilistic approach.

In spite of adoption of Intact Stability Code Part A as mandatory, the accidents have been occurred frequently caused by dynamic stability. Therefore International Maritime Organization (IMO) is revising the Intact Stability Code to prevent the cause of accidents. The result of 4th meeting of Sub-committee on Ship Design and Construction (SDC), five mode of the cause was defined and establishing each draft regulations. Recently we have been facing a number of safety accidents under various environments. Currently, if the people encountered a risk during maritime activities in offshore, rescue efforts have been made only rely on the human perspective and experience. As a result, there are a lot of deaths and disappearances accidents caused by accidents at offshore. These accidents are mainly seen to occur because of the delay in the rescue. In order to reduce the accident according to this rescue delay, we carried out a study on the designing a wireless transmitter and receiver devices that can be used easily with latest IoT technologies including GPS, RF, LoRa wireless communication technology for a rapid rescue.

The International Labor Organization (ILO) selected fishing as the most dangerous group of jobs in the world, and it is well known in Korea as a 4D industry. Offshore large purse seine accounts for the largest portion of the fishing industry in the coastal region with high death rate and the accident rate. The repeated disaster rate survey by the Korea Maritime Institute (KMI) shows offshore large purse seiner is the highest at 22.3 percent and coastal gill nets and offshore stow net are following at 12.4 percent and 11.9 percent in order. Therefore, risk factors occurring in offshore large purse seiners were analyzed based on data from the National Federation of Fisheries Cooperatives (NFFC) for three years from 2015 to 2017 and 4M (Man, Machine, Media, Management) 3E (Engineering, Education, Enforcement) techniques were used to provide a safe fishing environment. The number of accidents on offshore large purse seiners each year was more than 150, and the number of accidents on every fishing boats was as high as 17 percent in 2015. If the accident rate and the risk level were divided by insurance, the accident resulting from contact with machinery was the highest, and the risk of a contact with gear was low but frequently occurring. This was caused by collisions and contact with gear in situations where death and disappearance risk are not high, and accident types in situations where death and loss risk are considered to be contact with machinery, falls or other accidents. Through these analysis techniques, the frequency and risk of each type of accident on a offshore large purse seiners can be demonstrated, and it is expected to raise awareness of a safer fishing environment and contribute to the reduction of accidents.

The hydrocarbon gas leak in the offshore plant can cause large accidents and lead to significant damages to human, property and environment. For prevention of fire or explosion accidents from gas leak, a SIS(Safety Instrumented System) should be installed. In the early stage of the offshore design, required SIL(Safety Integrated Level) is determined and reliability analysis is performed to verify the design in reliability aspects. This study collected data, information related to reliability analysis and created knowledge model of safety design for the offshore system with MBSE(Model-Based Systems Engineering) concept. Knowledge model could support safety engineer's design tasks as the guidance of reliability analysis procedure of safety design and make good conversation with other engineers in yard, class, company, etc.

The offshore installations and ships are the structures most likely to be exposed to hazards such as hydrocarbon fire and/or explosion. Developing proactive measures to prevent the escalation of such events thus requires detailed knowledge of the related phenomena and their consequences. $CO_2$ extinguishing systems are extensively used for fire accidents of on-and offshore installations because of outstanding performance and low cost. There is, however, the risk of carbon dioxide system which enumerates many of the fatalities by suffocation associated with industrial fire protection requirements. Therefore, the aim of this study is to perform the prediction of fire suppression characteristics of the carbon dioxide system in realistic enclosed compartment area of ships and propose $CO_2$ extinguish fire fighting system for preventing suffocation accidents during fire fighting. According to CFD calculations, it can be observed and assessed that various fire profiles with $CO_2$ and $O_2$ mole fraction in the target enclosed compartment area are applicable within the proposed system. Additionally, the design of fire safety system of ships and offshore installations can utilize ventilation system and/or layout arrangement through the proposed system.
