한국판 아동용 Leyton 강박증 척도의 개발 (DEVELOPMENT OF THE KOREAN FORM OF LEYTON OBSESSIONAL INVENTORY-CHILD VERSION(LOI-CV))

  • 소아의 강박증 평가 도구인 아동용 레이톤 강박증 척도를 한국판 집단용 지필 검사로 개발하기 위하여 본 연구를 시행하였다. 국민학교 4학년부터 고등학교 3학년까지 127명을 대상으로 한국어로 번역한 지필 검사 양식의 아동용 레이톤 강박증 척도를 실시하였고, 검사-재검사 신뢰도 검증을 위하여 3주뒤에 82명에게 재검사를 시행하였다. 또한 전 대상 군에 신뢰도와 타당도가 이미 입증된 간이진단검사(SCL-90-R)를 함께 실시하여 공존타당도가 있는 지를 알아 보았고, 강박환아군과 정신과 환아대조군 그리고 정상대조군을 9명씩 비교하여 진단 변별력이 있는 지를 보았다. 검사-재검사 신뢰도, 내적 일관성 등은 상당히 만족스러운 수준이었고, 간이진단검사와의 공존 타당도도 비교적 높게 나왔다. 강박환아군이 정신과 환아대조군이나 정상 대조군보다 유의하게 높은 점수가 나와서 변별력 또한 높은 것으로 나타났다. 요인분석상 문항들을 5개의 요인으로 묶을 수 있었다. 한편 증상과 저항척도에서 여아가 남아보다 유의하게 높은 점수를 보고하였으며, 특히 국민학교 여아에서 높은 점수를 보여 주었다. 결론적으로 한국판 아동용 레이톤 강박척도는 실시하기 간편하고, 신뢰도와 타당도가 좋은 척도임이 입증되었다. 향후 전국 규모의 규준을 작성하고, 미흡한 부분을 보완한 후속 연구가 필요하다.

예비창업가의 열정이 창업의도에 미치는 영향 (The Effect of the Passion of Preliminary Entrepreneurs on Entrepreneurial Intention)

  • 최근 경제 부흥정책의 하나로 청년들의 창업에 대한 국가차원의 장려가 지속됨에 따라 청년창업에 대한 투자와 청년들의 지원이 증가하고 있다. 창업의도는 창업으로 이어지는 직접적인 요인으로 많은 연구들이 창업의도에 영향을 미치는 기업가의 개인적 특성에 대해서 연구하였다. 같은 맥락에서 본 연구의 목적은 창업의도에 영향을 미치는 요인으로서 창업가의 열정을 조화 열정과 강박 열정으로 나누어 조사하였다. 또한 열정에 영향을 미치는 창업가의 개인적 특성인 성취욕구, 감정, 주관적 규범을 검증하였다. 이를 위해 예비창업가를 대상으로 온라인 설문조사를 실시하였으며, 202명의 데이터를 수집하여 구조방정식 분석을 하였다. 분석결과 예비창업가의 성취욕구, 긍정적 감정은 조화 열정에 정(+)의 영향을 미치는 것으로 나타난 반면, 부정적 감정과 주관적 규범은 조화 열정에 유의미하지 않았다. 성취욕구는 강박열정에 부(-)의 영향을 보였고, 긍정적 감정과 부정적 감정은 강박 열정에 정(+)의 영향을 미쳤으나 주관적 규범은 강박 열정에 유의미하지 않았다. 조화 열정과 강박 열정은 창업의도에 정(+)의 영향을 나타내는 것을 확인하였다. 또한 조화 열정과 강박 열정이 성취욕구, 긍정적 감정, 부정적 감정과 창업의도 사이에서 매개효과가 있는지를 확인하였고, 그 결과 성취욕구, 긍정적 감정과 창업의도 사이에서 매개효과가 나타났고, 특히 성취욕구는 완전매개효과가 있었다. 긍정적 감정과 창업의도간의 관계에서 강박 열정 또한 매개효과가 확인되었다.

한방치료와 심리치료를 통한 강박장애 환자 치험례 (A Case Report of a Patient with Obsessive-Compulsive Disorder (OCD) Treated by Korean Medical Treatments and Psychotherapy)

  • Objectives The purpose of this study is to report that a patient with obsessive-compulsive disorder (OCD) significantly responds to Korean medical treatments and psychotherapy.Methods We treated the patient with Korean medical treatments (acupuncture and herbal medication) and Melonis Calyx Vomiting Therapy. We also used psychotherapy including Li-Gyeung-Byun-Qi therapy. The patient was diagnosed by DSM-IV diagnostic criteria for OCD. We used Obsessive-Compulsive Inventory-Revised-Korean (OCI-R-K), Visual Analogue Scale (VAS), Beck’s Depression Inventory (BDI), and Beck’s Anxiety Inventory (BAI) to evaluate the patient.Results OCI-R-K score was decreased from 39 points to 9 points. BDI score was decreased from 14 points to 6 points. BAI score was decreased from 25 points to 13 points.Conclusions This study suggests that Korean medical treatment is an effective way to treat patients who suffer from OCD.

중학교 일학년 학생에서 모즐리 강박척도를 이용한 탐색적 요인 분석 (Exploratory Factor Analysis of the Maudsley Obsessive-Compulsive Inventory in Middle School Students)

  • Objectives : The purpose of this study was to assess the factor structures related to the Maudsley Obsessive-Compulsive Inventory (MOCI) in middle school students. Methods : A total of 1,175 7th grade students from four middle schools in Paju city completed the MOCI, Center for Epidemiological Studies-Depression scale (CES-D), and Self Esteem Scale (SES). An exploratory factor (maximum likelihood method with oblique rotation) and correlation analyses were subsequently performed. Results : The two main factors related to the MOCI were 'intrusive thought' and 'contamination'. 'Intrusive thought' was positively correlated with the CES-D and negatively associated with the SES. 'Contamination' had a negative correlation with self-esteem as measured by the SES. Conclusion : The results of this study revealed a two-factor structure of obsessive and compulsive symptoms in middle school students.

강박장애 환자의 반응억제 결함 (Impaired Response Inhibition in Patients with Obsessive-Compulsive Disorder)

  • Objective : Impaired response inhibition has been suggested to play an important role in the pathophysiology of obsessive-compulsive disorder (OCD). The aim of this study was to evaluate the response inhibition in patients with OCD, by using the Go/NoGo paradigm, and to better understand its associations with clinical symptoms. Methods : The participants included 63 OCD patients and 80 healthy volunteers matched in age and sex. response inhibition was evaluated using computerized Go/NoGo task, in which their commission error rates, omission error rates, and mean response times were measured. The severity of clinical symptoms in the OCD patients was assessed using Montgomery-Asberg Depression Scale and Yale-Brown Obsessive Compulsive Scale. Result : OCD patients showed significantly impaired inhibition and higher omission errors rates despite their slower response time, compared to normal controls. Clinical symptoms were not correlated with commission errors and omission errors. Conclusion : The present results indicate that impairment in response inhibition may play a critical role in the pathophysiology of OCD as a trait. These findings suggest that deficit of response inhibition may contribute to developing and maintaining clinical symptoms such as compelling need to repeat certain actions in patients with OCD.

강박증 환자에서의 Mismatch Negativity 이상에 관한 연구 (Abnormal Mismatch Negativity in Patients with Obsessive-Compulsive Disorder)

  • Objective : Mismatch negativity (MMN) provides an electrophysiological index of an involuntary attention switching process. This study investigated MMN anomalies and their relationship to clinical variables in patients with obsessive-compulsive disorder (OCD). Methods : Study participants were 29 OCD patients and 24 normal controls. We used a 128-channel EEG to measure MMN during a passive oddball paradigm. Then we assessed the correlations between MMN amplitudes and clinical measures. Results : The OCD patients exhibited significantly greater MMN amplitude compared to the controls. Also, the right frontal lobe MMN amplitudes correlated significantly to the OCD patients' Yale-Brown Obsessive Compulsive Scale total scores (r=-0.520, p=0.005). Conclusion : These results suggest that MMN anomalies in OCD patients reflect a monitoring circuit dysfunction, raising the possibility that the OCD's pathophysiology includes glutamatergic dysfunction.

강박장애 치료에 대한 중의학 임상연구 동향 (Review of Clinical Studies on the Treatment of Obsessive Compulsive Disorder Using Traditional Chinese Medicine)

  • Objectives: The objective of this study is to analyze the Clinical reports of treatment of obsessive compulsive disorder (OCD) in traditional Chinese medicine (TCM). Methods: Clinical reports on obsessive compulsive disorder treatment from 2001 to 2017 were searched using the following key words in the China National Knowledge Infrastructure (CNKI). 'OCD', 'qiangpo zheng'. Results: A total of 22 papers were selected, 12 of which were clinical research papers and 10 case report papers. The acupuncture and moxa treatment was more effective in relieving the clinical symptoms of OCD patients as compared to only using western medicine, and it was also possible to reduce the adverse effects of western medicine. Conclusions: It was established that Traditional Chinese medicine has some effect on the treatment of OCD, but there were limitations as follows. The selected papers were retrieved from only one site of CKNI and did not follow a systematic review methodology. Therefore generalized results could not be derived, further studies should therefore be carried out.

탈근대 주체의 불안과 강박적 반복: 폴 오스터의 『리바이어던』 읽기 (Postmodern Subject's Anxiety and Obsessive Repetition in Paul Auster's Leviathan)

  • The purpose of this paper is to examine Paul Auster's Leviathan according to Slavoj Žižek's theory. Analyzing the characters in Leviathan, this paper chiefly discusses the postmodern subject's anxiety and obsessive repetition that the lack of the big Other led to. Section II explains the disintegration of the big Other and the subject's anxiety and obsessive repetition by the interpretation of the characters: Peter Aaron, Maria Turner, and Benjamin Sachs. Aaron wants to write on Sachs's life to overcome his uneasy subject's condition, and to establish the consistent and whole world. But his writing fails to meet his desire, owing to uncertainty of his understanding, and the incompleteness of his writing. In case of Maria, her uneasy subject's condition led to her obsessively repetitive picture-shooting herself and others, which proved to be a meaningless struggle for filling the void of the big Other and herself. Although Sachs already knows the lack and inconsistency of the big Other, he also repetitively tries to establish the consistent and whole Other. In Section III, this paper examines Sachs's terror as he struggles for the preservation of the big Other. His extreme striving also fails to reestablish the big Other as it loses its symbolic effectiveness in the postmodern era because he does not grasp the big Other as an empty Symbolic order, and rejects the premise of the big Other itself.

The Potential Usefulness of Magnetic Resonance Guided Focused Ultrasound for Obsessive Compulsive Disorders

  • Obsessive compulsive disorder is a debilitating condition characterized by recurrent obsessive thoughts and compulsive reactions. A great portion of the obsessive compulsive disorder (OCD) patients are managed successfully with psychiatric treatment such as selective serotonin-reuptake inhibitor and cognitive behavioral psychotherapy, but more than 10% of patients are remained as non-responder who needs neurosurgical treatments. These patients are potential candidates for the neurosurgical management. There had been various kind of operation, lesioning such as leucotomy or cingulotomy or capsulotomy or limbic leucotomy, and with advent of stereotaxic approach and technical advances, deep brain stimulation was more chosen by neurosurgeon due to its characteristic of reversibility and adjustability. Gamma knife radiosurgery are also applied to make lesion targeting based on magnetic resonance (MR) imaging, but the complication of adverse radiation effect is not predictable. In the neurosurgical field, MR guided focused ultrasound has advantage of less invasiveness, real-time monitored procedure which is now growing to attempt to apply for various brain disorder. In this review, the neurosurgical treatment modalities for the treatment of OCD will be briefly reviewed and the current state of MR guided focused ultrasound for OCD will be suggested.
