A Retrospective Study on Herbal Medication and Obesity for Children (소아의 한약 복용과 비만에 대한 후향적 연구)

  • Objective : As today obese children have increased rapidly in Korea, the interest has been much increased. And parents concern about the effect of Herbal Medication(HM) on gaining weight. Therefore we had a retrospective study HM and obesity for children. Method : We examined medical record of 38 patients who visited to Department of Pediatrics in 00 medical center from 2002 to 2006 and took HM. Their Hight, Weight, Obesity Index(OI) were estimated at two points: before and after HM treatment. And they were divided into two groups according to the HM type and analyzed for differences of OI. Result : There were no significant changes in 38 patients's OI before and after treatment. But only one case gained over weight(OI: 21.31). OI increased slightly after treatment but there were not statistically significance. OI according to the HM type had slightly difference but there were not statistically significance. There were no connections between HM type, the number of days taking HM and changes of OI. Conclusion : On the basis of this study, we thought there were no connections between HM and obesity for children. But this study's cases are small in number so more study is need.

  • Objective : There have been many programs to treat childhood obesity, since childhood obesity has shown up as a social problem. The purpose of this study was to find out considerable matters to manage Childhood Obesity Treatment Program (COTP). Methods : This study was made with reference to clinical progress notes of children with obesity, visiting the oriental medicine center, to take COTP from July 30th, 2003 to August 25th, 2004. COTP was consist of abdominal aroma massage, abdominal low frequency acupuncture therapy, auricular acupuncture therapy and behavior modification. Results : 1. Seven children who taken COTP started fat from young age and three started from about ten-year-old ages. 2. Six children have fat fathers, a child has fat parents and three children have no fat family member. 3. The obesity levels of Obesity Index(OI), Body Mass Index(BMI) and Rohler Index(RI) were different, used for evaluating the result of treatment, though they are used for a child. The result of OI reflected more sensitive from changes of weight than those of BMI and RI. 4. We got the better results with numbers of treatment and exercise. Conclusions : More correct standard are needed to estimate degree childhood obesity. And it is necessary to carry out obesity treatment program with behavior modification, for children with obesity. to be healthy adults.

The Study on the Serum Levels of Resistin, Adiponectin, and Leptin in Obese Children (비만 아동의 혈청 Resistin, Adiponectin 및 Leptin 농도에 관한 연구)

  • Resistin, adiponectin, and leptin are hormones secreted by adipose tissue and are known to play an important role in adipose tissue metabolism. The purpose of this study was to evaluate the levels of adipocyte-derived hormones (resistin, adiponectin, leptin) in obese children aged 10-12 years. The subjects were 102 obese children with obesity index (OI) over $120\%$ and 51 control children with obesity index less than $120\%$ were included for comparison. Anthropometric variables and serological parameters were assessed. Height, weight, OI, body mass index (BMI) were significantly higher in obese group than in control. Obese children showed significantly higher serum concentrations of triglyceride (TG), total cholesterol (TC), and LDL-cho1esterol and significantly lower HDL-cholesterol compared with control children, even though the values were within normal ranges for both groups. Concentrations of resistin and leptin were significantly higher in obese group than in control. Adiponectin and insulin levels were tended to lower in obese group even though the differences were not statistically significant. Resistin had significant positive correlation with OI and TG, and 1eptin with weight, OI, BMI, TG and TC. On the other hand, adiponectin showed significant negative correlations with height, OI and BMI. These finding showed that obese children had higher serum levels of resistin and leptin and lower adiponectin, and also these hormones had correlations with related factors of obesity, suggesting adipocyte-derived hormones has a role in child obesity.

Comparisons of obesity assessments in over-weight elementary students using anthropometry, BIA, CT and DEXA

  • Obesity was characterized in Korean elementary students using different obesity assessment tests on 103 overweight elementary students from three schools of Jeonbuk Province. The body mass index (BMI) and obesity index (OI) were compared, and the data using DEXA and CT were compared with the data using BIA and a tape measure. The results of this study are as follows: first, 27 students who were classified as obese by OI were classified as overweight by BMI, and 3 students who were classified as standard weight by BMI were classified as overweight by OI. Secondly, by DEXA and BIA measurements, there was 1.51% difference in body fat percentage (boys 1.66%, girls 1.17%) and the difference in body fat mass between boys and girls was 0.77 kg (boys 0.85 kg, girls 0.59 kg), but those differences in body fat percentage and mass were not statistically significant. Thirdly, the average total abdominal fat (TAF) measured by CT scans of obese children was more significantly related with subcutaneous fat (r = 0.983, P < 0.01) than visceral fat (r = 0.640, P < 0.01). Also, TAF were highest significant with waist circumference by a tape measure (r = 0.744, P < 0.01). In summary, as there are some differences of assessment results between two obesity test methods (BMI, OI), we need more definite standards to determine the degree of obesity. The BIA seems to be the most simple and effective way to measure body fat mass, whereas waist/hip ratio (WHR) using a tape measurer is considered to be the most effective method for assessing abdominal fat in elementary students.

Dietary Behaviors, Health-related Lifestyle and Blood Lipid Profile of Obese Children in Incheon (인천시 비만초등학생의 식행동, 건강관련 생활습관 및 혈중 지질농도)

  • The purpose of this study was to investigate the anthropometric and blood biochemical characteristics, the dietary behaviors and health-related lifestyles of obese children in Incheon. A cross-sectional survey was conducted using anthropometric measurements, biochemical assessments and questionnaire analysis. The subjects included 7,055 obese children residing in the Incheon area (from 106 elementary schools). The degree of obesity was classified using the Obesity Index (OI) as light 20% < OI < 30% ; Medium 30% < OI < 50% ; Severe 50% > OI. The statistical analysis was conducted using SPSS 10.0 program. Most subjects fell within the medium range of obesity. Most subjects had dietary problems such as overeating, unbalanced meals and skipping breakfast. The ratio of boy subjects eating green and yellow vegetables was lowe. when compared to that of the girl subjects. The boy subjects exercised more frequently and longer than the girls. As the Obesity Index increased, hypercholesterolemia significantly increased. The blood cholesterol levels of the subjects were positively correlated with body fat, waist/hip ratio and BMI; HDL cholesterol levels of the subjects were negatively correlated with the anthropometric data and the LDL cholesterol levels of the subjects were positively correlated with body fat. The blood triglycerides levels of the subjects were positively correlated with body weight, body fat, waist/hip ratio and BMI. Therefore, proper nutritional education and intervention are required for an improvement of obese children's dietary behavior, heath-related factors and blood lipid profiles.

A Comparative Study on Dietary Life according to the Obesity Assessment Methods of Higher Grade Elementary School Students in Jeonju (전주지역 고학년 초등학생의 비만판정 방법에 따른 식생활 비교연구)

  • This study was done for finding out if eating habits, eating behaviors were different between non-obese and obese elementary school students in Jeonju Area. Total 2568 students of 1364 male and 1204 female of the 4th, 5th, and 6th year in 5 elementary schools were surveyed and the statistics of the result was analyzed by SPSS program. The results are summarized as follows: 1. Obesity was defined as Body Mass Index(BMI) that exceeded 85th and Obesity Index(OI) that exceeded 110. First, subjects were divided into 4 groups : lean, normal, overweight and obese. Second subjects were reclassified into non-obese(lean and normal) and obese(overweight and obese) groups. Average height of male and female students were 142.5cm, 143.1cm and weight of those were 36.4kg and 37.9kg respectively. 2. As results of obesity computation, obese male students were 19.6%(overweight 11.3%, obese 8.3%) in BMI and obese male students were 25.0%(overweight 12.5.%, obese 12.5%) in OI. Especially Obesity percent rate of male student were significantly higher on that of female student in OI method. 3. Examining obesity between male and female, there were statistically different between male students and female students in OI, but there were not statistically different in BMI. With regard to grade level(4th, 5th, 6th), there were statistically different among grade levels. 4. Examining correlation between eating habits(eating behaviors) and obesity, there were statistically significant in some cases. For example, there were statistically significant correlation between fast eating habit and obesity. And the relation analysis of general environments and obesity showed that there were statistically significant in some cases. These results suggest that the number of overweight students can be increased due to the amount and kinds of food children have as well as the general causes of overweight such as genetic, environmental and psychological reason. Surveying about children's eating habits, eating behaviors this study methodically. Working with parents is necessary and comparison of eating habits, eating behaviors and nutrition knowledge between the past and their presents are also needed in a future.

The Effect of Balneotherapy on Obesity Index and Body Composition on Obese Children (온천요법이 비만 아동의 비만지수와 체성분에 미치는 영향)

  • Objectives The purpose of this study is to evaluate the effect of hot spring bath in obese children. Methods The study was conducted with 20 obese children whose BMI were over 20 ($kg/m^2$). The hot spring bath program was performed in the hot-spring facilities with temperature of $32{\sim}36^{\circ}C$. The children bathed four times from June to July, 2012. Prior to their bath, their heights, weights, and body compositions were measured every time. They were also confirmed their obesity index and obesity degree during the program. Results After the bathing program, degree of obesity has changed, three obesity judgment index (BMI, RI and OI) of the children have decreased (BMI, RI, OI on the $1^{st}$ day: $25.99{\pm}3.47$, $177.63{\pm}17.43$, $37.74{\pm}13.42$; on the $4^{th}$ day: $25.06{\pm}3.08$, $168.4{\pm}14.8$, $30.60{\pm}12.12$), so have body fat mass and percent body fat (BFM, PBF on the $1^{st}$ day: $21.83{\pm}7.03$, $38.24{\pm}3.36$; on the $4^{th}$ day:$19.55{\pm}6.35$, $34.20{\pm}3.77$), but skeletal muscle mass has increased (SMM on the $1^{st}$ day: $18.37{\pm}4.24$ ; on the $4^{th}$ day: $19.80{\pm}4.44$). Conclusions This study shows that hot spring bath could be an effective way of managing and treating obesity.

Comparisons of Effects of Biman-tang according to Administration Period in Childhood Obesity (비만환아에서 복약기간에 따른 비만탕의 치료효과 비교연구)

  • Objectives The purpose of this study is to investigate the safety and effect of Biman-tang (BMT) on two different administration periods in the treatment of childhood overweight and obesity. Methods In retrospective study, 39 overweight and obese ($85^{th}$ percentile ${\leq}$ Body Mass Index (BMI)) children were treated with BMT from January 2006 to April 2013 at Korean Medical Clinic in Suwon, Korea. The primary outcomes were the changes in BMI and Obesity Index (OI) from baseline to the treatment groups. Secondary outcomes included the changes in height, weight and safety of the medicine. Comparisons of BMI, OI, height and weight between the short-period (SP) treatment group and the long-period (LP) treatment group were done by using ANCOVA. Results The change of the mean of BMI ($-0.5{\pm}0.6kg/m^2$ vs $-1.4{\pm}0.8kg/m^2$, respectively; p=0.003) and OI ($-3.6{\pm}3.9%$ vs $-9.7{\pm}4.7%$, respectively; p<0.001) showed significant reduction both in SP (n=16) and LP treatment group (n=23). The mean height showed no significant difference in both groups. The mean weight of LP treatment group showed significant reduction compared to SP treatment group ($-0.2{\pm}1.3kg$ vs $-1.6{\pm}1.6kg$, respectively; p=0.006). Most of the children were compliant to the medication and no serious adverse events were found in two groups. Conclusions These findings emphasize that BMT is effective in the treatment of childhood obesity and it requires at least 45 days of treatment for the best result.

Indirect Assessment of Obesity and Correlation between Energy Intake and Energy Expenditure of Middle School Students (중학생의 간접적 비만판정과 에너지섭취 및 소비량과의 관계)

  • This study was accomplished to investigate the prevalence of obesity in 371 Kumi-city middle school students. The means of this study were anthropometric measurement and questionnaires about nutrient intake and energy expenditure. Anthropometric measurements were height, weight, arm circumference and skinfold thickness(biceps, triceps, subscapular, supraliac, calf). The results obtained are as follows: Energy, calcium, iron intake for boys were lower than RDA. Iron intake was low in girls. Energy intake is lower than energy expenditure and RDA for boys. Energy intake, energy expenditure and RDA for girls were balanced. The average height, weight and arm circumference of boys were respectively 160.0cm, 49.4kg, 23.1cm and those of girls were respectively 155.9cm, 48.9kg, 22.6cm. Prevalence of obesity was respectively 5%, 4%, 7%, 4%, 59%, 15%, 14%, 24% for boys and 6%, 10%, 13%, 9%, 30%, 7%, 1%, 3496 for girls by RBW, BMI, RI, OI, %Fat I, %FatII, %FatIII, triceps. Correlation between subscapular skinfold thickness and BMI, RI, OI and %FatII showed high.

A study on nutrition knowledge and dietary behavior of elementary school children in Seoul

  • The purpose of this study was to investigate the nutrition and diet related knowledge, attitude, and behavior of elementary school children in Seoul. The subjects included were 439 (male 236, female 203) elementary school children in the 4th to the 6th grades, The statistical analysis was conducted using SPSS 12.0 program. The average obesity index (OI) was 104.98 and 99.82 for male and female subjects, respectively. The average percentage of underweight, normal, overweight and obese of subjects was 33.7%, 32.8%, 12.3%, and 19.4%, respectively. The percentage of the underweight group of female subjects was higher than that of the male subjects. The percentage of the obese group of male subjects was higher than that of the female subjects. The average score of nutrition knowledge, nutrition attitude and dietary behavior was 6.8, 7.44, and 7.34, respectively. Dietary behavior of male subjects was positively correlated with parents' education levels, monthly household income and nutrition attitude. Dietary behavior of female subjects was positively correlated with monthly household income, nutrition knowledge and nutrition attitude. Dietary behavior of female subjects was positively correlated with obesity index (OI). Proper nutrition education and intervention are required for the improvement of elementary school children's nutrition knowledge, nutrition attitudes and dietary behaviors.
