Characteristics of Spatial Environment in Psychiatric Wards through the Analysis by Patient's Preferred Healing Environment in Observational Ward Structure Dividing Staff Area and Patient Area (정신요양 병동에서의 관리영역과 환자의 요양 공간영역의 관찰적 시선에 따른 병동구조와 요양 공간환경의 특성에 관한 연구)

  • Purpose: The relationship between the staff area and the patient's private area is the key factor in designing the structure and the environmental characteristics of ward spaces in a psychiatric hospital. Recent research has found that for the purpose of treatment and securing privacy, psychiatric patients need to be in an open space of relief rather than closed confined environment and under the watch of nursing staffs. Methods: A survey at three kinds of wards in a private psychiatric hospital in Japan was conducted in October 2002. These wards include an acute ward, a psychiatric convalescence ward, and a stress care ward. All three kinds of wards have the same structure. At each ward, spatial preferences of the 145 psychiatric inpatients were surveyed and data concerning the patient's diagnostic category, symptoms, and activities of daily living were recorded. Results: The patients in the stress care ward prefer to stay in private spaces than public spaces. On the other hand, in the acute ward the patients seem to have a preference between managed public spaces where are monitored by nursing staffs and their private rooms where the nurse station is close. In addition, the patients in the psychiatric convalescence ward spend most of their time in the public space, such as the hallways or the day room. Implications: Base on this research, the spaces at the acute ward that could be monitored by the nursing station serves effectively as a safety space for patients was concluded. However, in the stress care ward, the patients may perceive the monitoring by the nursing staff as interruption or nuisance to their relaxation. In order to design an ideal healing environment for psychiatric patients in psychiatric ward, it is important to consider how environmental characteristics of space affect the environmental sense of patients in each ward.

A Study on Causes and Reactions of Intending Turnover of Staff Nurses Working in Hospitals (임상간호원의 이직에 대한 반응 및 이에 미치는 요인에 관한 조사연구)

  • High turnover rate of nurses working in the hospitals is one of the serious problems in nursing society at the present time which can result in reducing the economic condition of hospital and providing low quality nursing care. This study was designed identify the causes and turnover rate of 248 clinical staff nurses in relation to their rewards and incentives in 6 hospitals located in Seoul from Oct.1 to Oct. 20, 1975. Questionnaires and analyzing items were formulated on the basic of Mallow's hierarchy of basic needs which were grouped in three categories such as safety and social and psychological rewards and incentives. The data were analyzed by means of mean, Chi square, Percentage and F- ratio. The following results were obtained: 1) There were 69% of respondents who have definitely decided or hoped to leave the hospital. 2) Those who are in the range of 26 to 30 years of age showed the strongest desire to leave the hospital. 3) There was no significant difference between those who want to leave the hospital and those who want to remain in relation to age, educational background, past experience and expecting period of clinical work. However, the married nurses tend to remain in the hospital than the single. 4) When the rewards scores in the three categories were calculated, psychological rewards were placed in the first rank by the mean score 14.09 ; and each of safety rewards and social rewards were 9.06 and 4.84. When the scores in three categories were compared it was found that there was significant difference. (F〈0.01).

The Moderating Effects of Social Support between Job Stress and Job Satisfaction in Staff Nurses (임상간호사의 직무 스트레스와 직무만족도에 대한 사회적 지지의 조절효과)

  • Purpose: This paper was attempted to focused on the analysis of job stress and job satisfaction according to the moderating effects of social support. Method: Data collected from 302 staff nurses in 2 general hospitals, with self-reporting questionnaires(Wolfgang's HPSI, modified Hagihara's social support, and MSQ). Descriptive statistics, Cronbach's alpha, Pearson correlation, ANOVA, and moderated regression analysis were used. Result: The subjects exhibited significantly highest level of 'the work and patients' conditions'. The perceived job stress was negatively correlated with supervisor and co-workers' support, and job satisfactions. Also supervisor and coworkers' support were positively correlated with job satisfactions. The main factor that affected the job satisfactions was 'work and patient conditions' and it was explained 35.5% out of the total variance of the job satisfactions with 'the work load and disturbing factor', 'supervisor support' and 'co-workers' support'. Supervisor support moderated the relationships between 'the patient care responsibilities and career development factor' and satisfaction. Conclusion: For developing the stress management program for hospital nurses, 'the work and patients' conditions', and the strategies of strengthening 'supervisor and co-workers' support' should be considered. In addition to, it may be considered to examine the effects of social support on the other criterion variables.

Participation in Decision-making and Expertise of Staff Nurses (일부종합병원 일반간호사의 의사결정 참여와 전문성)

  • The purpose of this study was to analyze the relationship of the participation in decisionmaking and expertise of staff nurses. The population for this study was the registered nurses(N=342) working in Chungnam and Chungbuk. The data were collected from April 26 to May 26, 1999. The survey instrument was Participation in Decision Activities Quesionnaire and Expertise scale developed by Anthony(1995), and Job Expertise scale of Van de Ven and Ferry. The results were as follows: 1) There was a significant difference for identification and design, and selection among the process of participation in decision making. 2) There was a significant difference between the participation in caregiving decisions and condition of work decisions. 3) (1) There was a significant difference among caregiving decisions between the expertise indicators and the variables such as education level. and the experience to told who is an expert. (2) There was a significant difference among condition of work decisions between the expertise indicators and the variables such as career, the spent keeping current per week, and self-rating of expertise.

Effect of Nurses' Job Stress on Job Satisfaction: Mediating Effect of Head Nurses' Emotional Leadership Perceived by Nurses (간호사의 직무 스트레스가 직무만족에 미치는 영향: 간호사가 지각한 수간호사의 감성리더십의 매개효과)

  • Purpose: The purpose of this study was to identify the mediating effect of head nurses' emotional leadership as perceived by nurses in the relationship between nurses' job stress and job satisfaction. Methods: Study participants were 278 staff nurses currently working at two university hospitals in Busan, Korea. SPSS/WIN 21.0 program was used for data analysis to analyze descriptive statistics, t-test, correlation, and stepwise multiple regression. The significance level was set at p<.05. Results: Nurses' job satisfaction had a significant correlation with job stress (r=-.31, p<.001) and emotional leadership of nurse managers as perceived by nurses (r=.53, p<.001). Also, head nurses' emotional leadership experienced by nurses had a partial mediating effect in the relationship between nurses' job stress and job satisfaction. Conclusion: The results of this study show that the nurse managers' emotional leadership has an important influence on nurses' job stress and job satisfaction. Therefore, head nurses' emotional leadership is very important for effective human resource management and the hospital should offer education and training to booster head nurses' leadership by developing emotional intelligence to promote staff job satisfaction.

Development of Web-based Learning Program on Cardiopulmonary Emergency Care Focused on Clinical Scenarios (웹기반 사례중심 심폐응급간호 학습 프로그램 개발)

  • Purpose: This study was conducted to develop a Web-based learning program on cardiopulmonary emergency care for clinical nurses and to evaluate learners' responses. Methods: Based on the assessment of learning needs of clinical nurses, a total of three self-directed learning modules were developed according to the procedure of the ADDIE (assessment, design, development, implementation, & evaluation) model. Results: Each learning module included the emergency treatments and drugs used in the real patients' situations with cardiopulmonary crisis, which had been adopted from the emergency department of a C University hospital located in G-city. Real video clips for endotracheal intubation and ACLS (advanced cardiac life support) were developed with the help of the staff of the department of emergency medicine using a human simulator, $SimMan^{(R)}$. The program published on the Web was evaluated by 20 clinical nurses who are working in the emergency department and wards of a C-University hospital. About 80% of the respondents were satisfied with the program contents, design, and learning strategy. Conclusions: Web-based learning programs on cardiopulmonary emergency care are needed for clinical nurses as educational material for staff education to increase their knowledge for making immediate clinical decisions and in giving skilled care in emergency situations.

The Number and Type of Microorganisms on the Ring Finger after Handwashing (반지착용이 손씻기 후의 미생물 수와 유형에 미치는 영향)

  • Background : There have been very few studies conducted on the number and type of microorganisms that remain on the ring finger after handwashing. This study was performed to investigate whether there were changes and differences in the type and number of microorganisms on the ring finger before and after handwashing. Method : The subjects of the study were 15 MICU and SICU staff nurses who were wearing rings. I swabbed two different fingers of the same hand with cotton balls. One finger which had a ring and the other with no ring. I swabbed the fingers of each subject three times(before handwashing, after handwashing with soap, and after handwashing with bethadine solution). After storing them for 48 hours in an incubator, I sent them to the laboratory and recorded the culture results. Results : There was no difference in the type of microorganism, but a major difference in the number of the microorganisms that existed on the finger ring. The results showed that there were much more microorganisms on the ring fingers than on the fingers that did not have rings both before and after handwashing. This tendency was consistent regardless of the handwashing agent. I therefore recommend that all nursing staff who work in general nursing units, as well as nurses who work in the ICU, remove their rings when taking care of the patients.

The Effect of Hospital SCM on Logistics Performance (병원 통합물류시스템(SCM)이 물류 업무성과에 미치는 영향)

  • Purpose: The purpose of this study was to examine the causal relationships among quality of hospital SCM (system quality, information quality, service quality), perceived usefulness, user satisfaction and logistics performance by hospital staff. Methods: A survey using a structured questionnaire was conducted with 276 hospital staff. Analysis of the data was with both SPSS Win 18.0 for descriptive statistics and AMOS 8.0 for the structural equation model. Results: The hypothetical model yielded Chi-square=8.121 (p=.087), df=4, $x^2$/df=2.030. GFI=.989, AGFI=.942, RMSEA=.066, NFI=.995, CFI=.997 TLI=.990 and showed good fit indices. Three dimensions of quality of SCM had significant direct effects on perceived usefulness. System and service of quality of SCM had significant direct effects on user satisfaction. Perceived usefulness had significant direct effects on user satisfaction. User satisfaction had significant direct effects on logistics performance. Conclusion: These results suggest that logistics performance can be enhanced by user satisfaction, user satisfaction can be enhanced by perceived usefulness, and perceived usefulness can be enhanced by quality of hospital SCM. Further study with various hospitals and various variables is necessary.

A Case Study on the Space Use Pattern and Behavior of the Elderly with Dementia in Common Space of Nursing Home (노인전문요양시설 치매노인의 공용공간 이용실태 및 행동특성에 관한 사례연구)

  • The purpose of this study was to clarify the space use pattern and the daily living activities of the elderly with dementia in the common space (living room) of nursing home. The subjects were 2 units(living room) of the nursing home. For this, researcher visited from June 20 to July 2 and collected data by observations of behavior of the elderly with dementia in common space(living room) and interviews with staff for the information about basic characteristics of the elderly with dementia and administration and operation of the facilities. The data were analyzed using the frequency and percentage. The results revealed as follows: first, the daily living activities of the elderly with dementia in common space tended to focus on self-concentration type and physical environment-interest type. Second, physical environmental -interest type. Second, physical environmental characteristics of common space affected space usage pattern of the elderly with dementia. And it was perceived like as living oriented space, social interaction space, facility space, staff oriented space. Conclusively it was suggested to plan dining space as semi-private space, day corner as semi-public space.

Pregnant Women's Labor Progress, Childbirth Outcome, and Childbirth Satisfaction according to the Presence or Absence of Labor Induction (유도분만 시행 여부에 따른 임산부의 분만진행과정, 분만결과, 분만만족도)

  • Purpose: To provide accurate information on induced labor and find strategies to enhance women's childbirth satisfaction. Methods: Participants were pregnant women expected to have normal vaginal delivery. A total of 113 women with induced labor and 61 women with spontaneous labor were surveyed. Data were collected using a questionnaire and electronic medical records. Results: The following variables related to labor progress showed significant differences between the induced labor group and the spontaneous labor group: length of the first stage of labor in primigravidas, use of analgesic, incidence of uterine hyperstimulation, incidence of fetal distress, and medical treatment for the expectant mother. Delivery type and the incidence of postpartum complications showed significant difference between the two groups. Induced labor women's childbirth satisfaction was mainly affected by the process of labor whereas spontaneous labor women's childbirth satisfaction was affected by the outcome of childbirth. Conclusion: Medical staff should have accurate information on the risk of induced labor and the benefits of a natural delivery. Moreover, medical staff should provide necessary information and environment for women to participate in the decision-making process.
