How image-processing parameters can influence the assessment of dental materials using micro-CT

  • Purpose: The aim of this study was to evaluate the influence of voxel size and different post-processing algorithms on the analysis of dental materials using micro-computed tomography (micro-CT). Materials and Methods: Root-end cavities were prepared in extracted maxillary premolars, filled with mineral trioxide aggregate (MTA), Biodentine, and Intermediate Restorative Material (IRM), and scanned using micro-CT. The volume and porosity of materials were evaluated and compared using voxel sizes of 5, 10, and 20 ㎛, as well as different software tools(post-processing algorithms). The CTAn or MeVisLab/Materialise 3-matic software package was used to perform volume and morphological analyses, and the CTAn or MeVisLab/Amira software was used to evaluate porosity. Data were analyzed using 1-way ANOVA and the Tukey test(P<0.05). Results: Using MeVisLab/Materialise 3-matic, a consistent tendency was observed for volume to increase at larger voxel sizes. CTAn showed higher volumes for MTA and IRM at 20 ㎛. Using CTAn, porosity values decreased as voxel size increased, with statistically significant differences for all materials. MeVisLab/Amira showed a difference for MTA and IRM at 5 ㎛, and for Biodentine at 20 ㎛. Significant differences in volume and porosity were observed in all software packages for Biodentine across all voxel sizes. Conclusion: Some differences in volume and porosity were found according to voxel size, image-processing software, and the radiopacity of the material. Consistent protocols are needed for research evaluating dental materials.

Evaluation of Scatter Reduction Effect of the Aft-Multiple-Slit (AMS) System Using MC Simulation (MC 시뮬레이션을 이용한 Aft-Multiple-Silt 시스템의 산란선 제거 효과 평가)

  • Purpose: We designed the aft-multiple-slit (AMS) system to reduce scatter in cone-beam computed tomography (CBCT). As a preliminary study, we performed a Monte Carlo N-Particle Transport Code (MCNP) simulation to verify the effectiveness of this system. Materials and Methods: The MCNPX code was used to build the AMS geometry. An AMS is an equi-angled arc to consider beam divergence. The scatter-reduced projection images were compared with the primary images only and the primary plus scatter radiation images with and without AMS to evaluate the effectiveness of scatter reduction. To obtain the full 2 dimensional (2D) projection image, the whole AMS system was moved to obtain closed septa of the AMS after the first image acquisition. Results: The primary radiation with and without AMS is identical to all the slit widths, but the profiles of the primary plus scattered radiation varied according to the slit widths in the 2D projection image. The average scatter reduction factors were 29%, 15%, 9%, and 8% when the slit widths were 5 mm, 10 mm, 15 mm, and 20 mm, respectively. Conclusion: We have evaluated the scatter reduction effect of the AMS in CBCT imaging using the Monte Carlo (MC) simulations. A preliminary study based on the MCNP simulations showed a mount of scatter reduction with the proposed system.

A Study of Bone Uptake According to Renal Function in the Whole Body Bone Scan (전신 뼈 검사에서 신장 기능에 따른 뼈 섭취율에 대한 고찰)

  • Whole body bone scan has been used to confirm bone metastasis and follow-up study with radio isotope. However, if the factors related to $^{99m}Tc$ uptake and waiting time for study are inappropriate, it would be image of low quality. The purpose of present study was to investigate correlation between the evaluation index of renal function and uptake of radiopharmaceuticals. The population for this retrospective study consisted of 387 patients who underwent whole body bone scan between June 2012 and December 2012. As a result of quantitative and qualitative analysis, we were able to confirm that GFR of less than normal range and creatinine levels in blood of more than average are more likely to be under the mean uptake rate. As a result of analysis on the indicator affecting soft-tissue and bone uptake, the correlation of all elements was somewhat low. Also there are no statistically significances due to the other parameters we did not deal with. Therefore, further research on additional factors is needed for exact study and improvement of the image quality.

Silk fibroin/hyaluronic acid blend sponge accelerates the wound healing in full-thickness skin injury model of rat (전층피부창상에서 실크피브로인과 하이알론산 혼합 스폰지의 창상치유효과)

  • The primary goal of the wound healing is rapid wound closure. Recent advances in cellular and molecular biology have greatly expanded our understanding of the biologic processes involved in wound repair and tissue regeneration. This study was conducted to develop a new sponge type of biomaterial to be used for either wound dressing or scaffold for tissue engineering. We designed to make a comparative study of the wound healing effect of silk fibroin/hyaluronic acid (SF/HA) blend sponge in full-thickness dermal injury model of rat. Two full-thickness excisions were made on the back of the experimental animals. The excised wound was covered with either the silk fibroin (SF), hyaluronic acid (HA) or SF/HA (7 : 3 or 5 : 5 ratio) blend sponge. On the postoperative days of 3, 7, 10 and 14, the wound area was calculated by image analysis software. Simultaneously, the tissues were stained with Hematoxylin-Eosin and Masson's trichrome methods to measure the area of regenerated epithelium and collagen deposition. In addition, we evaluated the degree of the epithelial cell proliferation using immunohistochemistry for proliferating cell nuclear antigen (PCNA). We found that the half healing time ($HT_{50}$) of SF/HA blend sponge treated groups were significantly decreased as compared with either those of SF or HA treatment group. Furthermore, SF/HA blend sponges significantly increased the size of epithelialization and collagen deposition as well as the number of PCNA positive cells on epidermal basement membrane as compared with those of control treatment. Especially, the 5 : 5 ratio group of SF/HA among all treatment groups was most effective on wound healing rate and histological studies. These results suggest that SF/HA blend sponges could accelerate the wound healing process through the increase of epithelialization, collagen deposition and basal cell proliferation in full thickness skin injury.

Evaluation of metabolic tumor volume using different image reconstruction on 18F-FDG PET/CT fusion image (18F-FDG PET/CT 융합영상에서 영상 재구성 차이에 의한 MTV (Metabolic tumor volume) 평가)

  • Recently, MTV(metabolic tumor volume) has been used as indices of the whole tumor FDG uptake on FDG PET image but it is influenced by image reconstruction. The purpose of this study was to evaluate the correlation between actual volume and metabolic tumor volume applying different SUVmax threshold for different reconstruction algorithm on phantom study. Measurement were performed on a Siemens Biograph mCT40 using a NEMA IEC body phantom containing different size six spheres filled with F18-FDG applying four SBRs (4:1, 8:1, 10:1, 20:1). Images reconstructed four algorithms (OSEM3D, OSEM3D+PSF, OSEM3D +TOF, OSEM3D+TOF+PSF) and MTV were measured with different SUVmax threshold. Overall, the use of increasing thresholds result in decreasing MTV. and increasing the signal to background ratio decreased MTV by applying same SUVmax threshold. The 40% SUVmax threshold gave the best concordance between measured and actual volume in PSF and PSF+TOF reconstruction image. and the 45% threshold had the best correlation between the volume measured and actual volume in OSEM3D and TOF reconstruction image. we believe that this study will be used when the measurement of MTV applying various reconstruction image.

Variation on Estimated Values of Radioactivity Concentration according to the Change of the Acquisition Time of SPECT/CT (SPECT/CT의 획득시간 증감에 따른 방사능농도 추정치의 변화)

  • With the recent development of precision medicine(Theranostics), interest and utilization of the quantitative function of SPECT/CT are increasing. This study aims to investigate the effect on the radioactivity concentration estimate by the increase or decrease in the total time of SPECT/CT imaging conditions. A standard image was obtained by the conditions of a total acquisition time of 600 sec(10 sec/f × 120 frames) by diluting 99mTc 91.76 MBq in a cylindrical phantom filled with sterile water, and a comparative image was obtained by increasing the total acquisition time by -90%, -75%, -50%, -25%, +50%, +100%. The CNR, radioactive concentration estimate(cps/ml), and the variation rate(%) of the recovery coefficient(RC) were analyzed by measuring the overall coefficient of interest in each image. The results[CNR, Radiation Concentration, RC] by the change in the number of projections for each increase or decrease rate(-90%, -75%, -50%, -25%, +50%, +100%) of total acquisition time are as follows. [-89.5%, +3.90%, 1.04] at -90%, [-77.9%, +2.71%, 1.03] at -75%, [-55.6%, +1.85%, 1.02] at -50%, [-33.6%, +1.37%, 1.01] at -25%, [-33.7%, +0.71%, 1.01] at +50%, [+93.2%, +0.32%, 1.00] at +100%. and also The results[CNR, Radiation Concentration, RC] by the acquisition time change for each increase or decrease rate(-90%, -75%, -50%, -25%, +50%, +100%) of total acquisition time are as follows. [-89.3%, -3.55%, 0.96] at -90%, [-73.4%, -0.17%, 1.00] at -75%, [-49.6%, -0.34%, 1.00] at -50%, [-24.9%, 0.03%, 1.00] at -25%, [+49.3%, -0.04%, 1.00] at +50%, [+99.0%, +0.11%, 1.00] at +100%. Image quality(CNR) showed a pattern of change in proportion to the increase or decrease in the total acquisition time of SPECT/CT, but the result at quantitative evaluation showed a change of less than 5% in all experimental conditions, maintaining quantitative accuracy(RC less than 0.05) without much influence.

Cortical Dysplasia: Tc-99m ECD SPECT Findings and Comparative Study with MRI according to Pathologic Grading (뇌피질 이형성증: Tc-99m ECD SPECT 소견과 병리적 등급에 따른 MRI와 비교 연구)

  • Purpose: Cortical dysplasia (CD) designates a diverse group of malformations resulting from one or more abnormalities in the development of the cerebral cortex. We investigated the findings of interictal SPECT and the diagnostic usefulness of interical and ictal SFECT according to pathological grading (PG) in comparison with MRI. Materials and Methods: This study included 16 patients (M:F=9:7, age: $19.9{\pm}11.8$ yrs) with pathologically proven CD. Tc-99m ECD SPECT was performed in all patients: interictal 11, interictal and ictal 3, ictal 2. MRI were obtained in all patients and image analysis was done blindly as to the result of SPECT. Pathologic findings of CD were classified into grade 1 G1, dyslamination), grade 2 (G2, dysplastic neurons) and grade 3 (G3, balloon cells). We compared SFECT with MRI in lesions-to-lesions and analyzed the result according to PG. Results: In SFECT and MRI. 38 and 27 lesions were visually recognized. In 14 interictal SPECT, variable findings in 35 lesions were demonstrated: 25 were hypoperfusion, 7 hyperperfusion, 2 heterotopic perfusion in the white matter. By comparison between two studios, missed lesions were founded: SPECT were 1 lesion, MRI 12. Review of missed 12 lesions of MRI were followed according to PG: G1 patients were 16.7% (4/19), G2 40.0% (6/15), and G3 50% (2/4). Conclusion: Interictal SFECT in CD showed variable findings such as hypoperfusion, hyperperfusion or heterotopic perfusion. However, for detection of missed CD on MRI, SFECT may help to detect a functional abnormality of the lesion with high PG.

Assessment of Viability in Regional Myocardium with Reversed Redistribution by Thallium Reinjection in Patients with Acute Myocardial Infarction (급성심근경색 환자에서 역재분포를 보인 심근의 Thallium 재주사에 의한 생존능의 평가)

  • Purpose: The aim of this study was to evaluate whether T1-201 reinjection distinguishes viable from non-viable myocardium in patients with reverse redistribution after acute myocardial infarction. Materials and Methods: We studied 42 patients with acute myocardial infarction (age, $55{\pm}12$ years). Eighteen (43%) out of 42 showed reverse redistribution on dipyridamole stress-4 hour redistribution T1-201 single photon emission computed tomography (SPECT). T1-201 reinjection was performed at 24 hours. Reverse redistribution was defined as worsening of perfusion defect at 4 hour delayed scan. All patients underwent follow-up echocardiography in 4 months to assess regional wall motion improvement. T1-201 uptake on reinjection images were analyzed for the prediction of myocardial wall motion improvement. Results: Of 36 segments with reverse redistribution, 17 segments showed normal wall motion on echocardiography, while 19 segments showed wall motion abnormalities. Of 19 the segments with reverse redistribution, 11 (58%) showed enhanced uptake after 24 hour reinjection. Myocardial wall motion was improved in 10 of 11 segments (90%) with enhanced uptake on reinjection. Wall motion improvement was not seen in 5 of 8 segments (63%) without enhanced thallium uptake. When myocardial viability was assessed by the uptake on reinjection image, nine of 10 segments (90%) with normal or mildly decreased uptake showed improved wall motion. Wall motion was not improved in 5 of 9 segments (16%) with severely decreased uptake. Conclusion: In patients with acute myocardial infarction, T1-201 reinjection imaging on myocardial segments with reverse redistribution has a high positive predictive value in the assessment of myocardial viability.

The Study of Error for Analysis in Dynamic Image from the Error of Count Rates in NaI (Tl) Scintillation Camera (NaI (Tl) 신틸레이션 카메라에서 계수율 오차에 따른 동적 영상 분석치 산출 오류에 관한 연구)

  • This study is aimed to evaluate the effect of $T_{1/2}$ upon count rates in the analysis of dynamic scan using NaI (Tl) scintillation camera, and suggest a new quality control method with this effects. We producted a point source with $^{99m}TcO_4^-$ of 18.5 to 185 MBq in the 2 mL syringes, and acquired 30 frames of dynamic images with 10 to 60 seconds each using Infinia gamma camera (GE, USA). In the second experiment, 90 frames of dynamic images were acquired from 74 MBq point source by 5 gamma cameras (Infinia 2, Forte 2, Argus 1). There were not significant differences in average count rates of the sources with 18.5 to 92.5 MBq in the analysis of 10 to 60 seconds/frame with 10 seconds interval in the first experiment (p>0.05). But there were significantly low average count rates with the sources over 111 MBq activity at 60 seconds/frame (p<0.01). According to the second analysis results of linear regression by count rates of 5 gamma cameras those were acquired during 90 minutes, counting efficiency of fourth gamma camera was most low as 0.0064%, and gradient and coefficient of variation was high as 0.0042 and 0.229 each. We could not find abnormal fluctuation in $x^2$ test with count rates (p>0.02), and we could find the homogeneity of variance in Levene's F-test among the gamma cameras (p>0.05). At the correlation analysis, there was only correlation between counting efficiency and gradient as significant negative correlation (r=-0.90, p<0.05). Lastly, according to the results of calculation of $T_{1/2}$ error from change of gradient with -0.25% to +0.25%, if $T_{1/2}$ is relatively long, or gradient is high, the error increase relationally. When estimate the value of 4th camera which has highest gradient from the above mentioned result, we could not see $T_{1/2}$ error within 60 minutes at that value. In conclusion, it is necessary for the scintillation gamma camera in medical field to manage hard for the quality of radiation measurement. Especially, we found a tendency that count rate changes over time at this study, and we proved that it can effect $T_{1/2}$. And also, there is need of appropriate phantoms and the method of quality management like this study, because there are not any advice or limitation degrees for domestic medical purpose scintillation camera.

Comparison of Radioactivity Measurement with Radionuclide Calibrators in Nuclear Medicine Centers (의료용 방사능측정기의 측정 정확도 평가)

  • To acquire good image quality and to minimize unnecessary radiation dose to patients, it is important to ensure that the radiopharmaceutical administered is accurately measured. Quality control of radionuclide calibrators should be performed to achieve these goals. The purpose of this study is to support the quality control of radionuclide calibrators in nuclear medicine centers and to investigate the level of measurement accuracy of the radionuclide calibrators. 58 radionuclide calibrators from 45 nuclear medicine centers, 74 radionuclide calibrators from 58 nuclear medicine centers, and 60 radionuclide calibrators from 45 nuclear medicine centers were tested with I-131, Tc-99m and I-123, respectively. The results showed that 81% of calibrators for I-131, 61% of calibrators for Tc-99m and 67% of calibrators for I-123 were within ${\pm}5%$. 17% of calibrators for I-131, 20% of calibrators for Tc-99m and 15% of calibrators for I-123 had a deviation in the range 5%< $|{\Delta}|{\leq}10%$. 2% of calibrators for I-131, 19% of calibrators for Tc-99m and 18% of calibrators for I-123 had a deviation of $|{\Delta}|$ >10%. Follow-up measurements were performed on the calibrators whose error exceeded the ${\pm}10%$ limit. As a result, some of the calibrator showed an improvement and their deviation decreased below the ${\pm}10%$ limit. The results have shown that such comparisons are necessary to improve the accuracy of the measurement and to identify malfunctioning radionuclide calibrators.
