Current Status and Improvements of Transfered PET/CT Data from Other Hospitals (외부 반출 PET/CT 영상 현황 및 개선점)

  • Purpose: This study was performed to find the current problems of PET/CT data from other hospitals. Materials and Methods: The subjects were acquired from 64 hospitals referred to our department for image interpretation. The formats and contents of PET/CT data were reviewed and the phone questionnaire survey about these were performed. Results: PET/CT data from 39 of 64 hospitals (61%) included all transaxial CT and PET images with DICOM (Digital Imaging Communications in Medicine) standard format which were required for authentic interpretation. PET/CT data from the others included only secondary capture images or fusion PET/CT images. Conclusion: The majority of hospitals provided limited PET/CT data which could be inadequate for accurate interpretation and clinical decision making. It is necessary to standardize the format of PET/CT data to transfer including all transaxial CT and PET images with DICOM standard format.

Effects of Activation Regimens of Recipient Cytoplasm, Culture Condition of Donor Embryos and Size of Blastomeres on Development of Reconstituted Bovine Embryos (수핵 난자의 활성화 방법과 공핵 수정란의 배양체계 및 할구의 크기가 소 핵이식 수정란의 발달에 미치는 영향)

  • To improve the efficiency of nuclear transplantation in bovine, in this study the development in vitro of nuclear transferred (NT) embryos was compared by different activation regimens of the enucleated oocytes. The effect of developmental stage and culture system of donor nuclei on fusion and development in vitro of NT embryos were also evaluated. Oocytes were collected from Hanwoo ovaries obtained from slaughterhouse and matured in Ham's F-10 supplemented with hormones. After 20~22 h maturation, the oocytes were vortexed to be free from cumulus cells and subsequently their nucleus and the first polar body were removed. Enucleated oocytes were divided into 3 groups for activation; the oocytes of group I were activated with ionomycin for 5 min and subsequently incubated in 6-dimetylarninopurine (DMAP) for 4 h, Those of group II were treated with DMAP for 4 h at 39 h after onset of in vitro maturation (IVM) and those of group III were kept in room temperature ($25^{\circ}C$) for 3 h at 39 h after onset of IVM. After in vitro fertilization (IVF) the embryos for muclear donor were cultured either by group culture (20 embryos /50 ${mu}ell$ drop) or individually (1 embryo /50 ${mu}ell$ drop) for 4 day and 5 day. At day 4 and 5 after IVF, blastomeres were separated in calcium-magnesium free medium, and then classified into small (day 5: $\leq$ 38 ${\mu}{\textrm}{m}$, day 4: $\leq$ 46 ${\mu}{\textrm}{m}$) and large (day 5 : $\geq$ 38 ${\mu}{\textrm}{m}$, day 4 ; $\geq$ 46 ${\mu}{\textrm}{m}$). The separated blastomeres were replaced into enucleated and activated recipient cytoplasm. The blastomere-oocyte complexes were fused by electrically. The NT embryos were cultured in TCM-199 containing 10% FCS in 39$^{\circ}C$, 5% $CO_2$ incubator for 7 day. The results obtained were summarized as follows; There were no differences in fusion and development to blastocyst between groups as group I (68%, 10%), group II (75%, 14%) and group III (73%, 9%), respectively. However, the cell number in blastocyst of NT embryos in group III were significantly fewer than in the other groups (P<0.05). No differences in fusion and development to blastocyst were found between individual or group cultured and between small or large blastomeres of day 4 and day 5 donor embryos. From these results, it was concluded that the combination of ionomycin and DMAP, or treatment of DMAP at 39 h after onset of IVM were useful for the efficient of production of NT bovine embryos, and the individual cultured embryos could be simply used as donor nuclei for NT bovine embryo.

Reduction of Injection Dose in 18F-FDG Fusion PET (PET-CT 검사에서 18F-FDG 투여량 감소에 대한 고찰)

  • Purpose With the recent rise of social issue regarding radiation exposure, attention to medical radiation use has been placed under a great spotlight. During PET-CT examination, generally about 40% more of $^{18}F$-FDG is used than EANM recommendation. While maintaining the diagnostic test result, we hope to find optimal injection dose to minimize the $^{18}F$-FDG in patients by utilizing the latest PET-CT scanner which is equiped with the newest technology. Materials and Methods During this experiment, the Biograph Truepoint 40 (siemens, USA) installed in 2007 and mCT 64 (siemens, USA) installed in 2011 were used and evaluated NECR (noise-equivalent counting rate) by using a scatter phantom. For the image quality evaluation of each scanner, we injected 3.7, 4.44 and 5.18 MBq/kg of $^{18}F$-FDG in NEMA IEC Body Phantom and also evaluated SNR between two scanners by using the data acquired at 60, 70, 80, 90, 100, 110 and 120 sec per bed. For the clinical evaluation, actual data of patients who were injected $^{18}F$-FDG 3.7, 4.44, 5.18 MBq/kg were used to compare SNR and draw a final result. Results As a result, mCT 64 peak NECR value was 1.65e+005, which is 10% higher than Turepoint 40. SNR values using the IEC body phantom was 17.9%, 17.4% and 17.1% higher in $^{18}F$-FDG 3.7 MBq/kg, 4.44 MBq/kg and 5.18 MBq/kg. In clinical patients, SNR values of the image mCT 64 was 16.5, which is 25% higher than Turepoint 40 scanner. Conclusion To draw a conclusion from the test result of this experiment, the same quality of SNR could be attained even with 10% reduced injection dose, if when the duration is extended by 10 sec/bed. This optimal result was possible due to enhanced equipment. The NECR (one of the equipment's performance assessment criteria for the scanner) increased by 10% and the SNR (one of the image quality assessment criteria) also increased by 17.5%. Therefore, we can expect to reduce the injection dose without deterioration of image quality. In consequence, it will also help to decrease the patient's anxiety of the radiation exposure.

  • Recently in nuclear medicine, to improve diagnostic value, SUV, semi-quantitative indicator used in PET-CT, is adopted in SPECT-CT. Therefore, this research analyzed correlation of $SUV_{max}$ of two scanners through phantom test, and evaluated possibility of clinical application. Radiopharmaceuticals ($^{18}F$, $^{99m}Tc$) were injected with the ratios of 8:1 and 4:1, considering background radioactivity, into the phantom manufactured with 6 globes of different sizes, and, based on clinical protocol, positive phases were acquired with PET-CT and SPECT-CT scanners, and interesting areas were divided into ROI and VOI, and $SUV_{max}$ of them were measured, and analyzed. Tests found out no statistically significant difference in $SUV_{max}$ measured with two scanners (P>0.05). Thus, $SUV_{max}$ of PET-CT and SPECT-CT had a certain correlation within significant levels, and were evaluated as the same. Accordingly, it seems that $SUV_{max}$ quantitative analysis using SPECT-CT can provide significant diagnostic information as the case of PET-CT.

Evaluation of metabolic tumor volume using different image reconstruction on 18F-FDG PET/CT fusion image (18F-FDG PET/CT 융합영상에서 영상 재구성 차이에 의한 MTV (Metabolic tumor volume) 평가)

  • Recently, MTV(metabolic tumor volume) has been used as indices of the whole tumor FDG uptake on FDG PET image but it is influenced by image reconstruction. The purpose of this study was to evaluate the correlation between actual volume and metabolic tumor volume applying different SUVmax threshold for different reconstruction algorithm on phantom study. Measurement were performed on a Siemens Biograph mCT40 using a NEMA IEC body phantom containing different size six spheres filled with F18-FDG applying four SBRs (4:1, 8:1, 10:1, 20:1). Images reconstructed four algorithms (OSEM3D, OSEM3D+PSF, OSEM3D +TOF, OSEM3D+TOF+PSF) and MTV were measured with different SUVmax threshold. Overall, the use of increasing thresholds result in decreasing MTV. and increasing the signal to background ratio decreased MTV by applying same SUVmax threshold. The 40% SUVmax threshold gave the best concordance between measured and actual volume in PSF and PSF+TOF reconstruction image. and the 45% threshold had the best correlation between the volume measured and actual volume in OSEM3D and TOF reconstruction image. we believe that this study will be used when the measurement of MTV applying various reconstruction image.

Studies on OsABF3 Gene Isolation and ABA Signal Transduction in Rice Plants Against Abiotic Stress (비 생물학적 스트레스 시 벼에서 OsABF3 유전자 분리와 ABA 신호전달 대한 연구)

  • Abscisic acid (ABA) is an important phytohormone involved in abiotic stress tolerance in plants. The group A bZIP transcription factors play important roles in the ABA signaling pathway in Arabidopsis but little is known about their functions in rice. In our current study, we have isolated and characterized a group A bZIP transcription factor in rice, OsABF3 (Oryza sativa ABA responsive element binding factor 3). We examined the expression patterns of OsABF3 in various tissues and time course analysis after abiotic stress treatments such as drought, salinity, cold, oxidative stress, and ABA in rice. Subcellular localization analysis in maize protoplasts using a GFP fusion vector further indicated that OsABF3 is a nuclear protein. Moreover, in a yeast one-hybrid experiment, OsABF3 was shown to bind to ABA responsive elements (ABREs) and its N-terminal region found to be necessary to transactivate a downstream reporter. A homozygous T-DNA insertional mutant of OsABF3 is more sensitive to salinity, drought, and oxidative stress compared with wild type plants & OsABF3OX plants. In addition, this Osabf3 mutant showed a significantly decreased sensitivity to high levels of ABA at germination and post-germination. Collectively, our present results indicate that OsABF3 functions as a transcriptional regulator that modulates the expression of abiotic stress-responsive genes through an ABA-dependent pathway.

Quantitative Feasibility Evaluation of 11C-Methionine Positron Emission Tomography Images in Gamma Knife Radiosurgery : Phantom-Based Study and Clinical Application

  • Objective : The functional information of $^{11}C$-methionine positron emission tomography (MET-PET) images can be applied for Gamma knife radiosurgery (GKR) and its image quality may affect defining the tumor. This study conducted the phantom-based evaluation for geometric accuracy and functional characteristic of diagnostic MET-PET image co-registered with stereotactic image in Leksell $GammaPlan^{(R)}$ (LGP) and also investigated clinical application of these images in metastatic brain tumors. Methods : Two types of cylindrical acrylic phantoms fabricated in-house were used for this study : the phantom with an array-shaped axial rod insert and the phantom with different sized tube indicators. The phantoms were mounted on the stereotactic frame and scanned using computed tomography (CT), magnetic resonance imaging (MRI), and PET system. Three-dimensional coordinate values on co-registered MET-PET images were compared with those on stereotactic CT image in LGP. MET uptake values of different sized indicators inside phantom were evaluated. We also evaluated the CT and MRI co-registered stereotactic MET-PET images with MR-enhancing volume and PET-metabolic tumor volume (MTV) in 14 metastatic brain tumors. Results : Imaging distortion of MET-PET was maintained stable at less than approximately 3% on mean value. There was no statistical difference in the geometric accuracy according to co-registered reference stereotactic images. In functional characteristic study for MET-PET image, the indicator on the lateral side of the phantom exhibited higher uptake than that on the medial side. This effect decreased as the size of the object increased. In 14 metastatic tumors, the median matching percentage between MR-enhancing volume and PET-MTV was 36.8% on PET/MR fusion images and 39.9% on PET/CT fusion images. Conclusion : The geometric accuracy of the diagnostic MET-PET co-registered with stereotactic MR in LGP is acceptable on phantom-based study. However, the MET-PET images could the limitations in providing exact stereotactic information in clinical study.

  • Plant Chloroplast have several advantages as an expression platform of biopharmaceuticals over conventional expression platforms such as mammalian cells, yeast and bacteria. First, plants do not serve as a host for mammalian infectious virus and have endotoxin like bacteria which can cause anaphylactic shock. In addition, high copy number of chloroplast genome allows for chloroplast transformants to reach the high level of expression of heterologous genes. Moreover, the integration of transgenes into specific region of chloroplast genomes makes chloroplast transformants unaffected by positional effect which can be frequently observed from nuclear transformants, resulting in loss of transgene expressions. Antimicrobial peptides (AMPs) are a kind of innate immunity which is found from bacteria to humans. Unlike conventional antibiotics, very less dosage of AMPs can have catastrophic effect on bacterial survival. Further, the repeated use of AMPs does not trigger the development of bacterial resistance. Moricin, one of the AMPs, was isolated from Bombyx mori, a silkworm moth. The C-terminal of moricin consists largely of basic amino acids, and the N-terminal has an α-helix structure. Moricin was chosen and expressed in a SUMO/SUMOase without leaving any unwanted amino acids which could potentially affect the anti-bacterial activity of the moricin. The transformation vector used in this study has already been created in this lab for the expression in both prokaryotic systems such as E. coli and chloroplast. The expressed moricin was purified using Ni columns and SUMOase, and the antibacterial activity of the purified moricin was confirmed using an agar diffusion assay.

A study on evaluation of the image with washed-out artifact after applying scatter limitation correction algorithm in PET/CT exam (PET/CT 검사에서 냉소 인공물 발생 시 산란 제한 보정 알고리즘 적용에 따른 영상 평가)

  • Purpose In PET/CT exam, washed-out artifact could occur due to severe motion of the patient and high specific activity, it results in lowering not only qualitative reading but also quantitative analysis. Scatter limitation correction by GE is an algorism to correct washed-out artifact and recover the images in PET scan. The purpose of this study is to measure the threshold of specific activity which can recovers to original uptake values on the image shown with washed-out artifact from phantom experiment and to compare the quantitative analysis of the clinical patient's data before and after correction. Materials and Methods PET and CT images were acquired in having no misalignment(D0) and in 1, 2, 3, 4 cm distance of misalignment(D1, D2, D3, D4) respectively, with 20 steps of each specific activity from 20 to 20,000 kBq/ml on $^{68}Ge$ cylinder phantom. Also, we measured the distance of misalignment of foley catheter line between CT and PET images, the specific activity which makes washed-out artifact, $SUV_{mean}$ of muscle in artifact slice and $SUV_{max}$ of lesion in artifact slice and $SUV_{max}$ of the other lesion out of artifact slice before and after correction respectively from 34 patients who underwent $^{18}F-FDG$ Fusion Whole Body PET/CT exam. SPSS 21 was used to analyze the difference in the SUV between before and after scatter limitation correction by paired t-test. Results In phantom experiment, $SUV_{mean}$ of $^{68}Ge$ cylinder decreased as specific activity of $^{18}F$ increased. $SUV_{mean}$ more and more decreased as the distance of misalignment between CT and PET more increased. On the other hand, the effect of correction increased as the distance more increased. From phantom experiments, there was no washed-out artifact below 50 kBq/ml and $SUV_{mean}$ was same from origin. On D0 and D1, $SUV_{mean}$ recovered to origin(0.95) below 120 kBq/ml when applying scatter limitation correction. On D2 and D3, $SUV_{mean}$ recovered to origin below 100 kBq/ml. On D4, $SUV_{mean}$ recovered to origin below 80 kBq/ml. From 34 clinical patient's data, the average distance of misalignment was 2.02 cm and the average specific activity which makes washed-out artifact was 490.15 kBq/ml. The average $SUV_{mean}$ of muscles and the average $SUV_{max}$ of lesions in artifact slice before and after the correction show a significant difference according to a paired t-test respectively(t=-13.805, p=0.000)(t=-2.851, p=0.012), but the average $SUV_{max}$ of lesions out of artifact slice show a no significant difference (t=-1.173, p=0.250). Conclusion Scatter limitation correction algorism by GE PET/CT scanner helps to correct washed-out artifact from motion of a patient or high specific activity and to recover the PET images. When we read the image occurred with washed-out artifact by measuring the distance of misalignment between CT and PET image, specific activity after applying scatter limitation algorism, we can analyze the images more accurately without repeating scan.

Interspecies Nuclear Transfer using Bovine Oocytes Cytoplasm and Somatic Cell Nuclei from Bovine, Porcine, Mouse and Human (소, 돼지, 생쥐, 사람의 체세포와 소 난자를 이용한 이종간 핵 이식)

  • This study was designed to examine the ability of the bovine (MII) oocytes cytoplasm to support several mitotic cell cycles under the direction of differentiated somatic cell nuclei of bovine, porcine, mouse and human. Bovine GV oocytes were matured in TCM-199 supplemented with 10% FBS. At 20h after IVM, recipient oocytes were stained with 5 $\mu\textrm{g}$/$m\ell$ Hoechst and their 1st polar body (PB) and MII plate were removed by enucleation micropipette under UV filter. Ear skin samples were obtained by biopsy from an adult bovine, porcine, mouse and human and cultured in 10% FBS added DMEM. Individual fibroblast was anlaysed chromosome number to confirm the specificity of species. Nuclear transferred (NT) units were produced by electrofusion of enucleated bovine oocytes with individual fibroblast. The reconstructed embryos were activated in 5 $\mu$M ionomycin for 5 min followed by 1.9 mM 6-dimethylaminopurine (DMAP) in CR1aa for 3 h. And cleaved NT embryos were cultured in CR1aa medium containing 10% FBS on monolayer of bovine cumulus cell for 8 days. Also NT embryo of 4~8 cell stage was analysed chromosome number to confirm the origin of nuclear transferred somatic cell. The rates of fusion between bovine recipient oocytes and bovine, porcine, mouse and human somatic cells were 70.2%, 70.2%, 72.4% and 63.0%, respectively. Also, their cleavage rates were 60.6%, 63.7%, 54.1% and 62.7%, respectively, there were no differences among them. in vitro development rates into morula and blastocyst were 17.5% and 4.3% in NT embryos from bovine and human fibroblasts, respectively. But NT embryos from porcine and mouse fibroblasts were blocked at 16~32-cell stage. The chromosome number in NT embryos from individual fibroblast was the same as chromosome number of individual species. These results show that bovine MII oocytes cytoplasm has the ability to support several mitotic cell cycles directed by newly introduced nuclear DNA.
