EFFICACY OF HAND REAMER AND ENGINE REAMER TO PREPARE ROOT CANAL (수동(手動)리머와 전동(電動)리머의 근관형성효과(根管形成效果))

  • This experimental study was made to evaluate the efficacy of root canal preparation of engine reamer versus hand reamer. Eighty extracted human teeth were prepared with the following treatments and devided into 4 groups; Group 1: Canal preparation with hand reamer, and irrigation with normal saline solution. Group 2: Canal preparation with engine reamer, and irrigation with normal saline solution. Group 3: Canal preparation with hand reamer, and irrigation with 3% hydrogen peroxide and 3.5% sodium hypochlorite solution. Group 4: Canal preparation with engine reamer, and irrigation with 3% hydrogen peroxide and irrigation with 3% hydrogen peroxide and 3.5% sodium hypochlorite solution. After decalcification, 5p. sections at levels 1-, 3-, and 5-mm from the apex were evaluated microscopically. The results were as follows; The effectiveness of hand reamer and engine reamer were equal in preparing the root canal at all levels from the apex, and were equal in normal saline solution groups, and 3% $H_2O_2$ and 3.5% NaOCl solution groups. Both in hand reaming groups and in engine reaming groups, some canal walls had still untouched portion, and canal irregularities and debris were still remained.

The Effects of Panax Ginseng Radix Pharmacopuncture and Zizyphi Spinosi Semen Pharmacopuncture on the Heart Rate Variability (인삼약침과 산조인약침이 정상인의 심박 변이도에 미치는 영향)

  • Objectives : We tried to investigate the effects of Panax Ginseng Radix pharmacopuncture and Zizyphi Spinosi Semen pharmacopuncture on human body with the Heart Rate Variability (HRV) in adult man. As well as we tried to investigate how safe Panax Ginseng Radix and Zizyphi Spinosi Semen pharmacopuncture on the the human being. Methods : We investigated on 44 healthy volunteers consisted of 16 subjects in Panax Ginseng Radix pharmacopuncture group, 13 subjects in Zizyphi Spinosi Semen pharmacopuncture group and 15 subjects in Normal Saline. We ruled out subjects who wasn't stable by rest, with in the limit of normal heart beat(60-120cycle/min). Study form was a randomized, clinical trial. Each group was injected Panax Ginseng Radix pharmacopuncture, Zizyphi Spinosi Semen pharmacopuncture and Normal Saline $0.2m{\ell}$ at $GB_{21}$(Kyonjong). We measured HRV on 7 times : before and after injection per 5 minutes during 30 minutes. The SPSS 13.0 for windows was used to analyze the data by the paired t-test(in the group) and Anova test(among the groups) were used to verify the result. Results : 1. After Panax Ginseng Radix pharmacopuncture injection, SDNN was significantly high, pNN50 was significantly low, Ln(TP) was significantly high, Ln(VLF) was significantly high, Ln(LF) was significantly high at 30 minutes. 2. After Zizyphi Spinosi Semen pharmacopuncture injection, Complexity was significantly low, Ln(VLF) was significantly high at 30 minutes. 3. HRV-index made a significant difference between the Zizyphi Spinosi Semen pharmacopuncture and normal saline at 5 minutes(p=0.02). 4. There wasn's any pain the injected region and whole body, rubefaction, uredo, nausea, vomiting, colic, headache, dizziness within 24 hours. Measured data and mean value of Mean-HRV was within normal range. Conclusions : The results suggested that and Panax Ginseng Radix pharmacopuncture in healthy adult man tended to activate the vagus nervous system within normal range.

Effect of Cynomorii Herba on the Sperm Concentration, Motility and Spermatogenesis in Male Rat (쇄양(鎖陽)이 백서(白鼠) 정자(精子)의 수(數), 운동성(運動性) 및 형성(形成)에 미치는 영향(影響))

  • Purpose : This study was conducted to investigate the effects of Cynomorii Herba (鎖陽) on the reproductive competence in male mice. Methods : We used 8-week-old Spraque-Dawley rats and administered the extract powder of Cynomorii Herba to 5 rats and normal saline to control group once in a day for 28 days. Then we observed the changes of body weight before and after administration of Cynomorii Herba extracts and normal saline. We isolated their testis surgically and observed the weight of testis, epididymis, vascular gland and prostate. Also we examined the total, normal and motile sperm concentration, the concentration of testicular catalase, peroxidase and configuration of testicular tissue before and after administration of Cynomorii Herba extracts and normal saline. Results : We found that the concentration of total, normal, motile sperm in testis of Cynomorii Herba treated group shows significant difference compared with the control group. The body weight, the weight of testis, epididymis and the concentration of testicular catalase and peroxidase were higher in the Cynomorii Herba treated group, on the other hand the weight of vascular gland was lower in the Cynomorii Herba. But the contents of above statement showed no significant difference. The weight of prostate showed lower in the control group statistically than that of the Cynomorii Herba treated group. We observed the seminiferous tubules taken shape minutely and the number of normal sperm increased in Cynomorii Herba treated group's testicular tissue. Conclusion This study shows that Cynomorii Herba may have an effect on the concentration, morphology and motility of sperm, the important factor in male fertility.

The Clinical Usefulness Evaluation of Normal Saline Injection in Coronary Artery Computed Tomography Angiography(Coronary CTA) (관상동맥 전산화단층촬영 조영검사에서 생리식염수 투여를 통한 임상 유용성 평가)

  • The purpose of this study is that in coronary artery angiography computed tomography (coronary CTA), to gain high quality of image and to use low dose radiation by administrating normal saline and converting the mode of scanning heart rate (HR) characteristics before infusing contrast media. All patients data (total specimens: 200, male: 108, female: 92) were measured by using appropriate mode of scanning the heart rate (HR) after injection of saline. in addition we measured radiation dose (CTDIvol, effective dose) in all examinations. CT number and noise, and blurring of coronary artery (proximal RCA, middle RCA, proximal LCA) were measured and compared. The result of this study after injection of saline, mean heart rate was decreased about $4.8{\pm}0.3bpm$ (beats per minute). 33 patients (13%) got converting scan mode due to reducing heart rate (HR). In prospective gating mode, radiation dose were measured less $6.0{\pm}1.0mSv$ (54.1%) than retrospective gating mode. Also showed a significant difference in heart rate decrease in image evaluation.

The Effects of Cervus elaphus Aquapuncture and Ginseng Radix Aquapuncture on the Growth of Animals (녹용약침과 인삼약침이 동물의 성장에 미치는 영향)

  • The effects of Cervus elaphus aquapuncuture and Ginseng Radix aquapuncuture on the growth, protein efficiency ratio, contents of total protein, total cholesterol, and triglyceride in serum were studied for thirty four days. The results were summarized as follows. 1. Weight significantly increased in $0.3\%$ Cervus elaphus aquapuncture group on Chunchu loci and in $0.2\%$ Ginseng Radix aquapuncture group on Chunchu loci compared to normal saline aquapuncture group on Chunchu loci. 2. Protein efficiency ratio significantly increased in $0.2\%$ and $0.3\%$ of Cervus elaphus aquapuncture group on Chunchu loci and $0.1\%$, $0.2\%$ and $0.3\%$ of Ginseng Radix aquapuncture group on Chunchu loci compared to nonnal saline aquapuncture on Chunchu loci. 3. Total protein significantly increased in $0.2\%$ and $0.3\%$ of Cervus elaphus aquapuncture group on Chunchu loci compared to nonnal saline aqriapuncture On Chunchu loci. whereas it had a tendency to increase in $0.2\%$ Ginseng Radix aquapuncture group on Chunchu loci compared to normal saline aquapuncture group on Chunchu loci. 4. Total cholesterol increased a little in $0.3\%$ Cervus elaphus aquapuncture group on Chunchu loci compared to normal saline aquapuncture group on Chunchu loci. 5. Triglyceride significantly increased in $0.2\%$ Ginseng Radix aqua puncture group on Chunchu loci compared to normal saline aquapuncture group on Chunchu loci. Significance was acknowledged in the effect by density difference of medicines. In the Cervus elaphus group, there was the most significant increase in density of $0.3\%$ aquapuncture, whereas in Ginseng Radix group, there was the most significant increase in density of $0.2\%$ aquapuncture. So setting up density of medicines is important and many various studies for clinical application are required.

Effects of Arctii Fructus on the Atopic dermatitis (우방자(牛蒡子)가 아토피 피부염에 미치는 영향)

  • Objective : Atopic dermatitis has a close relationship with degranulation of mast cell and separation of Histamine. As there's no experiment with herb, using Arctii Fructus, we investigated experimental influence of Arctii Fructus on degranulation of mast cell and separation of histamine in SD rat. Methods : The SD rats are classified into three groups. One group is normal one treated by normal saline before medical treatment. The other is control group prescribed to Compound 48/80 before normal saline treatment. And the third is experimental group prescribed to compound 48/80 after medical treatment of Arctii Fructus. Then, I investigated the experimental results by measuring the degree of degranulation and separation of histamine. The results of investigation on SD rat group showing the degree of inhibitory effect of degranulation of a mast cell are as follow, the normal group treated by normal saline reflecting the degree of degranulation is $6.10{\pm}0.20\;%$, the control group treated by only compound 48/80 is $87.56{\pm}11.00\;%$, the experimental group which treated by compound 48/80 and Arctii Fructus's medical treatment is $16.26{\pm}4.67\;%$. Results : The normal group treated by only normal saline reflecting the degree of degranulation is $6.10{\pm}0.20\;%$, the control group treated by only compound 48/80 is 87.56=11.00 %, the experimental group treated by compound 48/80 and Arctii Fructus's medical treatment is $16.26{\pm}4.67\;%$. This result indicates that the degree of degranulation of mast cell is obviously inhibited (p<0.0l) in the experimental group in comparison with control one. The analysis of data obtained from plasma, which collected from the experimented SDrats' hearts before their death, and the measurement of quantity of histamine secretion show the following results. The quantity of normal group and control one is $25.34{\pm}4.58$ nM, $348.59{\pm}30.77$ nM respectively, and experimental one prescribed to compound 48/80 after medical treatment of Arctii Fructus is $263.56{\pm}21.34$ nM. This result indicates that separation of histamine isobviously inhibited in the experimental group in comparison with control one (p<0.05). Conclusions : Arctii Fructus does obviously inhibit the degree of degranulation of mast cell (p<0.0l) and separation of histamine in the plasma (p<0.05).

Influence of Iodinated Contrast Media and Paramagnetic Contrast Media on Changes in Uptake Counts of 99mTc

  • The purpose of this study is to figure out how uptake counts of technetium ($^{99m}Tc$) among radioisotopes in the human body are affected if computed tomography (CT), magnetic resonance imaging (MRI) and isotope examination are performed consecutively. $^{99m}Tc$ isotope material, iodinated contrast media for CT and paramagnetic contrast media for magnetic resonance (MR) were used as experimental materials. First, $^{99m}Tc$ was added to 4 cc normal saline in a test tube. Then, 2 cc of CT contrast media such as $Iopamidol^{(R)}$ and $Dotarem^{(R)}$ were diluted with 2 cc normal saline, and 2cc of MRI contrast media such as $Primovist^{(R)}$ and $Gadovist^{(R)}$ were diluted with 2 cc normal saline. Each distributed contrast media was a total of 4 cc and included 10m Ci of $^{99m}Tc$. A gamma camera, a LEHR (Low energy high resolution) collimator and a pin-hole collimator were used for image acquisition. Image acquisition was repeated a total of 6 times and 120 frames were obtained and uptake counts of $^{99m}Tc$ were measured (from this procedure). In this study, as a result of measuring the uptake counts of $^{99m}Tc$ using the LEHR collimator, the uptake counts were less measured in all contrast media than normal saline as a reference. In particular, the lowest uptake counts were measured when $Gadovist^{(R)}$, contrast media for MRI, was used. However, the result of measuring the uptake counts of $^{99m}Tc$ using the pin-hole collimator showed higher uptake counts in all contrast media, except for $Iopamidol^{(R)}$, than normal saline as a reference. The highest uptake counts were measured particularly when $Primovist^{(R)}$, contrast media for MRI, was used. In performing the gamma camera examination using contrast media and $^{99m}Tc$, it is considered significant to check the changes in the uptake counts to improve various diagnosis values.

Effect of Intravenous Lidocaine on the Neuropathic Pain of Failed Back Surgery Syndrome

  • Background: An intravenous infusion of lidocaine has been used on numerous occasions to produce analgesia in neuropathic pain. In the cases of failed back surgery syndrom, the pain generated as result of abnormal impulse from the dorsal root ganglion and spinal cord, for instance as a result of nerve injury may be particularly sensitive to lidocaine. The aim of the present study was to identify the effects of IV lidocaine on neuropathic pain items of FBSS. Methods: The study was a randomized, prospective, double-blinded, crossover study involving eighteen patients with failed back surgery syndrome. The treatments were: 0.9% normal saline, lidocaine 1 mg/kg in 500 ml normal saline, and lidocaine 5 mg/kg in 500 ml normal saline over 60 minutes. The patients underwent infusions on three different appointments, at least two weeks apart. Thus all patients received all 3 treatments. Pain measurement was taken by visual analogue scale (VAS), and neuropathic pain questionnaire. Results: Both lidocaine (1 mg/kg, 5 mg/kg) and placebo significantly reduced the intense, sharp, hot, dull, cold, sensitivity, itchy, unpleasant, deep and superficial of pain. The amount of change was not significantly different among either of the lidocaine and placebo, or among the lidocaine treatments themselves, for any of the pain responses, except sharp, dull, cold, unpleasant, and deep pain. And VAS was decreased during infusion in all 3 group and there were no difference among groups. Conclusions: This study shows that 1 mg/kg, or 5 mg/kg of IV lidocaine, and palcebo was effective in patients with neuropathic pain attributable to FBSS, but effect of licoaine did not differ from placebo saline.

Effect of Administration of Sodium Carboxymethylcellulose and Dextran on Prevention of Adhesion Formation on Uterus and Embryo Collection in Rabbits (Sodium Carboxymethylcellulose 및 Dextran의 투여가 토끼 자궁의 유착방지와 수정란에 미치는 영향)

  • The effectiveness of sodium carboxymethylcellulose (SCMC) and deztran in the prevention of adhesion formation on the uterus and embryo collection in rabbits was elucidated. Following induction of adhesion on uterus and uterine horns by abrasion and retrograde flushing of embryos in gonadotropins primed rabbits, the solutions of saline (for control), 1% SCMC, 10% dextran and a synthetic solution of 1% SCNC and 10% dextran in saline were infused in the abdominal cavity at the dose of 5 ml/kg of body weight. The average percentage of adhesion was 11.1, 28.6, 41.7 and 73.3% in the rabbits infused with the synthetic, 1% SCMC, 10% dextran and saline solutions, respectively. The synthetic solution was more effective than other solutions in the rabbits. The average number and recovery rate of embryos were significantly (P<0.01) higher in the synthetic solution group than 1% SCMC, 10% dextran or saline solution groups. Among the collected embryos in the groups, the distribution of the normal embryos was higher in the synthetic solution group (99%) and the 10% dextran solution group (95.7%) than the 1% SCMC solution group (78.4%) and saline (66.2%). Theretore, a synthetic solution which is combined with 1% SCMC and 10% deztran in saline can be effectively used for the prevention of adhesion formation after uterine surgery and embryo collection.

Effect of the Exchange of Saline Used in Surgical Procedures on Surgical Site Infection (수술에 사용하는 생리식염수의 교환이 수술부위감염에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effect of the exchange of saline used in surgical procedures on surgical site infections. Method: Patients with stomach cancer were assigned to the experimental group or to the control group by random sampling, respectively. The experimental group received an exchange of saline during the operation right after the excision of the stomach in a gastrectomy but the control group did not. Data were collected from the medical charts of 34 patients from Dec. 1, 2002 through May 31, 2003. Result: The surgical site infection rate of the experimental group was 5.9% while surgical site infection rate of the control group was 17.6%. In total, the surgical site infection rate was 11.8%. The experimental group maintained a normal level of WBC on post operative day 3; however, the control group, showed an increase of WBC on post operative day 3. Conclusion: The exchange of saline used in an operation immediately after the excision of the stomach in a gastrectomy decreases the contamination level of saline used in the operation, and can prevent surgical patients from a surgical site infection.
