Human Epididymis Protein 4 Reference Intervals in a Multiethnic Asian Women Population

  • Background: Ovarian cancer is ranked as the fifth most common cause of cancer death in women. In Malaysia, it is the fourth most common cancer in females. CA125 has been the tumor marker of choice in ovarian cancer but its diagnostic specificity in early stages is only 50%. Hence, there is a critical need to identify an alternative tumor marker that is capable of detecting detect ovarian cancer at an early stage. HE4 is a new tumor marker proposed for the early diagnosis of ovarian cancer and disease recurrence. Currently, none of the normal ranges of HE4 quoted in the literature are based on data for a multiethnic Asian population. Therefore, the aim of this study was to determine reference intervals for HE4 in an Asian population presenting in University Malaya Medical Centre, a tertiary reference hospital. Materials and Methods: 300 healthy women were recruited comprising 150 premenopausal and 150 postmenopausal women, aged from 20-76 years. All women were subjected to a pelvic ultrasonograph and were confirmed to be free from ovarian pathology on recruitment. Serum HE4 levels were determined by chemiluminescent microparticle immunoassay (CMIA, Abbott Architect). The reference intervals were determined following CLSI guidelines (C28-A2) using a non-parametric method. Results: The upper limits of the $95^{th}$ percentile reference interval (90%CI) for all the women collectively were 64.6 pmol/L, and 58.4 pmol/L for premenopausal) and 69.0 pmol/L for postmenopausal. The concentration of HE4 was noted to increase with age especially in women who were more than 50 years old. We also noted that our proposed reference limit was lower compared to the level given by manufacturer Abbott Architect HE4 kit insert (58.4 vs 70 pmol/L for premenopausal group and 69.0 vs 140 pmol/L in the postmenopausal group). The study also showed a significant difference in HE4 concentrations between ethnic groups (Malays and Indians). The levels of HE4 in Indians appeared higher than in Malays (p<0.05), while no significant differences were noted between the Malays and Chinese ethnic groups. Conclusions: More data are needed to establish a reference interval that will better represent the multiethnic Malaysian population. Probably a larger sampling size of equal representation of the Malay, Chinese, Indians as well as the other native ethnic communities will give us a greater confidence on whether genetics plays a role in reference interval determination.

A Study on Client Selection Criteria for Home Health Care in Patients with Cerebral Vascular Disease (뇌혈관 질환 환자 사정에 대한 의료인간의 일치도 및 가정간호사업 대상자 선정에 관한 연구)

  • The primary purpose of this study is to promote and establish the development of home health care in Korea. It focuses on identifying and classifying the nursing activities that were provided by health professionals for patients who were admitted to two hospitals with cerebral vascular disease. And also. the study was conducted for comparison of client selection criteria between health professionals, and identifying patients who needed home health care The subjects of this study were 38 patients with cerebral vascular disease who were admitted to neuro-surgery wards at 2 hospitals with more than 500 beds in Daegu from November 1. through 30. 1991. Survey instruments were questionnaires to identify nursing activities and classify patients who needed home health care. Data was collelected by 1 doctor and 2 nurses per patient independently. They checked the same patient with the questionnaire on the same day and never communicated their information about patients with each other. All the questionnaires checked by doctor and nurses completely were 90. Statistical methods for analyzing data were non-parametric tests (Kruskal Wallice test and sign test). Correlation and percentages were used for further analysis. From this study. the following summarized conclusions have been drawn. 1. 10. 2 kinds of treatment and nursing activities were provided by health professionals for patients with cerebral vascular disease in hospital. 2. The points of nursing needs were between 32-37 out of a total of 500 as a result of the assessment about the health status of patients who were admitted to neuro-surgery ward with cerebral vascular disease. The points of Barthel Index of Functional Status Assessing Devices were between 24-34. Client Selection Criteria for Home Health Care was congruent between the Health professionals because the difference were not found to be statistically significant. 3. Patients classfied as home health care clients were $70-80\%$ of all patients who were admitted in hospital. There was not significant difference in patient selection criteria for home health care between health professionals statistically. As a result. the validity of different tools used in classifying home health care client were found to be congruent. 4. $80-85\%$ patients who could be discharged and sent to their homes early were identified as home health care clients. This study using client selection criteria. for home health care contributed to tool development because the validity of tools was verified. And also, this research represented that there was congruency in patient selection criteria for home health care between different health professionals. As a result, this study represented that many patients who were admitted to hospitals could be classified as a home health care clients. On the basis of the findings. further studies are required to develop client selection criteria using universal tools for classifying home health care clients in other chronic diseases. It is also recommended that comparative studies for client selection criteria between health professionals treating in other chronic diseases are necessary.

Analysis of Allowable Stresses of Machine Graded Lumber in Korea (국내 기계등급구조재의 허용응력 분석)

  • 365 pieces of domestic $38{\times}140{\times}3600mm$ Red pine structural lumber were machine graded conforming to a softwood structural lumber standard (KS F 3020). The allowable bending stresses calculated for each grade were compared with the values currently tabulated in the standard. Four calculation methods for lower $5^{th}$ percentile bending stress were non-parametric estimation with 75% confidence level, 2-parameter and 3-parameter Weibull distribution fit, and bending modulus of rupture (MOR)-modulus of elasticity (MOE) regression based method. Only the data set of Grades E8, E9, and E10 were statistically eligible for the $5^{th}$ percentile calculation. The MOR-MOE regression based method only was able to estimate the lower $5^{th}$ percentile values theoretically for the full range of grades. The results showed that all allowable bending stresses calculated were lower than the design values tabulated in the standard. This implies that the current machine grading system has the pitfall of structural safety. Improvement in current machine grading system could be achieved by introducing the bending strength and stiffness combination grade system.

Estimation of the Regional Future Sea Level Rise Using Long-term Tidal Data in the Korean Peninsula (장기 조위자료를 이용한 한반도 권역별 미래 해수면 상승 추정)

  • The future mean sea level rise (MSLR) due to climate change in major harbors of Korean Peninsula has been estimated by some statistical methods in this article. Firstly, Mann-Kendall non-parametric trend test to find some trend in the observed long-term tidal data has been performed and also Bayesian change point analysis has been used also to detect the location of change points and their magnitude quantitatively. Especially, in this study, the results from Bayesian change point analysis have been applied to combine 4 future MSLR scenario projections with local MSLR data at 5 tidal gauges. This proposed procedure including Bayesian change point analysis results can improve the step for the determination of starting years of future MLSR scenario projections with 18.6-year lunar node tidal cycle and effectively consider local characteristics at each gauge. The final results by the proposed procedure in this study have shown that the future MSLR in Jeju region (Jeju tidal gauge) is in the largest increment and also the future MSLRs in Western region (Boryeong tidal gauge) and Southern region (Busan tidal gauge) are in the second largest one. Finally, it has been shown that the future MSLRs in Southern region (Yeosu tidal gauge) and Eastern region (Sokcho tidal gauge) seem to be in the relatively smallest growth among 5 gauges.

  • Magnetic resonance image (MRI) is widely used in brain research field and medical image. Especially, non-invasive brain activation acquired image technique, which is functional magnetic resonance image (fMRI) is used in brain study. In this study, we investigate brain activation occurred by LED light stimulation. For investigate of brain activation in experimental small animal, we used high magnetic field 9.4T MRI. Experimental small animal is Balb/c mouse, method of fMRI is using echo planar image (EPI). EPI method spend more less time than any other MRI method. For this reason, however, EPI data has low contrast. Due to the low contrast, image pre-processing is very hard and inaccuracy. In this study, we planned the study protocol, which is called block design in fMRI research field. The block designed has 8 LED light stimulation session and 8 rest session. All block is consist of 6 EPI images and acquired 1 slice of EPI image is 16 second. During the light session, we occurred LED light stimulation for 1 minutes 36 seconds. During the rest session, we do not occurred light stimulation and remain the light off state for 1 minutes 36 seconds. This session repeat the all over the EPI scan time, so the total spend time of EPI scan has almost 26 minutes. After acquired EPI data, we performed the analysis of this image data. In this study, we analysis of EPI data using statistical parametric map (SPM) software and performed image pre-processing such as realignment, co-registration, normalization, smoothing of EPI data. The pre-processing of fMRI data have to segmented using this software. However this method has 3 different method which is Gaussian nonparametric, warped modulate, and tissue probability map. In this study we performed the this 3 different method and compared how they can change the result of fMRI analysis results. The result of this study show that LED light stimulation was activate superior colliculus region in mouse brain. And the most higher activated value of segmentation method was using tissue probability map. this study may help to improve brain activation study using EPI and SPM analysis.

Survey on Effect and Satisfaction of Korean Medicine Distillate Nasal Spray of Patient with Chronic Rhinitis (만성 비염 환자의 한방코분무제에 대한 효과 및 만족도 조사)

  • Objectives : This study was performed in order to investigate an immediate improvement effect of nasal obstruction after having patient with chronic rhinitis use nasal spray in nasal cavity after distilling Korean medicine, satisfaction for its fragrance and irritation and its effect continuance time. Methods : A questionnaire was performed for an immediate therapeutic effect of nasal spray 1, 2 and 3 weeks later after using spray by targeting 13 patients who used Korean medicine distillate spray for over 3 weeks among the patients who visited Sangji University Korean Medicine Hospital and whose symptoms were determined as chronic rhinitis and for whom Korean medicine distillate spray was prescribed. 3 weeks later after using spray, a questionnaire was performed for fragrance, irritation and continuance effect of Korean medicine. Immediate therapeutic effect being felt right after spraying was also surveyed and symptom scores was expressed from 10 to 0 and on the assumption that nasal obstruction symptom just before spraying was 10 points, symptom score after spraying was surveyed. In case of evaluation of satisfaction, satisfaction for fragrance and irritation of nasal spray was evaluated by scale based on full score of 5 point from 'very satisfied=5', 'satisfied=4', 'average=3', 'unsatisfactory=2' and 'very unsatisfactory=1', respectively. Effect continuance time after spraying was surveyed by dividing it into within 5 minutes, within 5-10 minutes, within 10-30 minutes and over 30 minutes. All the data was statistically analyzed by Friedman Test that is non-parametric test of Paired t-test and descriptive statistics. Results : Total surveyed patients were 13 persons including 6 males and 7 females and total average age was represented to be $27.1{\pm}17.7$ years old. Score of immediate nasal obstruction improvement effect at 1st week was $6.3{\pm}2.4$ points, at 2nd week $5.7{\pm}2.5$ points and at 3rd week $5.5{\pm}2.5$ points and when comparing 1st week, 2nd week and 3rd week statistically, p value was 0.047<0.05 and from this, it could be seen that the longer spray was used, the more was improvement effect represented with slight significance. In case of fragrance and irritation, both of its score was represented to be high as average $4.0{\pm}1.08 $points and average $4.4{\pm}0.8$ points, respectively. Effect continuance time below 10 minutes was represented in 8 persons who accounted for 61.5%. Conclusions: Immediate improvement effect of nasal obstruction being felt by the patients by using Korean medicine nasal spray could be confirmed and by having the patients use such spray for 3 weeks continuously, it could be realized that such effect was sustained and satisfaction for fragrance or irritation of Korean medicine nasal spray was significant. However, rather short sustained time was considered to be unsatisfactory and this shortcomings is required to be improved in the future.

Safety of Bojungikgi-tang Soft Extract after Single Oral Administration in Healthy Male Volunteers, Single Center Study (보중익기탕연조엑스의 1회 경구투여 후 안전성 평가에 관한 단일기관 연구)

  • Objectives This study is designed to evaluate the safety of Bojungikgi-tang soft extract in healthy male volunteers. Methods 12 healthy male volunteers were recruited and this study was carried out by a single center. Laboratory test results, vital signs of the volunteers were collected to evaluate safety. According to registration order, the 12 subjects were allocated by serial number. To evaluate safety, blood samples were taken and vital signs were checked 4 times - screening, pre administration, post administration and follow up-during the whole trial. The difference between pre (before medication [0 hr]) and post-administration (after medication [48 hr]) variables was summarized as mean±standard deviation. The normality test was performed using the Kolmogorov-Smirnov test and Shapiro-Wilk test. When the normality is satisfied, the paired t-test is applied. Otherwise, the non-parametric method, Wilcoxon signed rank test is applied. The significance level was p<0.05. The incidence of all adverse effects are shown in percentage. Results In the case of red blood cell, hemoglobin, hematocrit, lymphocytes, neutrophils, protein, γ-glutamyl transpeptidase values, the normality test result of the variable for the difference value before and after the dosing has a significance level <0.05. But most of values did not deviate from the normal range, and the deviation from the normal range could not be regarded as the significance associated with this clinical trial. And adverse event wasn't observed associated with the clinical trial drug. Conclusions Bojungikgi-tang soft extract were considered to be safe for healthy male volunteers.

Effect of environmental enrichment for piglets in the nursery phase

  • Objective: The effect of environmental enrichment on the behaviour of piglets in the nursery phase was evaluated. Methods: A total of 450 hybrid pigs (21 day old), including both females and uncastrated males, weighing approximately 6 kg, were distributed in a completely randomised design with 3 treatments and 3 replicates of 50 animals each. The treatments were: i) pen without environmental enrichment (control), ii) treatment consisting of continuous environmental enrichment (CEE) with rubber balls throughout the experimental period, and iii) treatment consisting of environmental enrichment with washed balls (EEWB) during the whole experimental period which were removed daily for washing. For the behavioural evaluation, 10 animals were randomly selected per replicate. The behavioural assessments were performed once a week, from 8 am to 6 pm, using images captured with a video camera. The data were submitted to non-parametric analyses, the means were compared using the Bonferroni test, and Person's correlations were also calculated. Results: A statistical difference (p = 0.001) was observed in the B5 (playful) behaviour; the animals in the EEWB treatment group had a higher frequency of this behaviour than animals in the control treatment group. The animals in the control group showed a higher frequency of B7 behaviour (lying down) (p = 0.026) than those in the EEWB and CEE treatment groups. The animals in the control group had a higher frequency of the B9 (belly nosing) behaviour than those in the EEWB group (p = 0.015). There was a tendency towards a higher frequency of behaviour B3 (walking in the pen) (p = 0.067) when the animals received the control treatment than treatments EEWB or CEE. Conclusion: The use of an enriching object improved the animal welfare and altered the correlation between the evaluated behaviours compared to the animals that did not have environmental enrichment.

Effectiveness and patient satisfaction of dental emergencies in Pitié Salpêtrière Hospital, Paris, during the COVID 19 pandemic

  • Background: A previous study reported the effectiveness and patient satisfaction in the dental emergency unit (DEU) of the Pitie Salpetrière Hospital in Paris before coronavirus disease 2019 (COVID-19). The same methodology was used during the COVID-19 pandemic to compare pain, anxiety, and patient satisfaction during the two periods. Methods: This prospective study was conducted in 2020 (NCT04354272) on adult patients. Data were collected on day zero (D0) on site and then by phone during the daytime on day one (D1), day three (D3), and day seven (D7). The primary objective was to assess the pain intensity at D1. Secondary objectives were to assess pain intensity at D3 and D7, anxiety intensity at D1, D3, and D7, and patient satisfaction. Patients were evaluated on a 0-10 numeric scale on D1, D3, and D7; mean scores were compared with non-parametric statistics (ANOVA, Dunn's). Results: A total of 445 patients were given the opportunity to participate in the study, and 370 patients consented. Seventy-one were lost during follow-up. Ultimately, 299 patients completed all the questionnaires and were included in the analysis. In the final sample (60% men, 40% women, aged 39 ± 14 years), 94% had health insurance. The mean pain scores were: D0, 6.1 ± 0.14; D1, 3.29 ± 0.16; D3, 2.08 ± 0.16; and D7, 1.07 ± 0.35. This indicates a significant decrease of 46%, 67%, and 82% at D1, D3, and D7, respectively, when compared to D0 (P < 0.0001). The mean anxiety scores were D0, 4.7 ± 0.19; D1, 2.6 ± 0.16; D3, 1.9 ± 0.61; and D7, 1.4 ± 0.15. This decrease was significant between D0 and D7 (ANOVA, P < 0.001). Perception of general health improved between D1 and D7. The overall satisfaction was 9.3 ± 0.06. Conclusion: DEU enabled a significant reduction in pain and anxiety with high overall satisfaction during COVID-19, which was very similar to levels observed pre-COVID-19 pandemic.

A Study of Anomaly Detection for ICT Infrastructure using Conditional Multimodal Autoencoder (ICT 인프라 이상탐지를 위한 조건부 멀티모달 오토인코더에 관한 연구)

  • Maintenance and prevention of failure through anomaly detection of ICT infrastructure is becoming important. System monitoring data is multidimensional time series data. When we deal with multidimensional time series data, we have difficulty in considering both characteristics of multidimensional data and characteristics of time series data. When dealing with multidimensional data, correlation between variables should be considered. Existing methods such as probability and linear base, distance base, etc. are degraded due to limitations called the curse of dimensions. In addition, time series data is preprocessed by applying sliding window technique and time series decomposition for self-correlation analysis. These techniques are the cause of increasing the dimension of data, so it is necessary to supplement them. The anomaly detection field is an old research field, and statistical methods and regression analysis were used in the early days. Currently, there are active studies to apply machine learning and artificial neural network technology to this field. Statistically based methods are difficult to apply when data is non-homogeneous, and do not detect local outliers well. The regression analysis method compares the predictive value and the actual value after learning the regression formula based on the parametric statistics and it detects abnormality. Anomaly detection using regression analysis has the disadvantage that the performance is lowered when the model is not solid and the noise or outliers of the data are included. There is a restriction that learning data with noise or outliers should be used. The autoencoder using artificial neural networks is learned to output as similar as possible to input data. It has many advantages compared to existing probability and linear model, cluster analysis, and map learning. It can be applied to data that does not satisfy probability distribution or linear assumption. In addition, it is possible to learn non-mapping without label data for teaching. However, there is a limitation of local outlier identification of multidimensional data in anomaly detection, and there is a problem that the dimension of data is greatly increased due to the characteristics of time series data. In this study, we propose a CMAE (Conditional Multimodal Autoencoder) that enhances the performance of anomaly detection by considering local outliers and time series characteristics. First, we applied Multimodal Autoencoder (MAE) to improve the limitations of local outlier identification of multidimensional data. Multimodals are commonly used to learn different types of inputs, such as voice and image. The different modal shares the bottleneck effect of Autoencoder and it learns correlation. In addition, CAE (Conditional Autoencoder) was used to learn the characteristics of time series data effectively without increasing the dimension of data. In general, conditional input mainly uses category variables, but in this study, time was used as a condition to learn periodicity. The CMAE model proposed in this paper was verified by comparing with the Unimodal Autoencoder (UAE) and Multi-modal Autoencoder (MAE). The restoration performance of Autoencoder for 41 variables was confirmed in the proposed model and the comparison model. The restoration performance is different by variables, and the restoration is normally well operated because the loss value is small for Memory, Disk, and Network modals in all three Autoencoder models. The process modal did not show a significant difference in all three models, and the CPU modal showed excellent performance in CMAE. ROC curve was prepared for the evaluation of anomaly detection performance in the proposed model and the comparison model, and AUC, accuracy, precision, recall, and F1-score were compared. In all indicators, the performance was shown in the order of CMAE, MAE, and AE. Especially, the reproduction rate was 0.9828 for CMAE, which can be confirmed to detect almost most of the abnormalities. The accuracy of the model was also improved and 87.12%, and the F1-score was 0.8883, which is considered to be suitable for anomaly detection. In practical aspect, the proposed model has an additional advantage in addition to performance improvement. The use of techniques such as time series decomposition and sliding windows has the disadvantage of managing unnecessary procedures; and their dimensional increase can cause a decrease in the computational speed in inference.The proposed model has characteristics that are easy to apply to practical tasks such as inference speed and model management.
