In this study, we have proposed a novel decomposition agent composed of Cu(II) and soluble chitosan for organophosphorus chemical agents. Compared to the autohydrolysis, the soluble Cu(II)-Chitosan complex hydrolyzed DFP more effectively. Results show that soluble Cu(II)-Chitosan complex enhances the hydrolysis of DFP in 4~6 folds compared to the autohydrolysis of DFP in buffer solution. This study provides the possibility of using this soluble Cu(II)-Chitosan complex as the environmental friendly decomposition agent which can substitute current DS-2 decomposition agent.

Objective: This study evaluated the feasibility of ultrasound-guided lumbar nerve root block (LNRB) and S1 nerve root block by identifying spread patterns via fluoroscopy in cadavers. Method: A total of 48 ultrasound-guided injections were performed in 4 fresh cadavers from L1 to S1 roots. The target point of LNRB was the midpoint between the lower border of the transverse process and the facet joint at each level. The target point of S1 nerve root block was the S1 foramen, which can be visualized between the median sacral crest and the posterior superior iliac spine, below the L5-S1 facet joint. The injection was performed via an in-plane approach under real-time axial view ultrasound guidance. Fluoroscopic validation was performed after the injection of 2 cc of contrast agent. Results: The needle placements were correct in all injections. Fluoroscopy confirmed an intra-foraminal contrast spreading pattern following 41 of the 48 injections (85.4%). The other 7 injections (14.6%) yielded typical neurograms, but also resulted in extra-foraminal patterns that occurred evenly in each nerve root, including S1. Conclusion: Ultrasound-guided injection may be an option for the delivery of injectate into the S1 nerve root, as well as lumbar nerve root area.
Background : Bell's palsy(BP) is defined as an idiopathic peripheral facial paralysis of sudden onset and account more than 50% of facial paralysis. It's etiology is unclear, but herpes simplex virus type-1(HSV-1) has been the most suspicious causative agent of BP that ever been studied. We evaluated the effect of add-on acyclovir in acute stage of BP. Methods : Subject consisted of 35 patients who developed acute idiopathic unilateral facial nerve palsy(16 men and 19 women with age 9-78 years old). The treatments were started within 10 days after onset of BP. Facial nerve function was assessed by the House-Brackman facial nerve grading scale and facial nerve conduction study including blink reflex. Follow-up evaluation were made 2 month after onset. Twenty of 35 patients were treated with combined therapy of acyclovir and prednisone. As a control group, 15 patients were treated with prednisone only. We compared the improvement of neurologic defects at recovery phase. Results : Compared with two groups, difference in grading scale at recovery phase is statistically significant(p<0.01). So, acyclovir-prednisone group showed a significant improvement in grading scale at recovery phase compared with prednisone group. Conclusion : We identified the benefits of add-on acyclovir in the acute stage of BP.
This study aims to pursue the multi-functional textile finishing method to detoxify chemical warfare agent by simply treating the well-known antimicrobial agent, chitosan, to cotton fabric. For this purpose, DFP(diisopropylfluorophosphate) was sele cted as a chemical warfare agent simulant and cotton fabric was treated with 0.5, 1.0, and 2wt% chitosan solution in 1wt% acetic acid. DFP decontamination properties of the chitosan treated cotton fabrics were evaluated and compared with the untreated cotton fabric. The chitosan treated cotton fabrics showed better DFP decontamination than the untreated cotton. Decontamination properties of the chitosan treated cotton fabrics improved with the increased chitosan solution used. Especially, the cotton fabrics treated with 2wt% chitosan solution showed 5 times more DFP decontamina tion than the untreated cotton fabrics. This suggested that the chitosan treated fabric has potential to be used as a material for protective clothing with chemical warfare agent detoxifying and antimicrobial properties.
Recently, it was reported that chitosan or APTMS(3-aminopropyltrimethoxysila ne) treatment to cotton using the simple pad-dry-cure method has potential to prepare textile materials for military chemical warfare protective clothing. However, it is not confirmed which method is more efficient. Therefore, this study aims to quantitatively compare detoxification properties of chitosan treated cotton fabric with those of APTMS treated cotton fabric. Detoxification properties were evaluated using the well-known organic phosphorous nerve agent stimulant, diisopropylfluorophosphate(DF P). With the same amount of chitosan and APTMS on the surface of the cotton fabrics, APTMS treated cotton fabric exhibited 10% higher detoxification properties than chitosan treated cotton fabric based on the rate of DFP hydrolysis and half-live of DFP calculated from the DFP decontamination ratios of the treated cotton fabrics through time. Therefore, APTMS treatment can be more efficient method to prepare the textile materials for military protective clothing than chitosan treatment.

Chemical warfare agents(CWA) such as nerve agents and vesicating agents show lethality by skin contamination. Skin protection, therefore, is one of the top priorities to deal with the growing threat from CWA. In an attempt to develop the most effective topical skin protectant(TSP), candidate substances including PFPE(perfluorinated polyether), PTFE(polytetrafluoroethylene), glycerin, and polysaccharides were evaluated in forms of various formulations against nerve agent simulant DMMP(dimethylmethyl phosphonate) penetration. The protective efficacy of the formulation against DMMP penetration was estimated as the onset time of color change of the KM9 chemical agent detection paper. Based on this study, it was found that several PFPE- and glycerin-based formulations exhibit remarkably superior efficacy as a protective cream. This protective cream is expected to be used as TSP for military application after further research.

A gene expressing organophosphorus acid (OPA) anhydrolases have been cloned from Alteromonas haloplanktis strain and expressed in bacterial strain BL21. Crude extract was prepared from the transformed bacterial strain BL2l and used in testing its degrading capability of real nerve gas, soman and sarin. Within 1 minute after the start of the reaction, nearly 65% of the soman added to the reactant(3mM) was degraded by adding 1 mg of the crude extract enzyme(20.0 Unit $mg^-{1}$ crude protein). In 6 minutes, the reaction reached at its steady state, which indicates that soman was completely degraded by that time. In the case of sarin, the degradation efficiency was observed to be about 0.7 times of that of soman. If the specific activity of OPAA is enhanced by both increased expression efficiency and purification, OPAA seems to be applied for the development of decontaminant of skin, especially of eye.

In this study, the influence of microporous structures of activated carbon fibers (ACFs) on dimethyl methylphosphonate (DMMP) gas sensing properties as a nerve agent simulant was investigated. The pore structure was given to carbon fibers by chemical activation process, and an electrode was fabricated for gas sensors by using these fibers. The PAN based ACF electrode, which is an N-type semiconductor, received electrons from a reducing gas such as DMMP, and then electrical resistance of its electrode finally decreased because of the reduced density of electron holes. The sensitivity of the fabricated DMMP gas sensor increased from 1.7% to 5.1% as the micropore volume increased. It is attributed that as micropores were formed for adsorbing DMMP whose molecular size was 0.57 nm, electron transfer between DMMP and ACF was facilitated. In conclusion, it is considered that the appropriate pore structure control of ACFs plays an important role in fabricating the DMMP gas sensor with a high sensitivity.

MiraDry®, a microwave thermolysis device, is comparably new non-surgical agent in the field of eradication of sweat glands for treating axillary hyperhidrosis and osmidrosis. So far, altered sensation, swelling, and compensatory sweating are widely known as adverse effects of MiraDry®. Of the few reported MiraDry®-induced neuropathy cases, median and ulnar neuropathies are common. Although, one case has described radial nerve and posterior cord damage with maximized stimulation intensity, musculocutaneous nerve damage induced by MiraDry® has not been reported. Here, we report a case of a 30-year-old woman experiencing left hand weakness after receiving MiraDry® at a local dermatology clinic. Left brachial plexopathy, mainly involving the median nerve and the musculocutaneous nerve with partial axonotmesis, was confirmed by electrodiagnostic studies. Ultrasound evaluation showed corresponding results. This is the first case report of the musculocutaneous neuropathy by MiraDry®.
