The Effect of Visual Cue Deprivation for the Head Alignment on Unilateral Neglect Patient: Case Report (편측 무시 환자에서 시각 정보 차단이 머리 위치 정렬에 미치는 영향: 사례 연구)

  • PURPOSE: The Unilateral neglect is characterized by difficulty shifting attention to the side of space opposite the brain lesion and frequently reducing use of contralesional extremities. This study was to identify whether the visual deprivation was responsible for head position on unilateral neglect patient after stroke. METHODS: A patient with left middle cerebral artery infarction participated in the study. We assessed neglect using line bisection and star cancellation test. Patient was instructed to maintain correct alignment of trunk and head in a sit position. We evaluated degree of head lateral tilting and rotation. Then, patient was blocked visual input. Also, we evaluated head position in the same way. RESULTS: He scored 3 points in the line bisection test and 9 points star cancellation test. In postural evaluation, he had deviated posture such as lateral head tilting and rotation. After visual cue deprivation, patient showed different head position which was decreased degree of head tilting and rotation. CONCLUSION: For vertical body orientation, it was used multiple sensory references including the vestibular, somatosensory, and visual system. This finding suggested that abnormal posture of neglect patient could be related to the visual input. It has important clinical implications in terms of understanding the neglect.

The effects of vibration stimulation applied to the left forearm on unilateral neglect in patients with stroke: A pilot randomized controlled trial

  • Objective : The purpose of this study was to investigate the effects of vibratory stimulation applied to the left forearm on unilateral neglect in patients with stroke. Methods : The subjects were 11 unilateral neglect patients with stroke. They were divided into 2 groups; 6 experimental subjects and 5 control subjects. Both groups received standardized rehabilitation program and occupational therapy for the intervention of unilateral neglect in a day. In random order, line bisection test (LBT), Albert test, and star cancellation test (SCT) were conducted at baseline, the next day. During the process of all assessments of the next day, vibration stimulation was provided for the 6 subjects in the experimental group (EG), while the 5 subjects in CG carried out all assessments without vibration stimulation. Result : No significant differences were found between groups on any demographic variable or baseline assessments scores. In the next day, there was significant improvement in the EG for the LBT(p<0.05), Albert's test(p<0.05), and SCT(p<0.05). However, there was no significant improvement in the CG for the LBT(p<0.05), Albert's test(p<0.05), and SCT(p<0.05). Conclusion : These result indicate that vibratory stimulation has a positive effect on the unilateral neglect, and may be considered as alternative choice in clinical occupational therapy for reducing unilateral neglect.

Effect of Visual neglect for hemiplegia to motor recovery (시각무시가 편마비 환자의 운동 기능회복에 미치는 영향)

  • The purpose of this study by examine the effect of visual neglect on hemiplegia motor recovery are showing trouble which can be raised by visual neglect and helping the patient's ADL and functional recovering. Among the patients who are being taken physical therapy in many other hospitals in Busan From February 1st 2002 to august 31st, we chose 20 patients as control group who did not have symptoms of visual neglect and 20 other patients who did have as case group. We used Albert's test and line bisection as visual neglect test, and MAS as motor recovery. The average age of the patient group is 51.55 and that of control is 44.9. The men's rate is higher than women' s in both groups. Although the rate of left hemiplegia is higher than right hemiplegia in case group, that of right hemiplegia is higher than left hemiplegia in control group. There is much lesion site of basal ganglia in case group. There is the most amount of transformation of MAS when visual neglect is shorter than 1.5centimeter. Each change of MAS point before and after therapy in case group and control group is revealed $6.5{\pm}4.37$ and $12.5{\pm}5.95$.

The Effects of Parents' Neglect and Abuse on the School Adjustment in Adolescents : The Mediating Effects of Social Withdrawal (부모의 방임 및 학대가 청소년의 학교생활적응에 미치는 영향 : 사회적 위축의 매개효과)

  • In this paper, the effects of parental neglect and abuse on school adjustment mediated by social withdrawal in adolescents were examined using a sample of 2,163 second grade middle school students(1,096 boys and 1,067 girls) from the Korean Child and Youth Panel Survey(KCYPS) data set. All research variables were measured using self-reported questionnaires. They were analyzed by means of Structural Equation Modeling. The results indicated that parents' abuse had no direct effect, but there was an indirect effect on adolescents' school adjustment mediated by adolescents' social withdrawal. Parental neglect had a direct effect on adolescents' school adjustment and also had an indirect effect on adolescents' school adjustment mediated by social withdrawal. The results indicate that parental neglect is a more significant contributor than adolescents' social withdrawal on adolescents' school adjustment. The results of this study show that parenting attitude, more specifically parental neglect, tend to be more important factors on school adjustment compared to adolescents' psychological problems, such as social withdrawal in adolescence. Taken together, these findings suggest that interventions aimed at improving parental neglect and adolescents' social withdrawal need to be emphasized if adolescents are to better adjust to school life.

The Mediating Effect of Self-Esteem in the Relation between Parental Abuse, Neglect and School Life Adjustment in Adolescent (부모의 학대, 방임과 청소년의 학교생활적응간의 관계에서 자아존중감의 매개효과)

  • This study investigates the effects of parental abuse and neglect on school life adjustment mediated by self-esteem in adolescents. Participants consisted of 2,259 middle school students from the Korean Children and Youth Panel Study 2012. Secondary questionnaire data on parental abuse and neglect, self-esteem, and school life adjustment are used for this study. Data were analyzed using descriptive analysis, Cronbach's a, Pearson's correlation analysis and multiple regression analysis. Baron and Kenny's method determined the mediation effect. SPSS ver 18.0 was adopted. The results were as follows. First, parental abuse and neglect were negatively correlated with the adolescent school life adjustment; however self-esteem was positively correlated with adolescent school life adjustment. Second, the results of the regression analysis revealed that adolescent school life adjustment was influenced both directly and indirectly; parental abuse had a direct effect on adolescents' school life adjustment and also had an indirect effect on mediated by self-esteem. Parental neglect had a direct effect on adolescent school life adjustment and had an indirect effect on adolescent school life adjustment mediated by self-esteem. The results indicated that self-esteem plays a crucial variable in the relationship of parental abuse and the school life adjustment. Also, self-esteem plays a crucial variable in the relationship between parental neglect and school life adjustment.

Short-Term Longitudinal Study on School Life Adjustment of Children and Parents' Neglect/Abuse as Perceived by Children according to Gender and Grade (성별과 학년 시기별 아동이 지각한 부모의 방임/학대와 아동의 학교생활적응에 관한 단기종단연구)

  • This study explored the longitudinal relationships between school life adjustment and the parents' neglect/abuse as perceived by children according to gender. The participants were 5th and 6th grade children and the instruments were parents' neglect/abuse scale and school life adjustment scale. Using Korea Youth Panel data of Child Youth Policy Institute of Korea, 5th and 6th grade data were analyzed. The results were as follows: Neglect/abuse as perceived by children were different according to their gender and grade. Children who had perceived more parents' neglect/abuse were more likely to have difficulty in school life adjustment. The results of this study suggest that the appropriate interventions for the parents' neglect and abuse is needed by showing the stability of the child's perception of the parents' neglect and abuse during the 5th-6th grade.

Effects of child neglect·abuse by parents, children's intimacy with teachers and peer attachment on boys' and girls' psycho-social maladjustment (부모의 방임 및 학대, 교사에 대한 친밀감과 또래애착이 초등학생의 심리·사회적 부적응에 미치는 영향 : 초등학생의 성차를 중심으로)

  • This study aimed to investigate how child neglect abuse by parents, children's intimacy with teachers and peer attachment influenced boys' and girls' psycho-social maladjustment(aggression, social withdrawal and depression). The subjects were 2,264 elementary school students(boys: 1,180, girls: 1,084) in upper grades participating in the Korea Child Youth Panel Surveys. The data were analyzed with descriptive statistics, correlations and hierarchical regressions. The main results of this study were as follows. Firstly, regardless of children's sex, as child abuse levels by parents are higher, so children's aggression levels are higher. Also, children's intimacy with teachers and peer attachment moderated the effects of child neglect abuse by parents on both boys' and girls' aggression. But the patters of moderating effects were differed in children's sex. Secondly, child neglect abuse by parents didn't influence boys' and girls' social withdrawal and boys' peer attachment moderated the effects of child neglect by parents on boys' social withdrawal. Finally, child abuse by parents strongly influenced boys' depression, but child neglect by parents strongly influenced girls' depression. Also, there were no moderating effects of children's intimacy with teachers and peer attachment on the relation between child abuse neglect by parents, and boys' and girls' depression.

  • Spatial neglect is a neurological disorder following stroke, a lesion that usually affects the right hemisphere, fail to process or attention on the contralateral side of body and space. Functional neuroimaging studies report that spatial neglect is associated with lesions of large middle cerebral artery, perisylvian network and attention network. Spatial neglect is associated with a poor outcome. For optimal diagnosis and intervention, Types and theories of spatial neglect should be considered, in addition to clinical assessment with the conventional test and functional test. The treatment for spatial neglect could be consist of top-down approaches and bottom-up approaches. Recent trends in rehabilitation intervention for spatial neglect have reported prism adaptation.

The Effects of Task-Oriented Training for Left Trunk Flexion Pattern Using Real-Time Ultrasound Imaging -A Single-Subject Experimental Study- (편측무시 환자에게 실시간 초음파 영상을 이용한 왼쪽 몸통 굽힘 패턴을 적용한 과제지향적 훈련의 효과 -단일 사례 연구-)

  • Purpose: This study investigates the effects of task-oriented training for a left trunk flexion pattern using real-time ultrasound imaging in a stroke patient with unilateral neglect symptoms. Methods: This study used the ABA experimental design, which is a single-subject research method among individual case research methods. For the ABA experimental design, changes in the degree of unilateral neglect, balance ability, and the thickness of the lateral abdominal muscle were visually analyzed during the baseline process, in the intervention period, and after the intervention. The experiments were performed 24 times in total for 8 times in each of the 3 periods. The unilateral neglect was measured using the Albert test, balance ability was measured using the Berg balance test, and the thickness of the lateral abdominal muscle was measured using ultrasound imaging. The subject was a 50-year-old male patient with unilateral neglect caused by right cerebral hemorrhage. He performed task-oriented training for a voluntary left trunk flexion pattern using real-time ultrasound imaging during the intervention period. Results: The result of comparing the data collected during the intervention period with the data point average of the baseline process showed that balance ability improved and the tendency line was above the baseline. The tendency line of unilateral neglect was below the baseline and showed a decreasing tendency. The thickness of the lateral abdominal muscle showed an increasing trend and the tendency line was above the baseline. Conclusion: The results of this study suggest that the task-oriented training for left trunk flexion pattern using real-time ultrasound imaging has a beneficial effect on balance ability, the degree of unilateral neglect, and the strength of the lateral abdominal muscle in unilateral neglect patients.

Influence of neglect and abuse by parents on the emotional problems of youth: The Mediating Effect of Self-esteem and relationship with teacher (부모의 방임과 학대가 청소년의 정서문제에 미치는 영향: 자아존중감과 교사관계의 매개효과를 중심으로)

  • This study investigated how abuse and neglect experience by parents affect self-respect, relationship with teachers and variance of emotional problem. Also we verified whether self-respect and relationship with teachers mediates relation between abuse and neglect experience and variance of emotional problem. The results of this study are as follows. First, adolescents who experienced abuse and neglect appear to have emotional problems such as depression, social withdrawal, and somatic symptoms. Furthermore, youth who have abuse and neglect experience have difficulties in self-respect and relation with teachers. Second, self-respect and relationship with teachers have significant indirect effect on relation between abuse and neglect experience and variance of emotional problem. Third, although relation between abuse and neglect experience and relationship with teachers represent positive correlation, the regression coefficients in the direct path are significant in the opposite direction, resulting in a suppression effect. Finally, implications for interventions, limitations of the study, and suggestions for future research were discussed.
