Multilevel Analysis of Health Care Service Utilization among Medical Aid Beneficiaries in Korea

  • Purpose: The current study was done to identify individual- and group-level factors associated with health care service utilization among Korean medical aid beneficiaries by applying multilevel modeling. Methods: Secondary data analysis was performed using data on health care service reimbursement and medical aid case management progress from 15,948 beneficiaries, and data from 229 regions were included in the analysis. Results: Results of multilevel analysis showed an estimated intraclass correlation coefficient (ICC) of 18.1%, indicating that the group level accounted for 18.1% of the total variance in health care service utilization, and that beneficiaries within the region are more likely to share common features with regard to health care service utilization. At the individual level, existence of disability and types of medical aid beneficiaries showed a significant association, while, at the group level, social deprivation index, and the number of beneficiaries and case managers within the region showed a significant association with health care service utilization. Conclusion: The significant influence of group level variables in health care service utilization found in this study indicate a need for group level approaches, such as policy change and/or promotion of community awareness.

Effectiveness of Community-based Case Management for Patients with Hypertension

  • Purpose: The purpose of this study was to evaluate the effectiveness of case management for patients with hypertension on their health status and medical service utilization. Methods: This study was a secondary analysis of data collected for a larger study of chronic disease management in 2008 using the National Health Insurance Corporation database. A total of 12,944 patients who received case management for hypertension were included in this analysis. The subjects of case management were classified into subgroups, namely, over-use, under-use, and non-use groups according to the amount of medical service utilization. To compare the medical service utilization, a control group was selected randomly. The data were analyzed through descriptive statistics, McNemar test, and ANOVA. Results: All the subgroups displayed significant differences in blood pressure, self-management, social support, and their characteristics of medical service utilization. The total medical expense of the under-use and non-use groups increased after case management. However, there was no decrease in the medical expense of the over-use group. Conclusion: This finding suggests that there is a need to re-examine why patients overuse medical services and to supplement specific strategies for encouraging appropriate medical service utilization, and enhancing case management efforts for the over-use group.

Development Strategy for Utilization of ECVAM using the User Survey (사용자 만족도 조사를 통한 국토환경성평가지도 발전방안 연구)

  • The purpose of this study is to work out strategy for utilization of Environmental Conservation Value Assessment Map (ECVAM) using the user survey. It surveyed system users of ECVAM about its recognition and satisfaction. The results of the survey, the ECVAM became more popular and were highly satisfied with updated data. Especially, the study found a relationship between the satisfaction of ECVAM and accuracy, utilization, and convenience of the system. However, the satisfaction has a difference between user groups, a government official and a agent for EIA including researchers. The satisfaction of the agent group was affected by the convenience, the accuracy, and the utilization in order. In the other hand, the satisfaction of the government official group was affected by the utilization, the convenience, the accuracy, and recognition in order. Therefore, we need to adopt different strategies for educations of ECVAM and publicity activities depending on user groups. To increase the satisfaction of ECVAM, we should research not only to attain pinpoint accuracy, but also to suggest the guideline to utilize the map for a government official.

Physician Utilization and its Determinants in Rural and Urban Slum Areas (일부 농촌주민과 도시영세민의 상병 및 의료이용에 관한 연구)

  • The household survey was performed in a urban slum and a rural Chonnam areas to measure the level of illness and medical care utilization and to find the determinants of physician utilization. The data revealed that age-adjusted prevalence rates of acute and chronic diseases were much the same in both areas ranged between 10.0 to 11.3%. But medical care utilization was more frequent in urban slum than in rural area. The facility of the first medical contact was also different. Some personal and disease related variables including disease severity and activity restricted day were significantly, but somewhat differently by area, associated with physician utilization pattern. When applying Anderson model, the medical need factors explained 42.2 and 40.4% of physician utilization in urban slum and in rural areas respectively, while the enabling factors explained 18.0 and 12.2% and the predisposing facotrs explained 17.1 and 8.9% correspondingly.

  • The renewed interest in the utilization of solar energy has revealed the need for reliable data concerning the irradiation of surfaces inclined at various angles, This report outlines the mantel in which these data were developed, presents the data in tabulated form and contains illustrative examples of how the data may be applied in the solution of solar energy utilization problems. The calculations performed and the data presented apply to latitudes 34 deg through 38 dog in South Korea.

A Study on the Management and Utilization and Application Method of Public Facility in Japanese (일본 분양공동주택의 공용시설 관리와 이용실태 및 활용방안에 관한 연구)

  • In Japan, apartment housing occupies a great proportion of residence due to high population density like Korea. Although Korea and Japan are similar in big proportions of apartment housing, and apartment housing management systems and utilization behaviors, there are considerable differences. This paper examined Japanese public facilities and the status of public facility management and utilization, which differ from those in Korea. In order to analyze the status of public facilities from a perspective of management companies, a preliminary survey was conducted targeting apartment management companies in the top 100 and a field study on public facilities was conducted targeting chief directors and managers of individual housing facilities to look into public facility utilization methods, problems with public facility management, demands of residents, and utilization situations. Also, the present state of facility utilization and utilization promotion plans were investigated, based on the drawings of facilities collected. As the result, results were found in three respects; 'elements, related to building plans', 'elements, related to utilization promotion', and 'elements, related to facility remodeling, including changes of uses'. In relation to the use of public facilities in the future, a more active communication between managers and users would be needed for planning. Also, there would be a need to provide public facilities for visitors when there are events for both occupants and visitors in order to promote the use of facilities and management companies would have to share information of excellent facility models.

The Continuum of Rehabilitation Care and the Rehabilitation Services that are Needed by People with Severe Disabilities (재가 중증 장애인의 지속적 재활 관리 실태와 재활 서비스 요구)

  • Objectives: As the number of people with disabilities is increasing and their needs for care are varied, the continuum and comprehensiveness of their rehabilitative care are getting more important. This study was performed to understand the utilization of rehabilitation services and requirements of care among the people with severe disabilities in Korea. Methods: We interviewed 578 disabled persons who had severe extremity and cerebral impairment in the urban and rural areas of Korea. The questionnaire included questions on their general characteristics, the type of disability, their physical function (ADL, IADL), their use of rehabilitation services after discharge and their requirements for rehabilitation care Results: Only 12.6% of people with disability in the community continuously used the available medical rehabilitation care. The associated factors for utilization of rehabilitation services were pain and admission for rehabilitative treatment in the acute phase. There was a great need for rehabilitation services in community and this varied according to gender, the socio-economic status, the functional status and the geographic region. The gap between utilization and need for rehabilitation services was largest in the economic support. The gap of primary health care was larger in the rural area than in the urban area. Conclusions: The needs for rehabilitation service were diverse according to the individual functional status, the regional characteristics and other general characteristics of people with disability. Strategies should be considered to eliminate the barriers to obtain rehabilitation services for the people with disability in the community.

Comparison of Teaching Status, Utilization of the Teaching Materials, and the Need to solve the Matters of Sex Education between the School Health Educators and Teachers in the Elementary Schools (보건교사와 일반교사의 성교육 수업실태, 자료 활용도 및 문제해결 요구도 비교)

  • Purpose: This descriptive study was conducted to compare the teaching status, utilization of the teaching materials, and the need to solve matters of sex education between the school health educators and teachers working in an elementary school in Busan. Method: 182 school health educators and 125 teachers participated in the research. Data was collected using a self-administered questionnaire, and analyzed by frequency and x2 -test using the SPSS WIN 10.0 Program. Results: While school health educators carried out most of the sex education in the extra curriculum and physical education class, teachers did in the regular curriculum and physical education class. Regarding the utilization of the teaching materials on sex education, although the majority of school health educators used the teacher's manual, only a few teachers used it. Most of the school health educators used the ICT teaching materials while only half of the teachers did. Regarding the methods to solve matters on sex education, school health educators responded that a sex-related subject should be combined with a health subject, and an independent sex-related subject was necessary. The teachers, however, responded that it was necessary to secure enough time for sex education, the sex-related subject combined with a health subject was necessary, and there is a need to designate a sex educator. Conclusion: School health educators utilized more teaching materials for sex education and suggested more active methods to solve matters related to sex education. Therefore, school health educators should be more active as sex educators, and the subject of sex education should be adopted as a regular course.

The Improvements for the Altitude Criteria related to the Adaptive Reuse Permission on Mountains District -with special emphasis on 'Management of Mountains District Act' and 'National Land Planning and Utilization Act' (산지전용허가 표고기준 개선방안에 관한 연구 - '산지관리법'과 '국토의 계획 및 이용에 관한 법률'을 중심으로)

  • The altitude criteria of 'Management of Mountains District Act' and 'National Land Planning and Utilization Act' are different because the adaptive reuse permissions on mountains district by two acts have been operated individually and each criteria has some problems for application. This study aims to suggest proposals for improvements of altitude criteria by two acts. The altitude criteria of the duel legal systems were researched by literature review and inherent issues were derived by interview with public officers and GIS tools applied to cases. The results are as follows : First, duel criteria systems need to be integrated based on the format by 'Management of Mountains District Act'. Second, the criteria index(50/100) by 'Management of Mountains District Act' need to be adjusted due to the preserved area ratio in mountains district and the each definition of 'the peak point of the mountain' and 'the tail of the mountain' can be changed as 'the highest point within the same slope(including ridge)' and as 'the average of the highest and the lowest point on boundary between the mountain district and the other land use district'. Third, the method of slope division may be determined by the conditions of local areas and the discrepancy between the two slopes in common boundaries must be adjusted. Finally, the maps containing preservation areas and development areas need to be notified.
