Glossopharyngeal neuralgia is a rare syndrome that involves episodic bursts of pain in the sensory distributuion of the ninth cranial nerve. The nature of the pain is characterized by excruciating shock-like pain in the region of the tonsillar fossa or pharynx and can radiate to the ear or the angle of the jaw. Like trigeminal neuralgia, glossopharyngeal neuralgia typically responds to anticonvulsant agents such as carbamazepine. However, dose of carbamazepine needs to be increased gradually to avoid side effects. If the patient can not tolerate until effective carbamazepine level is reached, phenytoin can be administered intravenously at the same time that oral carbamazepine therapy is begun. We present fifty-three year old female patient suffering from glossopharyngeal neuralgia who did not respond to initial carbamazepine therapy, but responded to concomitant intravenous infusion of phenytoin.

Objectives : To discuss the therapeutic properties of Daoyin, and suggest ways to apply the Spleen Daoyin Method clinically. Methods : The therapeutic properties of Daoyin were examined in medical texts, after which the movements and effects of the Spleen Daoyin Method were analyzed. Based on relevant contents in the Donguibogam, symptoms and diseases for application of the Spleen Daoyin Method were studied to suggest further clinical application possibilities. Results & Conclusions : Daoyin manages the body by treating diseases, while keeping the Shen intact to manage the mind. In the case of the Five Zhang Daoyin Method, its therapeutic nature is clear as the target is focused on the Five Zhang. The Spleen Daoyin Method is mostly comprised of two movements that enable overall restoration and improvement of the Spleen function. Therefore, it could be applied in cases of Spleen-Stomach diseases where there is functional damage.

The International Union for Conservation of Nature and Natural Resources (IUCN) considers the western/lowland bongo Tragelaphus eurycerus eurycerus to be a threatened species, and the eastern/mountain bongo Tragelaphus eurycerus isaaci an endangered species［1］. Although extinction is considered by many biologists to be a natural process during evolution, the exponential growth of the human population has drastically and prematurely reduced the numbers and genetic diversity of many species［2］. Species have evolved to adapt to a specific habitat or environment that meet their survival needs. Alteration or destruction of their habitat results in a species becoming incapable of adapting and hence becoming threatened with extinction. A widespread scientific and public consensus has emerged suggesting that governments should assign high priority to the maintenance of biological diversity via habitat preservation and management far species conservation［3］. Unfortunately, the loss of biological diversity far surpasses the available conservation resources and species are lost forever on a daily basis［4］. Notwithstanding the focus on habitat preservation and wildlife management, conservation biologists have also become increasingly interested in using the technologies of reproductive and developmental biology to help manage or rescue endangered species［5］.

담관암은 예후가 불량한 암으로 전암성 병변의 병리, 임상적 특징, 예후 등의 이해가 중요할 것으로 생각된다. 관내 유두상 종양은 드문 질환으로 만성적인 담관의 염증이 동반되어 있는 환자에서 발생 위험이 증가하며 비교적 느리게 성장하고 덜 침습적인 특징이 있으나 악성 전환 가능성이 높은 질병이므로 첫 진단 시 조직학적으로 양성이라 하더라도 적극적인 치료를 고려하도록 권고하고 있다. 이에 질병의 자연경과에 대한 보고는 적어, 저자들은 치료없이 8년간 경과 관찰하여 담관암으로 진행한 담관내 유두상 종양 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Primary cutaneous mucinous carcinoma (PCMC) is a rare malignant tumor of eccrine origin. Clinically, the carcinoma presents as a solitary, slow growing, and painless nodule. For this reason, this tumor is often considered to be a benign mass in the preoperative setting. The lesion is, however, malignant in nature and has a tendency for local recurrence and infrequent metastasis. Wide local excision is the treatment of choice. However, few reports exist with information regarding surgical margins and clinical outcomes. Herein, we report a case of PCMC excised with a narrow surgical margin and review the relevant literature. A 49-year-old man presented with a small cutaneous nodule of the right cheek. The mass was excised without any margin, but pathologic examination revealed histology of mucinous carcinoma. Because of this, the operative site was re-excised with a 5-mm margin, and the wound was closed using a V-Y advancement flap. Systemic work-up did not reveal other potential metastatic primary, for a final diagnosis of PCMC. We report this case of PCMC, treated with relatively narrow margin in a patient with good prognostic factors.

Granulomatous mastitis is a rare inflammatory breast lesion, usually presented with rapidly enlarging palpable mass in young parous women and can simulate carcinoma. Unnecessary surgical procedure can be avoided if the nature of the lesion were defined by fine needle aspiratio(FNA) cytology. We experienced a case of bilateral granulomatous mastitis diagnosed by FNA cytology. The patient was a 31-year-old woman with one month history of a rapidly enlarging mass in the left breast, considered clinically to be malignant. After 4 years, she presented with a palpable mass in the right breast. The FNA cytology smears contained numerous aggregates of epithelioid histiocytes admixed with Langhans' and foreign body giant cells, lymphoytes, neutrophils and apoptotic debris leading to a diagnosis of granulomatous mastitis. The subsequent lumpectomy of the left breast confirmed the diagnosis of granulomatous mastitis. The FNA cytology smears from right breast showed identical cytologic findings.

Background: Panax ginseng has been used as Korean medicine for various diseases. It has antioxidant, hypotensive, sedative, analgesic, and endocrine activities. Dammarane-type triterpenes from the plant have various beneficial effects. Methods: A dammarane-type triterpene saponin was isolated from P. ginseng root through chromatography such as repeated column chromatography and medium pressure liquid chromatography. Results and conclusion: New dammarane-type triterpene saponin was isolated for the first time from nature. The structure was elucidated as ginsenoside Rg12 (1) based on spectral data. There may be good materials from P. ginseng for the development of industrial applications such as nutraceutical, pharmaceutical, and cosmeceutical purposes.

We investigated how hydroxyapatite (HA) coating onto a porous super stainless steel (S.S.S, 22Cr-20Ni-6Mo-0.25N) affects bone ingrowth in a dog transcortical femoral model. Implants were histologically evaluated after 4 and 48 weeks of implantation, and the bone bonding strength at the bone/implant interface was examined by employing the pull-out test. The direct osseous tissue bonding onto the HA-coated S.S.S was observed, but the uncoated stainless steels had thin fibrous tissue layers. The mean interface strength of the HA-coated S.S.S was 1.5 and 2.5 times greater than those of the S.S.S and the 316L SS after one year of implantation, respectively. In preliminary studies, no toxic responce was observed from a cytotoxicity test of the S.S.S, having similar corrosion resistance to titanium. Our results suggest that early osteoconductive nature of HA coating may induce long term osteointegration for a bioinert substrate.

Objectives : Psychasthenia is a psychological disorder that is characterized by phobias, obsessions, compulsions, or excessive anxiety. Their thoughts can be scattered and require significant effort to organize, often resulting in sentences that do not come out as intended, making little sense to others. Methods : Two Patients with psychasthenia are unable to resist specific actions or thoughts, regardless of their maladaptive nature and have insufficient control over their conscious thinking and memory, sometimes wandering aimlessly and/or forgetting what they were doing. Results : The constant mental effort induces physiological fatigue, which worsens the condition. Nevertheless, psychasthenia has become a forgotten disorder. We observed two cases that are worthy of the original definition of psychasthenia. Conclusions : It can be concluded that patients with psychasthenia complain of sinking because of a reduction in psychological tension or an effort to recover the reduction in tension.

Asthma is traditionally regarded as a chronic airway disease, and recent literature proves its heterogeneity, based on distinctive clusters or phenotypes of asthma. In defining such asthma clusters, the nature of comorbidity among patients with asthma is poorly understood, by assuming no causal relationship between asthma and other comorbid conditions, including both communicable and noncommunicable diseases. However, emerging evidence suggests that the status of asthma significantly affects the increased susceptibility of the patient to both communicable and noncommunicable diseases. Specifically, the impact of asthma on susceptibility to noncommunicable diseases such as chronic systemic inflammatory diseases (e.g., rheumatoid arthritis), may provide an important insight into asthma as a disease with systemic inflammatory features, a conceptual understanding between asthma and asthma-related comorbidity, and the potential implications on the therapeutic and preventive interventions for patients with asthma. This review discusses the currently under-recognized clinical and immunological phenotypes of asthma; specifically, a higher risk of developing a systemic inflammatory disease such as rheumatoid arthritis and their implications, on the conceptual understanding and management of asthma. Our discussion is divided into three parts: literature summary on the relationship between asthma and the risk of rheumatoid arthritis; potential mechanisms underlying the association; and implications on asthma management and research.
