Problems in the Korean National Family Planning Program (한국가족계획사업(韓國家族計劃事業)의 문제점(問題點))

  • The success of the family planning program in Korea is reflected in the decrease in the growth rate from 3.0% in 1962 to 2.0% in 1971, and in the decrease in the fertility rate from 43/1,000 in 1960 to 29/1,000 in 1970. However, it would be erroneous to attribute these reductions entirely to the family planning program. Other socio-economic factors, such as the increasing age at marriage and the increasing use of induced abortions, definitely had an impact on the lowered growth and fertility rate. Despite the relative success of the program to data in meeting its goals, there is no room for complacency. Meeting the goal of a further reduction in the population growth rate to 1.3% by 1981 is a much more difficult task than any one faced in the past. Not only must fertility be lowered further, but the size of the target population itself will expand tremendously in the late seventies; due to the post-war baby boom of the 1950's reaching reproductive ages. Furthermore, it is doubtful that the age at marriage will continue to rise as in the past or that the incidence of induced abortion will continue to increase. Consequently, future reductions in fertility will be more dependent on the performance of the national family planning program, with less assistance from these non-program factors. This paper will describe various approaches to help to the solution of these current problems. 1. PRACTICE RATE IN FAMILY PLANNING In 1973, the attitude (approval) and knowledge rates were quite high; 94% and 98% respectively. But a large gap exists between that and the actual practice rate, which is only 3695. Two factors must be considered in attempting to close the KAP-gap. The first is to change social norms, which still favor a larger family, increasing the practice rate cannot be done very quickly. The second point to consider is that the family planning program has not yet reached all the eligible women. A 1973 study determineded that a large portion, 3096 in fact, of all eligible women do not want more children, but are not practicing family planning. Thus, future efforts to help close the KAP-gap must focus attention and services on this important large group of potential acceptors. 2. CONTINUATION RATES Dissatisfaction with the loop and pill has resulted in high discontinuation rates. For example, a 1973 survey revealed that within the first six months initial loop acceptance. nearly 50% were dropouts, and that within the first four months of inital pill acceptance. nearly 50% were dropouts. These discontinuation rates have risen over the past few years. The high rate of discontinuance obviously decreases the contraceptive effectiveness. and has resulted in many unwanted births which is directly related to the increase of induced abortions. In the future, the family planning program must emphasize the improved quality of initial and follow-up services. rather than more quantity, in order to insure higher continuation rates and thus more effective contraceptive protection. 3. INDUCED ABORTION As noted earlier. the use of induced abortions has been increase yearly. For example, in 1960, the average number of abortions was 0.6 abortions per women in the 15-44 age range. By 1970. that had increased to 2 abortions per women. In 1966. 13% of all women between 15-44 had experienced at least one abortion. By 1971, that figure jumped to 28%. In 1973 alone, the total number of abortions was 400,000. Besides the ever incre.sing number of induced abortions, another change has that those who use abortions have shifted since 1965 to include- not. only the middle class, but also rural and low-income women. In the future. in response to the demand for abortion services among rural and low-income w~men, the government must provide and support abortion services for these women as a part of the national family planning program. 4. TARGET SYSTIi:M Since 1962, the nationwide target system has been used to set a target for each method, and the target number of acceptors is then apportioned out to various sub-areas according to the number of eligible couples in each area. Because these targets are set without consideration for demographic factors, particular tastes, prejudices, and previous patterns of acceptance in the area, a high discontinuation rate for all methods and a high wastage rate for the oral pill and condom results. In the future. to alleviate these problems of the methodbased target system. an alternative. such as the weighted-credit system, should be adopted on a nation wide basis. In this system. each contraceptive method is. assigned a specific number of points based upon the couple-years of protection (CYP) provided by the method. and no specific targets for each method are given. 5. INCREASE OF STERILIZA.TION TARGET Two special projects. the hospital-based family planning program and the armed forces program, has greatly contributed to the increasing acceptance in female and male sterilization respectively. From January-September 1974, 28,773 sterilizations were performed. During the same time in 1975, 46,894 were performed; a 63% increase. If this trend continues, by the end of 1975. approximately 70,000 sterilizations will have been performed. Sterilization is a much better method than both the loop and pill, in terms of more effective contraceptive protection and the almost zero dropout rate. In the future, the. family planning program should continue to stress the special programs which make more sterilizations possible. In particular, it should seek to add the laparoscope techniques to facilitate female sterilization acceptance rates. 6. INCREASE NUMBER OF PRIVATE ACCEPTORS Among the current family planning users, approximately 1/3 are in the private sector and thus do not- require government subsidy. The number of private acceptors increases with increasing urbanization and economic growth. To speed this process, the government initiated the special hospital based family planning program which is utilized mostly by the private sector. However, in the future, to further hasten the increase of private acceptors, the government should encourage doctors in private practice to provide family planning services, and provide the contraceptive supplies. This way, those do utilize the private medical system will also be able to receive family planning services and pay for it. Another means of increasing the number of private acceptors, IS to greatly expand the commercial outlets for pills and condoms beyond the existing service points of drugstores, hospitals, and health centers. 7. IE&C PROGRAM The current preferred family size is nearly twice as high as needed to achieve a stable poplation. Also, a strong boy preference hinders a small family size as nearly all couples fuel they must have at least one or more sons. The IE&C program must, in the future, strive to emphasize the values of the small family and equality of the sexes. A second problem for the IE&C program to work. with in the: future is the large group of people who approves family planning, want no more children, but do not practice. The IE&C program must work to motivate these people to accept family planning And finally, for those who already practice, an IE&C program in the future must stress continuation of use. The IE&C campaign, to insure highest effectiveness, should be based on a detailed factor analysis of contraceptive discontinuance. In conclusion, Korea faces a serious unfavorable sociodemographic situation- in the future unless the population growth rate can be curtailed. And in the future, the decrease in fertility will depend solely on the family planning program, as the effect of other socio-economic factors has already been maximumally felt. A second serious factor to consider is the increasing number of eligible women due to the 1950's baby boom. Thus, to meet these challenges, the program target must be increased and the program must improve the effectiveness of its current activities and develop new programs.

  • The objective of this study is to suggest the utilization of educated dental hygienist as the solution to the problem of supply and demand in dental clinics that has been brought up recently. Through document research and National Health Personnel Licensing Examination Board homepage, we estimated the number of dental hygienist and the condition of employment as well as gotten a grasp of the current activities carried out by dental hygienists. Furthermore, through discussion of researchers, suggested reform bills to guarantee and extend the work of dental hygienists as well as to train dental assistant. The findings were as follows: As the result of the estimation of dental hygienist, in the year 2009, two dental hygienists structure will be formed in each dental clinic. Currently the practice ratio of non-law activities of dental hygienists is high and in order to increase the practical use of dental hygienists, there is a need to reform bills that guarantee and extend the work of dental hygienists. In order to train new labors, there is a need for cautious consideration to distinguish the activities of existing trained labors, reforming of bills, and considering from various other sectors. From present point of view, solving the problem of existing trained dental hygienists, researching for the plan of utilizing dental hygienists and carrying it into practice must be the priority.

Estimation of Representative Area-Level Concentrations of Particulate Matter(PM10) in Seoul, Korea (미세먼지(PM10)의 지역적 대푯값 산정 방법에 관한 연구 - 서울특별시를 대상으로)

  • Many epidemiological studies, relying on administrative air pollution monitoring data, have reported the association between particulate matter ($PM_{10}$) air pollution and human health. These monitoring data were collected at a limited number of fixed sites, whereas government-generated health data are aggregated at the area level. To link these two data types for assessing health effects, it is necessary to estimate area-level concentrations of $PM_{10}$. In this study, we estimated district (Gu)-level $PM_{10}$ concentrations using a previously developed pointwise exposure prediction model for $PM_{10}$ and three types of point locations in Seoul, Korea. These points included 16,230 centroids of the largest census output residential areas, 422 community service centers, and 610 centroids on the 1km grid. After creating three types of points, we predicted $PM_{10}$ annual average concentrations at all locations and calculated Gu averages of predicted $PM_{10}$ concentrations as representative Gu-estimates. Then, we compared estimates to each other and to measurements. Prediction-based Gu-level estimates showed higher correlations with measurement-based estimates as prediction locations became more population representative ($R^2=0.06-0.59$). Among the three estimates, grid-based estimates gave lowest correlations compared to the other two(0.35-0.47). This study provides an approach for estimating area-level air pollution concentrations and assesses air pollution health effects using national-scale administrative health data.

The health lifestyle of adults related to smoking, drinking and exercise (흡연, 음주, 운동과 건강생활양식)

  • This study is a descriptive survey to explore the health lifestyle of adults. The study subjects are teachers of elementary. middle and high school. and staffs of research institutes located in Chungchung Province and Daejon city. The data was collected from Jan. to march 1997 through self reporte for structured questionnaire. Fantastic check list of Wilson and Ciliska for Health Lifestyle Assessment and DSM-III-R for somatic symptom were used as tools. Data was analysed by frequency, $X^2_test$, t-test and Anova using SAS program. The results are as follows: 1. There were statistically significant differences In drinking(t=7.75, P=.000), exercise(t=-2.99, P=.003)and interpersonal relationship(t=2.22, P=.027) among 10 health lifestyle between smoking group and non-smoking group, in drinking(t=17.98, P=.000), exercise(-4.71. P=.000), and job satisfaction(t=2.22, P=.027) between drinking group and non-drinking group, and in eating habit(t=-2.00, P=.045), drinking (t=4.47, P=000), exercise (t= -16.49, P=000), keeping traffic law(t= -2.68, P=.007), personality (t= -2.05, P=.040) and anxiety/depression(t=-3.47, P=.000) between exercise group and non-exercise group. 2. There was statistically significant difference in cardiovascular symptom(F=4.22, P=.0l) among somatic symptoms of subjects according to exercise level. 3. There was statistically significance difference in lifestyle according to smoking level(F=, 3.33, P=.011), drinking level(F=9.17, P=.0001) and exercise level(F=11.93, P=.000l), and in somatic symptom according to sex(t=-3.93, P=.0001), weight(F=3.83, P=.022), exercise level (F=3.29, P=.03) among general characteristics. 4. There was statistically significant difference between sex in general (t= -3.64, P=.0001), gastrointestinal(t=-2.21, P=.02), musculoskeletal(t=-3.92, P=.001), and total symptom (t= -3.92, P=.0001). 5. There was statistically very highly signigicant difference In weight according to smoking($x^2=25.18,\; P=.001)$ and exercise$(x^2=16.46,\; P=001)$. 6. There was statistically significant difference in frequency between smoking group, drinking group and exercise group$(x^2=24.52,\;P=.001)$. Among a number of habit, smoking, drinking and exercise are important factors of human health to prevent related disease morbidity and death. It is essential for industrial health nurse to committ in this subject considering the influence of those factors and lifestyle on health. There is also a relationship of weight with smoking and exercise, the frequency of overweight/obesiy in smoking/ no-exercise group were high. It is quite necessary for the people having cardiovascular symptom to exercise to lower morbidity and mortality. The industrial health nurse has to keep In mind on this point and consider of time and facilities of fitness of employee. It needs to explore the cause by further research on somatic symptom of women. This research shows that concerning the relationship between smoking, drinking, and exercise, health care provider must take not only management of disease, but health behaviors and lifestyle into consideration.

Reliability and Validity of the Korean Version of the Post-Traumatic Stress Disorder Checklist in Public Firefighters and Rescue Workers (소방공무원과 구조대원에서 한국어판 Post-Traumatic Stress Disorder Checklist의 신뢰도와 타당도)

  • Objectives Firefighters and rescue workers are likely to be exposed to a variety of traumatic events; as such, they are vulnerable to the risk of post-traumatic stress disorder (PTSD). The psychometric properties of the Korean version of the PTSD Checklist (PCL), a widely used self-report screening tool for PTSD, were assessed in South Korean firefighters and rescue workers. Methods Data were collected via self-report questionnaires and semi-structured clinical interviews administered to 221 firefighters. Internal consistency, item-total correlation, one-week test-retest reliability, convergent validity, and divergent validity were examined. Content validity of the PCL was evaluated using factor analysis and receiver operating characteristic (ROC) analyses were used to estimate the optimal cutoff point and area under the curve. Results The PCL demonstrated excellent internal consistency (${\alpha}=0.97$), item-total correlation (r = 0.72-0.88), test-retest reliability (r = 0.95), and convergent and divergent validity. The total score of PCL was positively correlated with the number of traumatic events experienced (p < 0.001). Factor analysis revealed two theoretically congruent factors: re-experience/avoidance and numbing/hyperarousal. The optimal cutoff was 45 and the area under the ROC curve was 0.97. Conclusions The Korean version of the PCL may be a useful PTSD screening instrument for firefighters and rescue workers, further maximizing opportunities for accurate PTSD diagnosis and treatment.

Effects of Physical Stimulation at Pedical on Velvet Antler Growth in Sika Deer (육경에 대한 외과적 자극이 사슴뿔의 성장에 미치는 영향)

  • This study carried out to investigate the effect of cutting stimulation on time of cutting, yield and length of antler in Sika deer. There was no significance between surgical stimulation and length of antler, but length was the longest in one side cutting group and the shortest in both side cutting group. Development of point was increased as the number of wounds in the second year (P<0.05). Time of casting was 2 weeks earlier in one side cutting group (P<0.05) than in both sides cutting group. Yields of antler were $148{\pm}23g$ when pedicle was not cut, and $126{\pm}61g$, $179{\pm}63g$, 170g, and $48{\pm}25g$ when pedicle was cut by 0.3cm, 0.4cm, 0.5cm and 0.6cm, respectively. The most desirable thickness of pedicle cut was 0.4-0.5mm. Yield of antler was increased as the size of wound in the first year, and 588, 790 and 657g in control, one-cut and both-cut, respectively. Yields of antler in one-cut is significantly high (P<0.05).

  • This study attempts to analyze research Quality about home economics and education, based on the studies in the Journal of Korean Home Ecomics Education. The purpose of this study is to investigate the recent studies in hem economics and find out the desirable direction of it. The findings of study are as follow: While the numbers of articles appeared in The Korea Association of Practical Arts Education and Korean Home Economics Education Association are increasing, The Korean Home economics Association does not show the increase the number of articles. According to the historical changes and distribution, the majority was teaching-learning method and teaching materials. Also, from the point of view in research thesis, there were more articles in home economics and education than any others, In addition, among the various types of research, survey was most frequently used. There was significant differences of the sizes of groups according to the different journals, but the one group research was most frequent and one certain research area was dominant. The teams consist of two people showed the highest ratio, and junior high and high school were the most frequently researched institutions. Generally, it is hard to find research grant. Though there is much difference in numbers of references based upon various journals, almost all articles cited reference date not only from Korean articles, but also from international articles.

A Study on the Landscape Design for Sunchon National University Cultural Park (순천대학교 문화공원 설계)

  • College campus landscape plans once focused mainly on campus functionality and aesthetically pleasing buildings. Yet now, after the rise of greater emphasis on afforestation and eco-friendly planning, building spaces for the local culture and community has become the core of the plan. This study analyzed the design strategies and details of the landscape plan that was selected through the contest to select a design plan for the cultural park at Sunchon National University. The key considerations for the landscape Design for the cultural space at Sunchon National University areas follows. First, the design plan sought ways to reach out to the local community, going one step beyond just opening up campus facilities. This means more than just the opening of physical facilities and environments. It was designed to serve as a base to organize diversified programs by generations and groups with an aim to share the history and culture of the college, the local community and the region. Second, shapes and colors were designed to establish a unified image between buildings and outdoor facilities. "Three Books" was selected as the key motif as books were believed to be the most representative symbol of colleges while 6 straight lines, hexagons and circles inspired by the shape of three books were used in the design. In terms of colors, reddish-brown was used for buildings to enhance visibility along with harmony and esthetic appreciation. For facilities, black and blue were used as dominant colors and white and yellow as point colors to promote the image of Sunchon City. Third, with an aim to overcome the limitation of the overall college campus as a closed space, it was designed to be a barrier-free space, remaining open to everyone and encouraging visits and experiences for active communication with the local community.

A Study on Dietary Behaviors, Snack Habits and Dental Caries of High School Students in Gimhae, Kyungnam Province (경상남도 김해지역 고등학생의 식행동, 간식섭취 및 충치 실태조사)

  • This study was conducted to provide basic information snack intake frequency, eating patterns and current dental caries in high school students. A total of 626 students (333 males, 293 females) in Gimhae city participated in this study. The survey conducted by using a self-administered questionnaire in January, 2008. Dental caries-related information was obtained during the students' physical examination at the designated clinical hospital in 2007. The results were as follows. Sixty three point nine percent of the students were shown to have dental caries, which includes a sealant. The average number of dental caries was 3.1, which was greater in female than in male students (p < 0.001). Eighty six percent of the students were shown to maintain correct toothbrushing habits to prevent dental caries. Twenty percent of the participants brushed there teeth ‘instantly’ a meal or snack, and 27.3% of the participants brushed there teeth, gum and tongue, and 58.9% of the participants received toothbrushing education. In snack intake frequency, participants with no dental caries had preference to white milk whereas those with dental caries had preference to snacks such as chewing gums and carbonated beverages. Female students were shown to have more dental caries maybe because of their higher snack intake frequency in chocolate, cookies and chips, yoghurt and ice-cream. Dental caries did not affect eating behaviors. This study suggests that more education for correct toothbrushing and cariogenic and detergent foods are needed for better dental hygiene.

Implementation of Monitoring System of the Living Waste based on Artificial Intelligence and IoT (AI 및 IoT 기반의 생활 폐기물 모니터링 시스템 구현)

  • In this paper, we have implemented the living waste analysis system based on IoT and AI(Artificial Intelligence), and proposed effective waste process and management method. The Jeju location have the strong point to devise a stratagem and estimate waste quantization, rather than others. Especially, we can recognized the amount variation of waste to the residence people compare to the sightseer number, and the good example a specific waste duty. Thus this paper have developed the IoT device for interconnecting the existed CCTV camera, and use the AI algorithm to analysis the waste image. By using these decision of image analysis, we can inform their deal commend and a decided information to the map of the waste cars. In order to evaluate the performance of IoT, we have experimented the electromagnetic compatibility under a national official authorization KN-32, KN61000-4-2~6, and obtained the stable experimental results. In the further experimental results, we can applicable for an data structure for precise definition command by using the simulated several waste image with artificial intelligence algorithm.
