Gastric Plexiform Fibromyxoma with Two Different Growth Patterns on Histological Images: a Case Report

  • Plexiform fibromyxoma (PF) of the stomach is a very rare mesenchymal tumor of the gastrointestinal tract. We report the first case of PF with 2 different growth patterns pathologically confirmed after surgical resection. The tumor was characterized microscopically as infiltrative; it demonstrated diffuse growth into the smooth muscle bundles of the muscularis propria and was also multinodular and plexiform within the myxoid stroma. Immunohistochemical analysis revealed that the tumor cells were positive or weakly positive for smooth muscle actin, vimentin, and H-caldesmon and negative for desmin, CD117, CD34, CK-20, Pan-CK, Dog1, S100, ER, PR, and CD10. No mutations of C-kit and platelet-derived growth factor receptor alpha were detected. No genetic disruption of glioma-associated oncogene homolog 1 was detected by fluorescence in situ hybridization. The final diagnosis of PF was mainly based on the morphological and immunohistochemical findings.

Endoscopic Management of Gastric Subepithelial Tumor (위상피하종양의 내시경적 진단 및 치료)

  • Diagnosis of gastric subepithelial tumors (SETs) is sometimes difficult with conventional endoscopy or tissue sampling with standard biopsy, so non-invasive imaging modalities such as endoscopic ultrasound (EUS) and computed tomography are used to evaluate the characteristics of SETs features (size, location, originating layer, echogenicity, shape). However imaging modalities alone is not able to distinguish among all types of SETs, so histology is the gold standard for obtaining the final diagnosis. For tissue sampling, mucosal cutting biopsy and mucosal incision-assisted biopsy and EUS-guided fine-needle aspiration or biopsy (EUS-FNA or EUS-FNB) is commonly recommended. Endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD) are used for resection of SETs involving the mucosal and superficial submucosal layers, could not treat adequately and safely the SETs involving the deep mucosa and muscularis propria. Submucosal tunneling endoscopic resection (STER) and endoscopic full-thickness resection (EFTR) is used as a therapeutic option for the treatment of SETs with the development of reliable endoscopic closure techniques and tools.

Clinical Significance of Subdividing Gastric Cancer according to the Degree of Invasion of the Muscularis Propria (근층 위암 세분류의 임상적 의미)

  • Purpose: Some gastric cancer patients in whom the cancer has infiltrated up to the muscularis propria (mp) have a good postoperative course similar to that of early gastric cancer (EGC) patients (this does not match the general classification of gastric cancer). Therefore, we performed a retrospective analysis of 125 patients with mp gastric cancer based on the degree of mp invasion. Materials and Methods: The clinicopathologic features of 125 cases of mp gastric cancer were subdivided according to depth of invasion, and were retrospectively reviewed and compared with the surgical features of 222 patients with gastric cancer invading the submucosa (sm). For each tumor, using the section that showed the greatest extent of invasion, we evaluated the degree of tumor invasion into the mp layer at a magnification of $\times$100. The patients were classified into 2 groups: mp1, the tumor was limited to the first of the 3 mp layers, and mp2, the tumor had expanded beyond the first layer. Results: Patients with mp1 (n=50) had a significantly lower incidence of lymph node metastasis, and a smaller tumor size than patients with mp2 (n=75) (P=0.01 and P=0.029, respectively). The 5-year survival rate of mp1 patients was significantly better than that of mp2 patients ($95.3\%\;vs.\;77.6\%$, P=0.0282), but was similar to that ($91.2\%$) of the 222 sm patients. The 5-year survival rate of mp patients without lymph node metastasis (n=55) was significantly better than that of those with lymph node metastasis (n=70)($93.3\%\;vs.\;78.2\%$, P=0.0192). Patients with mp1 had a significantly higher incidence of lymph node metastasis ($42.5\%\;vs\;23\%$, P=0.006) than patients with sm. Conclusion: There were clear differences in clinical features between the mp1 and the mp2 patients. Subdivision of mp gastric cancer according to the depth of invasion may enable a more precise prognosis and a more pertinent treatment plan for mp patients. In particular, as the clinicopathological findings and surgical outcomes for mp1 patients were akin to those of the sm patients, mp1 patients may require treatment analogous to that administered to patients with sm gastric cancer.   • Primary malignant fibrous histiocytoma (MFH) is a malignant tumor of mesenchymal origin that rarely occurs in the urinary tract, particularly in the urinary bladder. Unlike urothelial carcinoma, which accounts for most bladder cancers, it occurs in the submucosal portion of the bladder wall and consists of the lamina propria, muscularis propria, and adventitia. It is presumed to originate from poorly differentiated pluripotent mesenchymal cells in which fibroblasts and histiocytes are partially differentiated. Radiologically, it is known as the "non-papillary tumor" and is commonly diagnosed as a large mass without necrosis, which shows invasion beyond the muscularis propia. Although the prognosis of this rare malignancy depends on pathological parameters, it generally has a poor prognosis with high local tumor recurrence. Here, we present a case of primary MFH in the urinary bladder with clinical symptoms of lower abdominal pain without gross hematuria that recurred rapidly and showed an aggressive disease course.

Morphological study on the development of the rectum in fetuses and neonates of Korean native goat (Capra hircus) (한국재래산양(Capra hircus)의 태아 및 신생아의 직장 발달에 관한 형태학적 연구)

  • The morphological studies on the rectal development in the 60-, 90- and 120-day-old fetuses and neonates of Korean native goats were investigated by light microscopy and scanning electron microscopy. The rectal wall appeared to be differentiated into the epithelium, lamina propria, tunica muscularis and serosa in 60-day-old fetus. The rectal epithelia were stratified in some areas and simple columnar in others at 60- and 90-day-old fetuses but was only observed simple columnar epithelium at 120-day-old fetus. The goblet cells and the intestinal glands appeared in the rectal mucosa at 90-day-old fetus and gradually increase in number during gestation period. The rectal villi appeared at 60-day-old fetus and increased in length and width until 90-day-old fetus but the villi appeared to be shorter and degenerated after 120-day-old fetus. Only the rudiment trace of the villi remained at birth. The tunica muscularis of the rectum were continuously developed during gestation period. The inner circular and outer longitudinal muscle layers of rectum were observed in the 120-day-old fetuses. Under scanning electron microsoopy, the rectal villi appeared at 60-day-old fetus. The pentagonal/hexagonal microridges and numerous microvilli were observed on the luminal surface of rectal wall. Low and thumb-like rectal villi were developed at 90-day-old fetus. The rectal villi became degenerated after 120-day-old fetus. Only the rudiment of villi was remained at birth. Many circular folds were well developed. Opening portions of intestinal glands appeared like slot.

Development on the colon of fetuses and neonates in Korean native goats (한국재래산양의 태아 및 신생아의 결장 발달에 관하여)

Scanning electron microscopically, the colonic villi developed in columnar shape or finger-like of fetuses at 60 days of gestation. The of the colonic villi became degenerated after 120 days of gestation. At birth only the rudiment trace of villi remained.

Study on the Respiratory Organ of Spined Loach, Iksookimia longicorpa (Pisces, Cobitidae), in Relation to the Air-breathing System (공기호흡과 관련된 왕종개(미꾸리과어류)의 호흡기관에 관한 연구)

  • To observe the respiratory system in relation to the air-breathing organ in Iksookimia longicorpa, micro-anatomical investigation was performed on the epidermis and on intestinal tract fragments. The epidermis was distinguished by two types of skin glands, a small mucous cell and a large club cell. The mucous cell was acid sulfomucins (some sialomucins), but the club cell did not give any histochemical tests for mucosubstances. The presence of a well defined lymphatic system with small lymphocytes was established in the stratum germinativum layer of the epidermis. A large number of blood capillaries run very close to each other just below the basement membrane. The straight intestinal tract is divided into an intestine and rectum, which consisted of a mucosa (epithelial layer), lamina propria-submucosa, muscularis, and serosa. The intestine and rectum have shorter mucosal folds and a thinner wall. The majority of the epithelial mucous cells contain acid sulfomucines. Based on the above results, I. longicorpa adapts to poor dissolved oxygen conditions by using an additional respiratory system using air through the epidermis, not the intestines.

Dietary Bovine Colostrum Increases Villus Height and Decreases Small Intestine Weight in Early-weaned Pigs

  • This experiment examined the effect of dietary spray-dried bovine colostrum on intestinal histology and organ weights in early-weaned pigs. In a randomised complete block design, twelve 14-day-old weaner pigs were offered a diet containing either 5% spray-dried bovine colostrum or no colostrum (control). Diets were formulated to contain 14.8 MJ/kg DE, 1.26% available lysine and to meet or exceed requirements for other nutrients. Piglets were offered the diets for a period of 14 days. No effect of diet on growth rate or feed intake was observed (p>0.10). Small intestine weight was reduced by 12% in piglets consuming dietary bovine colostrum (p< 0.05). Villous height and crypt depth were increased and decreased, respectively, in the proximal jejunum, mid jejunum and distal ileum of pigs consuming dietary bovine colostrum (p<0.05). Mid-jejunal lamina propria $CD4^+$ and $CD8^+$ T lymphocyte density was increased by 28 and 37%, respectively, in piglets consuming dietary bovine colostrum (p<0.05). Diet did not affect thickness of tunica muscularis externa or tunica submucosa (p>0.10). Collectively, these results suggest a positive effect of dietary bovine colostrum on intestinal morphology and immune status in early-weaned pigs.

An Overturn disease of Cultured Mud Loach (Misgurnus mizolepis) (미꾸라지 (Misgurnus mizolepis)의 뒤집힘병에 관한 연구)

  • In late summer of 2001, a new disease "overturn disease" occurred among mud loach (Misgurnus mizolepis) cultured in a farm Chonbuk Province. The fish exhibited a lethargic, erratic and spiral swimming at the water surface. Affected fish showed swollen abdomen with accumulation of air bubbles in the intestinal canal. The bubbles were much larger in the anterior part than in the posterior part. Hematological and histopathological examinations were done. No significant differences were found in RBC count, Ht value and Hb concentration between the diseased and the healthy fish. Gill filaments showed proximal hyperplasia of the respiratory epithelia and fusing of the gill lamella. Hemorrhage and necrosis occurred in the intestinal epithelia and within the lamina propria, and the mucosal epithelia were separated from the muscularis. Hepatocytes underwent atrophy. In the fish experimentally tied between the annal fin and the anus by a thread to confirm accumulation of air bubbles within intestine, accumulation of air bubbles was confirmed. This disease in mud loach appeared to be induced by circulatory disturbances in the intestinal wall owing to accumulation of air bubbles in the alimentary canal after fed high protein diet throughout all culturing period.

The Utility of Contrast Enhanced Ultrasound and Elastography in the Early Detection of Fibro-Stenotic Ileal Strictures in Children with Crohn's Disease

  • Purpose: Crohn's disease (CD) is a chronic, idiopathic bowel disorder that can progress to partial or complete bowel obstruction. At present, there are no reliable diagnostic tests that can readily distinguish between acute inflammatory, purely fibrotic and mixed inflammatory and fibrotic. Our aim is to study the utility of contrast enhanced ultrasound (CEUS) in combination with shear wave elastography (SWE) to differentiate fibrotic from inflammatory strictures in children with obstructive CD of the terminal ileum. Methods: Twenty-five (19 male) children between 2016-2021 with CD of the terminal ileum were recruited into the study. Among these patients, 22 had CEUS kinetic measurements of tissue perfusion, including wash-in slope (dB/sec), peak intensity (dB), time to peak intensity (sec), area under the curve (AUC) (dB sec), and SWE. In total, 11 patients required surgery due to bowel obstruction. Histopathologic analysis was performed by a pathologist who was blinded to the CEUS and SWE test results. Results: Patients that underwent surgical resection had significantly higher mean area under the curve on CEUS compared to patients responsive to medical therapy (p=0.03). The AUC also correlated with the degree of hypertrophy and the percent fibrosis of the muscularis propria, as determined by histopathologic grading (p<0.01). There was no difference in the mean elastography measurements between these two patient groups. Conclusion: CEUS is a useful radiological technique that can help identify pediatric patients with medically refractory obstructive fibrotic strictures of the terminal ileum that should be considered for early surgical resection.
