Resilience and Characteristics of Sleep and Defense among Shift Work Nurses (교대근무자의 회복력과 수면 및 방어 특성)

  • Objectives: Shift work is a stressful situation. It is important to know the factors associated with the ability to adapt to a shift work schedule. The aim of the present study was to investigate the association between sleep, as well as personality variables, and the resilience of shift work nurses. Method: Self-report questionnaires were administered to 95 nurses who worked in one national university hospital. Connor-Davidson resilience scale, hospital anxiety and depression scale, morningness-eveningness scale, Pittsburgh sleep quality index, other sleep-related questionnaires, and Korean defense style questionnaires were used. Results: Age, shift work duration, off-day oversleep, depression, anxiety, adaptive defense style, and self-suppressive defense style were significantly associated with resilience (p < 0.05). Multiple regression analysis showed that age (${\beta}=0.34$, p < 0.05), depression (${\beta}=-0.25$, p < 0.05), adaptive defense style (${\beta}=0.45$, p < 0.001), and self-suppressive defense style (${\beta}=-0.19$, p < 0.05) significantly predicted the resilience of shift work nurses. Concerning individual defense mechanisms, resignation (${\beta}=-0.20$, p < 0.05), sublimation (${\beta}=0.19$, p < 0.05), omnipotence (${\beta}=0.19$, p < 0.05), and humor (${\beta}=0.20$, p < 0.05) significantly predicted the resiliency. Conclusion: The findings indicate that a specific defense style and other mechanisms were associated with the resilience of shift work nurses. A future prospective study with more participants could further clarify the relationship between sleep-related variables, as well as personality factors, and resilience of shift work nurses.

Association between antioxidant vitamin intake and obesity among Korean women: using the Korea National Health and Nutrition Examination Survey 2007 ~ 2016 (우리나라 성인 여성의 항산화비타민 섭취량과 비만의 연관성 : 2007 ~ 2016년 국민 건강영양조사 자료를 이용하여)

  • Purpose: This study examined the association between the antioxidant vitamin intake and obesity in Korean women. Methods: Adult women aged ${\geq}19years$ who completed a health examination and nutrition survey from the Korea National Health and Nutrition Examination Survey between 2007 ~ 2016 were selected for the study (n = 30,425). A BMI ${\geq}25kg/m^2$ and waist circumference ${\geq}85cm$ were defined as obesity and abdominal obesity, respectively. The individual antioxidant vitamin intake was estimated by linking the antioxidant vitamin composition database of commonly consumed foods and the subjects' 24-hour recall food consumption data. Carotenoids, retinol, vitamin A (retinol activity equivalent), vitamin C, tocopherols, and vitamin E (${\alpha}$-tocopherol equivalent) were included in the analysis. Each vitamin intake was converted to the nutrient density per 1,000 kcal. Odds ratio (ORs) and 95% confidence interval (CI) for obesity according to each tertile of the nutrient density was obtained from multiple logistic regression adjusted for age, household income, education level, smoking, alcohol consumption, and physical activity. Results: The mean intake of ${\alpha}$-carotene, retinol, vitamin E, ${\alpha}$-tocopherol, and ${\gamma}$-tocopherol per 1,000 kcal was significantly lower in the obese group than in the normal group. A higher intake of lycopene was inversely associated with obesity (highest vs. lowest; OR = 0.89, 95% CI: 0.83-0.96) and abdominal obesity (highest vs. lowest; OR = 0.88, 95% CI: 0.81-0.95). Higher intakes of ${\alpha}$-carotene, total carotenoids, vitamin A, and ${\gamma}$-tocopherol also had a negative relationship with abdominal obesity. The antioxidant vitamin intakes from eggs, milk and dairy products, seasoning, and grains were significantly lower in the obese group than in the normal group. Conclusion: This study showed that the dietary intake of antioxidant vitamins was inversely associated with obesity and abdominal obesity among Korean women. Further study will be needed to examine the causal relationship between the antioxidant vitamin and obesity.

Impacts of Depression, Somatization, and Jaw Disability on Graded Chronic Pain in TMD Patients (우울, 신체화, 턱기능장애가 측두하악장애 환자의 만성통증척도에 미치는 영향)

  • The aim of this study was to investigate the impact of depression, somatization, and jaw disability on graded chronic pain of TMD using Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD). Ninety-three patients (17 men and 76 women, mean$\pm$SD age of 30.1$\pm$12.5 years) diagnosed with TMD based on RDC/TMD axis I criteria were administered RDC/TMD axis II history questionnaire. The relationships between depression, somatization, jaw disability, and each parameters of graded chronic pain (e.g. pain intensity, pain days, disability score, disability days, graded chronic pain scale) were analyzed by multiple regression analysis. The obtained results were as follows: 1. Among 93 TMD patients, the prevalence of low disability group of graded chronic pain scale was 42.0% and high disability group of graded chronic pain scale was 51.5%. 2. Depression did not show any significant influences on pain intensity, pain days, disability score, disability days, and graded chronic pain scale. 3. Somatization showed a significant effect on pain intensity (p<0.01), disability days (p<0.01), and graded chronic pain scale (p<0.01) except for both pain days and disability score. 4. Jaw disability also showed a significant effect on pain intensity (P<0.001), disability days (p<0.01), and graded chronic pain scale (P<0.001) except for both pain days and disability score. Somatization and jaw disability may closely relate to the pain intensity and degree of disability that TMD patients perceive. Therefore, comprehensive understanding of psychological profile and improvement of functional limitation of jaw movements in the patient should be considered to obtain an excellent outcome of chronic TMD management.

The Knowledge and Attitude on Breast Feeding of Female University Students (모유수유에 대한 여대생의 지식 및 태도)

  • The purpose of this study is to provide the basic data in order to develop of some educational programs for increasing breast feeding by studying the female university student's knowledge and attitude on breast feeding, who will become a mother in future. The respondents of this research were selected at random for 462 female students at the university in Seoul and Kyongki area, and it was the period collected the data from Oct 28, 2000 to Nov 8, 2000. The method of study distributed the measuring tools of knowledge with 33 items and the tools of measurement with 20 items on the attitude of breast feeding to the respondents directly, and collected them. The data were analyzed to use SPSS program. Unpaired t-test, ANOVA, Pearson correlation coefficient and Multiple regression analysis were used for the calculation of difference between groups and the results were as follows ; 1. The breast feeding was 50.6% in the period of lactation for the respondents and the nuclear families were 81.7% in family constituent unit. In the future the wisher of breast feeding was 91.5%, the medical personnel was a major informer who enjoyed their best confidence, Besides the respond-ents responded that the proper period for education of the breast feeding was in a high school. 2. The level of Knowledge on breast feeding. The respondents's knowledge on breast feeding was average $16.40{\pm}4.59$ points on the basis of 33 points and On the merits and demerits ratio of breast feeding has shown highest but there was low in the field of such a concrete and practical plan as the estimate of breast feeding and the method and mindfulness for breast feeding. The higher grader, the college of the natural science showed significantly the higher points in the knowledge degree by respondents's characters and in such cases the persons of breast feeding or the informed of breast feeding by a medical personnel or the women of strong will for breast feeding action in the future. 3. The Attitude on breast feeding. There was relatively shown a positive attitude of the total average $60.50{\pm}7.59$ points and the average evaluation $3.04{\pm}.36$ points in the attitude on breast feeding. The attitude by each factors has the highest points in the practical action aspect but the lowest in the emotional aspect. The attitude on breast feeding by respondents's characters significantly showed a positive attitude in such cases the persons of breast feeding or the informed of breast feeding or the women of strong will for breast feeding action in the future. 4. Relation to knowledge and attitude on breast feeding. There was shown a correlation of definition in the relation to knowledge and attitude on breast feeding, 5.Factors which have an effect on knowledge and attitude on breast feeding. The factors which have an effect on knowledge of breast feeding were attitudes on breast feeding, graders, the college of natural science and the informed of breast feeding. Also the factors which have an effect on attitude on breast feeding were on will and knowledge on breast feeding, a large family, the informed of breast feeding. In conclusion, it will have to enforce a systematic education on the method of a practical breast feeding enlarged by a medical personnel and professional early enough as the information provision on breast feeding enables one to increase knowledge and attitude on it, besides it has relations with their practical will.

Cephalometric differences in obstructive sleep apnea between obese and non-obese Korean male patients (한국인 성인 남성 폐쇄성수면무호흡 환자의 측모 두부 방사선계측학적 비교)

  • Objective: The purpose of this study was to compare the cephalometric measurements of obese and non-obese Korean male patients with obstructive sleep apnea syndrome (OSA). Methods: Eighty-seven adults who had visited the Sleep Disorder Clinic Center in Keimyung University, Daegu, Korea were examined and evaluated with polysomnography (PSG) and lateral cephalogram. They were divided into 4 groups (non-obese simple snorers, obese simple snorers, non-obese OSA patients, obese OSA patients) according to AHI (Apnea-Hypopnea Index) and BMI (Body Mass Index). Results: The obese OSA group had the highest AHI among the 4 groups. The non-obese OSA group had a significantly steeper mandibular angle and shorter tongue length than the obese OSA group. The hyoid bone of the obese OSA group was positioned anterior and inferior as compared with the non-obese OSA group. Multiple regression analysis showed that tongue length in the obese OSA group and retroposition of hyoid bone in the non-obese OSA group were significant determinants for the severity of AHI. Conclusions: From a cephalometric point of view, the obese and non-obese pateints with OSA may be characterized by different pathogeneses. Therefore, they have to be managed by individualized treatment. For the obese OSA patients, weight control must be advised as a first choice and for the non-obese OSA patients, oral appliance, nasal CPAP, UPPP and others could be chosen according to the obstructive sites.

Quality Evaluation of Take-out Services at Restaurants in Chungbuk Province (충청북도지역 외식업체의 테이크아웃서비스 품질특성 분석)

  • The purpose of this research was to evaluate the quality of take-out services at restaurants in Chungbuk Province. A questionnaire survey by 450 customers who had experience in take-out service at the restaurants was conducted and 378 completed questionnaires were available for statistical evaluation. Statistical analyses were made of raw data by SAS V8.2. The scale for analyzing the importance and performance of the service quality was composed of 5-point Likert scales. The main results of this study are as follow: The quality attributes of take-out service were rearranged into four factors in terms of food, sanitation, access and service. The importance score was higher than performance score. IPA showed that 'freshness of food material', 'cleanliness and hygiene in food', 'sanitation of facilities', 'neatness of employees' and 'price in food' was included in 'focus here' area. There was significantly positive correlation between factors such as food, sanitation, access, service and overall customer satisfaction (p<.001); between factors and repurchasing intentions (p<.001); and between customer satisfaction and repurchasing intentions (p<.001). According to multiple regression analysis, 26.27% of the variance in respondents' overall satisfaction score and 9.21% of the variance in respondents' repurchasing intention score could be explained by factors such as food, sanitation, access and service.

Dietary Behaviors, Body Satisfaction and Factors Affecting the Weight Control Interest According to Gender of Middle School Students in Wonju Area (원주지역 중학생들의 성별에 따른 식행동과 신체만족도 및 체중조절 관심도에 영향을 미치는 변인)

  • The purpose of this study was to investigate the BMI, dietary behaviors, body satisfaction and factors affecting the weight control interest according to gender of middle school students (195 boys and 251 girls) in Wonju area. The average height and weight of the boys were $164.8{\pm}8.3\;cm$ and $55.5{\pm}12.3\;kg$ respectively. Those of the girls were $158.8{\pm}5.0\;cm$ and $48.7{\pm}7.4\;kg$ respectively. The average body mass index (BMI) of the boys and girls was 20.3 and 19.3 respectively. In both boys and girls, 69.7% of them had regular meals and the main reason for skipping meals was insufficient time to eat due to oversleeping (33.6%). Most of the subjects consumed snacks more than once a day (95.5%). The dietary intake attitude score of the girls ($35.08{\pm}5.91$) was higher than that of the boys ($33.81{\pm}5.45$) (p<0.01). The eating disorders score of the girls ($12.92{\pm}6.22$) was higher than that of the boys ($10.70{\pm}9.82$) (p<0.01). Also, 87.2% of the subjects showed normal behavior and 12.8% of the subjects had eating disorders, but there was no significant difference according to gender. The body satisfaction score of the boys ($30.16{\pm}5.08$) was higher than that of the girls ($28.00{\pm}5.52$) (p<0.001). In general, body satisfaction score correlated positively with dietary intake attitude score, but correlated negatively with eating disorders score (p<0.05). Multiple regression analysis revealed that body satisfaction score was the most significant variable which affected the weight control interest. Therefore, development of the nutrition education program for adolescents will be effective not only for the improvement of dietary behaviors but also for the positive change in the misleading perception about the ideal body shape.

A Study on Estimation of Caring Demand for Extended Care Facilities by Activities of Daily Living (ADL(Activities of Daily Living)을 이용한 무료노인요양시설 수용노인의 돌봄필요도에 관한 연구)

  • Most extended care facilities have admitted both the healthy and unhealthy elderly, among which members' average caring demand vary. The Aged Welfare Law, however, currently provides no reasonable basis on the staffing policy for extended care facilities. It just reflects the admitted number of the elderly rather than differences in members' average caring demand among facilities. This study is designed to estimate the need for caring staff on the basis of the correlation between the individual health status measured by various tools including Activities of Daily Living and caring demand by actual service time for each one. The sample included all of the admitted elderly(187 persons) in 4 extended care facilities, two in Seoul and the other two in Kangwon-Do over the survey period October 5 through October 20, 1996. The survey process consisted of 3 stages. (1) The current staffing information was collected through self-completed written questionnaires left for head of official in each facility. (2) Six graduate students at School of Public Health interviewed all residents to collect information on their health status and sociodemographics. The response rate for the interview was relatively high(85%). (3) Information on direct and indirect caring time consumed for each residents came from self-completed written questionnaires given to nurses and helpers in each target facilities. Analysis of the data was made using Pearson's correlation and multiple regression technique through SAS program. Based on this procedure, the following was found. 1. No facility meet the staffing standard in the Aged Welfare Law completely. 2. It is actual service time that is most correlated with ADL(Activities of daily living). 3. When all of the elderly are divided by four groups based on the level of ADL, the mean values of needed caring time in each group are 15, 21, 36 and 88 minutes respectively. 4. There is no significant difference among facilities in distribution of elderly person by group. 5. No facility meets the estimated number of nurses and helpers which reflects health status of the admitted elderly. Therefore, it is required that severity of the admitted elderly be considered in establishing staffing standard for extended care facility.

A Study of Serum Lipid Levels, Blood Sugar, Blood Pressure of Vegetarians and Non-Vegetarians (IV) - Based on age, abnormal serum lipid level, blood pressure, blood sugar - (채식인과 비채식인의 혈중 지질수준, 혈당, 혈압에 관한 연구(IV)-연령대별 비정상자수를 중심으로-)

  • The purpose of this study is to verify the relation between vegetarian diet and the risk factors of cardiovascular disease. Results were summarized as follows: Levels of total cholesterol, LDL-cholesterol, atherogenic index (AI), diastolic blood pressure, blood sugar and HDL-cholesterol were significantly higher (p<0.01) in non-vegetarians than those of vegetarians, but ratio of HDL cholesterol/total-cholesterol was lower in non-vegetarians. The number of subjects with abnormal serum total-cholesterol, LDL-cholesterol, atherogenic index (AI) and systotic blood pressure were much more in non-vegetarians than those in vegetarians, it were confirmed clearly as the increment of ages. In the multiple regression analysis, we acknowledged that the main factors affecting on total-cholesterol in vegetarians were age, BMI and the systolic blood pressure (R-square: 0.21), while important factors were age, BMI, the systolic blood pressure and physical activity (R-square: 0.81) in the non-vegetarians. In the vegetarians, age and BMI were the main factors having influence on LDL-cholesterol (R-square: 0.18), while age, BMI, systolic blood pressure and the physical activity (R-square: 0.82) were main factors in the non-vegetarian. In conclusion, vegetable diet may improve blood lipid level, blood pressure, blood sugar.

Factors Associated with Active Participation in Health Promotion Programs at a Public Health Center (보건소 건강증진 프로그램 신청자의 참여 상태와 관련 요인)

  • Objectives: We identified factors associated with any participation and with "good participation" (as assessed by frequency of attendance) in health promotion programs at a public health center in Korea. Methods: The subjects included 199 women who attended a baseline examination of health promotion programs during the first half of 2009. We collected data by structured interviews and physical examinations. Participation status was quantified by the frequency of attendance to the program. We classified the subjects as non-participants (0) and participants (1+ times), and as poor participants (0-29 times) and good participants (30+ times). Results: Of the 199 subjects, there were 57 (28.6%) non-participants, while 56 (28.1%) were classified as good participants. The factors that significantly affected participation status, as identified by univariate analysis, were personal factors (age, educational level, marital status, religion, living with someone, monthly income), environmental factors (method of access, accessibility of other facilities), body mass index, hypertension, perceived barriers to health, emotional salience, affectionate domain of social support, and depression. Multiple logistic regression analyses indicated that method of access was the most significant factor affecting participation in the health promotion program, and that the factors most highly associated with good participation were emotional salience, hypertension and body mass index. Conclusions: Our findings suggest that specific factors determine and enhance participation in health promotion programs offered by public health centers. These factors should be considered during the design and evaluation of health promotion programs that are offered by public health centers.
