Congenital miliary tuberculosis in an 18-day-old boy

  • Congenital tuberculosis (TB) is a rare disease that is associated with high mortality. Mycobacterium tuberculosis, the causative agent, may be transmitted from the infected mother to the fetus by the transplacental route or by aspiration of infected amniotic fluid. Clinical symptoms and signs are not specific. Miliary patterns are the most common findings in the chest X-rays of many infants with congenital TB. In this case, an 18-day-old boy had jaundice on the fifth day of birth, and fever and respiratory distress appeared on the 18th day. Chest X-ray showed diffuse fine bilateral infiltration. Clinically, pneumonia or sepsis was suspected. Respiratory symptoms and chest X-ray findings worsened despite empirical antibiotic therapy. The lungs showed miliary infiltration suggestive of TB. Gastric aspirates were positive for M. tuberculosis. Respiratory distress and fever were gradually improved after anti-TB medication. Congenital TB is difficult to detect because of minimal or no symptoms during pregnancy and nonspecific symptoms in neonates. Hence, clinicians should suspect the possibility of TB infection even if neonates have non-specific symptoms. Early diagnosis and meticulous treatment are required for the survival of neonates with TB.

  • Human breast milk (HBM) is essential for the infant's growth and development right after birth and is an irreplaceable source of nutrition for early human survival. Various infant formulas have many similarities to HBM in many components, but there is no perfect substitute for HBM. Recently, various breast milk components and their roles have been studied according to the development of various analysis techniques. As is already well known, HBM contains about 87%-88% water, and 124-g/L solid components as macronutrients, including about 7% (60-70 g/L) carbohydrates, 1% (8-10 g/L) protein, and 3.8% (35-40 g/L) fat. The composition may vary depending on the environmental factors, including maternal diet. Colostrum is low in fat but high in protein and relatively rich in immuneprotective components. Although HBM contains enough vitamins to ensure normal growth of the infant, vitamins D and K may be insufficient, and the infant may require their supplementation. Growth factors in HBM also serve as various bioactive proteins and peptides on the intestinal tract, vasculature, nervous system, and endocrine system. In the past, HBM of a healthy mother was thought to be sterile. However, several subsequent studies have confirmed the presence of rich and diverse microbial communities in HBM. Some studies suggested that the genera Staphylococcus and Streptococcus may be universally predominant in HBM, but the origin of microbiota still remains controversial. Lastly, milk is the one of most abundant body fluid of microRNAs, which are known to play a role in various functions, such as immunoprotection and developmental programming, through delivering from HBM and absorption by intestinal epithelial cells. In conclusion, HBM is the most important source of nutrition for infants and includes microbiomes and miRNAs for growth, development, and immunity.

Development of High-yielding Mutants of Streptomyces avermitilis for Avermectin B_{1a} Production through Protoplast Fusion. (원형질체 융합에 의한 Avermectina B_{1a} 고생산성 Streptomyces avermitilis 균주 개발)

  • In order to enhance the productivity of AVM $B_{la}$ produced by Streptomyces avermitilis as a secondary metabolite, we established a basic protocol necessary for protoplast fusion with high-producing strains as a fusion partner, and then obtained various kinds offusants by adopting a massive strain-development procedure (a miniaturized strain screening system). An alternative fusion method using UV and/or NTG mutation of protoplasts was developed to screen genetic recombinants without specific selectable markers. In this method, the mutants obtained by protoplast fusion after UV and/or NTG treatment (95% death rate) of the respective fusion partner (protoplasts of the respective mutants resistant against L-isoleucine antimetabolites such as O-methylthreonine and/or azaleucine) were regarded as DNA-recombined protoplast fusants. Notably it was demonstrated that most of the protoplast recombinants obtained by the UV mutation method were able to biosynthesize higher amount of AVM $B_{la}$ , reaching almost three times higher level (almost equal to the industrial productivity), compared to the average AVM Bla amount of the parallel mother strains.

Comparative study on differences of knowledge and attitude level of breast feeding between breast feeding mothers and bottle feeding mothers (일부 농촌지역 영아모의 수유형태에 따른 모유수유에 대한 지식과 태도 비교 연구)

  • The purpose of this study was to investigate the differences of knowledge and attitude level of breast feeding between breast feeding mothers and bottle feeding mothers. The subjects were mothers of 65 infants 1-6 months of age. Data were collected by a questionnaire on knowledge of breast feeding developed by Jung et al.(l994) and on attitude of breast feeding developed by Shin and Jeon(l996). The data was statistically analyzed according to Frequency, t-test, ANOVA and Pearson correlation coefficients. The results are as follows. 1) Degree of knowledge of breast feeding was significantly different according to child number(F=3.72, P<0.05) and delivery pattern(T=2.28, P<0.05) in breast feeding mothers. 2) There was a Significant difference on knowledge of breast feeding between breast feeding mothers and bottle feeding mothers(t=2.34, p<.05). 3) There was a statistical Significant difference on attitude of breast feeding between breast feeding mothers and bottle feeding mothers(t=3.05, p<.01). 4) The correlation between the mother's knowledge and attitude level of breast feeding was not found. Based on the results of this study, we suggest that systematic and supportive breast feeding program including knowledge and attitude of breast feeding have to be provided by nurse to mothers of infants to take an important role in breast feeding.

  • In accordance with wide spread of personal computer and the expansion of network access, the use of internet has been popular and communication by text message is much more normal than that of voice and image. Accordingly, the side effect of communication language brings about gap between diverse social class, the isolation of communication between generations, abusive expressions, obstacles of juvenile mental development and so on. It appears by the form of slang and vulgar word and has a negative effect on education of mother tongue and usage of children's real language. To deal with these problems, we developed new web-based education system through the analysis of learners' requirement; "Barun Mal, Goeun Mal class". So we verified its efficiency to apply to real class. We also found that this system increased the learners' interest and educational effectiveness. Also, this system contributed to the proper use of language.

The Relationship between Early Neo-maternal Exposure, and Maternal Attachment, Maternal Self-esteem and Postpartum Depression in the Mothers of NICU Infants (신생아 중환자실에서의 초기 모아상호노출과 모아애착, 모성자존감 및 산후우울간의 관계분석)

  • Purpose: This study was performed to investigate the quantities of three neo-maternal exposures; visiting frequency, auditory contact and physical contact, and to examine the relationship between the quantities of each exposure and maternal attachment, maternal self-esteem and postpartum depression in 40 mothers of NICU babies during the first week in the NICU. Method: Each neo-maternal exposure was counted at every mother's visit to the newborn and maternal attachment, maternal self-esteem and postpartum depression were measured using the maternal attachment inventory, the maternal self-report inventory and Edinburgh Postpartum Depression Scale (EPDS) on the first and seventh day in the NICU. Result: The Mean of each neo-maternal exposure was 8.77(2.81) for the visiting frequency, 5.82(3.66) for the auditory contact and 5.60(2.89) for the physical contact during 7 days in the NICU. No significant changes were found in the scores of maternal attachment, maternal self-esteem and postpartum depression between the first and the seventh day in the NICU. The quantities of neo-maternal exposures were positively related to the scores of maternal attachment and maternal self-esteem but not related to postpartum depression. Conclusion: The results of the study suggest the lack of early neo-maternal exposure in cases of NICU hospitalization negate its beneficial effects on maternal psychological well-being in increasing maternal attachment and self-esteem. More efforts are neededfor the neo-maternal interaction and the reevaluation of NICU visitation hours in order to promote maternal-infant interaction.

ANALYSIS OF FAMILIAL TENDENCY IN SKELETAL CLASS III MALOCCLUSION (골격성 하악전돌증 환자의 유전적 경향에 대한 분석)

  • The purpose of this study was to examine the familial tendency of the patients with mandibular prognathism in three generations and to define the relationship between the familial tendency and the skeletal class III morphology. The probands of this study were 103 orthognathic surgery patients with skeletal Class III malocclusions who had undergone (48 men, 55 women) mandibular set-back surgery. A questionnaire was given to patients who sought surgical treatment for excessive mandibular length, and all answers were confirmed in interviews. Lateral cephalograms were analyzed in cranial base parameters, mandibular positional parameters and mandibular skeletal parameters. In the examined families, 58.3% had at least one member other than the proband who had mandibular prognathism. The affected ratio of total relatives was 4.5%, and the value was higher in first-degree (13.4%) than second-degree (5.9%) and third-degree relatives (1.7%). The affected ratio was 51.9% in the offsprings who had at least one affected father or mother. The comparison of the groups according to the familial tendency showed no significant craniofacial skeletal measurments. In conclusion, skeletal class III malocclusion showed high familial tendency, suggesting a significant genetic influence in the etiology. However, the patient's familial tendency did not show the special craniofacial patterns compare to the subjects without familial tendency.

The Clinical Effect of Gikwuibosin-tang on Nocturnal Enuresis (기귀보신탕 투여가 야뇨증 환아에 미치는 영향에 관한 임상적 연구)

  • Objective : The objective of this study is to assess the clinical effect of Gikwuibosin_tang on nocturnal enuretic children. Methods : This clinical study was carried out with 15 cases of children aged 5 to 10 years old who visited the 00 Oriental Clinic between February 2005 and August 2005 and who were diagnosed as the nocturnal enuresis. The data of present symptom and improvement progress was collected a telephone interviews with their mother. Results : The total number of children was 15, 10 of them were male and 5 were female. Sex ratio was 2:1. Patients under 6 years old were 9(60%). After the treatment of oriental medicine der $2{\sim}3$ months, 6 of the children were improved completely and 7 of them were cured quite well, 2 of them were recovered in a half way. the treatment performed very well so there's no case which were not improved at all. And a ratio of the perfect cure was 40%. Following-up survey by telephone, the treatment were performed completely for 8 of them and 4 of them were almost improved but 3 of them were not improved. And a ratio of the perfect cure was 53.3%. Conclusion : Gikwuibasin-tang is effective in the treatment of nocturnal enuretic children. Further study is needed with more cases.

Characteristics and Origin of Salts in the Black Surface Layer of Stone Monuments (석조기념물 흑색표면층에 있는 염의 특성과 기원)

  • It is well known that the salt in stoneworks impact on damage of stone by various methods. Various kind of salt is detected in high content in the black surface layer of stone monuments and it is required about origin and roles of salts on blackening to considerate. In this study have been investigated the characteristics of salt in the black crust of sandstone and limestone which used in historical monuments in Berlin, Germany. Salt was analyzed for cation by Atomic Absorption and Atomic Emission Spectroscopy and for anion by ion Chromatograph. The representative ions are $Ca^{2+}\;and\;SO_4^{-2}$ in all samples and the most obvious combinations was proved in calcium sulfate by statistical methods. Because gypsum was detected by X-ray diffraction analyses in the black crusts, this calcium sulfate was confirmed by 2 hydrate type. The probable origin of Ca is air particle in atmosphere around sandstone and calcite in mother rock in limestone, and that of S is polluted air and mortar which used as conservation works.

  • Restless legs syndrome (RLS) is a common neurological sleep disorder in adults characterized by the following diagnostic criteria: an urge to move that is usually associated with unpleasant sensations and symptoms that are worse at rest, relieved by movement, and most severe at night. The definite diagnosis of RLS in children is stricter and consists of self-description of leg discomfort or the presence of 2 of 3 supportive criteria combined with 4 essential criteria for diagnosis in adults. RLS in childhood has often been misdiagnosed as growing pains or a part of normal development. As a result, physicians have often missed the chance for proper management. We diagnosed a case of RLS in a 5-year-old boy presenting with growing pains, whose mother was found to have had RLS since childhood. We confirmed RLS by using a polysomnograph, in which the indices of periodic limb movement syndrome (PLMS) and periodic limb movement during wakefulness (PLMW) were recorded to be compatible with RLS criteria. The patient's ferritin level was low normal, and his symptoms improved after taking iron supplements.
