Behavioral and Psychological Symptoms of the Elderly with Mild Dementia in Local Communities and Care-giving Stress and Self-efficacy of Family Members (지역사회 경증치매노인의 행동심리증상과 가족의 부양스트레스 및 자기효능감)

  • The purpose of this study is to provide a basic data for reducing care-giving stress of family members of the elderly with mild dementia by determining the relationship between the behavioral and psychological symptoms of the elderly with dementia and their family members' care-giving stress and self-efficacy. The descriptive statistics, t-test, and ANOVA were used for analysis in order to investigate the relationship. The behavioral and psychological symptoms of the elderly with mild dementia and their family members' care-giving stress had a positive correlation (r=.655, p<.001), and as their family members' care-giving stress and the members' self-efficacy had a negative correlation (r=-.334, p<.001). Based on the result of this study, it is necessary to reduce the care-giving stress of family members of the elderly with mild dementia and improve the self-efficacy of family members by developing proper measures to cope with the behavioral and psychological symptoms of the elderly with mild dementia.

  • Purpose: The purpose of this study was to explore the barriers and solution of providing long-term care (LTC) services at home for the beneficiaries with mild dementia. Methods: The data were collected by interviewing three focus groups consisting of 10 home-visit nurses and analyzed through the analytic process by Morgan and Krueger. Results: The barriers of providing LTC services for clients were identified as follows: inadequateness of the current LTC model for elderly individuals with mild dementia and inappropriateness of the personal environment for home care. The solutions for these barriers were suggested as follows: building up a safe environment for home care and maintaining an appropriate standard utilization plan of LTC service. Conclusion: The current service model for the beneficiaries with mild dementia should be modified in order to provide comprehensive long-term care services based on their complex needs.

Effects of cognitive rehabilitation program for the elderly with mild dementia (경증치매 노인을 위한 인지재활 프로그램의 효과)

  • The purpose of this study was to investigate the effects of cognitive programs on cognitive function improvement in patients with mild dementia. The subjects were 30 patients with mild dementia. The subjects were applied the cognitive rehabilitation program three times a week for 6 weeks and analyzed the pre-experiment and post-experiment values through the Korean simplified mental state examination (MMSE-K). The results were as follows: The mean of test was increased and statistically significant. Thus, cognitive rehabilitation programs for patients with mild dementia are effective interventions to improve cognitive function in patients with dementia.

Stress and Burn-Out Experience in Caregivers of Patients with Senile Dementia (치매환자 부양자의 스트레스와 소진경험)

  • Purpose: This study was done to identify the experience of stress and burn-out in caregivers of patients with senile dementia. Method: To evaluate the degree of stress and burn-out in caregivers of patients with senile dementia, 64 caregivers and matched to 64 patients with senile dementia at a Primary Health Care Post in South Kyung Sung Province were selected. The study was carried out from March 6 to March 30, 2001. Data on the degree of dementia in the patients was measured by the MMSE-K (Mini-Mental State Examination-Korea) and caregiver characteristics such as, sex, age, marital status, educational level, job, socioeconomic status, religion, number in family, relationship with patient, duration of care, and chronic disease in caregiver were collected by direct interview with a questionnaire. Results: Of 64 patients with senile dementia, 15.6% were classified as mild dementia (MMSE score 20-24) and 84.4%, as severe dementia. There were no significant characteristics of caregivers associated with the degree of stress and burn-out experience. The degree of burn-out in these caregivers of patients with severe dementia (mean value 94.3) was significantly higher than the 81.4 for those caring for patients with mild dementia (p<0.05). However, the degree of stress was not significantly related with the degree of dementia. The proportion experiencing severe burn-out (above score 4) was 54.7% in the physical domain, 90.6% in the emotional domain, and 73.4% in psychiatric domain, respectively. Conclusion: The above findings suggest that the degree of stress and burn-out experienced by caregivers of patients with senile dementia are high. Also the degree of burn-out experienced by in caregivers of patients with severe dementia was higher than for those caring for patients with mild dementia.

The Effect of IADL on life satisfaction in the elderly with dementia and mild cognitive impairment: Multiple Mediating Effects of Subjective Expectation and Depression

  • In this study, using KLoSA data, it was verified whether subjective expectations and depression had a multi-mediated effect on life satisfaction of the elderly with dementia and mild cognitive impairment. As a result of the analysis, first, it was analyzed that the higher the IADL of the elderly with dementia and mild cognitive impairment, the lower the subjective expectation, the higher the depression, and the lower the life satisfaction. Second, it was analyzed that as the subjective expectations of the elderly with dementia and mild cognitive impairment increased, depression decreased and life satisfaction increased. Third, it was analyzed that IADL in the elderly with dementia and mild cognitive impairment not only directly affects life satisfaction, but also indirectly affects life satisfaction through subjective expectations and depression, so it has a multi-mediated effect. Therefore, it is necessary to expand subjective expectations by expanding service providers that can support daily life restrictions for the elderly with dementia and mild cognitive impairment, providing regular programs by experts to reduce depression, and establishing new national pension benefits for dependents.

A Study of Jowiseungchungtang in patients with mild Dementia of Alzheimer type (조위승청탕(調胃升淸湯)이 초기 Dementia of Alzheimer type 환자의 인지기능 변화에 미치는 효과)

  • Objective : The clinical efficacy of Jowiseungchungtang(oriental herbal medication) over 6-month on the cognitive function in patients with mild Dementia of Alzheimer type(DAT) was investigated in this study. Method : The subjects for this study consisted of dementia patients who visited to the outpatient dementia department of Oriental Neuropsychiatry of the Kyunghee Oriental hospital. Patients were required to have at entry: a dignosis of mild DAT; Biochemical test, CBC, Urine analysis, Chest X -ray, EKG, Brain- MRI, Mini-Mental States-Korea(MMSE-K), Korean-Dementia Rating Scale(K-DRS), Samsung Dementia Questionaire(SDQ;to their caregiver). Patients(n=31) were assigned to treatment with Jowiseungchungtang (fourth/day for 6-month). The primary outcome measure was K-DRS. Result : 1. Patients' mean age was $71.4{\pm}6.6$ years(range 59 to 86 years). 2. Patients' mean education was $6.1{\pm}4.9$ years. K-DRS scores at baseline was $110.5{\pm}12.2$. K-DRS scores at 6-month was $109.0{\pm}17.0$. K-DRS scores at 6-month was not significantly different with K-DRS scores at baseline(t=.791 r=.435 p<.05). Conclusion : Treatment with Jowiseungchungtang for 6-month protected the cognitive function decline in patients with mild DAT.

The Effects of Task Oriented Circuit Exercise on Balance and Cognition in Mild Dementia Patients (순환식 과제 지향 운동이 경증치매환자의 균형능력과 인지기능에 미치는 영향)

  • PURPOSE: The purpose of this study was to investigate the effects of task oriented circuit exercise(TOCE) on the balance and cognition in mild dementia patients. METHODS: The subjects of the study were 30 patients with mild dementia and assigned to the TOCE(task oriented circuit exercise) group(n=15) and aerobic exercise(AE) group(n=15). TOCE group performed six task three times weekly for 12 weeks. AE group underwent the cycle ergometer and treadmill exercise three times a week during the experimental period respectively. Timed up and go test(TUG), Berg balance scale(BBS) and AP1153 Biorescue were used to assess the parameters for the balance. Korean-mini mental state examination and Global deterioration scale(GDS) were used to assess the parameters for the cognition. For the statistical analysis, paired t-test and independent t-test were used to compare the differences among two groups. RESULTS: Each group showed improvement in balance and possibility for improving cognitive function. TOCE group especially, there were significant improvements in limit of stability. CONCLUSION: The results of the study suggest that TOCE is a more diverse exercise programs introduced in the study. Furthermore, TOCE can be a helpful to improve the balance and cognition in mild dementia patients.

The Effect of the Integrated Cognitive Rehabilitation Program on the Cognitive Function and Depression of Elderly with Mild Dementia (경증치매노인에서 통합적 인지재활 프로그램의 인지기능 및 우울정도 개선효과 연구)

  • Purpose: This study aimed to investigate the effect of the integrated cognitive rehabilitation program in elderly patients with mild dementia. Methods: A total of 20 elderly patients with mild dementia participated in the integrated cognitive rehabilitation program. The program was conducted three times per week for a total of 4 periods (10 wks per period) from February 6, 2018 to December 13, 2018. Each session lasted for 1 hour, and the cognitive function and depression of the subjects were measured before and after they participated in the program. Results: After the program, there was a significant increase and decreases in participants' cognitive function and depression respectively. There was a statistically significant difference in cognitive function score over time and period, except for the 2nd period. The difference in the degree of depression over time and period was statistically significant. Conclusion: The results suggest that an integrated cognitive rehabilitation program could help improve cognitive function and the degree of depression in elderly patients with mild dementia.

Effects of Bibliotherapy of Dotard Dementias Old Adults (독서요법이 경증 치매노인에게 미치는 효과)

  • This study was conducted to investigate the effectiveness of bibliotherapy on the levels of cognitive function, depression, and social behavior in the elderly with mild dementia. For this quasi-experimental research, a total of 166 subjects the age of from 90 to 65 were selected of the 166, were divided into the experimental group and control group for six months. These findings confirmed that the dementia Bibliotherapy program made a contribution to improve cognitive function and social behavior, and to reduce depression in the elderly with mild dementia. Therefore, it is recommended that this program be used in clinical practice as an effective Bibliotherapy program for the elderly with mild dementia.

  • The purpose of this study was to find out whether the cognitive function, depression, and health promotion lifestyle changes are maintained or improved through the dementia prevention program when applying the dementia prevention program along with the daily life management program to the elderly with mild cognitive impairment. The study was conducted from April 1 to August 9, 2019, and 30 elderly people with mild cognitive impairment who participated in the dementia prevention program at the Dementia Safty Center in Seoul were studied. The study found that cognitive function (MMSE-DS) and health promotion lifestyle (HPLP-II) were maintained and improved compared to control groups in the experimental group that voluntarily implemented daily life management programs along with dementia prevention programs, and that depression (SF-GDS) decreased. Therefore, based on the results of this study, in future studies, we hope to develop a variety of voluntary daily life management programs to maintain and improve the effectiveness of the dementia prevention program for a long period of time. I hope that the prospective study will be done.
