A Study of the Change of Breast Uptake in Menstrual Cycle on $^{18}F$-FDG PET/CT (월경 주기에 따른 $^{18}F$-FDG PET/CT에서의 유방 섭취 변화에 관한 고찰)

  • Purpose: $^{18}F$-FDG PET/CT has been known a useful modality to diagnose high-glucose-using cells such as cancer cells by glucose metabolism of FDG. Mainly, FDG takes on cancer and inflammatory cells; However, There have been FDG uptakes on normal tissues by individual physiological characteristics, occasionally. Especially, in fertile females, unusual FDG uptake of breast changes as the menstrual cycle, and disturb diagnosis. Therefore, the study aimed to evaluate the change of breast FDG uptake in menstrual cycle on $^{18}F$-FDG PET/CT. Materials and Methods: 160 females ($34{\pm}3.5$ years old) who do not undergo a gynecologic anamnesis and have regular menstrual cycle over the previous 6 months were examined, from March 2009 to February 2010. They were divided 4 groups (each 40 patients) as flow phase, proliferative phase, ovulatory phase and secretory phase using Pregnancy Calculator 0.14. and history taking. Discovery Ste (GE Healthcare, Milwaukee, Mi, USA) was used as PET/CT. We analyzed SUVs on accumulated region on breast, and 3 nuclear medicine specialists did the Blind test. Results: SUVs on the Breast were flow phase ($1.64{\pm}0.25$), proliferative phase ($0.93{\pm}0.28$), ovulatory phase ($1.66{\pm}0.26$) and secretory phase ($1.77{\pm}0.28$). It showed high uptake value in secretory, flow phase and ovulatory phase (p<0.05). In gross analysis, the accumulation of breast was divided into 3 grades as comparing with lung and liver. The breast's uptake was equal to lung (Grade I); between lung and liver (Grade II); equal to or greater than liver (Grade III). The results showed high uptake value in secretory, flow phase and ovulatory phase (p<0.05). Conclusion: In fertile females, FDG uptake of breast changed as menstrual cycle, and it available to diagnose breast disease. Therefore, we consider reducing false-negative finding of breast disease, by doing examination on appropriate period through history taking about individual menstrual cycle.

Influence of Menstrual Cycle on Inflammatory Markers in Gingival Crevicular Fluid : Pilot Study (월경주기에 따른 치은열구액 내 염증지표의 변화: 예비 연구)

  • Sex Hormones exert significant influence in body physiology throughout life. Some reports suggested increased sex hormone levels correlate with an increased prevalence of gingivitis. The objectives of this pilot study are to(1) address the link between menstrual cycle and MMP-9, MMP-8, IL-$1{\beta}$ of gingival crevicular fluid during 4 weeks and (2) discuss the major biomarker of periodontal status. 7 female and 3 male volunteer who didn't have smoking habit, medical and dental disease after informed consent, were seen twice a week for 4 weeks. GCF samples were collected and GCF levels of MMP-9, MMP-8, IL-$1{\beta}$ were measured by enzyme-linked immunosorbent assay. The GCF levels of MMP-9, MMP-8, IL-$1{\beta}$ had fluctuation according to menstrual cycle, but the changes of those were not significant by Friedman matched samples test. There is no difference between female and male subjects by Mann-Whitney U test. The correlation of MMP-8, MMP-9 and IL-$1{\beta}$ showed strong by Spearman correlation(0.644-0.945). This study confirms menstrual cycle doesn't influence the periodontium of healthy female subjects.

Research of Relationship Between Cold Hypersensitivity and Sasang Constitution (냉증을 호소하는 여성 환자의 사상체질과 다빈도질환에 대한 조사연구)

  • Objectives: The purpose of this research is to investigate the relationship between Sasang constitution and cold hypersensitivity. Methods: We investigated 391 outpatients who visited Dong-Eui Oriental Hospital OB & GY from June 12, 2013 to April 18, 2014. Among 134 patients who complained feeling of cold, we analyzed 107 patients whose Sasang constitution is confirmed. Results: 1. There were 21 persons 19.8% of under age 27, 28 persons 26.4% of age 28-34, 23 persons 21.7% of age 35-41, 18 persons 17% of age 42-48, 16 persons 15.1% of age over 49. 2. Among 107 patients, there were 52 persons 48.6% of Soeumin, 29 persons 27.1% of Taeeumin, 25 persons 23.4% of Soyangin, 1 person 0.9% of Taeyangin. And Taeyangin interior disease was 1 case 100%, Soyangin exterior disease was 22 cases 88% and interior disease was 3 cases 12%, Taeeumin exterior disease was 11 cases 37.9% and interior disease was 18 cases 62.1%, Soeumin exterior disease was 19 cases 36.5% and interior disease was 33 cases 3.5%. 3. In the distribution of cold-hypersensitive part, hands and feet with chills were 58 cases 4.7% by largest number, and the following was hands and feet in 24 cases 22.6%. 4. In the distribution of chief complaint, there were 25 cases 23.58% of menstrual pain, 19 cases 17.92% of postpartum symptoms, 17 cases 16.04% of oligomenorrhea and hypomenorrhea, 14 cases 13.21% of cold hypersensitivity. 5. In the distribution of chief complaint depending on age, menstrual pain of under age 27, postpartum symptoms of age 35-41 and cold hypersensitivity of over age 49 were at a high rate. Conclusions: Results suggest that patients with symptom of cold hypersensitivity have some tendencies in age, Sasang constitution, chief complaint, cold-hypertensive part.

  • The purpose of this study is to present basic data to be used in the development of a sexual education program to create an affirmative sexual egoism. We classified the types of subjectivities and revelation attitudes of menstrual experience in 5th and 6th elementary school girls. The analytic method of data was a "Q-method" (William Stephenson). The results of this study classified four-types. The first type is an "open type". Girls of this type accepted menstrual experience as neither disease nor an embarrassment but as an important and natural course for a woman. The second type is the "concealing type". Girls of this type keep silent and conceal menstruation from persons because they consider it shameful or disgrace. The third type is the "conceited type". Girls of this type think that menstruation is painful-but only for them. Also, they accept such pains being 'grown-up'. The fourth type is the "tomboy type". Girls of this type want to be an eternal child; they hate being born a woman and think menstruation is troublesome. As a result of this research, we have concluded that it is necessary to educate girl students before they experience menstruation in order to develop positive attitudes towards sexuality and in so doing, aid them through adolescence.

The Effects of Thera-Tainment SSP Intervention on Dysmenorrhea Among of Women in Their 20s (테라테인먼트 SSP 중재가 20대 여성의 생리통에 미치는 효과)

  • The purpose of this study was to provide basic data on the thera-tainment intervention method and to relieve dysmenorrhea by identifying the effects of silver spike point therapy on menstrual pain, muscle tone, and muscle stiffness among 30 women in their 20s. The subjects had a regular menstrual cycle, had a visual analog scale score of more than 4 for dysmenorrhea pain, and did not have any gynecological disease. SSP therapy was applied for 20 minutes on San-Yin-Jiao on the first day of their menstrual cycles. Muscle tone and stiffness were measured with Myoton before treatment, immediately after treatment, and 3 hours after treatment. SPSS version 19 was used to analyze the collected data. Using Friedman test, muscle tone and stiffness were measured before treatment, immediately after treatment, and 3 hours after treatment. The results indicate that after treatment dysmenorrhea pain steadily decreased and muscle tension and stiffness decreased significantly. This shows that using SSP therapy is effective on reducing primary dysmenorrhea and improving women's health care. This shows that using SSP therapy is effective at reducing primary dysmenorrhea and improving women's health care.

A Review of Dysmenorrhea Related Articles in Literature of Oriental Medicine (한의학 관련 학회지의 월경통 관련 논문에 대한 종설)

  • Purpose: Dysmenorrhea is a common gynecologic condition among reproductive-age women. It consisted with menstrual clamps and other symptoms such as nausea. vomiting, diarrhea, and fatigue. Pain control for this disease mainly through NASIDs. but recently CAM therapies, acupuncture and others for painful menstruation are widely used in western countries. But RCT articles about dysmenorrhea in Korea, are not enough to lead such global research trends. The purpose of this study is to review treatment method and other research tendency about dysmenorrhea in Korean Medicine(KM) related dominant journals. Methods: Searching was done through web site and directly searched dysmenorrhea related articles in journals of Korean medicine Gynecology, published during 1996-2007. Results: Twenty five articles were searched in KM related journals, then two of them were simple case report and ten were paper of clinical trials. But there no RCT or control group-designed study. All of them present positive effect on dysmenorrhea but drop out rates were relatively high. Conclusion: KM therapies including acupuncture and herbal medicine have some beneficial effect to resolve menstrual cramps, but KM therapy related Korean articles don't have strong objective power as a evidence in the viewpoint of EBM. So continuous clinical trials such as RCT and multi center trials are needed.

The Effect of Endogenous and Exogenous Estrogen on Risk Factors of Coronary Heart Disease -Part II : On Platelet Aggregation- (내인성 및 외인성 Estogen이 관상동맥질환 위험인자에 미치는 영향 -제2보: 내인성 및 외인성 Estrogen이 혈소판 응집에 미치는 영향-)

  • In the atherosclerotic subjects, arterial endothelial cell injury and plaque formation are suspected to be strong causable factors in developing acute coronary syndrome, and it was revealed that platelets have a very important role in this case. Women are exposed to atherosclerosis at a different degree after menopause or oral contraception. The purpose of this study was to determine the effects of endogenous and exogenous estrogen on the degree of platelet aggregation in platelet rich plasma(PRP) in twenty nonsmoking healthy Korean women for 12 weeks. The subjects were assigned to three groups: (1) eight women aged 49 to 60(yr) for the postmenopausel(Pst) group, (2) eight, aged 22 to 30(yr) for the premenopausa(Pre) group, (3) four, aged 23 to 30(yr) for the oral contraceptive (OC) group which used triphasic OC formulation. Fasting blood sample were obtained from the subjects, (1) once per 6 weeks in the Pst group, (2) every phase of the menstrual cycle in the Pre group, (3) each once during and after OC administration in the OC group. ADP, collagen and epinephrine were used as aggregating reagents, and platelet aggregation and time(Δt: time reaching to the maximum point of aggregation) in PRP were measured at the maximum point of aggregation in five minutes. All the data were adjusted for dietary effects, personality type and body mass index(BMI) by using analysis of covariation(ANCOVA). Platelet aggregation to ADP and collagen(MADP and MCOLL) were not significantly different among the three groups, and Δt to ADP and collagen(TADP and TCOLL) were not either. But maximum platelet aggregability and Δt to epinephrine(MEPIN and TEPIN) were significantly different among the three groups, and the OC group showed the lowest value (p<0.01). Maxtimum platelet aggregability and Δt during the menstrual cycle were not significantly different in the Pre group. Any other significant differences in the maximum platelet aggregability and Δt were found between oral contraception phase and washing out phase(menstruation) in the OC group. In results, maximum platelet aggregability and aggregation time to ADP and collagen seemed not to be affected by endogenous and exogenous estrogen, even though MEPIN and TEPIN showed significantly low in the OC group among the three groups.

Early menarche and its consequence in Korean female: reducing fructose intake could be one solution

  • The mean age at menarche (AAM) of Korean females has been rapidly decreasing over the last 50 years; currently, the prevalence of early menarche (<12 years) is 22.3%. Female adolescents who experience early menarche are known to be at greater risk of psychosocial and behavioral problems along with several physical health problems such as menstrual problems. They also tend to achieve a shorter final height and develop obesity. Population-based Korean studies have shown a strong association between early menarche and the risk of obesity, insulin resistance, metabolic syndrome, nonalcoholic fatty liver disease, diabetes, breast cancer, and cardiovascular disease in adulthood. Although the exact mechanism of how early menarche causes cardiometabolic derangement in later adulthood is unknown, childhood obesity and insulin resistance might be major contributors. Recent studies demonstrated that an excessive consumption of fructose might underlie the development of obesity and insulin resistance along with an earlier AAM. A positive association was observed between sugar-sweetened beverages (a major source of fructose) intake and obesity, metabolic syndrome, insulin resistance, and cardiometabolic risk in Korean females. In pediatrics, establishing risk factors is important in preventing disease in later life. In this regard, early menarche is a simple and good marker for the management of cardiometabolic diseases in adulthood. Decreasing one's fructose intake might prevent early menarche as well as the development of obesity, insulin resistance, and cardiometabolic diseases.

  • Despite enormous efforts, no effective medication has been found to significantly halt or even slow the progression of neurological diseases, such as acquired (e.g., traumatic brain injury, spinal cord injury, etc.) and chronic (e.g., Parkinson's disease, Alzheimer's disease, etc.) central nervous system disorders. So, researchers are looking for alternative therapeutic modalities to manage the disease's symptoms and stop it from worsening. Concerning disease-modifying capabilities, stem cell therapy has emerged as an expanding domain. Among different types of stem cells, human endometrial regenerative cells have excellent regenerative properties, making them suitable for regenerative medicine. They have the potential for self-renewal and differentiation into three types of stem cells: epithelial stem cells, endothelial side population stem cells, and mesenchymal stem cells (MSCs). ERCs can be isolated from endometrial biopsy and menstrual blood samples. However, there is no comprehensive evidence on the effects of ERCs on neurological disorders. Hence, we initially explore the traits of these specific stem cells in this analysis, followed by an emphasis on their therapeutic potential in treating neurological disorders.

An Analysis of Clinical Application of Guibi-tang for Obstetrics and Gynecological Disease (동국대학교 일산한방병원 여성의학과에서의 귀비탕(歸脾湯) 임상 적용 사례 분석 연구)

  • Objectives: This study was to investigate clinical usage of Guibi-tang based on actual application in obstetrics and gynecology. Methods: This study investigated the number of patients who were prescribed Guibi-tang-gami-bang and what herbal medicines were adjusted according to their chief complaint from January 1st, 2007 to August 31th, 2008. Results: The number of patients who were prescribed Guibi-tang for obstetrics and gynecological diseases at the Department of Obstetrics and Gynecology was 721. Guibi-tang has been used to treat menstrual irregularity, postpartum general weakness, postpartum pantalgia, perimenopausal symptoms and postmenopausal syndrome most frequently. And Guibi-tang has been widely used in obstetric and gynecologic diseases such as general weakness after abortion or gynecological surgery, pelvic pain, dysmenorrhea, uterine myoma, endometriosis, infertility, premenstrual syndrome, urinary incontinence and breast disease. Conclusions: Guibi-tang can be used in obstetric and gynecologic diseases as described in classical oriental medicine literatures, and it is necessary to research clinical usage of Guibi-tang through randomized controlled trial.
