A Study on Improvement of Emergency Medical Service System in Kwangju (광주광역시 응급의료체계 개선에 관한 연구)

  • The purpose of this study was to provide the basic data for improvement of Emergency Medical Service System in Kwangju. The EMSS can be defined as the complete chain of human and physical resources that provides patient care in cases of sudden illness and injury. To provide effective emergency care through the EMSS in a region, the issue of training especially as it relates to EMT in EMSS delivery is more important than emergency medical equipment and facilities for pre-hospital emergency care. The transport of emergency medical patients carried out almost by 119 Emergency Medical Services. But out of all the employees at 119 EMS only 19.0% have graduated with a major in Emergency Medical Technology. It would seem prudent then that the graduates of EMT programs should gradually replace employees working at 119 EMS that do not have an EMT degree to ensure the best possible pre-hospital care for emergency medical patients. Therefore it can be expected that in the future there should an enormous demand for qualified EMT professionals to meet the growing needs for a superior level of emergency medical care for civilian.

  • Various types of high-precision radiotherapy, such as intensity-modulated radiation therapy (IMRT), tomotherapy (Tomo), and stereotactic body radiation therapy have been available since 1997. After being covered by insurance in 2015, the number of IMRT cases rapidly increased 18-fold from 2011 to 2018 in Korea. IMRT, which uses a high-beam irradiation monitor unit, requires higher shielding conditions than conventional radiation treatments. However, to date, research on the shielding of facilities using IMRT and the current understanding of its status are insufficient, and detailed safety regulation procedures have not been established. This study investigated the recommended criteria for the shielding evaluation of facilities using medical linear accelerators (LINACs), including 1) the current status of safety management regulations and systems in domestic and international facilities using medical LINACs and 2) the current status of the recommended standards for safety management in domestic and international facilities using medical LINACs. It is necessary to develop and introduce a safety management system for facilities using LINACs for clinical applications that is suitable for the domestic medical environment and corresponds to the safety management systems for LINACs used overseas.

  • In the tele-medical system, the broadband network for multimedia telecommunication and the multimedia terminal equipment for the remote access of the tele-medical information are essential. Especially, the tele-medical terminal equipment should provide the multimedia GUI environment in order to support the similar medical process by the tele-medical system. In this paper, we present a multimedia GUI (Graphic User Interface) for a Multimedia Tele-Medical System (TeleMedi_GUI) based on ATM/B-ISDN. In the tele-medical system, one workstation is used for the multimedia data server that is supporting multiple client terminals that are connected by the ATM network. The client terminals are based on Multimedia Personal Computers, and provide the remote access environment of the tele-medical database. We also developed the remote access protocols among the clients and the server to access multimedia medical information of the multimedia server. With using the TeleMedi_GUI, the doctors can examine and treat patients efficiently, using image data like X-ray/CT and voice data such as the S-ray diagnosis. The result of this paper can be applied to the following areas: 1) the implementation of the advanced medical service system interconnecting the small-scale health center and general hospitals, 2) the development of a fully computerized medical information system within the hospital.

Cross Calibration of Dual Energy X-ray Absorptiometry Equipment for Diagnosis of Osteoporosis: between Domestic Manufacturers and Global Manufacturers (골밀도 장치의 교차분석 ; 국내 제조사와 해외 제조사 비교)

  • Dual energy X-ray absorptiometry is mainly used as an X-ray test method. For equipment manufactured GE and Hologic, cross-calibration analyses (CCA) of machines from the same manufacturer and between units from different manufacturers have been conducted, but the CCA of equipment manufactured in Korea are inadequate. Through CCA, we present a formula of the intersections between the Korean medical equipment company (KEC) with GE and Hologic manufactured DXA, and among the KEC DXA. The CCA was conducted for the European Spine Phantom on DXA from four KEC and three global medical equipment company (GEC) manufacturers. We compared bone mineral density (BMD) values and calculated the CCA equation by linear regression analysis. The standard-deviations (SD) of the BMD values were highest for the Dexxum T for the low, medium, and high spine, which were 0.030, 0.029, and 0.037, respectively. The smallest SD in the low and medium vertebrae were 0.005 and 0.004 for the Horizon Ci, respectively, and 0.005 for the Osteo Pro Max in the high vertebrae. Based on the intersection equations of the KEC DXA established in this study, CCA of various KEC DXA should be established for more accurate follow-up of BMD tests in clinical environments.

  • Offer of medical treatment service for recovery or preservation of physical spiritual function of elderly people by sudden population graying developing is essential. Therefore, according to increase of elderly's medical treatment demand, medical treatment service request is augmented. Number of medical treatment service utilization wishes to grope medical treatment service support way for elderly residing cloth elevation laying stress on elderly's Aging in Place in increase trend in this research. If decide, is as following : First, national hospital and public health center were concentrated most on Seoul and kyonggi, and there were many hospitals to south of a river nine, Songpagu, Seochogu, and public health center was expose that is one by one to each nine. Second, in the case of Seoul, elderly population ratio was expose that comparatively high Yeongdeungpo, Chongno, Yongsan, west passage nine is few hospital number relatively in elderly's residential area. Third, need that establish elderly full text clinic on part of general hospital or university hospital equipment. Fourth, must do so that can use access as is easy little more in local community to all elderly who need medical treatment service of visit nursing, visit medical examination and treatment etc.. that consist in present public health center.

  • Advancement in the medical field provides an opportunity for the development of medical equipment and also enable accurate diagnosis for the inspection and the treatment of diseases. The aging of equipment due to the frequent operation produce uncertainty in the patient exposure dose, so a quality control was implemented by establishing a safety management system on a regular basis. The x-ray tube voltage (kVp), which is directly involved in the patient exposure dose and the life of the equipment, needs periodic performance for the quality control, but it has a limitation due to the time and the cost. In this study, we proposed a new method for measuring the kVp with ease by using the Y and Cu to solve the problem of the time and cost. We also evaluated the usefulness and the effectiveness of the new method and its accuracy through the kVp measurement. After securing the condition, the amount of the tube current and evaluating the usefulness and effectiveness of kVp measured using Y and Cu. The density range used was at 0.5-1.0, kVp was in the range of ${\pm}10%$, then, we recorded the change in concentration as % of Y and Cu. This experiment showed that when Cu and Y was at 75 kVp, concentration was the same, and when the kVp was changed to 65 ~ 85 %, a gradual increase in concentration to 85 ~ 110 % was noted.

  • Mammography equipment is an essential detector for making an early diagnosis of female's breast lesion. Recently, in most hospitals, a digital mammography detector is used due to the wide and consistent supply of digital mammography equipment. However, the average effective radiation is increasing due to the indiscreet use of CR or DR mammography. The purpose of this study is to recognize the possible indirect radiation damage, which can be occurred due to an excessive effective exposure of radiation, by evaluating spacial radiation rate of the digital mammography detector used for female patient. Consequently, the high mount of spacial radiation showed digital mammography equipment on the horizontal direction. Considering the result, digital mammography equipment should be installed by avoiding along the horizontal direction.

The Radiation Safety Management in the Animal Hospital Using Inspection Standard of Diagnosis Radiation System (진단용 방사선발생장치의 검사기준을 적용한 동물병원의 방사선 안전관리)

  • The purpose of this study was to investigate the actual conditions of radiation safety supervision in animal clinics using inspection standard of X-ray generator for diagnosis. The surveys for inspection standard system, equipment condition, and safety supervision were carried out in 18 animal clinics randomly. The inspection standard included reproducibility of dose exposure, kVp, mAs, collimator accuracy test, collimator luminance test, X-ray view box luminance test, grounding system equipment test and external leakage current test. The surveys of equipment condition and safety supervision used one-on-one interview with 5 points measurement. As a result, 44.44% of reproducibility of dose exposure was proper, 81.25% of kVp test was good, and 100% of mAs test was appropriate. Also, 66.66% of collimator accuracy test was proper, 61.11% of collimator luminance test was good, 53.13% of X-ray view box luminance test was suitable. In addition, only 5.55% of grounding system equipment and ground resistance was proper, 63.64% of external leakage current test was appropriate in grounding system equipment test. The 100mA electric capacity of X-ray generator for diagnosis was popular with 44.44%, and its 55.56% was purchased used equipment. Monthly average of less than 50 times (61.11%) was top frequency in use, and no animal clinic had a thermo-luminescence dosimeter(TLD). The 16 animal clinics with radiation safety zone and 2 without radiation safety zone were appeared.

Anticipating the Need for Healthcare Resources Following the Escalation of the COVID-19 Outbreak in the Republic of Kazakhstan

  • Objectives: The lack of advance planning in a public health emergency can lead to wasted resources and inadvertent loss of lives. This study is aimed at forecasting the needs for healthcare resources following the expansion of the coronavirus disease 2019 (COVID-19) outbreak in the Republic of Kazakhstan, focusing on hospital beds, equipment, and the professional workforce in light of the developing epidemiological situation and the data on resources currently available. Methods: We constructed a forecast model of the epidemiological scenario via the classic susceptible-exposed-infected-removed (SEIR) approach. The World Health Organization's COVID-19 Essential Supplies Forecasting Tool was used to evaluate the healthcare resources needed for the next 12 weeks. Results: Over the forecast period, there will be 104 713.7 hospital admissions due to severe disease and 34 904.5 hospital admissions due to critical disease. This will require 47 247.7 beds for severe disease and 1929.9 beds for critical disease at the peak of the COVID-19 outbreak. There will also be high needs for all categories of healthcare workers and for both diagnostic and treatment equipment. Thus, Republic of Kazakhstan faces the need for a rapid increase in available healthcare resources and/or for finding ways to redistribute resources effectively. Conclusions: Republic of Kazakhstan will be able to reduce the rates of infections and deaths among its population by developing and following a consistent strategy targeting COVID-19 in a number of inter-related directions.

Patient's Satisfaction with Medical Care Services in Hospital (병원 이용자의 의료서비스 만족도 조사)

  • The purpose of this study was to determine factors influencing patient satisfaction with medical services in hospital, which is classified into environmental aspect, human services and procedural services. Based on the results of literature review, the study focused on effects of social-demographical factors on patient satisfaction. The environmental aspect of medical care services included medical equipment and facilities, hygiene, ventilation, heating and air-conditioning, waiting and resting space, ward space and parking facilities. Procedural service included registration process, bill payment, waiting time after registration, examination and prescription as well as appointment process. Human services consisted of physicians listening to stories of patients, examination duration, physicians' explanation and physicians' service. As for nurses, explanation about disease, examination procedure and results, kindness and nursing care were evaluated. Services provided by other staff members were also evaluated. Patient satisfaction, defined as individual attitude toward medical service as a whole, was measured using a questionnaire. A total of 700 in-or out-patients were surveyed in 6 hospitals with more than 300 beds in North Gyeongbuk Province. 1. The level of patient satisfaction varied with characteristics of patients. Male patients and those in their 30s had a low level of satisfaction. Dissatisfaction level was positively related to education level but negatively related to economic condition. 2. As for patient satisfaction with medical service providers and other employees in hospital, satisfaction level with physician's explanation about treatment was higher. But dissatisfaction levels with treatment duration and the lack of explanation about examination procedures were high, calling for improvement. Dissatisfaction level with nursing care was high, calling for training of nurses for better service. Given the low level of satisfaction with human services, hospital employees need to be trained to improve their service. 3. It Was found that administrative service was also a significant factor influencing patient satisfaction in addition to medical service. It is therefore important for hospitals to provide patients with prompt and convenient procedural service. 4. Environmental factors such as medical equipment and amenity facilities also affected patient satisfaction. Thus environmental condition, procedural service and human service are all important to improve medical service in hospital. In summary, procedural service was the most significant factor for patient satisfaction. The level of satisfaction in patients was also affected by human service and environmental condition. It is therefore necessary to take patient-oriented approach in providing medical service in an effort to improve patient satisfaction. The finding of a lower level of satisfaction with human service signifies the need for training of healthcare providers and other hospital employees for better services. The introduction of advanced management programs is also needed to improve procedures that patients go through in hospitals.
