Radiological Risk Assessment for $^{99m}Tc$ Generator using Uncertainty Analysis (불확실성 분석을 이용한 $^{99m}Tc$ 발생기 사용의 방사선위험도 평가)

  • Recently, much attentions are paid to the risk associated with increased uses of medium size radiation sources in medical and industrial fields. In this study, radiation risks to the worker and to the general public due to $^{99m}Tc$ generator were assessed for both normal and accident conditions. Based on the event tree technique, exposure scenarios for various situations were derived. Uncertainty analysis based on the Monte-Carlo technique was applied to the risk assessment for workers and members of the public in the vicinity of the work place. In addition, sensitivity analysis was performed on each of the five independent input parameters to identify importance of the parameters with respect to the resulting risk. Because the frequencies of normal tasks are fat higher than those of accidents, the total risk associated with normal tasks were higher than the accident risk. The annual dose due to normal tasks were $0.6mSv\;y^{-1}$ for workers and $0.014mSv\;y^{-1}$ for public, while in accident conditions $3.96mSv\;y^{-1}\;and\;0.0016mSv\;y^{-1}$, respectively. Uncertainty range of accident risk was higher by 10 times than that of normal risk. Sensitivity analysis revealed that source strength, working distance and working time were crucial factors affecting risk. This risk analysis methodology and its results will contribute to establishment of risk-informed regulation for medium and large radioactive sources.

The Odd Pair Family's Health management in rural, Korea -Comparison with the Pair Family- (농촌거주 외짝가족의 건강관리-부부가족과의 비교)

  • Family has emerged as a key concept for health, and it has been identified as one of the most important conditions. The relationship between health habit and its management is different depending on family. The odd pair family, mostly rural lower income class, worry to have poor health because of no spouse and small family size. One thousand eight hundred and seventy(1870) subjects were collected in 9 provinces through the sampling of Probability Proportional to Size (PPS). Questionnaire method was conducted on health checking, bath states, alcohol consumption, cigarette smoking, and the prevalence of farmer's health related problems. The main results were as follows: 1) The characteristics of odd pair families are that the head of household is female(77% ), the size of family is small(1.76 persons), the education level is low(7.5 years for male, 3.1 years for female) and the age group is old (male: 89.78 year old, female: 73.69 year old). 2) For the odd pair family, the frequency of health checking is quite low with one or two times per year(l0.2%) and the rate of no-health checking is much higher(35.8%) .3) Bathing utility is not available 29.6% of the odd pair family and only cold water is supplied at home for the 11.5 % of them. However, for the paired family, 9.8 % of them has no bathing utility and the rate of the family supplied with only cold water is just 7.9%. 4) The bathing frequency score of odd pair family is l.74points for male and 1.25 points for female. 5) The rate of smoking habits for odd pair family is 68.5 % and specially it is 7.6% for female, which is higher comparing with that of pair family. 6) The smoking frequency score of odd pair family is 1.57 points. 7) Alcohol drinking frequency score of odd pair family is 1.79 points for male, and 3.24points for female. 8) Farmers' syndrome(FS) revealed 38.7% of odd pair family and it is lower than that of pair family(57.3%). Special pain of FS was huckle bone and muscle(28.4%) and articular pain(24.l %). The pain rate of huckle bone and muscle(43.l %) and articular pain(33.5%) were higher in a year in odd pair family were lower than those of pair family: farming machine caused accidents(6.5%) and pesticide poisoning(5.7%). l0) The odd pair family use more frequently medical clinic or public health center for the treatment of FS(74.7%) and pesticide poisoning(62.5%) than the pair family for FS(69.0%) and for pesticide poisoning(.53.6%). The score of FS treatment is 5.70 points for odd pair family and it is not significantly different from 5.62 points of the paired family. The result of pesticide poisoning treatment score is as same as that of FS.

An Analysis of Internet based Child Care Portal sites (인터넷 육아전문 사이트의 육아정보 분석)

  • Web sites on the internet are getting major resources to gain information related to child care. Though the numerous web sites deliver child care information, they have never been evaluated with criteria before. The purpose of this study is to identify existence and organization of child care portal sites and to analysis their contents, therefore to suggest guidelines for parents. The survey was conducted from Sep. 1. to Oct. 30., 2001. by means of Lycos Korea and Daum search engine and finally 45 portal sites related to child care were selected eliminating the commercial and personal homepages. The results were as follows: 1. Most of the sites(95.6%) were operated by corporations without registration(82.2%). Consultants were mostly professional (71.1%). 2. The contents were analyzed 4 categories including 19 themes. 3. Diet & Nutrition category include weaning food, breast feeding, bottle feeding and snack. The recipe and type of weaning food on months were topped(64.44%). 4. Infant Care category include bathing, sleeping, clothes, skin care and cord care. bathing method and heat of bathing water were topped(44.44%). 5. Growth & Development category was consisted of developmental characteristics, dental growth, play & exercise and learning & guidance. Developmental stage and motor development were topped (62.22%). 6. Health maintenance & promotion category was consisted of emergency care, prevention of accidents, vaccination, common pediatric disease and parental role. Care of diarrhea, constipation and fever, and precautions for vaccination were topped(48.89%).

  • A disaster can be defined in many ways based on perspectives, in addition, its types are able to classify differently by various standards. Considering the different perspectives, the disaster can be occurred by natural phenomenon that is like typhoon, earthquake, flood, and drought, and by the accident that is like collapse of facilities, traffic accidents, and environmental pollution, etc. Into the modern society, moreover, the disaster includes the damages by diffusion of epidemic and infectious disease in domestic animals. The disaster was defined by natural and man-made hazards in the past. As societies grew with changes of paradigm, social factors have been included in the concept of the disaster according to new types unexpected by new disease and scientific technology. Change the concept of social disasters, Ministry of Public Safety and Security (MPSS) has provided the regional safety index, which measures the safety level of a local government. However, this regional safety index has some limitation to use because this index provides the information for city unit which is a unit of administrative districts of urban. Since these administrative districts units are on a different level with urban and rural areas, the regional safety index provided by MPSS is not be able to direct apply to the rural areas. The purpose of this study is to determine the regional safety index targeting rural areas. To estimate the safety index, we was used for 3 indicators of the MPSS, a fire, a crime, and an infectious disease which are evaluable the regional safety index using an accessibility analysis. For determining the regional safety index using accessibility from community centers to public facilities, the safety index of fire, crime, and infectious disease used access time to fire station, police office, and medical facility, respectively. An integrated Cheongju, targeting areas in this study, is mixed region with urban and rural areas. The results of regional safety index about urban and rural areas, the safety index in rural area is relatively higher than in the urban. Neverthless the investment would be needed to improve the safety in the rural areas.

Clinical and Statistical Analysis with Age in Cases of Pediatric Burn Patients (소아 화상 환자 2,795례에 대한 연령별 임상 통계학적 분석)

  • Purpose: The objective of this study is to analyze the epidemiological characteristics of pediatric burn patients and to determine the targets for a pediatric burn prevention program. Methods: A retrospective review of all medical records of acute pediatric burn patients (age < 15 years old) admitted to our hospital between January 2005 and December 2009 was performed. Results: 1472 males and 1323 females were investigated, with a male to female ratio of 1.11 : 1. The greatest number of burn patients were those with an age of 1~2 years (1,463, 52.3%). Scalding burn was the most common cause of injury, which accounted for 2183 (78.1%) patients, followed by contact burns (10.5%), flame burn (4.9%), steam burn (3.6%). Especially steam burn was the second cause of injury in the age under 1 year, while flame burn was the second cause of injury in the age over 7 years. During recent 5 years, incidence of flame burn, steam burn, electrical burn gradually decreased. Variation of seasonal incidence is minimal and most of the patients (2,716 cases, 97.2%) had burns less than 20% TBSA (Total body surface area). The median hospital stay was 18.79 days, and the rate of operation was 28.6% with a high rate in electrical burn (76.2%), flame burn (50.0%), steam burn (46.1%). 6 patients died in this series, which yielded a mortality rate of 0.2%. Conclusion: Prevention efforts should reflect recent study results. Focused prevention program and campaign to make people aware of risk factors and their avoidance is required to reduce the number of burn accidents in children.

Factors Affecting Hemodynamic Instability in Patients with Pelvic Bone Fracture (골반골 골절 환자의 혈역학적 불안정화에 미치는 영향요소)

  • Purpose: Treatment and prognosis in patients with pelvic bone fracture depend on the characteristics of the fracture and the stability of the pelvic ring. The purpose of this study is to analyze the characteristics of and the relationships between fracture patterns, injury mechanisms, clinical courses, and prognoses according to the hemodynamic pattern. Methods: Between January 2004 and September 2006, 89 patients under diagnosis of pelvic bone fracture were retrospectively analyzed on the basis of medical records and radiologic examinations. Patients with confirmed hemorragic shock with a systolic pressure of less than 90 mmHg were defined as the shock group. Young's classification was used to characterize fracture patterns. Factors relating to the clinical manifestation and to treatments such as transfusion and surgery were analytically compared. Results: The mean age of the patients was $48.8{\pm}18.7$, among which 49 (55.1%) were male. The numbers of shock and non-shock patients were 35 (39.3%) and 54 (60.7%) respectively. Eighteen (51.4%) of the shock patient had injuries resulting from pedestrian accidents (p=0.008). According to Young's classification, lateral impact fractures amounted to 20 and 33, front-rear impact fractures to 9 and 20, and multiple fractures to 6 and 1 among the shock and non-shock patients, respectively (p=0.027). Thirty-nine (39) cases in non-shock injuries were conservatively managed while 18 cases in shock injuries were surgically treated. In the shock group, the liver and the kidney were often damaged, as well. Among the shock patients, the average admission period was $7.5{\pm}8.7days$ in intensive care and $55.1{\pm}47.9days$ in total, which were longer than the corresponding numbers of days for the non-shock patients (p<0.05). No deaths occurred in the non-shock group while 5 deaths (14.2%) occurred in the shock group (p=0.007). Conclusion: The factors affecting hemodynamic instability in patients with pelvic bone fracture are injury mechanism, classification of fracture, and associated injuries.

  • This study has examined how the welfare system has changed as it has passed through the most controversial period in Korean modern history. The welfare system has changed in a way that adapts to the need for export-led economic growth. Industrialization centered on light industry, which started in the mid-1960s, absorbed the labor force that existed in the rural areas and commodified them, thereby creating a momentum for Korean society to get out of poverty. However, the public de-commodification, ie social security system, adapted to the commodification of the labor force has been institutionalized only in a very limited area and people. Indeed, the de-commodification system was confined to the area directly linked to the reproduction of the labor force. Even so, the target was very limited in the abundance of labor in rural areas. Compulsory medical insurance was rejected because of corporate burden, and industrial accidents insurance was introduced centering on large-scale workplaces. As the Korean economy began to move from the light industry to the heavy industry in the 1970s, the commodificated labor force changed from a low skilled labor force to a skilled male labor force. It is at this time that dual structures have begun to be created between workers employed in export-oriented large enterprises and workers employed in domestic-oriented SMEs. Therefore, the system of de-commodification that supports the reproduction of labor power in response to social risks has also been institutionalized centering on large-scale workplaces.

Whiplash Injury Conditions of Rear-End Collisions at Low-Speed (저속 추돌사고에서 목 상해 조건에 대한 연구)

  • As the number of reported injuries has tended to increase over time, large hospitalization expenditure from excessive medical treatments and hospitalization, and insurance frauds associated with moral hazard in minor collisions have caused a global societal problem. Many occupants of rear-ended vehicles involved in rear-end collisions complain of whiplash injury, which is also known as neck injury, without any anatomical and radiological evidence. With only clinical symptoms, stating that a whiplash injury is a type of injury defined by the Abbreviated Injury Scale would be difficult. Therefore, this study focuses on minor rear-end collisions, where the rear-ender vehicle collides with the rear-ended vehicle at rest. The mathematics dynamic model is employed to simulate a total of 100 rear-end collision scenarios based on various weights and collision speeds and identify how the weights and speeds of both vehicles influence the risk of whiplash injury in occupants involved in minor rear-end collisions. The possibility of an injury is very high when the same-weight vehicles are involved in accidents at collision speeds of 15 km/h or higher. The possibilities are 36% and 84% with collision speeds of 15 km/h and 20 km/h, respectively, if weights are disregarded.

An Empirical Study on Firefighters' Health Hazard Factors -Focused on Fire Fighters, Rescue Workers and Emergency Medical Technicians Perception in Busan Fire Fighters- (소방공무원 건강장해 유해인자에 대한 실증연구 -화재진압대원, 구조대원, 구급대원의 인식조사를 중심으로-)

  • This study is intended to provide basic data for health management of firefighters in the future by empirically looking into health hazard factors of firefighters in Busan City. It was revealed that firstly, the danger of harmful chemicals in a fire was perceived the most by firefighters, who extinguish a fire in person on the scene of a fire, and it was followed by the danger of falling while putting out a fire. This study is intended to provide basic data for health management of firefighters in the future by empirically looking into health hazard factors of firefighters in Busan City. It was revealed that firstly, the danger of harmful chemicals in a fire was perceived the most by firefighters, who extinguish a fire in person on the scene of a fire, and it was followed by the danger of falling while putting out a fire. Moreover, the danger of shift work was perceived the most by paramedics. This corresponds to the existing studies arguing that shift work is harmful to health. Next, the overload of patient transport was recognized as the second biggest hazard factor. This demonstrates they are worried about various second accidents that may happen due to a lot of patient transport works. In addition, the possibility of causing a traffic accident was perceived as a hazard factor too, since they must drive ambulance cars quickly to transport patients. Lastly, rescue workers regarded these hazard factors to be most dangerous. This is associated with their occupational characteristics, because rescue workers are the closest to diverse risks including a fire.

  • The life expectancy of people has been steadily extended due to the improvement of living standards and the development of medical technology. Besides, the low birth rate and long life expectancy decrease the youth population while the elderly population is steadily increasing. Many elderly people have enough time and financial allowance, and the interest in health care is increased all over society. There are also increasing numbers of elderly people who exercise lightly in the playgrounds and parks such as running and walking. Outdoor fitness equipment that can be easily seen in neighbourhood parks, public spaces, etc. can be good health promotion tools that can be easily accessed in the daily life of the elderly. Outdoor fitness equipment Description Signs require a design that takes into account the declining vision of older users. Proper design is also important for the prevention of safety accidents in use. The purpose of this study is to find out the perception of the elderly about outdoor fitness exercise equipment sign system and to find the problem of recognition. To do this, we conduct user observation and interview surveys for the elderly. As a result, the emphasis of the caveat or problems with the size of the writing were pointed out, and it was necessary to explain the picture rather than the text. Also, there was a high demand for refraining from using foreign words, using universal words and including motor effects in signs. Based on the results of the user observation and interviews, we propose guidelines for the use of outdoor exercise equipment.
