A Study on the Current Status and System of the Elderly Medical and Welfare Facilities with the Subjects in Busan (노인 의료복지시설의 실태분석 및 시설체계에 관한 연구 - 부산광역시 노인 의료복지시설을 대상으로 -)

  • This is the study on the current status and system of the elderly medical and welfare facilities with the subjects in Busan. I investigated the problems related to those by analysing the current status and implementing a survey and then suggested a new facility system as a salvation. At present, the elderly medical and welfare facilities can not receive variety of inmates and also they are operated separatedly so the elderly is not able to receive special treatment and nursing at the same time in one facility. Some nursing facilities can give special treatment by connecting specific hospital when it is necessary, but the connection is not activated than our expectancy at the present. On the base of these problems, this study suggested 'one-stop service system' that can afford to give nursing and special treatment in a place ranging wider areas as the solution. The five types of facility system by integrating the results of survey are as followings. 1) Free special nursing facility + The geriatric hospital. 2) The profit special nursing facility + The geriatric hospital. 3) The geriatric hospital + The hospital rooms with silver town residence type. 4) The actual expense special nursing facility + The geriatric hospital + The hospital rooms with silver town residence type. 5) The geriatric hospital + The residential welfare service. It seems that the preparation for the elderly is urgent when considering the facilities related to those are not special any more because every one is supposed to use them some day. The kinds and the degree of elderly's diseases will be more duplicated and significant afterwards. Therefore it is expected that more various and specialized facilities are to be required.

Development of the System for Home Visiting Physical Therapy (가정방문 물리치료 시행을 위한 시스템 개발)

  • The most of patients and protectors point that the Home Visting Physical Therapy is necessary and compulsive. A Public Health Center and a Welfare Center have to help a home visiter for treatment cure to ask for professional medical services in general hospital. On getting a name lists of patients a treatment of doctors, they must remark the conditions of the patients to keep the mutual relation general hospital. A home visiting physical therapists in the welfare center and public health center need to discuss a main doctors in a same center for revaluation of patients. The system in a general hospital consists of the medical department and the administration with the doctor of hospital as the central figure. A department of home visiting physical therapy has a physical therapy team closely connected with various medical office in hospital. The system in a public health center is composed of the health executive office, health direction medicine office, and the community health office. Department of home visiting physical therapy belongs to community health office. Home visiting physical therapy in a welfare center belongs to home visiting service office. The qualifications of a physical therapist is intended for people who have received clinic experience of three years and regular education. The theory (352 hours) and practice (248 hours) total to 600 hours. They can develop professional skills through these education courses. The frequency of home visiting is proper third a week after talking with a medical attendant about the state of patient. The care time is proper from 30 to 60 minutes.

Study on optimal treatment payment by cost accounting in the artificiality kidney center in medical institutions (의료기관 인공신장실의 원가계산에 의한 적정수가에 관한 연구)

  • This study is to research cost accounting practice and to analyze propriety of patients' medical payment in artificiality kidney center. The researched cost datum of the year 2012 are as follows. - Hemodialysis medical treatment was reimbursed as much as 158,001 won in case of health insured patients, but payed-off as much as 135,810 won. - The average figure of the total hospitals and clinic center is 1,603,303 won, and one time cost of hemodialysis treatment is 154,487 won. Optimal treatment pay are suggested as follows. First, Regardless of the notified classification from MOHW(Ministry of Health and Welfare), 136,000 won of fixed price payment classification needs to be reclassified by patients, severity and tobe rearranged by fixed price payment system of hospitals. Second, Fixed payment code notified by the Ministry of Health and Welfare is recommended to be simplifies and to reflect according to contents of the medical treatment rendered to patients. Third, Establishment of artificial kidney center has to be risk managed because of its huge investment. Fourth, Cost analysis model has to be maintained as basis together with appropriate application of conversion index model mixed with SGR model.

  • Purpose: The purpose of this study is to investigate the impact of job stress, self-efficacy and empowerment of 119 Emergency medical technicians in Jeollanam-do. Methods: This study used the data collected from 346 respondents working for first aid in a fire station. To answer research questions, this study employed frequency analysis, reliability test, F (t)-test, ANOVA, Scheffe' post-hoc test, and multiple-regression analysis. Results: In terms of demographic characteristics, marital status and age had significant impact on job stress, but sex and education did not. In terms of job characteristics, first placement, rank, and tenure made significant difference on job stress, whereas certificate, department, and area did not. These results address that general characteristics, self-efficacy and job stress are critical variables on empowerment. Conclusion: From the research findings, it is concluded that fire fighters' job stress can be reduced if professional development programs are given to them, which is likely to provide better public services such as first aids and welfare, as well as to increase job satisfaction.

Proposed ICT-based New Normal Smart Care System Model to Close Health Gap for Older the Elderly

  • At the time of entering the super-aged society, the health problem of the elderly is becoming more prominent due to the rapid digital era caused by COVID-19, but the gap between welfare budgets and welfare benefits according to regional characteristics is still not narrowed and there is a significant difference in emergency medical access. In response, this study proposes an ICT-based New Normal Smart Care System (NNSCS) to bridge the gap I n health and medical problems. This is an integrated system model that links the elderly themselves to health care, self-diagnosis, disease prediction and prevention, and emergency medical services. The purpose is to apply location-based technology and motion recognition technology under smartphones and smartwatches (wearable) environments to detect health care and risks, predict and diagnose diseases using health and medical big data, and minimize treatment latency. Through the New Normal Smart Care System (NNSCS), which links health care, prevention, and rapid emergency treatment with easy and simple access to health care for the elderly, it aims to minimize health gaps and solve health problems for the elderly.

Factors Related to Physical Health Monitoring in Community-Dwelling Patients with Schizophrenia Spectrum Disorder (지역사회 거주 조현병 범주 장애 환자의 신체건강관리 관련 요인)

  • Objectives: To identify factors related to physical health monitoring of patients with schizophrenia spectrum disorder. Methods: A total of 172 patients with schizophrenia spectrum disorder registered in mental health welfare centers and rehabilitation facilities in Gwangju were recruited. Physical health monitoring was defined by two health behaviors; fasting blood tests within recent 2 years in all participants and routine medical check-ups covered by national insurance within recent 5 years in participants aged 40 years or older. Demographic and clinical characteristics including overweight, metabolic syndrome and knowledge about physical illness were compared according to physical health monitoring. Results: Prevalence of overweight and metabolic syndrome were 62.8% and 40.1%, respectively. The rates of fasting blood tests and routine medical check-ups were 34.9% and 67.9%, respectively. The rates of fasting blood tests were significantly higher in general hospital and university hospital compared to mental hospital or private clinic. Rates of routine medical check-ups were significantly lower in individuals using daily rehabilitation service and smokers. Knowledge about cancer and chronic illness were significantly better in individuals receiving routine medical check-ups compared with those not receiving it. Conclusion: Education about physical health should be integrated to mental health service in community mental health center.

Factors Influencing the Health Status and Life Satisfaction of Elders in Free and Charged Welfare Facilities (무료 및 유료 노인복지시설 노인의 건강상태와 생활 만족도에 영향을 미치는 요인)

  • Objectives: The purpose of this study was to investigate factors influencing the health status and life satisfaction of elders in welfare facilities. Methods: The subjects of this study were selected randomly among those without cognitive impairment from free (140 persons) and charged (140 persons) welfare facilities in the Yeongnam area. Data was analyzed using t-test and stepwise multiple regression. Results: Health status and life satisfaction were 2.52 and 1.98, respectively, in the elders from free welfare facilities, and 2.67 and 2.08 respectively, in the elders from charged welfare facilities. In those from free facilities, life satisfaction and motivation for getting into the welfare facility were the influencing factors of health status. In those from charged facilities, life satisfaction, gender, motivation for getting into the welfare facility, limited service such as physiotherapy, age, and lack of staff and professionalism were the influence factors of health status. In those from free facilities, health status, relationship conflict with fellow elders, lack of staff and professionalism, insufficient facilities and inadequate environment, and indifference of sons and daughters were the influence factors of life satisfaction. In those from charged facilities, health status, education and age were the influence factors of life satisfaction. Conclusions: It was found that both health status and life satisfaction of elders in charged welfare facilities were higher than those in free welfare facilities.

A Study of Social Program Space Layout in the Elderly Welfare Facility - Case study of 22 elderly welfare facilities in the Honam Province - (노인복지시설의 이용서비스공간의 면적구성에 관한 연구 - 호남지역의 노인복지회관 22개소를 중심으로 -)

  • Since elderly welfare centers were designated as leisure facilities for elders by the Welfare of the Age Act enacted in 1981, they have been built in cities and provinces, but services provided by the facilities have not been diverse. Furthermore, thanks to the full enforcement of the local autonomy system, the number of elderly welfare centers is increasing rapidly. In the Honam area, 36 elderly welfare centers were in operation in 2005, but most of them do not reflect the characteristics of the aged population and the corresponding locality. Neither are there service programs nor detailed plans for spatial structure or layout for such sonics programs. Therefore, this study classified program spaces and analyzed space locations in elderly welfare centers in the Honam area, and investigated the spatial layout of each facility including counseling spaces, medical and rehabilitative spaces, social education spaces, welfare spaces and management spaces. The ultimate purpose of this study is, based on the results of the analysis and investigation, to present basic materials necessary for designing the spatial structure of elderly welfare centers that satisfies various services in consideration of the local characteristics of the Honam area.

  • Objectives: In this study, we strived to determine the possibility of socioeconomic welfare in oral healthcare by analyzing the National Health Insurance (NHI) coverage rate. To date, efforts to realize the "social economy" of healthcare are active. While oral disease is common and chronic among Koreans, the rate of NHI coverage of dental clinics is substantially lower than that of the medical clinics. Methods: We defined the NHI coverage of dental clinics as a proxy for "social skills" to improve oral health problems. The data were collected through a comparative analysis of the NHI coverage of dental clinics and that of non-dental clinics, in health welfare social cooperatives. Results: The NHI coverage rate of the dental clinics in health welfare social cooperatives ranged from 0.97 to 2.62 times that of the non-dental clinics in health welfare social cooperatives. Conclusions: In conclusion, responsible management is recommended for making health welfare social cooperatives meaningful as a social economy.

A Study on the Establishment of Welfare Facilities for the Aged Using the Facilities Characteristics of Closed Schools - Focused on the Northern Area of Gyeonggi-Do - (폐교의 시설 특성을 활용한 노인복지시설 구축에 관한 연구 - 경기 북부지역을 중심으로 -)

  • Changes in the population structure caused by a decrease in the birthrate and an extension of life expectancy cause problems in utilization inefficiencies of closed schools and shortage welfare facilities for the aged. The closed schools have location characteristics that are close to the existing residence and facilities such as land and school buildings, so it is expected that the schools will be used as welfare facilities for the aged in the community. The purpose of this study is to compare and analyze the facility characteristics of closed schools in northern Gyeonggi-Do and present the types of aged welfare facilities that can be built, providing basic data for developing customized aged welfare facilities. The results of this study are summarized as follows: 1) The proportion of aged people in northern Gyeonggi-Do is higher than in southern Gyeonggi-Do, but welfare facilities for the aged are insufficient, so it is necessary to expand welfare facilities for the aged in northern Gyeonggi-Do. 2) Urban areas such as Paju and Goyang City have a lot of residential proximity, Dongducheon and Yangju have a lot of residential proximity, while schools located in urban and rural areas such as Yeoncheon have a lot of residential proximity. 3) Closed schools in urban areas such as Goyang City and Paju City are desirable to use them as residential facilities for senior citizens, day and night protection and short-term protection facilities, while closed schools in Namyangju City, Pocheon City, and Gapyeong County are desirable to use them as senior citizen centers and medical welfare facilities.
