진료역량 중심의 기본의학교육 학습성과 (Clinical Competency-Centered Learning Outcomes in Basic Medical Education)

  • Outcome-based learning is a global trend in medical education. The Korean Association of Medical Colleges (KAMC) has been developing learning objectives for basic medical education (BME) in Korea. In 2012, KAMC published the 1st edition of "Learning outcomes of basic medical education: Clinical competency-centered" in order to promote outcome-based medical education. KAMC has recently revised and updated the learning outcomes of basic medical education in the clinical competency-centered 2nd edition to reflect the suggestions of all medical schools in Korea and improve application of the published learning outcomes for BME in the field of medical education. KAMC has been making efforts to integrate clinical competency-centered learning outcomes with scientific concepts and principle-centered learning outcomes in addition to basic clinical skills and performance in BME.

  • This study aimed to review the expected changes in the medical educational environment and to evaluate approaches to coping with the abolition of the postgraduate intern training system. It is expected that after the intern training system is dismantled, postgraduate medical students will be deprived of the opportunity to practice opportunity for clinical practice and to inquire into their medical specialization. Therefore, major improvements in the clinical education curriculum must be made so that students can do so through the clinical education program. Offering students the opportunity to perform clinical practice through the clinical education program might require a revision in the laws and regulations on clinical education as well as the standardization of the clinical education curriculum in line with international practices. Reform measures to provide students the opportunity to inquire their specializations might be the introduction of a medical curriculum containing diverse fields and the establishment of a matching program to assign medical students to their residency programs after medical school. Finally, the fact that the basic concern of postgraduate medical education is the cultivation of primary care physicians must not be forgotten even after the dismantling of the postgraduate intern training system.

졸업 후 의학교육제도의 역사성 고찰 (Taking into Account the History of Korean Graduate Medical Education)

  • During the Japanese colonial period in the Korean Peninsula, Chosun (ethnic Korean) physicians were trained in vocational clinical schools, but Japanese physicians in medical school. Therefore, the Japanese government treated the Japanese physicians as medical doctors but Chosun physicians as dealers or traders in clinical services. This colonial discriminatory policy became a habitual concept to Korean physicians. Because of these traditional concepts regarding physicians, after the colonial period, the newly established Korean government also had the same concept of physicians. Therefore, in 1952, the Korean graduate medical education system was launched under a government clearance system with the claim of supporting medical specialties as clinical dealers or clinical businesspeople. During the last 60 years, this inappropriate customary concept and the unsuitable system have evolved into medical residency training education, and then into graduate medical education. Today graduate medical education has become inextricably linked to postdoctoral work in Korean hospitals.

의료시스템과학 교육의 임상실습 적용 사례 개발과 적용 (Development and Implementation of Health Systems Science Education in the Clinical Learning Environment )

  • Health systems science is a new medical educational field added to the traditional medical education curricula of basic and clinical sciences. Health systems science emphasizes a more comprehensive approach utilizing systems thinking to care for patients, including interactions between multiple healthcare systems. In this review, I explore how health systems science education can be applied when medical instructors teach students in clinical clerkships through representative case studies. This study first looks at examples of health systems science education in clinical clerkship in the United States and suggests how to develop the curriculum of health systems science for clinical learning environments in Korea by combining Kotter's 8-step change management model and Kern's 6-step curriculum development model. Finally, based on practical examples from actual clinical practice education situations, suggestions are made regarding how to develop the entire educational program of a medical school from the stage of applying health systems science at the individual level to clinical practice education.

의과대학 학생 임상실습을 위한 교육병원의 역할 제고 방안 (Improving the Role of the Teaching Hospital in the Clinical Clerkship)

  • The mission of the modern medical school includes education, research, and patient care. The clinical clerkship is an important part of the core curriculum, and hospital facilities are needed for the clinical clerkship. However, unfortunately, education has moved to the periphery during the past several decades because of the dominance of research and patient care. This may lead to obstacles in the education of future physicians in the long term. To promote their education mission, teaching hospitals need to recognize and share the importance of this mission. In addition to the certification of teaching hospitals, a new paradigm for teaching hospitals should be introduced to produce a high quality clinical clerkship and postgraduate medical education. The relevant government departments need to allocate and expand financial support to medical schools and teaching hospitals, and to unify supervision of basic and postgraduate medical education.

  • The clinical clerkship focuses students on developing their ability to perform comprehensive diagnosis and management of patients with common undifferentiated problems by the integration of knowledge and clinical reasoning. Therefore, the clerkship evaluation system should assess their actual problem solving and professional behavior. However, concern remains that clerkship evaluations are imprecise and highly variable. This review is designed to provide faculty members with concepts, options, and a methodology to actively teach and evaluate the clinical clerkship, as well as offer encouragement and inspiration to medical students. We reviewed past and current clinical clerkship evaluations and discuss several tips to improve clinical excellence such as continuity, transparency of the evaluation process, a faculty development program, practical examination of clinical skills, implementation of a checklist for recording exposure and skills, providing prompt and constructive feedback to students, self-evaluation of professional performance, varying multi-faceted assessment combinations, being outpatient clinic-centered, and having dedicated faculty members who give students one-on-one contact with a preceptor.

일개 의과대학 본과 4학년의 롤 모델링 경험과 성찰수준 분석: 임상실습을 중심으로 (Analysis of Role Modeling Experiences and Reflection Level of Fourth-Year Medical Students at a Single Medical College: A Focus on the Clinical Clerkship)

  • This study aimed to analyze the subjects, situations, and reflection levels related to role modeling experienced by medical students during their clinical clerkship and their own reflections. This study intends to suggest ways of improving how residents and clinical faculty should treat and teach medical students. Written interviews were conducted regarding senior medical students' role modeling experiences during their clinical clerkships in 2018 and 2019. Content analysis was conducted for a total of 224 cases from 196 students. Content analysis revealed three types of role modeling content: subjects (faculty, residents, nurses, peer students), situations (clinical competence, personal qualities, teaching skills), and the level of reflection (critical reflection, reflection, thoughtful action, and habitual action) in each case. As role model subjects, faculty were found to be the paramount role model (n=142, 62.83%). Role modeling was the most frequently performed for clinical competence (n=103, 45.98%). Clinical competence was frequently shown in communication and empathic listening during rounds and outpatient relationships between the patient and doctor. Regarding the level of reflection for role modeling, the number of critical reflections was 86 (38.39%) and that of reflections was 80 (35.71%). In particular, negative role modeling showed a high level of critical reflection in relation to faculty (64.44%) and nurses (8.89%). In conclusion, role modeling of medical students participating in clinical clerkships occurs in situations that the role models are not aware of, with positive or negative effects on the formation of professional identity among medical students.

의사면허 실기시험 제도의 성과와 과제 (The Effects and Challenges of Clinical Skills Assessment in the Korean Medical License Examination)

  • Clinical skills assessment was recently introduced to the Korean Medical License Examination to test medical school graduates' competencies in clinical skills. Various measures, including research and rehearsals, had been undertaken to prepare for the assessment for several years before the clinical skills assessment was first implemented. The assessment has been repeated annually for about 3,500 examinees over the course of 50 days per year for the past 4 years. The introduction of the assessment had significant effects on education in Korea's medical schools. Many schools have established clinical skills labs and the teaching of clinical skills has also been strengthened. The residents who have taken the clinical skills exam now express more confidence in caring for patients. To improve the quality of the assessment, it should be performed on a year-round schedule and a pilot test and various forms of the items should be introduced.

의과대학 교육과정중 임상영양교육의 필요성 평가 (Evaluation of Necessity of Clinical Nutrition Education in the Medical School Curriculum)

  • This study was conducted to evaluate the necessity of clinical nutrition education in the medical school curriculum. One hundred and eighty five medical school students, residents, nurses, and forty undergraduate students majoring in food science and nutrition were involved in this study. The survey included questions about nutritional knowledge, necessity of nutrition counseling, necessity of clinical nutrition and nutritional attitudes. The nutritional knowledge scores of the subjects in medical school and residents were significantly lower than those of the students majoring in food science and nutrition. Subjects of 91.3% strongly agreed to the necessity of nutrition counseling for patients ; 78.9% agreed that the present nutrition education in medical schools was inadequate. Most subjects (94.1%) agreed that clinical nutrition was an important field in the treatment of diseases. The medical profession is facing the challenges of the 1990s with the shift to outpatient care and preventative services. Clinical nutrition is an essential element in the process of curriculum change. The nutrition educator will be expected to take a leading role in integrating clinical nutrition into medical school curriculum.

응급구조(학)과 학생들의 임상현장실습 시 감염관리에 대한 인지도와 수행도 (Paramedic student's awareness and performance of infection control on clinical field training)

  • Purpose: This study aimed to provide basic data for infection control education plans based on infection control awareness and performance of paramedic students during clinical field training. Methods: Data were collected from paramedic students with experience in clinical field training. The data collection period was from May 4, 2023, to June 4, 2023, and 132 copies of the collected survey were analyzed using the SPSS27.0 program. Results: Infection control awareness and performance were 4.80±0.24 points and 4.49±0.55 points out of 5, respectively. The infection control awareness of the participants according to clinical field training-related characteristics differed significantly in university education before clinical field training (t=2.100, p=.038). In addition, there were significant differences in performance in the number of clinical field training sessions (F=9.149, p=.000), hospital education before clinical field training (t=5.365, p=.000), and hospital education during clinical field training (t=3.094, p=.002). Conclusion: Before clinical field training, schools should provide infection control education that combines theory and practice suitable for hospital practice so that students can complete the infection control education organized by the hospital. Furthermore, if a university develops infection control in the clinical field training guidelines, it will have a positive impact on students' infection control performance through prior education.
