The Effects of Tetracycline-loaded Silk Fibroin Membrane on Guided Bone Regeneration in a Rabbit Calvarial Defect Model (가토 두개골 결손부 모델에서 테트라사이클린 함유 실크 파이브로인 차폐막의 골유도 재생 효과)

  • Purpose: The aim of this study was to evaluate the bone regeneration ability of 1% tetracycline (TC)-loaded silk fibroin membrane (SFM), in a rabbit calvarial defect model. Methods: Twenty New Zealand white rabbits were used for this study. Bilateral round defects were made on the rabbit parietal bone, using trephine bur with an 8 mm diameter. TC-loaded SFM or SFM was covered on the right parietal bone defect, and the left parietal bone defects were uncovered for the control. The animals were humanely sacrificed at 4 or 8 weeks postoperatively. A micro-computerized tomography (${\mu}$-CT) of each specimen was taken for analysis of bone regeneration. Hematoxylin and Eosin stain were done to observe histological findings. Results: From the ${\mu}$-CT results, regenerated bone volume ($mm^3$) of 1% TC-loaded SFM, SFM, and control were $7.80{\pm}5.87$, $8.79{\pm}3.44$, and $10.61{\pm}5.3$ at 4 weeks postoperatively, respectively (P>0.05). Regenerated bone volume ($mm^3$) of 1% TC-loaded SFM, SFM, and control were $36.56{\pm}8.50$, $25.86{\pm}8.17$, and $19.09{\pm}5.07$ at 8 weeks postoperatively, respectively (P<0.05). Conclusion: The 1% TC-loaded SFM showed more bone regeneration than the SFM and the uncovered control, in guided bone regeneration.

  • Introduction: Hydroxyapatite ($Ca_{10}(PO_4)_6(OH)_2$, HA) is the main inorganic phase of human hard tissue that is used widely as the repair material for bones. When HA is applied to a bony defect, however, it can be encapsulated with fibrous tissue and float in the implanted area due to a lack of consolidation. Bioceramics as allogenic graft materials are added to HA to improve the rate and bone healing capacity. Fluoridated hydroxyapatite ($Ca_{10}(PO_4)_6(OH,F)_2$, FHA), where F- partially replaces the OH- in hydroxyapatite, is considered a good alternative material for bone repair owing to its solubility and biocompatibility. Materials and Methods: This study was designed to determine the bone healing capacity of FHA newly produced as a nanoscale fiber in the laboratory. HA and FHA with bioglass was implanted in a rabbit cranium defect and the specimen was analysed histologically. Results: 1. At 4 weeks, fibrous connective tissue and little bone formation was observed around the materials of the experimental group I implanted HA and bioglass. Newly formed bone was observed around the materials in the experimental group II implanted FHA and bioglass. 2. At 8 weeks, the amount of newly formed and matured bone was higher in experimental group II than in experimental group I and the control group. Conclusion: These results suggest that FHA and bioglass is a relatively favorable bone substitute with biocompatibility and better bone healing capacity than pure HA and bioglass.

  • As an efficient controlled release system for rhBMP-2, a functional nanoparticle-hydrogel complex, incorporated with matrix metalloproteinase(MMP) sensitive peptide cross-linker, was developed and used as a bone transplant. In vivo bone formation was evaluated by soft x-ray, histology, alkaline phosphatase(ALP) activity and mineral contents analysis, based on the rat calvarial critical size defect(8mm in diameter) model. Significantly, effective bone regeneration was achieved with the rhBMP-2 loaded MMP sensitive hyaluronic acid(HA) based hydrogel-Nanoparticles(NP) complex, as compared to only MMP HA, the MMP HA-NP without rhBMP-2, or even with the rhBMP-2. These improvements included the formation pattern of bone and functional marrow, the degree of calcium quantification, and the ALP activity. These results indicate that the MMP sensitive HA with nano-particle complex can be a promising candidate for a new bone defect replacement matrix, and an enhanced rhBMP-2 scaffold.

  • We tested the bone regenerating capacity and histologic response of bioresorbable matrix-type implant, which was made with Poly(lactide-co-glycolide)(PLGA) and bone apatite for the carrier of bone morphogenetic protein(BMP). The critical size defect of 8mm in diameter was created at the calvaria of SD rats(n=18), and repaired with polymer implant with $15{\mu}g$ of rhBMP-2(n=9) or without it(n=9). At 2 weeks, 1 months after implantation, the animals were sacrificed(3 animals at every interval and group) and histologically evaluated. The calvarial defect which was repaired with polymer with BMP healed with newly formed bone about 70% of total defect. But that without BMP showed only 0 to under 30% bony healing. Inflammatory response was absent in both group through the experimental period, but there's marked foreign body giant response though it was a little less significant in polymer with BMP group. As the polymer was resorbed, the space was infiltrated and replaced by fibrovascular tissue, not by bone. In conclusion, our formulation of bioresorbable matrix implant loaded with bone morphogenetic protein works good as a bone regenerating material. However, it is mandatory to devise our system to have better osteoinductive and osteoconductive property, and less multinucleated giant cell response.

  • Hemifacial microsomia is a term used to describe a facial anomalies caused by the defect of anatomic structures originated from the first and the second branchial arches. The defect area encompasses some facial areas including mandible, facial muscles, masticatory muscles, cranial nerves, auricles, etc., and the degree of manifestations of the anomalies is extmely diverse. A 20-year-old man complaining of facial asymmetry and malocclusion visited our hospital. An orthognathic surgery was performed for the correction of hard tissue anomalies and then autogenous dermis-fat autotransplantation was done for the improvement of remaining soft tissue defect. The result was esthetically good and the case was presented here.

  • The present study reports a case of developmental salivary gland defect. On panoramic examination, large radiolucent lesion surrounded by a sclerotic wall was noted in the right posterior mandible. On CT examination, large lingual bone concavity with extra-osseous course of the inferior alveolar nerve bundle was detected. In atypical cases, it is suggested that confirmatory imaging using CT or MRI should be taken.

Evaluation of bone formation and membrane degradation in guided bone regeneration using a 4-hexylresorcinol-incorporated silk fabric membrane

  • Background: The aims of present study were (1) to evaluate new bone formation among the 4-hexylresorcinol (4HR)-incorporated silk fabric membrane (SFM), conventional SFM, and uncovered control groups and (2) to compare the amount of residual membrane between the 4HR-incorporated SFM and conventional SFM in a rabbit parietal defect model. Methods: Nine New Zealand white rabbits were used for this animal study. After the formation of a bilateral parietal bone defect (diameter 8.0 mm), either 4HR-incorporated SFM or conventional SFM was grafted into the defect. The defect in the control was left uncovered. New bone formation and the amount of residual membrane were evaluated by histomorphometry at 8 weeks after the operation. Results: The total amount of new bone was $37.84{\pm}8.30%$ in the control, $56.64{\pm}15.74%$ in the 4HR-incorporated SFM group, and $53.35{\pm}10.52%$ in the conventional SFM group 8 weeks after the operation. The differences were significant between the control and 4HR-incorporated SFM group (P = 0.016) and between the control and conventional SFM group (P = 0.040). The residual membrane was $75.08{\pm}10.52%$ in the 4HR-incorporated SFM group and $92.23{\pm}5.46%$ in the conventional SFM group 8 weeks after the operation. The difference was significant (P = 0.039). Conclusions: The 4HR-incorporated SFM and conventional SFM groups showed more bone regeneration than the control group. The incorporated 4HR accelerated the partial degradation of the silk fabric membrane in a rabbit parietal defect model 8 weeks after the operation.

  • In 1942, Stafne described 35 "bone cavities" at the angle of the mandible. They appeared as unilocular, well-circumscribed, round or elliptical radiolucencies located below the inferior dental canal and between the angle of the mandible and first molar tooth. Since 1942, these lesions have been frequently described under various terms: aberrant or ectopic salivary gland; static or latent or idiopathic defect, cavity or cyst; mandibular salivary gland inclusion; lingual mandibular cavity; and Stafne's cyst, defect or cavity. Usually they were asymptomatic, with a predilection for men between age 50 and 70 years, and almost unilateral. At surgical exploration, they appeared as concavities on the lingual cortex and contained salivary gland tissue, often in continuity with the submandibular gland. In 1957, Richard and Ziskind were the first to report the appearance of a Stafne's cyst in the premolar region. Contrary to posterior defects, the anterior defects are difficult to diagnose clinically because the mandibular canal is not present, and the unilocular radiolucency can be confused with other cysts (radicular, residual, odontogenic, lateral periodontal,etc). The purpose of the present report is to describe an unusual case of Stafne's cyst in the anterior region of the mandible in 58-years-old woman.

THE USE OF MEDPORTM(POROUS HIGH-DENSITY POLYETHYLENE) IN ORAL AND MAXILLOFACIAL REGION (구강악안면영역에서의 MedporTM의 임상적용)

  • Recently, for the reconstruction of bony defect and cosmetic improvement, many graft materials and implants have been widely used in the various surgical situations. The alloplastic materials have many advantages such as simplicity of operation, no additional need of surgery, and easy manipulation. The $Medpor^{TM}$(porous high-density polyethylene, Porex Co., USA) was initially studied in 1972 for surgical implant and introduced as an implant material for oral and maxillofacial region by Sauer and King in 1988. This material permits full ingrowth of bone into the implants, substantially increasing the implant's incorporation into the recipient site. It can be shaved during the surgery, which results in an improvement and prefabricated various size and shapes to fit into the surgical defect. The $Medpor^{TM}$ was used in 32 patients from 1995 to 1997 at the maxillofacial region. It was used for paranasal augmentation in 24 cases, for malar augmentation in 2 cases, for infraorbital augmentation in 2 cases, for mandibular angle augmentation in 2 cases, for mandibular body augmentation in 2 cases, for chin vertical augmentation in 1 case. It was mainly fixed with miniplate or screw. There were few complications except one infection and one exposure of the implant.

DEVELOPMENT OF A BIOACTIVE CELLULOSE MEMBRANE FROM SEA SQUIRT SKIN FOR BONE REGENERATION - A PRELIMINARY RESEARCH (멍게와 미더덕 피부의 천연 셀룰로오스 각질을 이용한 골재생 효능을 가진 생활성막의 개발 - 예비 연구)

  • Objectives : To develop a bioactive membrane for guided bone regeneration (GBR), the biocompatibility and bone regenerating capacity of the cellulose membrane obtained from the Ascidians squirt skin were evaluated. Materials and methods : After processing the pure cellulose membrane from the squirt skin, the morphological study, amino acid analysis and the immunoreactivity of the cellulose membrane were tested. Total eighteen male Spraque-Dawley rats (12 weeks, weighing 250 to 300g) were divided into two control (n=8) and another two experimental groups (n=10). In the first experimental group (n=5), the cellulose membrane was applicated to the 8.0 mm sized calvarial bone defect and the same sized defect was left without cellulose membrane in the first control group (n=4). In the another experimental group (n=5), the cellulose membrane was applicated to the same sized calvarial bone defect after femoral bone graft and the same sized defect with bone graft was left without cellulose membrane in the another control group (n=4). Each group was sacrificed after 6 weeks, the histological study with H&E and Masson trichrome stain was done, and immunohistochemical stainings of angiogenin and VEGF were also carried out. Results : The squirt skin cellulose showed the bio-inductive effect on the bone and mesenchymal tissues in the periosteum of rat calvarial bone. This phenomenon was found only in the inner surface of the cellulose membrane after 6 weeks contrast to the outer surface. Bone defect covered with the bioactive cellulose membrane showed significantly greater bone formation compared with control groups. Mesenchymal cells beneath the inner surface of the bioactive cellulose membrane were positive to the angiogenin and VEGF antibodies. Conclusion : We suppose that there still remains extremely little amount of peptide fragment derived from the basement membrane matrix proteins of squirt skin, which is a kind of anchoring protein composed of glycocalyx. This composition could prevent the adverse immunological hypersensitivity and also induce bioactive properties of cellulose membrane. These properties induced the effective angiogenesis with rapid osteogenesis beneath the inner surface of cellulose membrane, and so the possibilities of clinical application in dental field as a GBR material will be able to be suggested.
