Contrast-Enhanced High-Resolution Intracranial Vessel Wall MRI with Compressed Sensing: Comparison with Conventional T1 Volumetric Isotropic Turbo Spin Echo Acquisition Sequence

  • Objective: Compressed sensing (CS) has gained wide interest since it accelerates MRI acquisition. We aimed to compare the 3D post-contrast T1-weighted volumetric isotropic turbo spin echo acquisition (VISTA) with CS (VISTA-CS) and without CS (VISTA-nonCS) in intracranial vessel wall MRIs (VW-MRI). Materials and Methods: From April 2017 to July 2018, 72 patients who underwent VW-MRI, including both VISTA-CS and VISTA-nonCS, were retrospectively enrolled. Wall and lumen volumes, signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR) were measured from normal and lesion sites. Two neuroradiologists independently evaluated overall image quality and degree of normal and lesion wall delineation with a four-point scale (scores ≥ 3 defined as acceptable). Results: Scan coverage was increased in VISTA-CS to cover both anterior and posterior circulations with a slightly shorter scan time compared to VISTA-nonCS (approximately 7 minutes vs. 8 minutes). Wall and lumen volumes were not significantly different with VISTA-CS or VISTA-nonCS (interclass correlation coefficient = 0.964-0.997). SNR was or trended towards significantly higher values in VISTA-CS than in VISTA-nonCS. At normal sites, CNR was not significantly different between two sequences (p = 0.907), whereas VISTA-CS provided lower CNR in lesion sites compared with VISTA-nonCS (p = 0.003). Subjective wall delineation was superior with VISTA-nonCS than with VISTA-CS (p = 0.019), although overall image quality did not differ (p = 0.297). The proportions of images with acceptable quality were not significantly different between VISTA-CS (83.3-97.8%) and VISTA-nonCS (75-100%). Conclusion: CS may be useful for intracranial VW-MRI as it allows for larger scan coverage with slightly shorter scan time without compromising image quality.

판정행렬분석을 통한 PET-MRI의 간암 진단성능 평가 (Evaluation of the Liver Cancer Diagnosis Function of PET-MRI Based on Decision Matrix Analysis)

  • 최근 임상에서 활용하고 있는 일체형 PET-MRI의 간암 진단능력을 평가하기 위해 $2{\times}2$ 판정행렬을 이용하여 유용성을 평가하였다. 실험대상의 PET-MRI 검사 결과를 통해 간암 판정 여부 즉 비정상과 정상 판정을 받은 경우는 각각 98건, 51건 이었으며, 세포병리학적 결과가 양성과 음성 판정을 받은 경우는 각각 103건, 62건으로 나타났다. 이 중 두가지 검사에서 진양성의 경우는 95건, 위양성은 3건으로 나타났으며, 진음성은 62건, 위음성의 경우는 5건으로 분석되었다. 실험결과 PET-MRI 검사의 예민도는 95.00%, 특이도는 95.38%, 위음성률은 0.05%, 위양성률은 0.05%, 정확도는 95.15%로 분석되었다. 따라서 간암의 진단에 있어 수술 전 병기 결정이나 치료 후 재발 및 원격전이의 발견, 불분명한 원발 림프절 전이 등의 평가 등에 활용 가능성이 높을 것으로 판단되며, 특히 병리학적 검사와의 복합적 진단 및 추적검사를 통해 간암 진단을 위한 PET-MRI 임상적 유용성은 충분할 것으로 사료된다.

MRI와 동시에 측정한 뇌전도 신호로 전류원 국지화를 할 때 경사자계 유발 잡음의 영향 분석 (Effects of Gradient Switching Noise on ECD Source Localization with the EEG Data Simultaneously Recorded with MRI)

  • 목적 : MRI를 촬영하면서 동시에 측정한 뇌전도 신호에 가장 크게 영향을 미치는 것은 경사자계 유발 잡음이다. MRI와 동시에 측정한 뇌전도 신호에서 전류원 국지화를 수행할 때, 경사자계 유발 잡음이 미치는 영향을 분석하고자 한다. 대상 및 방법 : 사람의 머리와 유사하게 만든 뇌전도 팬텀과 MR compatible 뇌전도 측정시스템, 그리고 3.0 Tesla MRI 시스템을 실험에 사용하였다. 3.0 Tesla MRI 시스템 안에 전류원이 설치되어 있는 뇌전도 팬텀을 놓고, EPI 촬영을 하는 동안 뇌전도 신호를 측정하였다. 경사자계의 세기와 전류원의 위치를 조절하면서 뇌전도 신호 측정을 하였고, 측정된 뇌전도 신호에 대하여 전류원 국지화를 수행할 때 나타나는 국지화 오차를 평가하였다. 결과 : 경사자계 유발잡음에 의한 국지화 오차는 경사자계의 세기와 전류원의 위치에 따라 변화하는 것을 관찰하였다. 또한 경사자계 유발잡음이 Gaussian 잡음보다 전류원 국지화에 미치는 영향이 큰 것을 관찰하였으며, 경사자계 유발 잡음의 전력이 뇌전도 신호 전력의 $10\%$ 이하일 때는 전류원 국지화에 미치는 영향이 미미함을 관찰하였다. 결론 : 경사자계 유발 잡음 전력을 뇌전도 신호 전력의 $10\%$ 이하로 줄인다면 MRI를 하면서 동시에 측정한 뇌전도 신호로도 전류원 국지화를 할 수 있음을 알 수 있었다.

  • Infectious and inflammatory bone diseases include a wide range of disease process, depending on the patient's age, location of infection, various causative organisms, duration from symtom onset, accompanied fracture or prior surgery, prosthesis insertion, and underlying systemic disease such as diabetes, etc. Bone infection may induce massive destruction of bones and joints, results in functional reduction and disability. The key to successful management is early diagnosis and proper treatment. Various radionuclide imaging methods including three phase bone scan, Ga-67 scan, WBC scan, and combined imaging techniques such as bone/Ga-67 scan, WBC/bone marrow scan add complementary role to the radiologic imaging modalities including plain radiography, CT and MRI. F-18 FDG PET imaging also has recently been introduced in diagnosis of infected prosthesis and chronic active osteomyelitis. Selection of proper nuclear medicine imaging method will improve the diagnostic accuracy of infections and inflammatory bone diseases, based on understading of pathogenesis and radiologic imaging findings.

종양내 출혈을 동반한 뇌수막종 - 증례보고 - (Meningioma with Intratumoral Hemorrhage - A Case Report -)

  • Meningioma associated with intratumoral hemorrhage is rarely reported. We present two patients with intratumoral hemorrhage. One 70-year-old man was admitted to our department with a decreased level of consciousness and left hemiparesis. CT scan and MRI scan revealed huge tumor with intratumoral hemorrhage in the frontal lobe. After surgical removal of the tumor, histopathological diagnosis was a meningotheliomatous meningioma. The other seemed patients was 56-year-old woman with headache and vomiting. She showed no specific neurological deficit. CT and MRI scan revealed large size tumor with intratumoral hemorrhage that looks like meningioma. The patient died suddenly before surgery. We reviewed the relevant literature and discussed the possible mechanism of hemorrhage in the meningioma.

Effect of Gd-based MR contrast agents on CT attenuation of PET/CT for quantitative PET-MRI study

  • We evaluate the influence of MR contrast agent on positron emission tomography (PET) image using phantom, animal and human studies. Phantom consisted of 15 solutions with the mixture of various concentrations of Gd-based MR contrast agent and fixed activity of [$^{18}F$]FDG. Animal study was performed using rabbit and two kinds of MR contrast agents. After injecting contrast agent, CT or MRI scanning was performed at 1, 2, 5, 10, and 20 minutes. PET image was obtained using clinical PET/CT scan, and attenuation correction was performed using the all CT images. The values of HU, PET activity and MRI intensity were obtained from ROIs in each phantom and organ regions. In clinical study, patients (n=20) with breast cancer underwent sequential acquisitions of early [$^{18}F$]FDG PET/CT, MRI and delayed PET/CT. In phantom study, as the concentration increased, the CT attenuation and PET activity also increased. However, there was no relationship between the PET activity and the concentration in the clinical dose range of contrast agent. In animal study, change of PET activity was not significant at all time point of CT scan both MR contrast agents. There was no significant change of HU between early and delayed CT, except for kidney. Early and delayed SUV in tumor and liver showed significant increase and decrease, respectively (P<0.05). Under the condition of most clinical study (< 0.2 mM), MR contrast agent did not influence on PET image quantitation.

The Unnecessity of Positron Emission Tomography Computed Tomography in the Etiologic Evaluation of Neurodevelopmental Delay in Craniosynostosis Patients

  • Background: In evaluation of craniosynostosis patients in terms of neurodevelopmental delay, positron emission tomography computed tomography (PET-CT) scan can be used to assess brain abnormalities through glucose metabolism. We aimed to determine the unnecessity of PET-CT in this study. Methods: Thirty-eight patients diagnosed with craniosynostosis who underwent distraction osteogenesis from October, 2010 to November, 2013 were reviewed. Magnetic resonance imaging (MRI) and PET-CT scan were carried out for evaluation of the brain structure and function, whereas X-ray and CT scan were taken for evaluation of the skull. Results: Nine patients reported abnormal MRI findings which were not significant, and five patients showed local problem on brain on PET-CT scan. No correlation was found among them. Conclusion: PET-CT evaluation of possible abnormal brain findings do not affect surgical planning or require additional therapy. Preoperative PET-CT scan is not the essential study to get any etiologic information of the disease consequences or to establish the treatment plan.

Investigation of light stimulated mouse brain activation in high magnetic field fMRI using image segmentation methods

  • Magnetic resonance image (MRI) is widely used in brain research field and medical image. Especially, non-invasive brain activation acquired image technique, which is functional magnetic resonance image (fMRI) is used in brain study. In this study, we investigate brain activation occurred by LED light stimulation. For investigate of brain activation in experimental small animal, we used high magnetic field 9.4T MRI. Experimental small animal is Balb/c mouse, method of fMRI is using echo planar image (EPI). EPI method spend more less time than any other MRI method. For this reason, however, EPI data has low contrast. Due to the low contrast, image pre-processing is very hard and inaccuracy. In this study, we planned the study protocol, which is called block design in fMRI research field. The block designed has 8 LED light stimulation session and 8 rest session. All block is consist of 6 EPI images and acquired 1 slice of EPI image is 16 second. During the light session, we occurred LED light stimulation for 1 minutes 36 seconds. During the rest session, we do not occurred light stimulation and remain the light off state for 1 minutes 36 seconds. This session repeat the all over the EPI scan time, so the total spend time of EPI scan has almost 26 minutes. After acquired EPI data, we performed the analysis of this image data. In this study, we analysis of EPI data using statistical parametric map (SPM) software and performed image pre-processing such as realignment, co-registration, normalization, smoothing of EPI data. The pre-processing of fMRI data have to segmented using this software. However this method has 3 different method which is Gaussian nonparametric, warped modulate, and tissue probability map. In this study we performed the this 3 different method and compared how they can change the result of fMRI analysis results. The result of this study show that LED light stimulation was activate superior colliculus region in mouse brain. And the most higher activated value of segmentation method was using tissue probability map. this study may help to improve brain activation study using EPI and SPM analysis.

적응 디지털 필터 기반의 MRI Cardiac Gating을 위한 심전도 신호의 MR Gradient 잡음 최소화 방법 (Minimizing MR Gradient Artefacts on ECG Signals for Cardiac Gating based on an Adaptive Digital Filter)

  • In Magnetic Resonance Imaging(MRI), the QRS complex of ECG is used as a trigger signal for MRI scan. But, gradient and RF(radio frequency) artifacts which are caused to static and dynamic field in MRI scanner cause interference in the ECG. Also, the signal shape of theses artifacts can be similar to the QRS-complex, causing possible misinterpretation during patient monitoring and false gating of the MRI. In case of using general FIR or IIR band-pass filters for minimizing the artifacts, artifact-reduction-ratio is not excellent. So, an adaptive real-time digital filter is proposed for reduction of noise by gradient and RF(radio frequency) artifacts. The proposed filter for MRI-Gating is based on the noise-canceller with NLMS(Normalized Least Mean Square) algorithm. The reference signals of the adaptive noise canceller are a combination of the noisy three channel ECG signals. In conclusions, the proposed method showed the acceptable quality of ECG signal with sufficient SNR for gating the MRI and possibility of real time implementation.
