  • Modern hospitals have been greatly facilitated with information technology (IT) such as hospital information system (HIS). One of the most prominent achievements is medical imaging and image data management so-called Picture Archiving and Communication Systems (PACS). Due to inevitable use of diagnostic images (such as X-ray, CT, MRI), PACS made tremendous impact not only on radiology department but also nearly all clinical departments for exchange and sharing image related clinical information. There is no doubt that better use of PACS leads to highly efficient clinical administration and hospital management. However, due to rapid and widespread acceptance of PACS storage and management of digitized image data in hospital introduces overhead and bottleneck when transferring images among clinical departments within and/or across hospitals. Despite numerous technical difficulties, financing for installing PACS is a major hindrance to overcome. In addition, a mirroring or a clustering backup can be used to maximize security and efficiency, which may not be considered as cost-effective approach because of extra hardware expenses. In this study therefore we have developed a new based on grid of distributed PACS in order to balance between the cost and network performance among multiple hospitals.

Study of Nuclear medical imaging tests patient of Claustrophobia (폐소공포증 환자의 핵의학 영상검사에 관한 연구)

  • Patients who had claustrophobia tend to feel fear when they were scanned by an MRI, CT, PET-CT, or using a gamma camera scan. In this paper, claustrophobic patients were tested to find effective ways by changing patient's positions. For this paper, PET-CT scan in patients who had claustrophobia were used in the prone position. Prone position helped to maintain stable position and to get a h0igh quality of inspection without failure. Thus, as claustrophobic patients were requested taking prone position, they could feel comfortable. In a confined space, prone position for the claustrophobic patients who had a fear of the PET-CT examination would be expected to reduce the failure rate of inspection.

Assessment of the Synovial Inflammation in Rheumatoid Arthritis with $^{99m}Tc$-labelled Polyclonal Human IgG(HIG): Prospective Comparison with Gadolinium Enhanced MRI ($^{99m}Tc$-labelled HIG 스캔을 이용한 류마티스 관절염 환자에서 활막염증의 평가 : 조영증강 자기공명영상과의 전향적인 비교)

  • Many clinical and laboratory tests have been employed to evaluate disease activity in rheumatioid arthritis. $^{99m}Tc$-labelled polyclonal IgG(HIG) has been demonstrated to accumulate in focal sites of infection or inflammation in both animals and human subjects. The purpose of this study was to distinguish arthritis with active inflammation from those without active inflammation and to correlate relative intensities of $^{99m}Tc$-labelled HIG uptake of the rheumatoid arthritis with clinical and MR indices of the joint inflammation. This study included twelve patients with active rheumatoid arthritis, two with ankylosing spondylitis and one with degenerative osteoarthritis without active inflammation. A Whole-body and spot images were obtained 4 hours after intravenous injection of 20mCi of $^{99m}Tc$-labelled HIG. Scintigrams were assessed visually by 3 experienced radiologists, and graded as normal or mildly and markedly increased uptake within the joints, and the degree of uptake was compared with clinical and radiologic severity of synovial inflammation. MRI studies were done on the involved joints consisted of wrist(n = 11), knee(n = 2) and hip joint(n= 2). Active synovitis was defined when marked elevation of ESR and gadolinium enhancement of synovium on MRI were demonstrated. Markedly increased radiotracer uptake was seen in 10 of 11 rheumatoid arthritic patients with active synovitis whereas normal or mildly increased uptakes were noted in others, including rheumatoid arthritic patient(n=1) and non-rheumatoid patients(n = 3) without active synovitis. This study showed that the localization of involved joints in rheumatoid arthritis could be detected with $^{99m}Tc$-labelled HIG and that the degree of uptake correlated well with the degree and activity of inflammation. In conclusion, $^{99m}Tc$-labelled HIG scan is a useful method in the evaluation of active inflammation in rheumatoid arthritis.

  • Line scan diffusion weighted imaging (LSDI) pulse sequence for 0.32 T magnetic resonance imaging (MRI) system was developed. In the LSDI pulse sequence, the imaging volume is formed by the intersection of the two perpendicular planes selected by the two slice-selective $\pi$/2-pulse and $\pi$-pulse and two diffusion sensitizing gradients placed on the both side of the refocusing $\pi$-pulse and the standard frequency encoding readout was followed. Since the maximum gradient amplitude for the MR system was 15 mT/m the maximum b value was $301.50s/mm^2$. Using the developed LSDI pulse sequence, the diffusion weighted images for the aqueous NaCl solution phantom and triacylglycerol solution phantom calculated from the line scan diffusion weighted images gives the same results within the standard error range (mean diffusivities = $963.90{\pm}79.83({\times}10^{-6}mm^2/s)$ at 0.32 T, $956.77{\pm}4.12({\times}10^{-6}mm^2/s)$ at 1.5 T) and the LSDI images were insensitive to the magnetic susceptibility difference and chemical shift.

High Resolution MR Images from 3T Active-Shield Whole-Body MRI System (3T 능동차페형 전신 자기공명영상 장비로부터 얻어진 고해상도 자기공명영상)

  • Purpose : Within a clinically acceptable time frame, we obtained the high resolution MR images of the human brain, knee, foot and wrist from 3T whole-body MRI system which was equipped with the world first 37 active shield magnet. Materials and Methods : Spin echo (SE) and Fast Spin Echo (FSE) images were obtained from the human brain, knee, foot and wrist of normal subjects using a homemade birdcage and transverse electromagnetic (TEM) resonators operating in quadrature and tuned to 128 MHz. For acquisition of MR images of knee, foot and wrist, we employed a homemade saddle shaped RF coil. Topical common acquisition parameters were as follows: matrix=$512{\times}512$, field of view (FOV) =20 cm, slice thickness = 3 mm, number of excitations (NEX)=1. For T1-weighted MR images, we used TR = 500 ms, TE = 10 or 17.4 ms. For T2-weighted MR images, we used TR=4000 ms, TE = 108 ms. Results : Signal to noise ratio (SNR) of 3T system was measured 2.7 times greater than that of prevalent 1.5T system. MR images obtained from 3T system revealed numerous small venous structures throughout the image plane and provided reasonable delineation between gray and white matter. Conclusion The present results demonstrate that the MR images from 3T system could provide better diagnostic quali\ulcorner of resolution and sensitivity than those of 1.5T system. The elevated SNR observed in the 3T high field magnetic resonance imaging can be utilized to acquire images with a level of resolution approaching the microscopic structural level under in vivo conditions. These images represent a significant advance in our ability to examine small anatomical features with noninvasive imaging methods.

Correlation between Cephalhematomas and Intracranial Hematomas (신생아 두혈종과 두개내 혈종과의 연관성)

  • Purpose : Cephalhematomas rarely lead to serious complications, such as skull fractures and intracranial hematomas, so CT and/or MRI scans are indicated only in cases in which depressed fractures are suspected or neurologic symptoms develop. Nevertheless, we have experienced several cases of cephalhematomas associated with intracranial hematomas in the absence of remarkable neurologic symptoms. The aim of this study was to evaluate the correlation between cephalhematomas and intracranial hematomas and determine the need for neuroimaging in infants with cephalhematomas. Methods : Infants who were admitted to the NICU with cephalhematomas and underwent neuroimaging (CT and/or MRI) between January 2002 and July 2006 were evaluated. Neuroimaging was done when the symptoms suggested the development of an intracranial hematoma. Results : Among 54 infants with cephalhematomas, 18 infants underwent neuroimaging. Six of 18 infants (33.3%) had intracranial hematomas, 4 infants had epidural hematomas, and 2 infants had subdural hematomas. Four of these 6 infants had neurologic symptoms or depressed skull fractures; 2 infants had no neurologic symptoms or depressed skull fractures. The neuroimaging was done to evaluate the cause of an excessive elevation of serum bilirubin and unexplained anemia. There were no remarkable differences between the infants with and without intracranial hematomas with respect to gestational age, birth weight, head circumference, diameter of the cephalhematoma, neurologic symptoms, and other clinical signs and symptoms. Conclusion : Based on this study, intracranial hematomas are common complications of cephalhematomas, thus more careful inspection and neuroimaging may be needed in cases of cephalhematomas in newborns.

Anatomical Analysis of Superior Glenoid and Glenoid Labrum (상부 관절와 및 관절와 순의 해부학적 분석)

  • Purpose: The results of the repair for the superior labrum lesions attaching at the superior glenoid have not been satisfactory in every cases. We wanted to analyze the shoulder MRI and the anatomical morphology and pattern of the superior glenoid at which the superior labrum attaches to get anatomical information helpful in treating the superior labrum. Materials and Methods: We analyzed the coronal images of the shoulder MRI of 108 cases taken at our hospital. Average age was 52 years (range, 17~71 years), 55 males and 53 females. On two coronal images behind the attachment of the long head of biceps that the repair of the SLAP was performed at, the length of the attachment of superior labrum and the angle of the supero-lateral glenoid were measured. Results: The average length of the attachment of superior labrum was $9.78{\pm}1.64\;mm$. The average length was $10.1{\pm}1.61mm$ in male, $9.43{\pm}1.6\;mm$ in female. The angle of the supero-lateral glenoid was $89.6{\pm}7.6$ degrees. Conclusion: The attachment of the superior labrum in coronal plane was shorter in posterior spot than anterior. The angle of the supero-lateral glenoid was less in posterior spot.

Arthroscopic Release of the Extensor Carpi Radialis Brevis Tendon for Chronic Recalcitrant Lateral Epicondylitis (만성 불응성 외 상과염에서 시행한 관절경적 단 요 수근 신건 유리술)

  • Purpose: The objective of this study was to assess the clinical outcome of arthroscopic release of the extensor carpi radialis brevis (ECRB) tendon in chronic recalcitrant lateral epicondylitis and tried to determine any prognostic factors. Materials and Methods: A retrospective review of 24 patients with lateral epicondylitis treated by arthroscopic ECRB release was performed. Outcome measures included a patient self rating and visual analog scale (VAS). Functional evaluation was made with Quick-disabilities of the arm, shoulder and hand (DASH) score system to identify preoperative factors that might be associated with the outcomes. Results: Twenty one (88%) elbows received benefit from the procedure. A mean preoperative VAS pain score and the mean Quick-DASH were significantly improved at final follow up. Age, sex, dominant arm and duration of symptom, presence of capsular tear or calcification did not correlate significantly with the clinical outcome. Three of four patients without T2-weighted high signal focus of the ECRB origin on preoperative magnetic resonance imaging (MRI) showed higher Quick-DASH score. Conclusion: Arthroscopic release of the ECRB is an effective option for chronic recalcitrant lateral epicondylitis. Lack of high signal focus on preoperative MRI is significantly associated with a poor surgical outcome and a considerable factor for the proper surgical indication.

  • Recently the medical field to efficiently process the vast amounts of information to decision trees, neural networks, Bayesian Networks, including the application method of various data mining techniques are investigated. In addition, the basic personal information or patient history, family history, in addition to information such as MRI, HRCT images and additional information to collect and leverage in the diagnosis of disease, improved diagnostic accuracy is to promote a common status. But in real world situations that affect the results much because of the variable exists for a particular data mining techniques to obtain information through the enemy can be seen fairly limited. Medical images were taken as well as a minor can not give a positive impact on the diagnosis, but the proportion increased subjective judgments by the automated system is to deal with difficult issues. As a result of a complex reality, the situation is more advantageous to deal with the relative probability of the multivariate model based on Bayesian network, or TAN in the K2 search algorithm improves due to expansion model has been proposed. At this point, depending on the type of search algorithm applied significantly influenced the performance characteristics of the extended Bayesian network, the performance and suitability of each technique for evaluation of the facts is required. In this paper, we extend the Bayesian network for diagnosis of diseases using the same data were carried out, K2, TAN and changes in search algorithms such as classification accuracy was measured. In the 10-fold cross-validation experiment was performed to compare the performance evaluation based on the analysis and the onset of high-risk classification for patients with HRCT images could be possible to identify high-risk data.

Functional MR Imaging of Language System : Comparative Study between Visual and Auditory Instructions in Word Generation Task (언어 중추 영역에 대한 기능적 자기공명영상: 시각적, 청각적 지시 과제에 관한 비교)

  • To evaluate the usefulness if functional MR imaging(MRI) for the determination of language dominance system and to assess differences in the visual and auditory instrument language generation task according to activation task or activated area. Functional maps of the language area were obtained during visual and auditory instructions in word generation tasks in 6 healthy volunteer with right-handness were examined on a 1.5T scanner and the EPI BOLD technique, and three pulse sequence technique get of the true axial planes. Both task consisted of 96 phases including 6 activations and rests contents. Postprocessing were done on MRDx program by using cross correlation method. Two task compare the blain activation area surveyed of 1anguage lateralization index. To evaluated of the detection rates of Broca. Wernicke, pre-frontal lobe, Supplementary Motor Area (SMA) and pre-motor cortex areas and the differences of language lateraliaztion among two word generation task To lateralization index survey in 1anguage area on right and left in brain get to activation area pixel in brain. Compared to visual and auditory instrument task in the language areas get to the lateralization index. Two language generation task high detection rates of Broca and Wernicke areas. The visual instruction no detected in the auditory area, and auditory instruction no detected in the visual area. There was statistics significant different of them among language generation task. 1'his indicated that language area obtained image of the brain functional MR imaging usefulness in the visual and auditory task instrument.
