Diagnosis of Unruptured Intracranial Aneurysms Using Proton-Density Magnetic Resonance Angiography: A Comparison With High-Resolution Time-of-Flight Magnetic Resonance Angiography

  • Objective: Differentiating intracranial aneurysms from normal variants using CT angiography (CTA) or MR angiography (MRA) poses significant challenges. This study aimed to evaluate the efficacy of proton-density MRA (PD-MRA) compared to high-resolution time-of-flight MRA (HR-MRA) in diagnosing aneurysms among patients with indeterminate findings on conventional CTA or MRA. Materials and Methods: In this retrospective analysis, we included patients who underwent both PD-MRA and HR-MRA from August 2020 to July 2022 to assess lesions deemed indeterminate on prior conventional CTA or MRA examinations. Three experienced neuroradiologists independently reviewed the lesions using HR-MRA and PD-MRA with reconstructed voxel sizes of 0.253 mm3 or 0.23 mm3, respectively. A neurointerventionist established the gold standard with digital subtraction angiography. We compared the performance of HR-MRA, PD-MRA (0.253-mm3 voxel), and PD-MRA (0.23-mm3 voxel) in diagnosing aneurysms, both per lesion and per patient. The Fleiss kappa statistic was used to calculate inter-reader agreement. Results: The study involved 109 patients (average age 57.4 ± 11.0 years; male:female ratio, 11:98) with 141 indeterminate lesions. Of these, 78 lesions (55.3%) in 69 patients were confirmed as aneurysms by the reference standard. PD-MRA (0.253-mm3 voxel) exhibited significantly higher per-lesion diagnostic performance compared to HR-MRA across all three readers: sensitivity ranged from 87.2%-91.0% versus 66.7%-70.5%; specificity from 93.7%-96.8% versus 58.7%-68.3%; and accuracy from 90.8%-92.9% versus 63.8%-69.5% (P ≤ 0.003). Furthermore, PD-MRA (0.253-mm3 voxel) demonstrated significantly superior per-patient specificity and accuracy compared to HR-MRA across all evaluators (P ≤ 0.013). The diagnostic accuracy of PD-MRA (0.23-mm3 voxel) surpassed that of HR-MRA and was comparable to PD-MRA (0.253-mm3 voxel). The kappa values for inter-reader agreements were significantly higher in PD-MRA (0.820-0.938) than in HR-MRA (0.447-0.510). Conclusion: PD-MRA outperformed HR-MRA in diagnostic accuracy and demonstrated almost perfect inter-reader consistency in identifying intracranial aneurysms among patients with lesions initially indeterminate on CTA or MRA.

Usefulness of Silent MRA for Evaluation of Aneurysm after Stent-Assisted Coil Embolization

  • Objective: To determine the usefulness of Silent MR angiography (MRA) for evaluating intracranial aneurysms treated with stent-assisted coil embolization. Materials and Methods: Ninety-nine patients (101 aneurysms) treated with stent-assisted coil embolization (Neuroform atlas, 71 cases; Enterprise, 17; LVIS Jr, 9; and Solitaire AB, 4 cases) underwent time-of-flight (TOF) MRA and Silent MRA in the same session using a 3T MRI system within 24 hours of embolization. Two radiologists independently interpreted both MRA images retrospectively and rated the image quality using a 5-point Likert scale. The image quality and diagnostic accuracy of the two modalities in the detection of aneurysm occlusion were further compared based on the stent design and the site of aneurysm. Results: The average image quality scores of the Silent MRA and TOF MRA were 4.38 ± 0.83 and 2.78 ± 1.04, respectively (p < 0.001), with an almost perfect interobserver agreement. Silent MRA had a significantly higher image quality score than TOF MRA at the distal internal carotid artery (n = 57, 4.25 ± 0.91 vs. 3.05 ± 1.16, p < 0.001), middle cerebral artery (n = 21, 4.57 ± 0.75 vs. 2.19 ± 0.68, p < 0.001), anterior cerebral artery (n = 13, 4.54 ± 0.66 vs. 2.46 ± 0.66, p < 0.001), and posterior circulation artery (n = 10, 4.50 ± 0.71 vs. 2.90 ± 0.74, p = 0.013). Silent MRA had superior image quality score to TOF MRA in the stented arteries when using Neuroform atlas (4.66 ± 0.53 vs. 3.21 ± 0.84, p < 0.001), Enterprise (3.29 ± 1.59 vs. 1.59 ± 0.51, p = 0.003), LVIS Jr (4.33 ± 1.89 vs. 1.89 ± 0.78, p = 0.033), and Solitaire AB stents (4.00 ± 2.25 vs. 2.25 ± 0.96, p = 0.356). The interpretation of the status of aneurysm occlusion exhibited significantly higher sensitivity with Silent MRA than with TOF MRA when using the Neuroform Atlas stent (96.4% vs. 14.3%, respectively, p < 0.001) and LVIS Jr stent (100% vs. 20%, respectively, p = 0.046). Conclusion: Silent MRA can be useful to evaluate aneurysms treated with stent-assisted coil embolization, regardless of the aneurysm location and type of stent used.

  • This study analyzed that the effects of AEO MRA benefit on decreasing of trade cost and the strategies for expanding of trade. It uses the system GMM for effective solutions of endogenous matter with lagged dependent variable. In terms of the result of analysis, AEO MRA has a positive effect on decreasing of trade cost, especially this study proved the result of previous study AEO MRA expanded the trade through improving the time required for customs clearance and deregulation of non-tariff barriers. In conclusion, this study proposes the policy fo AEO MRA by analyzing the trade cost of AEO MRA by using the system GMM.

  • Multiresoluton analysis(MRA) of space of square integrable functions defined on whole entire line has been well-known. But for many applications, MRA on bounded interval was required and studied. In this paper we give a MRA for $L^2$(0, 1) by means of periodic wavelets based on regular MRA for $L^2$(R) and give the convergence of partial sums.

Effects of Dietary Allium fistulosum L. and Tannic Acid on in vitro Ruminal Fermentation Characteristics and Methane Emission (국내산 파(Allium fistulosum L.)와 탄닌산을 이용한 사료첨가제가 in vitro 반추위 발효성상과 메탄 발생에 미치는 영향)

  • This study was conducted to investigate for the natural methane emission inhibitor as a feed additive no adversely effect on rumen fermentation. Five different Control (Wheat barn (0.05 g), MRA(Methane Reduction Additive)-1 (Allium fistulosum L. (0.05 g)), MRA-2 (Sodium Lauryl Sulfate (0.025 g) + Wheat barn (0.025 g) mixed), MRA-3 (Sodium Dodecyl Sulfate (0.025 g) + Wheat barn (0.025 g) mixed), and MRA-4 (Allium fistulosum L. (0.02 g) + Tannic acid (0.02 g) + Wheat barn (0.01 g) mixed) contents were used to perform 3, 6, 9, 12, 24 and 48 h incubation for in vitro fermentation. Ruminal pH values were ranged within normal ruminal microbial fermentation. Dry matter digestibility was not significantly different across the treatments during the whole fermentation time. Also, the result of microbial growth had no adversely effect on during the whole fermentation time. At 24 h, methane emission was significantly lower (P<0.05) than all treatments except to MRA-1. Especially, MRA-4 carbon dioxide emission was significantly lower (P<0.05) than control at 9, 24 and 48 h incubation. In addition MRA-4 propionate concentration was significantly higher (P<0.05) than control at 24 h incubation. The result of RT-PCR Ciliate-associated methanogens were significantly lower (P<0.05) at MRA-1, MRA-3 and MRA-4 than control at 24 h incubation. Based on the present results, MRA-4 could be suggestible methane emission inhibitor as a natural feed additive.

  • 뇌 혈관 영상은 2D로 되어있어 임상에서 뇌의 이상 유무와 질병의 진행 정도를 판별하는데 어려움이 있다. Volume Rendering은 2차원 데이터를 3차원 영상으로 재구성하여 오브젝트의 내부 모습을 3차원으로 볼 수 있게 해주는 장점이 있어 진단에 도움을 줄 수가 있다. MRA(Magnetic Resonance Angiography) 는 MRI(Magnetic Resonance Imaging)을 이용하여 Vascular Imaging 하는 기법이다. MRA 혈관 영상을 가시화하는 방법으로 MIP(Maximum Intensity Projection)를 이용하였다. 본 논문에서는 256×256 크기의 MRA영상 48장을 MIP 로 볼륨 랜더링하여 뇌 혈관 영상을 3차원으로 가시화 하였다.

eMRA: Extension of MRA Considering the Relationships Between MDR Concepts (eMRA: MDR의 개념간 관계성을 고려한 MRA 확장)

  • Metadata registry (MDR) is the international standard, developed by ISO/IEC for exchange and sharing data between databases. Many MDR systems are used in diverse domains such as medical service, bibliography, environment for sharing and integrating data. However, those systems have different physical structures individually because the MDR standard defines only the metamodel for registering and storing metadata. It causes heterogeneity between the system structures and requires additional cost to maintain interoperability. ISO/IEC 13249-8 Metadata Registry Access (MRA) is developing as an international standard to provide a consistent access facility to data stored in different metadata registries. However, MRA does not consider the relationships between the concepts (classes) defined in the MDR specification. It causes that incorrect query results returned from MDR systems. It also requires additional cost of modeling and rewriting queries to reflect each physical model. Therefore, this paper suggests eMRA which considers the relationships between the concepts in MDR. The comparative evaluations are described to show the advantages of eMRA. eMRA has superior performance in query modeling and referential integrity than MRA defined by the relationship between the concept of MDR.

Maternal Role Attainment of Primiparous During the Postpartum Period (산욕기 초산모의 어머니 역할획득에 관한 연구)

  • This study was undertaken to identify the levels and affecting factors of the maternal role attainment(MRA) in the primipara during the postpartum period. The healthy ninety primiparous from the one university hospital and two local clinics in KwangJu city were selected and two Semantic Differential Scales (SD-Myself as Mothers, SD-My Baby) and the Pharis Self Confidence Scale were used in this study. Questionnaires were distributed at the 3rd days and the 4-6 weeks of the primiparous not showing any complication after normal delivery. The data collected were analysed statistically using t-test, Pearson's Product Moment Correlation Coefficient and ANOVA. The results obtained were summarized as follows; 1) On the 3rd day after the delivery, the scores of SD-myself as mother, SD-baby and Pharis Self Confidence were 70.6 points, 73.6 points and 78.6 points, respectively, showing the low level of MRA. 2) On the 4-6 weeks after delivery, the score of SD-myself as mother, SD-baby and Pharis Self Confidence were 72.8 points, 77.9 points, and 86.9 points, respectively, indicating the moderate level of MRA. 3) The mean scores of the SD scale and the Pharis Self Confidence during the postpartum periods were higher than those of the 3rd days, showing the SD-myself as mother (t=-2.09, P<.05), SD-baby(t=-4.12, P<.001), Pharis Self Confidence(t=-6.59, P<.001), respectively. 4) Positive correlations (r=.24$\sim$.69) were shown in the concepts related to the MRA and the cognitive-motor skill components and cognitive-affective skill components of the MRA became harmonious over time. 5) The relationships between the score of the MRA and the demographic and obstetric variables were as follows ; a) the score of the MRA in the twenties was higher than those of the thirties. b) the group with higher educational background showed higher MRA socres than the group with lower one. c) those who wanted pregnancy sustenance had higher MRA scores than those who did not. d) the group that did think of festus-feature represented higher MRA scores than those who did not. e) the group of mothers who have the daughters showed higher MRA scores than those who have boys. It can be concluded from the results that the MRA in the primiparous increased gradually, and that the cognitive-motor skills and cognitive-affective skills became harmonious over time. The level of the MRA was affected partly by the mothers general, obstetrical variables. Following suggestion were made oil the basis of the present study ; a) The longitudinal study on the MRA is needed. b) Multivariate analyses should be done for the identification of the factors influcening on the MRA. c) Education program for primiparous mother should be designed and developed to improve the MRA.

  • In this paper, it is implemented that Sub- MRA PWM techinque which is applied to MRA PWM technique using the Digital Signal Processor. Unstable element of analog is reduced for Sub - MRA PWM technique by digital signal pressing. And harmonic is analized by simulation to verify that. It is afford the process induction motor control with real time by minimizing the delay time of digital system. Time delay which is a defect of digital control can by minimized using fast caculation. Therefore, real time control is implemented in the induction motor

Combined effect of lightweight fine aggregate and micro rubber ash on the properties of cement mortar

  • Exterior walls in buildings are exposed to various forms of thermal loads, which depend on the positions of walls. Therefore, one of the efficient methods for improving the energy competence of buildings is improving the thermal properties of insulation plaster mortar. In this study, lightweight fine aggregate (LWFA) and micro rubber ash (MRA) from recycled tires were used as partial replacements for sand. The flow ability, unit weight, compressive strength, tensile strength, thermal conductivity (K-value), drying shrinkage and microstructure scan of lightweight rubberized mortar (LWRM) were investigated. Ten mixtures of LWRM were prepared as follows: traditional cement mortar (control mixture); three mixes with different percentages of LWFA (25%, 50% and 75%); three mixes with different percentages of MRA (2.5%, 5% and 7.5%); and three mixes consisting both types with determined ratios (25% LWFA+5% MRA, 50% LWFA+5% MRA and 75% LWFA+5% MRA). The flow ability of the mortars was 22±2 cm, and LWRM contained LWFA and MRA. The compressive and tensile strength decreased by approximately 64% and 57%, respectively, when 75% LWFA was used compared with those when the control mix was used. The compressive and tensile strength decreased when 5% MRA was used. By contrast, mixes with determined ratios of LWFA and MRA affected reduced unit weight, K-value and dry shrinkage.
